摘要
目的探讨替罗非班对进展性脑梗死(PCI)患者神经功能及血小板功能的影响。方法将106例PCI患者依据随机数字表法分为对照组(53例)和观察组53例,对照组采用常规治疗,观察组在常规治疗基础上联合替罗非班治疗。对比两组临床疗效及不良出血情况,观察两组患者治疗前后神经功能及血小板功能差异。结果观察组治疗总有效率为88.68%,显著高于对照组71.70%(P<0.05);两组血尿、牙龈出血等不良出血发生率差异无统计学意义(P>0.05)。与治疗前相比,两组治疗后美国国立卫生研究院卒中表(NIHSS)、改良RANKIN表(mRS)、血清神经肽Y(NPY)、神经元特异性烯醇化酶(NSE)水平及血浆血小板黏附率(PAdT)、血小板聚集率(PAgT)、血小板膜糖蛋白CD62P水平均明显降低,且观察组各指标均低于对照组(P<0.05)。结论替罗非班用于PCI的临床疗效确切,安全性高,能够有效改善血小板功能,促进神经功能恢复。
Objective To investigate the effect of tirofiban on neurological function and platelet function in patients with progressive cerebral infarction(PCI).Method 106 patients with PCI were divided into control group(53 cases)and observation group(53 cases)according to random number table method.The control group received conventional treatment,while the observation group received tirofiban on the basis of conventional treatment.The clinical efficacy and adverse bleeding of two groups were compared,and the neurological and platelet functions of two groups were observed before and after treatment.Results The total effective rate of observation group was 88.68%,significantly higher than that of control group 71.70%(P<0.05).There was no significant difference in incidence of hematuria,gingival bleeding and other adverse bleeding between two groups(P>0.05).Compared with before treatment,the national institutes of health stroke score(NIHSS),modified RANKIN table(mRS),serum neuropeptide Y(NPY),neuronspecific enolase(NSE)levels and blood platelet adhesion rate(PAdT),platelet aggregation rate(PAgT),platelet membrane glycoprotein CD62P levels were significantly lower in two groups were significantly decreased,and the indexes in observation group were lower than control group(P<0.05).Conclusion Tirofiban has definite clinical efficacy and high safety in treatment of PCI,which can effectively improve platelet function and promote neurological function recovery.
