摘要
目的研究自拟安神养血汤联用艾司唑仑治疗心脾两虚型失眠患者的疗效以及对心率变异性的影响。方法将107例心脾两虚型失眠患者随机分为对照组(54例)和观察组(53例),对照组患者予服艾司唑仑治疗,观察组在此基础上予服自拟安神养血汤,疗程4周。观察两组患者临床疗效、治疗前后匹兹堡睡眠质量指数量表(PSQI)、SPIEGEL量表、疲劳量表(FS-14)、心脾两虚证型评分、心率变异性(HRV)指标,记录研究中出现的不良反应。结果观察组临床总有效率为90.6%(48/53),对照组临床总有效率为74.1%(40/54),经比较,观察组疗效优于对照组(P<0.05),治疗后两组患者的PSQI、SPIEGEL、FS-14及心脾两虚证型评分均下降(P<0.05),且观察组下降程度多于对照组(P<0.05),治疗后两组患者HRV三角指数均上升,且观察组升高多于对照组(P<0.05),观察组治疗后VLF及LF/HF均上升(P<0.05),两组患者不良反应出现情况比较无显著性差异(P>0.05)。结论自拟安神养血汤联用艾司唑仑治疗心脾两虚患者有更好疗效,且有益于改善心率变异性。
Objective To study the effect of Self-Made Anshen Yangxue Decoction(自拟安神养血汤)combined with estazolam on insomnia of sydnrome of deficiency of heart and spleen and its influence on heart rate variability.Methods A total of 107 insomniacs with deficiency of heart and spleen were randomly divided into control group(54 cases)and observation group(53 cases).The control group was treated with estazolam,and the observation group was additionally treated with Anshen Yangxue Decoction for 4 weeks.The clinical efficacy,Pittsburgh Sleep Quality Index(PSQI),SPIEGEL,Fatigue Scale(FS-14),heart and spleen deficiency syndrome score and heart rate variability(HRV)were observed before and after treatment,and the adverse reactions were recorded.Results The total clinical effective rate of the observation group was 90.6%,and that of the control group was 74.1%.The curative effect of the observation group was better than that of the control group(P<0.05).After treatment,the scores of PSQI,SPIEGEL,FS-14 and deficiency of heart and spleen syndrome of the two groups were all decreased(P<0.05),and the decrease degree of the observation group was more than that of the control group(P<0.05).After treatment,the HRV triangular index of the two groups was increased,and the increase in the observation group was more than that in the control group(P<0.05).After treatment,the VLF and LF/HF in the observation group were increased(P<0.05).There was no significant difference in the occurrence of adverse reactions between the two groups(P>0.05).Conclusion The Self-Made Anshen Yangxue Decoction combined with estazolam has better curative effect on patients with deficiency of heart and spleen,and is beneficial to improving heart rate variability.
