摘要
目的:评估循证护理可以减少或预防脑卒中患者并发压疮的危险性,为预防脑卒中压疮发生提供参考依据。方法:计算机检索PubMed、EBsco、Wan Fang Data、VIP、CBM、CNKI等数据库,检索数据库的时间是建库至2020年4月循证护理干预脑卒中患者合并压疮,评估纳入的文献偏倚风险后,采用RevMan5.3软件进行Meta分析。结果:纳入的文献有12篇,涉及到1427例患者。Meta分析结果显示,循证护理干预长期卧床脑卒中患者压疮发生率[OR=0.18,95%CI=(0.12,0.27),P<0.00001],出现压疮的时间[SMD合并=2.47,95%CI=(1.75,3.19),P<0.00001],护理满意度[OR=8.16,95%CI=(2.68,24.86),P=0.0002],生活质量评分[SMD合并=3.51,95%CI=(2.48,4.53),P<0.00001]。结论:循证护理在脑卒中并发压疮方面起到了改善作用。考虑到纳入文献的评价质量不高,以上结论仍需高质量和大样本随机对照试验进行验证。
Objective:To assess reducing and preventing effect of evidence-based nursing on the pressure ulcers in patients with stroke using meta-analysis.Methods:The CNKI,VIP,CBM,Wan Fang data,PubMed,EBSco were searched from inception to April,2020 for articles about the effect of the intervention of evidence-based nursing reduce and prevent the pressure ulcers in patients with stroke.Before evaluating risk of bias about incorporated literature,they are analyzed by using RevMan5.3 software.Results:Totally 12 randomized controlled trials,involving 1427 patients.Meta-analysis showed that the intervention of evidenced-based nursing on the effect of pressure ulcers in patients with stroke[OR=0.18,95%CI=(0.12,0.27),P<0.00001],the time of occurrence of pressure ulcers[SMD=2.47,95%CI=(1.75,3.19),P<0.00001],nursing satisfaction[OR=8.16,95%CI=(2.68,24.86),P=0.0002],quality of life score[SMD=3.51,95%CI=(2.48,4.53),P<0.00001].Conclusion:The evidence-based nursing had certain effects on reducing and preventing the pressure ulcers in patients with stroke.Nevertheless,the quality of incorporated papers is low and number of documents are pretty less,thus,we need randomized controlled trials with enough samples and high quality to satisfy above conclusions.
