摘要
目的探讨给予社区膝关节骨性关节炎(KOA)患者补肾活血方治疗的临床效果。方法选取2016年7月至2018年8月北京市顺义区李遂社区卫生服务中心门诊接受诊治的94例社区KOA患者分为两组,给予对照组(n=47)常规西药治疗,观察组(n=47)加服中药汤剂-补肾活血方治疗,疗程均为4周,观察患者关节疼痛症状、体征、关节功能改善情况等。结果经治疗后2、4周及3、6个月,观察组视觉模拟评分法(VAS)评分均显著低于对照组(P<0.05);治疗后6个月,观察组白介素-6 (IL-6)、血清基质蛋白酶3 (MMP-3)、骨性关节炎指数评分表(WOMAC)各项评分及总评分均显著低于对照组(P<0.05);观察组膝关节功能优良率为85.11%、治疗总有效率为97.87%,均显著高于对照组的65.96%、85.11%,差异有统计学意义(P<0.05)。对照组、观察组不良反应总发生率分别为8.51%、4.26%,两组比较差异无统计学意义(P>0.05)。结论在常规西医治疗基础上再给予社区骨关节炎患者补肾活血方治疗,可使患者关节疼痛获得有效减轻,促进患者临床指标、膝关节功能得到更大程度改善,提高总体治疗效果。
Objective To investigate the clinical effect of Bushen Huoxue Prescription onpatients with knee osteoarthritis(KOA) in community.Methods Ninety-four community KOA patients who received treatment in Community Health Service Centre of Lisui in Shunyi,Beijing from July 2016 to August 2018 were enrolled and divided into two groups.The control group(n=47) was given routine western medicine treatment,while the study group(n=47) was given traditional Chinese medicine decoction Bushen Huoxue prescription on the basis of routine western medicine treatment,and the duration of treatment was all 4 weeks.The symptoms,signs of joint patin and improvement of joint function were observed.Results After 2-,4-week and 3-,6-month treatment,the Visual Analogue scale(Vas) scores of the study group were significantly lower than those of the control group(P<0.05).Six months after treatment,the levels of Interleukin-6(IL-6),matrix metalloproteinase-3(MMP-3), the total scores and the score of every item of Osteoarthritis/osteodystrophy Index Score Sheet(WOMAC) scale in the study group were significantly lower than those in the control group(P<0.05).The excellent and good rate of knee joint function was 85.11% and the total effective rate was 97.87% in the study group,which were significantly higher than 65.96% and 85.11% in the control group(P<0.05).The total incidence of adverse reaction in control group and study group was8.51% and 4.26%,respectively,and there was no significant difference between the two groups(P>0.05).Conclusion On the basis of routine western medicine treatment,Bushen Huoxue prescription can effectively alleviate the joints pain,greatly improve the clinical indicators and knee joint function,and the overall therapeutic effect in the patients with KOA in the community.
