摘要
目的:了解法乐氏四联症(tetralogyofFallot,TF)儿童手术后运动功能的自然恢复及其与运动量的关系。方法:对44例TF根治手术后4.0~8.5年的儿童运动量分级同时进行踏车运动试验,记录最大运动功量(maximumworkcapacity,MWC)及MWC时的心率,收缩压和舒张压,并与25例年龄,性别相当的正常儿童进行比较。结果:TF儿童手术后的MWC犤(92.9±32.4)W犦明显低于对照组犤(136.4±39.1)W犦,差异有显著性(t=3.55,P<0.01),达MWC时的心率犤(165±19)次/min犦和收缩压犤(129±14.2)mmHg犦亦明显低于对照组(t=2.09,2.32,P<0.05)。MWC及达MWC时的心率和收缩压与运动耐量分级有关,运动量为Ⅰ~Ⅱ级的儿童,其MWC犤(73.8±21.3)W犦、心率犤(155±21)次/min犦和收缩压犤(121±7.2)mmHg犦不仅低于对照组(t=2.73~3.48,P<0.01)而且低于运动量为Ⅲ~Ⅳ级的儿童(t=2.12~3.24,P<0.05~0.01)。结论:手术治疗不能使TF儿童的运动耐量达到正常,康复训练可能有利于整体运动和心血管功能的恢复。
AIM:To comprehend the changes of exercise capacity and its relationship with self restoring in children with Tetralogy of Fallot(TF) after operation. METHODS:Totally 44 TF patients who received operation 4.0-8.5 years ago were divided according to the amount of exercise and received bicycle ergometer exercise test.Maximum work capacity(MWC) and the heart rate,systolic blood pressure and diastolic blood pressure at MWC were recorded.The data were compared with those of 25 aged and sex matched normal children. RESULTS:The postoperative MWC of TF patients[(92.9±32.4)W] was evidently lower than that in the control group[(136.4±39.1)W],with significant difference(t=3.55,P< 0.01).At MWC,heart rate[(165±19)beats per minute] and systolic blood pressure[(129±14.2)mm Hg] were detected remarkably lower than those in the control group(t=2.09,2.32,P< 0.05).MWC and heart rates,systolic blood pressure at MWC were related to exercise tolerance grade.For children with motion amount of Ⅰ-Ⅱ,their MWC[(73.8±21.3)W],heart rate[(155±21)beats per minute] and blood pressure[(121±7.2)mm Hg]were not only lower than those of the children in the control group(t=2.73-3.48,P< 0.01),but also lower than those of the children with motion amount of Ⅲ Ⅳ(t=2.12-3.24,P< 0.05-0.01). CONCLUSION:Operation cannot make children's exercise tolerance normal,while,rehabilitation training may help recover their exercise capacity and cardiovascular function.
