摘要
目的探讨互联网+指导下居家护理在脑卒中出院带气管切开套管患者中的应用效果.方法选择2019年6月—2020年10月该院收治的脑卒中治疗后生命体征平稳但需带气管切开套管出院患者80例为研究对象,按随机数字表法分为两组,每组40例.对照组采用常规居家护理,观察组采用互联网+指导下居家护理,比较两组患者护理前后的奥马哈KBS评分、居家护理评分及护理后看护人的相关知识与能力评分,统计两组患者护理期间的并发症发生率及再入院情况.结果护理1周后和护理1个月后,观察组患者的奥马哈KBS评分高于对照组,差异有统计学意义(P<0.05).护理1个月后,观察组的居家护理评分高于对照组,且看护者对敷料更换、保持气道通畅、更换套管指征把握及家庭护理熟练程度的评分均高于对照组,组间差异有统计学意义(P<0.05).观察组护理期间的并发症发生率为5.00%,再住院率为5.00%,低于对照组的32.50%及35.00%,组间差异有统计学意义(P<0.05).结论脑卒中带气管切开套管出院患者采用互联网+指导下居家护理可更好地改善其生活质量,提高患者居家护理效果及其看护人的主观认知,减少并发症发生率及再入院率,值得推广使用.
Objective To explore the application effect of home nursing under the guidance of Internet+in patients with stroke discharged from hospital with tracheotomy trocar.Methods A total of 80 patients with stable vital signs after stroke treatment who needed tracheal trocar in the hospital from June 2019 to October 2020 were selected as the study subjects,and they were divided into two groups according to random number table method,with 40 patients in each group.The control group received routine home nursing,and the observation group received home nursing under the guidance of Internet+.The Omaha KBS score,home nursing score and related knowledge and ability score of caregivers were compared between the two groups before and after nursing,and the incidence of complications and readmission during nursing were counted between the two groups.Results After 1 week and 1 month of nursing,the Omaha KBS seore of the observation group was higher than that of the control grpup,the difference was statistically significant(P<0.05).After 1 month of nursing,the score of ho me nursing in the observation group was higher than that in the control group,and the scores of caregivers on dressing change,keeping airway unobstructed,indication of casing replacement and home nursing proficiency were higher than those in the control group,the differences between the groups were statistically significant(P<0.05).The complication rate and re-hospitalization rate of the observation group were 5.00%and 5.00%,respectively,lower than 32.50%and 35.00%of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Home nursing under the guidance of Internet+can better improve the quality of life of stroke patients discharged from hospital with tracheostomy cannulas,improve the effect of home nursing and the subjective cognition of caregivers,reduce the incidence of complications and readmission rate,which is worth promoting.
