摘要
目的 寻找多柔比星 (阿霉素 )血管外渗有效的防护方法。方法 用 2 4只新西兰大白兔 ,制作 3个部位静脉外渗模型 ,分别用云南白药乙醇膏、硫酸镁湿敷和碳酸氢钠局部封闭治疗 ,各组分别于 3d、5d和 10d行肉眼及组织病理学观察。结果 5~ 10d后云南白药组表面渗出物明显减少 ,真皮内少量炎细胞浸润 ,充血水肿消失 ;碳酸氢钠组表面大量炎性渗出物附着 ,伴有毛囊坏死 ,少许淋巴细胞浸润 ,充血水肿消失 ;硫酸镁组表面渗出物附着 ,真皮内少许淋巴细胞、嗜酸粒细胞浸润 ,充血水肿 ,组织变性、坏死。结论 云南白药乙醇对多柔比星血管外渗具有良好防护作用 ,碳酸氢钠次之 。
Objective To find an effective protective method for preventing doxorubicin vascular leakage.Methods 24 rabbits were used to establish venous leakage models at three different parts.Partial block therapy was carried out by using Yunnan Baiyao ethanol cream, magnesium sulfate wet compress and sodium bicarbonate separately. Eye-observation and pathological observation were carried out on the 3rd, 5th and 10th day.Results After 5-10 days,surface leakage was less in Yunnan Baiyao group than that in the other two groups. In Yunnan Baiyao group, surface ooze decreased markedly, there was less inflammatory cell infiltration in the derma and swelling edema disappeared.In sodium bicarbonate group, large amounts of inflammatory ooze was found to adhere to the surface, with necrosis of hair follicles, a few lymph cells infiltrated and hyperemia oedema disappeared. In magnesium sulfate group,ooze was found to adhere to the surface, some lymph cells and acidthirst white cells infiltrated in the derma, hyperemia edema appeared in striated muscles, tissues extensively changed, and necrosis appeared.Conclusion Yunnan Baiyao ethanol cream has excellent protection against doxorubicin vascular leakage.Sodium bicarbonate is not as good as Yunnan Baiyao but better than magnesium sulfate.
