肾动态显像评价2型糖尿病肾功能的应用价值

摘要 目的:探讨肾动态显像对糖尿病肾功能评估的应用价值,为临床诊断和治疗提供相关依据。方法将66例2型糖尿病患者根据24h尿微量白蛋白排泄率(UAER)测定结果分为三组:DM1组,DM2组,DM3组,将30例健康者设为对照组(NC组)。各组均进行肾动态显像并测定血清肌酐(Scr)、尿素氮(BUN),观察肾小球滤过率(GFR)、高峰时间(tb)、半排时间(T1/2)、20min残留率(C20)以及 Scr、BUN各项指标。结果糖尿病各组 GFR分别为(95.06±20.78)ml/min、(82.64±18.65)ml/min、(33.77±20.50)ml/min,与NC组GFR(111.95±15.24) ml/min 比较,GFR均降低,差异有显著性意义(P<0.05、P<0.01、P<0.01)。仅 DM3组 BUN、SCr较NC组升高(P<0.01);DM1、DM2、DM3组与NC组比较,tb、T1/2延长(P<0.01),C20增高(P<0.01)。结论肾动态显像能够早期发现2型糖尿病患者肾功能变化并了解肾功能受损程度,为临床预防及治疗糖尿病肾病(DN)提供可靠依据。 Objective To evaluate renal dynamic imaging in judgment of renal functions in patients with type 2 diabetes mel itus so as to provide relevant basis for clinical diagnosis and treatment.Methods Sixty-six patients with type 2 diabetes mel itus classified by the urinary albumin excretion rate(UAER)into 3 grope:grope 1,grope 2,grope 3.Thirty normal people without diabetes mel itus were included into control grope.Renal dynamic imaging were performed on al these cases as wel as the serum creatinine(SCr)and blood erea nitrogen(BUN)had been done,the survey of the glomerular filtration rate(GFR),the renal function curve peak time(tb),the half activity time(T1/2),the 20 min residual rate(C20),the SCr and BUN had been done;each grope of type 2 diabetes mel itus compared with control grope,just the grope of DM3 whose BUN and SCr were higher than the control grope;every grope of type 2 diabetes mel itus the tb、T1/2 were longer than the control grope,and the C20 was higher than control grope.Results The GFR of each grope of type 2 diabetes mel itus were 95.06±20.78,82.64±18.65,33.77±20.50(ml/min),were al statistical y lower than the control grope which GFR was 111.95±15.24(ml/min),(P<0.05,P<0.01 and P<0.01).Conclusion Renal dynamic imaging is helpful for the early detection of the change of renal functions in patients with type 2 diabetes mel itus,judge the injury of the renal function and provide relevant basis for clinical prevention and diagnosis the Diabetic Nephropathy(DN).
