摘要
目的通过观察急诊科至消化内科院内交接急性重症胰腺炎(severe acute pancreatitis,SAP)患者的满意度与院内各转运环节的关系,探寻是否存在量化影响交接满意度的环节。方法选取2019-01/2019-06月经作者医院急诊科运送SAP患者/消化内科接受SAP患者的医护人员为受试者。以问卷调查形式考评双方医护人员在本次转运进程的满意度,并采集转运环节(运送/交接)中的各种信息。使用Spearman相关性分析处理双方交接成员的交接满意度与交接各环节的相关性。结果本研究最终获取87次有效的SAP患者院内交接,运送方(急诊科)及接收方(消化内科)分别有99人次、178人次医护人员参与了本次观察性研究。Spearman相关性分析提示:(1)交接所耗时间与运送满意度呈负相关(rs=-0.481,P<0.05);交接进程被其他事件打断次数与运送满意度呈负相关(rs=-0.502,P<0.05)。(2)运送前完成六项基本准备与接收满意度呈正相关(rs=0.542、P<0.05);运送前采取SBAR模式沟通与接收满意度呈正相关(rs=0.611,P<0.05)。结论影响急诊科/消化内科评定院内交接SAP患者的满意度因素可量化。
Objective To observe the relationship between the satisfaction degree of patients with severe acute pancreatitis(SAP)who transferred in the emergency department and the department of digestive medicine and the transfer links in the hospital,to explore whether there are quantitative factors that affect transfer satisfaction.Methods The medical staff who transported SAP patients through the emergency department/patients who received SAP in the department of digestive medicine were selected as the subjects from January 2019 to June 2019.The overall satisfaction of the medical staff of the two sides in the transfer process were evaluated by the questionnaire survey,and the various information during the transfer process(transportation/transfer)collected.The correlation between the transfer satisfaction of the transfer members and each link in the transfer was conducted by Spearman correlation analysis.Results A total of 87 valid in-patient transfers of SAP patients were obtained.A total of 99 person-times and 178 person-time medical staff of transporter(emergency department)and the receiver(gastroenterology)were enrolled in this observational study.Spearman correlation analysis showed that:(1)The time spent in the transfer was negatively correlation with the satisfaction of the transfer(r_s=-0.481,P<0.05).The number of interruptions of transfer process was negatively correlated with delivery satisfaction(r_s=-0.502,P<0.05).(2)The completion of six basic preparations before delivery was positively correlated with the reception satisfaction(r_s=0.542,P<0.05).SBAR mode communication before delivery was positively correlated with reception satisfaction(r_s=0.611,P<0.05).Conclusion The factors influencing the satisfaction degree of SAP patients in the department of emergency gastroenterology can be quantified.
