摘要
目的分析血清胎盘生长因子(PLGF)在孕早期唐氏综合征(DS)产前筛查中的参考价值。方法本研究选取2018年10月至2019年12月在于我院接受孕早期DS筛查的1806位孕妇为研究对象,48位DS孕妇纳入DS组,其余的1758位孕妇设为对照组。检测和对比两组研究对象中的血清PLGF、AFP、β-hCG和PAPP-A水平,比较各检测方法的诊断敏感性。结果DS组和对照组的研究对象的年龄、孕周和体重均无显著的统计学差异(P均>0.05);孕早期DS组研究对象的PLGF水平显著低于正常对照组(P<0.05);孕早期DS组的PLGF水平显著低于对照组(P<0.05),而两组的AFP、β-hCG、PAPP-A水平均无显著的统计学意义(P均>0.05);孕早期联合PLGF和血清学诊断的AOC面积显著高于单独的PLGF检测、AFP+β-hCG+PAPP-A检测方法(P均<0.05)。结论母体血清PLGF水平DS孕早期筛查的一个敏感指标,值得在临床上结合常规的血清学检测指标行DS的早期筛查。
Objective:To analyze the reference value of serum placental growth factor(PLGF)in prenatal screening for Down′s syndrome(DS)in early pregnancy.Methods:From October 2018 to December 2019,1806 pregnant women who were screened for early pregnancy in our hospital were selected as subjects.48 DS pregnant women were included in the DS group,and the remaining 1758 pregnant women were included in the control group.The levels of serum PLGF,AFP,β-hCG and PAPP-A in the two groups were examined and compared,and the diagnostic sensitivities of each test method were compared.Results:There were no significant differences in age,gestational age and body weight between the DS group and the control group(P>0.05).The PLGF level in the DS group was significantly lower than that in the normal control group(P<0.05).The level of PLGF in DS group was significantly lower than that in the control group(P<0.05),but there was no statistically significant difference in AFP,β-hCG and PAPP-A levels between the two groups(P>0.05).The AOC area of PLGF and serological diagnosis was significantly higher than that of PLGF detection and AFP+β-hCG+PAPP-A assay(P<0.05).Conclusions:The maternal serum PLGF level is a sensitive indicator for DS screening in early pregnancy,it is worthy of clinical use for early screening with conventional serological markers.
