摘要
目的调查和分析疗养军事飞行人员甲状腺疾病流行病学特点,为新形势下航卫保障工作提供科学指引。方法收集近10年15580名疗养军事飞行人员的体检鉴定资料,按甲状腺疾病传统触诊筛查模式(2009-01/2017-12月)和普遍筛查模式(甲状腺B超和功能普查,2018-01/2019-03月)进行分组,对甲状腺疾病按照筛查模式、年龄、性别、飞行时间、飞行机种、飞行职务进行分析比较。结果普遍筛查下飞行人员甲状腺疾病检出率35.74%,远高于传统筛查模式(5.49%),其中甲状腺结节、自身免疫性甲状腺炎、甲状腺功能减退症、甲状腺功能亢进症检出最多,总共发现甲状腺癌14例。因甲状腺疾病造成的停飞率为0.76%。甲状腺疾病检出率随年龄增大而增加(P<0.05);随飞行时间增长而增加(P<0.05);女性飞行人员检出率高于男性(P<0.05);不同机种间检出率差异具有统计学意义(P<0.05);而与飞行职务无关(P>0.05)。结论疗养军事飞行人员甲状腺疾病发病率高,严重影响飞行安全,需要进行普遍筛查,飞行人员甲状腺疾病的防治工作将作为未来航卫保障工作的重点。
Objective To investigate and analyze the epidemiological characteristics of thyroid diseases in convalescent military flight personnel,and to provide scientific guidance for the protection of aviation and health in the new situation.Methods The medical examination data of 15580 convalescent military flight personnel over the past 10 years were collected and divided into two groups according to the traditional screening model for thyroid diseases(January 2009 to December 2017)and the universal screening model(January 2018 to March 2019),and the screening model,age,sex,flight time,flight aircraft type and flight position of thyroid diseases were analyzed and compared.Results The detection rate of thyroid diseases among flight personnel under universal screening was higher than that of traditional screening mode(35.74%vs.5.49%),among which thyroid nodules,autoimmune thyroiditis,hypothyroidism and hyperthyroidism were accounted for the most,and a total of 14 cases of thyroid cancer were found.The grounding rate due to thyroid disease was 0.76%.The detection rate of thyroid disease increased with age and flight time(P<0.05),the detection rate of female was higher than male(P<0.05).The detection rate of thyroid disease were significantly difference among the different flight aircraft type(P<0.05).Conclusion The high incidence of thyroid disease in convalescent military flight personnel has a serious impact on flight safety,the universal screening model is needed,and the prevention and treatment of thyroid diseases in flight personnel will be one of the focus of future aerospace medical care.
