摘要
目的:对输尿管软镜碎石术(flexible ureteroscopic lithotripsy,FURL)前双J管预置时间进行系统评价。方法:由2名研究者运用计算机独立检索Pubmed、Embase、CochraneLibary、中国知网、万方、维普数据库中与输尿管软镜碎石术前预置双J管时间的相关研究。根据FURL术前双J管预置时间进行分组,双J管预置时间为1周设为观察组,预置时间为2周设为对照组;观察两组在放置双J管期间并发症发生或解痉药物使用、术后结石清除率、手术时间以及术后并发症的差异。对所纳入的文献进行质量学评价,采用RevMan5.3软件进行Meta分析。结果:纳入病例对照研究5篇、随机对照试验2篇,共计7篇文献。共包含1281例患者,观察组704例,对照组577例。Meta分析结果显示,观察组在放置双J管期间并发症发生或解痉药物使用上低于对照组(OR=0.45,95%CI:0.29-0.71,P<0.05),在术后结石清除率(OR=0.88,95%CI:0.64-1.22,P=0.46)、手术时间(WMD=0.00,95%CI:-3.80-3.81,P=1.00)、术后并发症(OR=1.13,95%CI:0.34-3.76,P=0.84)三方面比较上,观察组与对照组的差异无统计学意义。结论:输尿管软镜碎石术前将双J管预置时间控制在1周,可以降低置管期间并发症的发生或解痉药的使用,而不影响术后结石清除率,在术后并发症的发生和手术时间上无明显影响。
Objective:To systematically evaluate the pre-placement time of double J stents before flexible ureteroscopic lithotripsy.Methods:Two researchers used computer to independently retrieve the pre-placement time of double J stents before flexible ureteroscopic lithotripsy from Pubmed,Embase,CochraneLibary,CNKI,WanFang and VIP database.FURL was grouped according to the pre-placement time of double J stents.The pre-placement time of double J stents was one week as the experimental group,and the pre-placement time was two weeks as the control group.To observe the differences between the two groups in the incidence of complications or the use of spasmolysis drugs during the placement of double J stents,postoperative stone-free rate,operative time and postoperative complications.The included literature was evaluated qualitatively,and RevMan5.3 software was used for Meta analysis.Results:Five case-control studies and two randomized controlled trials were included,and a total of 7 literatures were included.A total of 1281 patients were included,704 in the experimental group and 577 in the control group.Meta analysis results showed that the incidence of complications or the use of spasmolysis drugs during the placement of double J stents in the experimental group was lower than that in the control group(OR=0.45,95%CI:0.29-0.71,P<0.05),the postoperative stone-free rate(OR=0.88,95%CI:0.64-1.22,P=0.46),the operative time(WMD=0.00,95%CI:-3.80-3.81,P=1.00),and the postoperative complications(OR=1.13,95%CI:0.34-3.76,P=0.84).There was no statistically significant difference between the experimental group and the control group.Conclusion:The pre-placement time of double J stents before flexible ureteroscopic lithotripsy for 1 week can reduce the incidence of complications or the use of spasmolysis drugs during the placement of double J stents without affecting the postoperative stone-free rate,and it has no significant influence on the postoperative complications and operation time.
