摘要
目的 探讨参黄膏贴敷兔神阙穴对小肠吻合口愈合的促进作用.方法? 将24只新西兰兔随机均分为对照组及参黄膏组,均行回肠部分切除吻合术,参黄膏组术后给予参黄膏外敷兔神阙穴.在术后3d和7d分别测定小肠吻合口破裂压、胶原蛋白光密度百分值水平、吻合口羟脯氨酸水平、吻合口YAP、ZAT和TGF-β1蛋白表达.结果? 两组术后均未出现腹腔脓肿和吻合口漏;术后第7天参黄膏组吻合口破裂压力增加,差异具有统计学意义(P<0.05);术后第3天和第7天参黄膏组羟脯氨酸水平明显增高,差异有统计学意义(P<0.05,P<0.01);术后7d参黄膏组胶原蛋白光密度百分值水平显著增加(P<0.01);术后7d参黄膏组吻合口组织YAP、ZAT和TGF-β1蛋白表达明显增加.结论? 参黄膏外敷神阙穴可通过Hippo信号通路促进兔小肠吻合口胶原纤维合成和吻合口愈合.
Objective To study the positive effect of Shenhuang ointment administered transdermally by Shenque point on rabbit intestinal anastomotic wound healing after bowel resection.Methods 24 New Zealand rabbits were randomly divided into Shenhuang ointment group in which Shenhuang ointment were administered transdermally by Shenque point(q12h)during intestinal anastomosis after bowel resection,and control group in which only intestinal resection and anastomosis was performed.Then the incidence of anastomotic leakage,the anastomotic bursting pressure,the degree of collagen deposition,the level of hydroxyproline and expression of YAP,ZAT and TGF-β1 at anastomotic stoma were observed.Results There was no abdominal abscess and anastomotic leakage after operation in both groups.The bursting pressure of small bowel anastomoses was significantly higher in the Shenhuang ointment group than in the control group 7 d after anastomosis(P<0.05).The level of hydroxyproline in anastomotic tissue 3d and 7 d after anastomosis in the Shenhuang ointment group was more than that in the control group(P<0.05,P<0.01).The area of collagen fibrils 7d after intestinal anastomosis in the Shenhuang ointment group were significantly more than that in the control group(P<0.01).The expression of YAP,ZAT and TGF-β1 proteins in the anastomotic tissue of SHG group increased significantly 7 days after intestinal anastomosis.Conclusion Shenhuang ointment administered transdermally by Shenque point may therefore positively affect the rigidity of intestinal anastomosis by Hippo pathway.
