创面封闭负压引流技术促进伤口愈合的作用机制研究进展 被引量:7

摘要 创面封闭负压引流术(VAC)是近年来兴起的一种创面治疗新技术,它能加速各类急、慢创面愈合,尤其适用于慢性难愈性创面和不能立即实施确定性外科闭合手术的伤口,并与传统的负压引流术存在区别。其促进创面愈合的确切作用机制尚未完全清楚,涉及到增加局部血流、消除局部水肿、减少创面渗液的积聚、抑制创面细菌生长、促进细胞增生和肉芽组织生长、保持创面及创周组织的湿润环境、调节胶原酶及明胶酶活性、减轻创伤后免疫抑制等多个方面的综合作用。随着其机理的进一步阐明,VAC 将会得到更加广泛有效的临床应用。 Vacuum-assisted closure(VAC)is a novel wound-healing technique which appeared and developed recently.It can promote the healing of various kinds of wounds including acute and chronic ones,especially the healing of those wounds which is difficult to heal or can not be closed by traditional definite surgical operations.VAC is different from traditional negative-pressure drainage.Though the exact mechanisms of VAC are not yet fully understood,the mechanism of VAC is correlated with the integrative function in increasing localized blood flow, decreasing the localized edema,reducing the accumulation of wound exudation,inhibition of the growth of bacteria, increasing the rates of granulation tissue formation and cellular proliferation,retention of moisture,modulation of the activity of MMPs and TIMPs and alleviation the immunosuppression after trauma.With the further elucidation of its mechanism,VAC is supposed to be clinically applied even more widely and effectively.
