甘氨双唑钠在恶性肿瘤放疗中的增敏作用 被引量:13

摘要 目的 观察放射增敏剂甘氨双唑钠 (CMNa)配合放疗治疗恶性肿瘤的疗效及放射反应的变化。方法 对 2 0 6例肿瘤患者随机分为增敏组和常规放疗组。常规放疗组 1 0 3例 (对照组 ) ,常规外照射总量DT5 0~70Gy/ 5~ 7周 ,每次 2Gy ,每周 5次 ,均采用 6MVΧ线照射。增敏组 1 0 3例 ,外照射合并甘氨双唑钠 ,外照射同常规放疗组 ;甘氨双唑钠给药方案 :生理盐水 1 0 0ml+CMNa 80 0mg/m2 ,静脉滴注 ,每周三次。结果 ①完全缓解率增敏组和对照组分别为 4 9.6 %和 32 .1 % (χ2 =4 .5 5 ,P =0 .0 2 82 ) ;差异有显著性。②达到完全缓解 (CR)和部分缓解 (PR)增敏组所用放射剂量明显少于对照组。③增敏组放疗期间血常规、心电图、肝肾功能等同对照组 ,无异常改变。④放射不良反应两组差异无显著性。结论 放疗合并甘氨双唑钠疗效优于单纯放疗并减少放射剂量 ; Objective To evaluate the effect of external radiotherapy combine CMNa Sodium on malignant tumor and the change of the radiation reaction.Methods 206 cases of malignant tumor were randomly divided into two groups. Radiosensitization group(RSG 130 cases): routine radiotherapy same as the conventional fraction group. The CMNa dosage regimen: NS 100ml+ CMNa 800mg/m 2 ivgtt, per week 3 times; The conventional fractionation group(CF 130 cases)received routine radiotherapy 2Gy per day, 5times a week to a total dose DT of 50~70Gy in 5~7 weeks. All patients were irradiated by 6MVΧ ray. Results ①The CR: RSG and CF separate were 49.6% and 32.1% (χ 2=4.55,P=0.0282).The CR rate of RSG group is better than CF group,there was significant difference. ②The radiation dose of RSG group is lower than CF group when the CR and PR was achieved. ③RSG group was on schedule of all patients. In time of radiotherapy the blood routine, cardiogram, hepatic and renal function and so on in the RSG group, there is no abnormalities change. ④In aspects of radiation reaction in two groups there was no difference. Conclusion The RSG group curative effect is better than conventional radiotherapy, and it can notably decrease the radiation dose and the radiation reaction and the adverse reaction was not increased.
