摘要
目的 探讨维生素D受体 (VDR)基因B/b多态性与慢性肾功能衰竭 (CRF)继发性甲状旁腺功能亢进的关系。方法 慢性肾衰患者 10 1例 ,分为二组 :①高全段甲状旁腺素 (iPTH)组 5 8例 ,iPTH >6 0pmol/L ;②低iPTH组 4 3例 ,iPTH <12pmol/L。正常健康对照 10 0例。采用限制性片段长度 (RFLP)方法分析VDR基因多态性 ,同时检测血清碱性磷酸酶 (ALP)、Ca2 +、P3 -、Mg2 +。结果 维生素D基因型的分布在慢性肾衰高iPTH组BB型 5例 ( 8.8% ) ,Bb型 2 0例 ( 35 .1% ) ,bb型 32例 ( 5 6 .1% ) ;低iPTH组BB型 10例 ( 2 2 .7% ) ,Bb型 19例 ( 4 3.2 % ) ,bb型 15例 ( 34.1% )。正常人群与CRF患者BB、Bb、bb基因型及B、b等位基因分布无明显差异 (P >0 .0 5 ) ;而CRF高iPTH组与低iPTH组相比 ,BB基因型和B等位基因的频率明显减低 (P <0 .0 5 ) ;bb基因型和b等位基因的频率明显升高 (P <0 .0 5 )。
Objective To investigate the relationship between vitamin D receptor(VDR)gene polymorphism and secondary hyperparathyroidism in patients with chronic renal failure(CRF).Methods One hundred and one cases with CRF were divided into two groups.①The high iPTH group included 58 CRF patients whose serum iPTH was above 60 pmol/L .②The low iPTH group included 43 patients whose serum iPTH was below 12 pmol/L.The VDR gene sequences were PCR amplified,and the product was cleaved with the restriction enzyme BsmⅠ.At the same time we examined the blood alkaline phosthatase and electrolyte.Results In the high iPTH group 5 persons were BB type,20 persons were Bb type and 32 persons were bb type.And in the low iPTH group 10 persons were BB type,19 persons were Bb type and 15 persons were bb type.No difference in genotype and allelic gene distribution were found between the control group and the patients with CRF(P>0.05).However,genotypic distribution was significantly different between the high iPTH group and the low iPTH Group.BB genotype and B alleles frequencies were significantly decreased (P<0.05),bb genotype and b alleles frequencies were significantly increased(P<0.05).Conclusion Different iPTH is greatly related with the VDR gene polymorphism in Chinese patients (the Han nationality) with CRF.
