摘要
目的对外阴阴道念珠菌病的临床感染菌种分布及药敏结果进行分析,为临床抗真菌性阴道炎的治疗及快速诊断方法的建立提供实验室依据。方法对299例阴道分泌物标本进行湿片镜检、聚合酶链式反应(polymerase chain reaction,PCR)核酸扩增及真菌培养,采用MA120全自动细菌鉴定仪及配套鉴定板进行真菌培养鉴定及药敏试验。结果湿片镜检阳性率为14.05%,PCR核酸扩增法阳性率为22.07%,真菌培养阳性率为21.74%;培养分离出念珠菌属5种共65株,其中以白色念珠菌为主(80.00%),其次为光滑念珠菌(15.38%);念珠菌属对伊曲康唑耐药性较高(41.54%),其次为氟康唑(30.77%),白色念珠菌与光滑念珠菌对氟康唑、伊曲康唑的耐药性差异有统计学意义(P<0.01)。结论PCR核酸扩增法和真菌培养法阳性率明显高于湿片镜检法;阴道念珠菌病的临床感染以白色念珠菌和光滑念珠菌最多;念珠菌属对伊曲康唑有较高的耐药性,光滑念珠菌对氟康唑和伊曲康唑耐药性高于白色念珠菌。
Objective To anaysis the distribution and drug sensitivity of clinical infection of vulvovaginal candidiasis,provide a laboratory basis for the treatment and rapid diagnosis of clinical antifungal vaginitis.Methods A total of 299 cases of vaginal discharge specimens were examined by wet film microscopy,polymerase chain reaction(PCR)nucleic acid amplification and fungal culture.MA120 automatic bacterial identification instrument were used for fungal culture identification and drug sensitivity test.Results The positive rate was 14.05%by wet film microscopy,22.07%by PCR nucleic acid amplification and 21.74%by fungal culture.A total of 65 strains were isolated from 5 species of candida and candida albicans was the main species(80.00%),followed by candida smooths(15.38%).The resistance of candida to itraconazole was highly(41.54%),followed by fluconazole(30.77%).The resistance of candida albicans and candida albicans to fluconazole and itraconazole was statistically significant(P<0.01).Conclusion The positive rate of PCR nucleic acid amplification and fungus culture is higher than that of wet film microscopy.The clinical infection of vulvovaginal candidiasis is mainly candida albicans and candida smooths.Candida has a high resistance to itraconazole,and candida slates have a higher resistance to fluconazole and itraconazole than candida albicans.
