摘要
目的探讨经鼻留置主支气管吸痰管在有气管插管拔除指证且咳痰能力欠佳的危重患者的应用效果。方法选取2017-01—2020-01间在石河子大学医学院第四附属医院急诊监护病房中气管插管已拔除的205例患者,根据住院号单双数随机分为对照组102例、实验组103例,对照组按常规方法经口或经鼻插入吸痰管吸痰,实验组组从鼻腔插入留置主气管吸痰管固定,按需吸痰。比较两组患者的治疗效果。结果血气指标变化、严重并发症、气管插管复插率、气管切开例数、机械通气时间、平均住院天数,实验组均优于对照组(P<0.05)。结论经鼻留置主支气管吸痰管可以通过增加分泌清除率来优化拔管后的呼吸功能,改善缺氧及二氧化碳潴留,降低严重并发症、再次气管插管、气管切开及平均住院时间。
Objective To investigate the effect of nasal indwelling main bronchial sputum suction tube in critically ill patients with indications of tracheal intubation removal and poor sputum ability.Methods A total of 205 patients with tracheal intubation removed in the emergency ward of the fourth affiliated hospital of medicine college of shihezi university from January 2017 to January 2020 were selected and randomly divided into control group 102 cases and experimental group 103 cases according to the hospital number.In the control group,the sputum suction tube was inserted orally or nasally to suck sputum according to the conventional method,and the experimental group was inserted into the nasal indwelling main tracheal suction tube from the nasal cavity and fixed,and suction was performed as needed.Compared the treatment effect of the two groups of patients.Results The blood gas index changes,serious complications,tracheal intubation reintubation rate,number of tracheotomy cases,mechanical ventilation time,and average days of hospitalization in the experimental group were better than the control group(P<0.05).Conclusion Nasal intubation of the main bronchial sputum suction tube can optimize the respiratory function after tracheal intubation removal by increasing secretion clearance rate,improve hypoxia and carbon dioxide retention,reduce serious complications,reintubation,tracheotomy and average hospital stay.
