摘要
目的复发性口腔溃疡为临床常见疾病,治疗以口腔局部用药为主,虽可缓解溃疡症状,但存在复发风险。为提高疗效和减少复发,本研究探讨掺钕钇铝石榴石(neodymium yttrium aluminum garnet,Nd:YAG)激光联合康复新液和西瓜霜喷剂对复发性口腔溃疡治疗效果。方法选取新乡市第四人民医院2016-02-01-2019-02-01收治的98例复发性口腔溃疡患者为研究对象,根据患者性别、年龄、病程及溃疡特征等因素分为观察组和对照组,各49例。观察组采用Nd:YAG激光联合康复新液和西瓜霜喷剂治疗,对照组采用康复新液联合西瓜霜喷剂治疗。连续治疗5d后,评估两组临床疗效和溃疡相关指标(愈合时间、愈合等级和复发率),观察治疗前后免疫指标[免疫球蛋白A(immunoglobulin A,IgA)、免疫球蛋白G(immunoglobulin G,IgG)和免疫球蛋白M(immunoglobulin M,IgM)]及疼痛(visual analogue scale/sore,VAS)评分。结果观察组总有效率为95.92%,高于对照组的75.51%,χ2=8.333,P=0.004。观察组溃疡愈合时间为(2.96±1.08)d,低于对照组的(5.26±2.11)d,t=6.792,P<0.001;溃疡愈合等级优于对照组,u=3.391,P<0.001;复发率为2.01%,低于对照组的22.45%,χ2=9.496,P<0.001。治疗5d后,观察组血清IgA水平为(189.57±39.42)g/L,高于对照组的(160.32±34.87)g/L,t=3.890,P<0.001;血清IgG水平为(1436.11±213.11)g/L,高于对照组的(854.98±119.10)g/L,t=16.663,P<0.001。观察组治疗后第1天(t=31.886,P<0.001)、第3天(t=30.414,P<0.001)和第5天(t=36.998,P<0.001)VAS评分均低于对照组。结论Nd:YAG激光联合康复新液和西瓜霜喷剂治疗复发性口腔溃疡,可有效减轻疼痛程度,缩短溃疡愈合时间,提高溃疡愈合等级,降低复发率,提高免疫功能。
OBJECTIVE Recurrent oral ulcer is a common clinical disease.It is mainly used in oral cavity.Although it can relieve ulcer symptoms,there is a risk of recurrence.In order to improve the curative effect and reduce recurrence,this study explores yttrium-aluminum garnet(Nd:YAG)Therapeutic effect of laser combined rehabilitation new liquid and watermelon cream spray on recurrent oral ulcer.METHODS A total of 98 patients with recurrent oral ulcers who were admitted to Xinxiang Fourth People’s Hospital from February 1,2016 to February 1,2019 were enrolled.The patients were divided into observation group and control group according to factors such as gender,age,duration of disease and ulcer characteristics.The observation group(n=49,Nd:YAG laser combined with rehabilitation fluid,watermelon cream spray treatment)and the control group(n=49,rehabilitation new fluid combined with watermelon cream spray treatment),after5 days of continuous treatment,the clinical efficacy of the two groups was evaluated.Ulcer related indicators(ulcer healing time,ulcer healing grade,recurrence rate),immune indicators before and after treatment[immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)]and pain(VAS)score were analyzed.RESULTS The total effective rate of the observation group was 95.92%,higher than that of the control group 75.51%(χ2=8.333,P=0.004).The ulcer healing time of the observation group was shorter than that of the control group(t=6.792,P<0.001).Ulcer healiy grade of the observation group better than that(u=3.391,P<0.001).The recurrence rate of the observation group was 2.01%lower than that of the control group(22.45%,χ2=9.496,P<0.001).The serum IgA level in the observation group was higher than that in the control group[(189.57±39.42)vs(160.32±34.87),t=3.890,P<0.001],and the serum IgG level in the observation group was higher than that in the control group[(1436.11±213.11)vs(854.98±119.10),t=16.663,P<0.001].The observation group was treated on the first day(t=31.886,P<0.001),the 3 rd day(t=30.414,P<0.001)and the 5 th day(t=36.998,P<0.001)after treatment were all lower than the control group.CONCLUSION Nd:YAG laser combined with rehabilitation fluid and watermelon cream spray for the treatment of patients with recurrent oral ulcers has a definite curative effect,which can effectively reduce the degree of pain,shorten the healing time of ulcers,improve the healing level of ulcers,reduce the recurrence rate and improve immune function.
