摘要
目的探究两种预冲方法在无肝素血液透析中的抗凝不同效果.方法将2018年9月-2020年1月入选的30例血液透析患者为主要研究对象,采用自身对照方式,在1周内进行两次无肝素透析治疗,即改良密闭式循环预冲法无肝素透析和常规浸泡法无肝素透析.对比两种不同冲洗方式治疗参数与压力指标及滤器凝血情况.结果改良组置换液总量多于常规浸泡组,改良组跨膜压和静脉压值明显低于常规浸泡组,差异有统计学意义(P<0.05).改良组凝血程度低于常规浸泡组,差异有统计学意义(P<0.05).结论采用采用无肝素改良预冲方法能够有效保持滤器的湿度使治疗更加安全,并能够减少凝血发生的次数.
Objective To investigate the anticoagulation effect of two kinds of pre-flushing methods in heparin-free hemodialysis.Methods 30 hemodialysis patients enrolled from September 2018 to January 2020 were selected as the main research objects.They were treated with heparin-free dialysis twice within one week by self-control method,that is,heparin-free dialysis by modified closed circulation pre-flushing method and heparinfree dialysis by conventional immersion method.The treatment parameters,pressure index and coagulation condition of filter were compared between two different irrigation methods.Results The total amount of replacement fluid in the modified group was higher than that in the conventional group,and the values of transmembrane pressure and venous pressure in the modified group were significantly lower than that in the conventional group,with statistical significance(<0.05).The degree of coagulation in the modified group was lower than that in the conventional immersion group,and the difference was statistically significant(<0.05).Conclusion The heparin-free modified preflush method can effectively maintain the humidity of the filter and make the treatment safer and reduce the number of coagulation.
