Differences of cognitive function between genders in patients with obsessive-compulsive disorder

摘要 目的:了解性别对强迫症患者认知功能的影响。方法:对符合DSM-IV及CCMD-III强迫症诊断标准的52例强迫症患者(男33例,女19例)按性别分组,在用药前或停药5个半衰期以上采用神经心理测验(包括知识、算术、数字符号、木块拼图、数字广度、逻辑记忆、视觉记忆、Strooptest、词汇流畅、划痕测验A、B,汉诺塔、威斯康辛卡片分类改良版)进行评定。53例健康人在年龄、性别、受教育年限与研究组匹配后作为对照组,按性别分组后在同期做相同项目的神经心理测验。结果:强迫症组女性仅stroop-c时间犤(161.90±37.43)s犦一项指标优于男性犤(121.99±33.07)s犦(t=3.86,P<0.01),但逻辑即时记忆,stroop-cw时间,stroop-cw错误数,划痕A错误结果未显示出正常女性的优越性(t=-2.11~3.16,P<0.05~0.01)。结论:女性强迫症患者的语言工作记忆、干扰抑制能力、注意转移及选择性注意的损害重于男性。 AIM:To explore the effect of gender on the cognitive function in patients with obsessive compulsive disorder(OCD). METHODS:Totally 52 patients with OCD(33 males and 19 females) were grouped by gender according to Diagnostic and Statistical Manual of Mental Disorders(DSM IV) and the third revision of Chinese Classification of Mental Disorders(CCMD III),and were evaluated by the neuropsychological test before medication or more than 5 half lives after drug withdrawal.The neuropsychological test included the knowledge,mathematics,digit symbol, jigsaw puzzle,digit span,logical memory,visual memory,Stroop test,verbal fluency,trial making A and B,tower of Hanoi and modified version of Wisconsin card sorting test.Another 53 normal persons in the control group,who were paired with the OCD patients by age,gender and years of education,were tested by the same neuropsychological test after being grouped by gender. RESULTS:In the OCD group,only the time of stroop cw was better in females[(161.90±37.43) s]than in males [(121.99±33.07) s](t=3.86,P< 0.01),while the results of logical memory immediately,time of stroop c,error of stroop cw and error of trail making A in females were not better than those in males(t=-2.11-3.16,P< 0.05-0.01). CONCLUSION:The impairment of language work memory,the ability of inhibiting interference,attention shifting and selective attention in females is severer than that in males with OCD.
