中国胃肠胰神经内分泌肿瘤病理学诊断共识在临床诊断中的运用 被引量:2

摘要 目的:运用2011年4月我国病理学家发布的'中国胃肠胰神经内分泌肿瘤病理学诊断共识'对胃肠胰神经内分泌肿瘤(GEP-NEN)进行分类,并探讨其临床病理及相关免疫组织化学特点。方法对47例(GEP-NEN)的临床病理及免疫表型进行进行回顾性分析。结果47例GEP-NEN中,发病年龄12~79岁,发病部位依次为直肠17例,食道12例,阑尾10例,十二指肠4例,胃3例,结肠1例,暂无胰腺病例。运用'中国胃肠胰神经内分泌肿瘤病理学诊断共识'分类后,NET35例(G1级22例\G2级13例)、NEC 7例(G3级)、 MANEC 5例。结合相应的组织学特点及免疫组织化学结果 CgA 35/47+;Syn+47\47;CD56+47\47;Ki67+≤2%22例、Ki67+3-20%13例、Ki67+>20%7例(不含MANEC)结论①使用统一规范诊断对NEN进行诊断及分类,不仅能让病理诊断更加规范和易于掌握,同时也便于临床进行规范化治疗,以确保患者得到最理想的治疗。为中国GEP-NEN 随访数据库建立提供规范的数据;②免疫组织化学染色项目 CgA、Syn、Ki67为必选,同时根据笔者相关统计,建议加选CD56.同时针对功能性神经内分泌肿瘤可选择相应的抗体。 Objective:by 2011 April our pathologists published'China gastroenteropancreatic neuroendocrine tumor pathology diagnosis consensus'of gastroenteropancreatic neuroendocrine tumors(GEP-NEN)classification,and to explore the clinical pathological and immunohistochemical features.Methods:47 patients(GEP-NEN)clinical pathology and immunophenotype were analyzed retrospectively.Results:47 cases of GEP-NEN,age 12-79 years old,disease found in rectum in 17 cases,esophagus in 12 cases,10 cases of appendix,4 cases of duodenum,stomach in 3 cases,1 cases of colon,pancreatic cases No.The use of'Chinese gastroenteropancreatic neuroendocrine tumor pathology consensus'after classification,NET35 cases(22 cases of grade G1 in 13 cases of grade\G2,7 cases),NEC(G3),5 cases of MANEC.Features and immunohistochemical results of CgA 35/47+with the corresponding tissue;Syn+47\47;CD56+47\47;Ki67+=2%22,Ki67+3-20%13 cases,Ki67+>20%7 cases(excluding MANEC).Conclusions;①using standardized diagnosis conclusion to carry on the diagnosis and classification of NEN,not only can make the disease diagnosis standard and easy to grasp,and at the same time for clinical standard treatment,to ensure that patients receive treatment is the most ideal.Provide standard data set for China GEP NEN fol ow-up database.②immunohistochemical staining of CgA,Syn,Ki67 project is required,at the same time,according to the relevant statistics,suggest to add CD56.at the same time for functional neuroendocrine tumors can select corresponding antibody.
