摘要
目的探讨经皮测血氧饱和度的高低对不同胎龄早产儿视网膜病的影响。方法回顾性的选择2017年10月1日到2019年9月30,由南宁市第二人民医院,南宁市妇幼保健院NICU收治的513例出生胎龄≤34周的早产儿和出生体重<2000 g的早产儿作为研究对象,根据经皮测血氧饱和度(TcSO2)水平和胎龄将早产儿分为A组(n=114):胎龄≤32周,85%≤TcSO2≤93%;B组(n=157):胎龄32≤周,93%<TcSO2≤100%;C组(n=134):胎龄>32周,88%≤TcSO2≤95%;D组(n=108):胎龄>32周,95%<TcSO2≤100%。所有患儿均由眼科医生进行视网膜病的筛查,对比不同组别的发病情况。结果A、B、C、D四组患儿ROP总患病率分别为9.6%,17.8%,7.5%以及12.0%,C组早产儿ROP患病率最低,B组早产儿ROP患病率最高,各组间患病率差异具有统计学意义(χ^(2)=8.216,P<0.05)。结论ROP的发生率与胎龄及TcSO2高低相关,胎龄越小,TcSO2越高,发病率越高.。不同胎龄的早产儿应选择合适的血氧饱和度范围,<32周早产儿血氧饱和度应维持在85%~93%,>32周早产儿血氧饱和度应维持在88%~95%。
Objective To Analysis the effect of different arterial oxygen saturation on incidence of retinopathy(ROP)of premature infants with different gestational age(GA).Method 513 cases of premature infants in NICU from January 2015 to December 2018 were selected as research subjects.According to the average level of oxygen saturation(Sa O2)and GA,all infants were divided into four groups,group A(infants with GA≤13 and SaO285-93%,n=114),group B(infants with GA≤=114),group B(infants with GA≤32,Sa O293%-100%,n=157),group C(infants with GA>32 weeks and Sa O288-95%,n=134)and group D(infants with GA>32 weeks and Sa O295-100%,n=108).The data about the prevalence of ROP was collected.Result The prevalence of ROP in group A,B,C and D were 9.6%,17.8%,7.5%and 12.0%,respectively,and the prevalence of ROP of preterm infants in group C was the lowest,but that of in group B was the highest(χ^(2)=8.216,P=0.0418).The prevalence rate among four group were significantly different.Conclusion The appropriate level of SaO2 for preterm infant with<32 gestational age was 85-93%,and preterm infant with>32 gestational age was 88-95%.
