摘要
目的冻融胚胎移植是人类辅助生殖技术发展期间较为重要的组成部分之一,而玻璃化冷冻技术是其中最为有效的方法,具有安全、可靠和易操作等特性,是目前胚囊及卵裂期胚胎的首选冷冻手段。为提高冻融胚胎移植成功率,本研究分析玻璃化冷冻胚胎解冻后卵裂球损伤与增殖对移植结局影响。方法收集2016-01-20-2019-01-20焦作市妇幼保健院生殖中心玻璃化冷冻胚胎解冻1700个,按照复苏后卵裂球损伤情况的不同分为两组,存在损伤情况的作为观察组(1401个),无损伤情况的作为对照组(299个),对各组移植后的临床妊娠率和移植率进行对比分析。结果观察组移植成功率为49.18%,低于对照组的59.53%,差异有统计学意义,χ2=10.568,P=0.001;妊娠率为57.18%,低于对照组的67.98%,差异有统计学意义,χ2=5.823,P=0.016。1401个复苏后卵裂球中,有胚胎卵裂球生长占比最高,为48.11%;无胚胎卵裂球生长占比最低,为6.00%;有胚胎卵裂球融合占21.48%;无胚胎卵裂球融合占24.41%。结论玻璃化冷冻胚胎解冻后,其妊娠率和移植率会受到卵裂球损伤影响。
OBJECTIVE Freeze-thaw embryo transfer is one of the most important components during the development of human assisted reproductive technology.The vitrification technology is the most effective method.It is safe,reliable and easy to operate.It is the current embryo sac and the preferred means of freezing for cleavage stage embryos.In order to improve the success rate of freeze-thaw embryo transfer,this study analyzed the effect of blastomere damage and proliferation on the outcome of graft after thawing of vitrified frozen embryos.METHODS A retrospective analysis of1700 thawedfrozen embryos was carried out.The samples were collected from 20 January,2016 to 20 January,2019 in Reproductive Medicine Center of the Jiaozuo Maternal and Child Health Hospital.They were divided into two groups according to the blastomere injury after resuscitation(1401),as a control group(299)without injury,the clinical pregnancy rate and transplantation rate of each group were compared and analyzed.RESULTS The success rate of transplantation in the observation group was 49.18%,lower than 59.53%in the control group,the difference was statistically significant,χ2=10.568,P=0.001.The pregnancy rate was 57.18%,lower than 67.98%in the control group,the difference was statistically significant,χ2=5.823,P=0.016.Among the 1401 blastomeres after resuscitation,blastomeres with embryos accounted for 48.11%,blastomeres without embryos accounted for 6.00%,blastomeres with embryos accounted for21.48%,and blastomeres without embryos accounted for 24.41%.CONCLUSION After thawing of vitrified frozen embryos,the pregnancy rate and transplantation rate will be affected by blastomere damage.
