摘要
目的通过1例腰椎间盘突入硬膜内患者探讨其发病机制、影像学表现和临床治疗方法。方法患者术前接受体检及多种影像学检查;确诊后施以椎管探查减压术及椎间盘切除手术;然后笔者结合文献复习讨论。结果患者治愈,出院。结论既往手术可导致硬膜囊内型椎间盘突出症的发病,但其发病机制目前还不明确。GDDTPA加强MRI可用于硬膜囊内型椎间盘突出症的诊断,其他影像学方法诊断硬膜囊内型椎间盘突出症目前还存在争议。硬膜囊内型椎间盘突出症的临床治疗主要通过手术的方法,应尽早治疗。
Objective Through one case of intradural disc herniation( IDDH) in reality to explore its pathogenesis,imaging findings,and clinical treatment. Methods The patient received physical examination and multiple imaging inspections( X-ray,CT,MRI,…and so on) preoperatively,after confirmed diagnosis,was subjected to exploration spinal canal decompression,and resection of nucleus pulposus of intervertebral disc; Then authors also took relevant literature review. Results The patient was cured,then discharged. Conclusion Although the privous operation can lead to the IDDH,but the mechanism of IDDH is still not clear at present. The GD-DTPA enhanced MRI can be used for the diagnosis of IDDH,while it is controversial that if other imaging methods can be used for the diagnosis of IDDH until now. The operation can be used as the main treatment of IDDH as early as possible in clinical practice.
