摘要
背景:新型冠状病毒肺炎疫情发展迅速,波及广泛,已成为国际关注的突发公卫事件。目的:总结新型冠状病毒肺炎疫情对脊柱外科在日常疾病诊疗方面的影响,结合国家和医院制定的诊疗方案提出针对性的就医流程和科学防控策略。方法:回顾性分析我科2020年春节假期结束后起30 d内脊柱外科门诊量、病房收治患者数量、手术量、手术类型等数据,并与2018年、2019年同期数据进行对比,分析疫情对日常诊疗活动的影响,结合脊柱疾病诊疗原则及新型冠状病毒肺炎诊疗方案,优化脊柱外科疾病诊疗流程、手术室适应证及防控预案。结果:脊柱外科病区2020年春节假期结束后的30 d门诊量为745人次,2018年和2019年同期分别为2407人次和3085人次,受疫情影响,2020年较2019年明显下降,下降约75.9%。病房仅收治患者38例,而2018年和2019年同期分别为160例和183例,整体下降超过75%。相对于2018年的141台手术和2019年的166台手术,2020年同期手术量仅36台,且微创手术占总手术量的比例由2018年的27.5%和2019年的24.6%上升至2020年的68.4%,差异有统计学意义(P=0.001)。通过配合医院制定科学、规范的收住院流程、手术适应证和防控预案,门诊及住院患者均未出现新型冠状病毒感染者。结论:新型冠状病毒肺炎疫情对脊柱外科病区门诊、住院及手术各个诊疗环节均产生了显著的影响。合理、科学、高效的诊疗流程及救治方案对于疫情防控至关重要。
Background: Corona virus disease 2019(COVID-19) spread worldwide in just one month, and it was listed as aPublic Health Emergency of International Concern(PHEIC) by the World Health Organization(WHO). Objective: To analyzethe influence of COVID-19 outbreak on the diagnosis and treatment of spinal diseases, and to put forward correspondingsolutions for scientific prevention and control. Methods: We analyzed the post-outbreak clinical data of the spine surgerydepartment and compared them with those during the same period in 2018 and 2019. According to the treatment principles ofspinal disorders and prevention guidelines of COVID-19 pneumonia, the standard procedures of treatment for spinal surgerypatients were formulated. Results: In 2018 and 2019, 2407 and 3085 out-patient visited to the spine surgery department in ourhospital within 30 days after the spring festival, respectively. But the number of out-patient visits decreased to 745 in 2020,which was about 75.9% lower than that in 2019. There were 160 and 183 patients admitted to our department in 2018 and2019 in this period, respectively, while only 38 patients in 2020, which was decreased by more than 75%. Compared with 141 operations in 2018 and 166 operations in 2019, only 36 operations were performed during the same period in 2020. Theproportion of minimally invasive surgeries in the total number of operations increased from 27.5% in 2018 and 24.6% in 2019 to 68.4% in 2020(P=0.001). No outpatients or inpatients was infected COVID-19. Conclusions: The COVID-19 outbreak hasa significant impact on the daily work of the spine surgery department. A scientific medical process and prevention measuresare of utmost importance.
