摘要
目的分析部队中老年良性前列腺增生(benign prostatic hyperplasia,BPH)患者下尿路症状(lower urinary tract symptoms,LUTS)的临床特点及其对生活质量的影响。方法调查300例55岁以上军休干部前列腺增生患者,采用国际前列腺症状评分表(international prostate symptom score,IPSS)和生活质量评分表(quality of life,QOL)进行评估。结果随着年龄增长,患者IPSS总评分升高,LUTS发病率增加。IPSS评分、LUTS症状程度、梗阻程度、刺激程度和QOL评分与年龄均呈正相关(r值依次为0.437、0.413、0.395、0.349、0.244,P均<0.001)。除夜尿增多外,其他6项LUTS症状随增龄呈增高趋势。LUTS中以夜尿增多,排尿不尽、尿频发生率较高。LUTS严重程度增加,患者的QOL评分显著上升(r=0.466,P<0.001)。结论BPH患者年龄越大LUTS越重,对生活质量影响越明显。需加强部队中老年BPH患者中LUTS群体的筛查及宣教工作,早期治疗,调整生活方式,以改善患者的症状及生活质量。
Objective To analyze the clinical characteristics of lower urinary tract symptoms(LUTS)in middle-aged and elderly patients with benign prostatic hyperplasia(BPH)and its impact on quality of life.Methods A total of 300 cases of BPH patients over 55 years old were selected and investigated by international prostate symptom score(IPSS)and quality of life(QOL).Results With increasing of age,the total IPSS score increased and the incidence rate of LUTS increased.IPSS score,LUTS symptom degree,obstruction degree,stimulation degree and QOL score were positively correlated with age(r=0.437,0.413,0.395,0.349 and 0.244,all P<0.001).In addition to increase in nocturia,the other 6 LUTS symptoms increased with age.The incidence of nocturia increased,incomplete urination and frequent urination was higher in LUTS.The severity of LUTS increased and QOL score increased significantly(r=0.466,P<0.001).Conclusion The older the age of BPH patients is,the heavier the LUTS is,and the more obvious the impact on the quality of life is.It is necessary to strengthen the screening and education of LUTS group in middle-aged and elderly people with BPH in the army.Early treatment and adjustment of their lifestyle should be carried out to improve the patient′s symptoms and quality of life.
