摘要
目的探讨食管床内胃代食管颈胸部吻合治疗食管癌的适用范围与改进措施。方法采用食管床内胃代食管颈胸部吻合术治疗食管癌61例。分析该术式的优缺点及手术操作的改进措施。结果本组手术成功59例,4例(6.6%)发生并发症,其中2例(2.3%)死亡,1例死于奇静脉损伤大出血,另1例因术后严重肺部感染并吻合口瘘死亡;其余2例分别为肺不张与心律失常,经治疗痊愈。结论该术式适用于胸部各段食管癌,只要注意并发症就可取得良好疗效。
Objective To study the indication and improvement of the operative method for stomach replacing esophagus in esophageal bed with the esophagogastrostomy to treat thoracicesophageal carcinoma.Methods To study the ad- vantages and improvement of cervical—thoracicesophageal carcinoma.Results Fifty—nine cases were cured,and 4 cases had complication (the incidence is 6.6%).Two of them were dead (the mortality is 3.3%):one case died of haemorrhage due to injury of azygosrein in the operation and the other case died of serious infection of the lungs and anastomosis leakage after operation.The next two cases who had arrhythmia and pulmonary atelectasis were cured.Conclusion This opeation manner can be used to treat upper,middle and lower thoracic esophageal carcinoma and the curative effect is good but it must attend to complication.
