摘要
目的探讨早期肠内营养支持对胃癌患者术后恢复的影响。方法选取2018年10月-2019年10月在解放军联勤保障部队第901医院肿瘤内科住院的180例胃癌患者作为研究对象,采用随机抽样方法将其分为肠内营养组(n=90例)和肠外营养组(n=90例),观察两组术前1 d和术后7 d的营养状况指标、免疫功能指标以及胃肠道功能恢复情况。结果180例患者中,男101例(56.2%),女79例(43.8%),平均年龄(59±10.5)岁,术前1 d两组患者体质量、总蛋白、白蛋白无统计学差异(P>0.05),术后7 d肠内营养组体质量[(53.1±1.6)kg vs(50.8±1.6)kg]、总蛋白[(59.3±1.5)kg vs(56.4±1.6)kg]、白蛋白[(37.5±1.7)kg vs(35.1±1.2)kg,P<0.05]明显高于肠外营养组。肠内营养组术后7 d IgA值明显高于肠外营养组[(3.1±0.9)g/L vs(1.9±0.6)g/L,P<0.05],其他免疫功能指标无统计学差异(P>0.05)。肠内营养组首次肛门排气时间[(1.9±0.2)d vs(2.6±0.2)d]、首次肛门排便时间[(2.7±0.2)d vs(3.2±0.8)d]、住院时间[(12.6±1.7)d vs(15.4±1.6)d]明显早/短于肠外营养组(P<0.05);两组患者并发症发生率无统计学差异(校正χ~2=0.000,P=1.000)。结论早期肠内营养对胃癌者术后的营养支持安全有效,能够有效促进患者胃肠道功能恢复和改善营养状况。
Objective To explore the effect of early enteral nutrition support on postoperative recovery in patients with gastric cancer.Methods One hundred and eighty gastric cancer patients admitted to the 901th hospital from October 2018 to October 2019 were selected as subjects.They were randomly divided into enteral nutrition group(n=90)and parenteral nutrition group(n=90).The nutrition status index,immune function index,and gastrointestinal function recovery status of the patients were observed at 1 day before surgery and 7 days after surgery.Results Among the 180 patients,101 were male(56.2%)and 79 were female(43.8%),with an average age of 59.0±10.5 years.There was no significant difference in body weight,total protein,and albumin between the two groups on the day before surgery(all P>0.05).The body weight([53.1±1.6]kg vs[50.8±1.6]kg),total protein([59.3±1.5]kg vs[56.4±1.6]kg)and albumin([37.5±1.7]kg vs[35.1±1.2]kg)of the enteral nutrition group were higher than those of the parenteral nutrition group at 7 days after operation,and the differences were statistically significant(all P<0.05).The IgA value([3.1±0.9]g/L vs[1.9±0.6]g/L)of the enteral nutrition group was significantly higher than that of the parenteral nutrition group at 7 days after operation(P<0.05).There was no significant difference in other immunological indicators(P>0.05).The time to passing gas([1.9±0.2]d vs[2.6±0.2]d),the time to pass stool([2.7±0.2]d vs[3.2±0.8]d),and hospitalization time([12.6±1.7]d vs[15.4±1.6]d)in the enteral group were shorter than those in the parenteral nutrition group,the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Early enteral nutrition is safe and effective for postoperative nutritional support for patients with gastric cancer,which can effectively promote gastrointestinal function recovery and improve nutritional status.
