摘要
[目的]回顾性分析双柱固定法重建肱骨髁间粉碎性骨折的手术效果。[方法]对12例肱骨髁间粉碎性骨折病人,采用切开复位双柱钢板内固定技术治疗,术后进行临床和影像学检查,评介治疗效果。[结果]其中按Mehne-Matta分型:低T型9例,Y型6例,H型21例,术后1年摄片检查,骨折愈合30例,占83%,骨不愈合2例,占17%,骨折不愈合均为鹰嘴窝,内外柱已愈合且关节面形态未见改变,肘关节功能恢复根据美国特种外科医院,HSS肘关节功能评介标准,优良率为91.7%。[结论]应用双柱钢板治疗肱骨髁间粉碎性骨折,能够做到肱骨远端关节面近乎完全的恢复,是一种安全有效的内固定技术。
[Objective] To summarize the surgical techniques and results of reconstruction of severely comminuted intercondylar fracture of humerus with two columns internal fixation.[Methods] Twelve patients suffered severely comminuted intercondylar fractures were treated with reconstruction of intercondyle with open reduction and internal two columns fixation,depending on the x-ray films and clinical symptoms and findings of physical examination; the results were analyzed. [Results]According to mehne-matte classification system,low T-shaped in 3 cases,Y-shaped in 2 cases and H-shaped 7 cases,all patients were followed up more than 1 year,x-ray films showed 10 cases were union and 2 cases were nonunion. Based on scale of HSS system of elbow joints. the rate of excellent or good was 91.7%. [Conclusion]Open reduction and internal fixation with two columns technique can perfectly restore the articular surfaces and offer early passive or active motion.
