摘要
目的 :探讨退行性腰椎疾病棘间韧带MRI表现及其病理学意义 .方法 :对 30例健康志愿者 (对照组 )和 30例退行性腰椎疾病患者 (实验组 )进行腰椎功能性X线摄影和MRI检查 .根据MRI信号强度将L1 S1 棘间韧带的MRI特征分为 5种类型 :1A ,1B ,2 ,3和 4型 ,并选择 1 0例棘间韧带标本进行组织学检查 .结果 :对照组 94 %棘间韧带为 1A型 ;实验组棘间韧带表现为 :1A型 6 0个 ,1B型 31个 ,2型 1 8个 ,3型 1 6个和 4型 2 5个 .1B型患者的平均年龄和椎间盘退变程度显著高于其他类型 ,腰椎不稳与 2型存在相关 ,而很少发生于 1A和 1B型 .组织学检查结果表明 1B型主要为纤维束骨化和坏死 ,2型为炎性细胞弥漫增生 ,3型为脂肪变性 .结论
AIM: To explore the magnetic resonance imaging (MRI) features and their pathologic significance of the interspinous ligaments in degenerative lumbar spinal diseases. METHODS: Thirty healthy volunteers (normal group) and 30 patients with degenerative lumbar spinal diseases (study group) underwent examinations of functional radiography and MRI. The MRI features of the interspinous ligaments L 1 S 1 were classified as five categories according to their signal intensities: type 1A, 1B, 2, 3 and 4. Ten interspinous ligament specimens were selected for the histological examination. RESULTS: In the normal group, 94% interspinous ligaments were classified as type 1A, while in the study group interspinous ligaments were 60 type 1A, 31 type 1B, 18 type 2, 16 type 3 and 25 type 4. The average age and the disc degeneration grade of the patients with type 1B were significantly higher than those of other types. Instability was found to be associated with type 2 rather than types 1A and 1B. The histological examination showed that the ossification and necrosis of fiber bundle predominated over type 1B, proliferation of inflammatory cells over type 2 and fatty degeneration over type 3. CONCLUSION: Lumbar spinal MRI is helpful in identifying normal and various kinds of pathological changes in the interspinous ligaments.
