摘要
目的探讨疏肝滋肾清心方对慢性心肾综合征情绪障碍患者神经营养因子、外周血肌钙蛋白Ⅰ(TnⅠ)、氨基末端脑钠肽前体(NT-proBNP)、心脏脂肪酸结合蛋白(H-FABP)、脑钠肽(BNP)水平及心肌能量代谢的影响。方法选择2018年1月—2020年1月唐山市中医医院就诊的93例慢性心肾综合征情绪障碍患者为研究对象,根据随机数字表法将纳入研究的病例分为研究组和对照组,对照组采取休息、合理运动、低盐低脂和优质低蛋白饮食、纠正电解质代谢紊乱和服用常规西药的基础对症治疗方案,研究组在此基础上服用疏肝滋肾清心方,两组患者均连续治疗4周。观察两组患者临床疗效,中医证候积分,外周血TnⅠ、NT-proBNP、H-FABP、BNP含量,心肌能量代谢标志物含量以及神经营养因子含量变化情况。结果治疗后两组各项中医证候积分相较于治疗前均明显降低,且研究组低于对照组(P<0.05);经秩和检验显示,研究组疗效分级情况明显优于对照组(P<0.05);治疗后两组TnⅠ、NT-proBNP、H-FABP、BNP含量相较于治疗前均明显降低,且研究组低于对照组(P<0.05);治疗后两组肌酸激酶(CK)、乳酸脱氢酶(LDH)含量相较于治疗前均明显降低,且研究组低于对照组(P<0.05);治疗后两组脑源性神经营养因子(BDNF)含量相较于治疗前均明显升高,且研究组高于对照组(P<0.05),治疗后两组神经营养因子-3(NT-3)含量相较于治疗前均明显降低,且研究组低于对照组(P<0.05)。结论疏肝滋肾清心方对慢性心肾综合征情绪障碍患者临床疗效较好,可有效降低心肌损伤程度,改善心肌能量代谢并调节神经营养因子含量。
Objective To explore the effects of Shugan Zishen Qingxin Decoction(疏肝滋肾清心方)on neurotrophic factors,peripheral blood TnⅠ,NT-proBNP,H-FABP,BNP levels and myocardial energy metabolism in patients with chronic cardiorenal syndrome mood disorders.Methods 93 patients with chronic cardiorenal syndrome and mood disorders who were treated in the hospital between January 2018 and January 2020 were taken as the research objects.According to the random number table method,the included cases were divided into research group and control group,and the control group took the basic symptomatic treatment plan of rest,reasonable exercise,low-salt,low-fat and high-quality low-protein diet,correction of electrolyte metabolism disorders and taking conventional Western medicine.On this basis,the research group took Shugan Zishen Qingxin Decoction,and both groups of patients were treated for 4 consecutive weeks.Compared the clinical efficacy,TCM syndrome score,peripheral blood TnⅠ,NT-proBNP,H-FABP,BNP content,myocardial energy metabolism marker content,and neurotrophic factor content changes were compared between the two groups of patients.Results After treatment,the scores of various TCM syndromes in the two groups were significantly lower than before treatment,and the research group was lower than the control group(P<0.05);the rank sum test showed that the curative effect classification of the research group was significantly better than the control group(P<0.05);after treatment,the contents of TnⅠ,NTpro BNP,H-FABP,and BNP in the two groups were significantly lower than those before treatment,and the research group was lower than the control group(P<0.05);the contents of CK and LDH in the two groups were significantly lower than those before treatment,and the research group was lower than the control group(P<0.05);after treatment,the BDNF content in the two groups were significantly higher than before treatment,and the research group was higher than the control group(P<0.05);after treatment,the NT-3 content in the two groups was significantly lower than before treatment,and the research group was lower than the control group(P<0.05).Conclusion Shugan Zishen Qingxin Decoction has a good clinical effect on patients with chronic cardiorenal syndrome and mood disorders.It can effectively reduce the degree of myocardial damage,improve myocardial energy metabolism and regulate the content of neurotrophic factors.
