There were similar but not as th...We report on an unusual case presenting with a cardiac pseudo-tumour on echocardiogram, which corresponded to a large soft tissue mural thickening around the mid-right coronary artery. There were similar but not as thick mural lesions around other parts of the coronary arteries. The so-called “pigs-in-a-blanket” sign on computed tomography (CT) scan was pathognomonic of IgG4 coronary arteritis. The IgG4 level was grossly elevated at more than 10 times the upper limit of normal. Positron emission tomography (PET)-CT scans with 18F-fluoro-deoxy-glucose (FDG) and 68Ga-Fibroblast Activation Protein Inhibitor (FAPI) were performed to assess the extent of organ involvement of the IgG4-related disease. The patient was treated with 8 injections of rituximab with good serological response. However, the coronary arteritis findings on CT scan remained unchanged.展开更多

Previously reported patients with uterine incarceration have different symptoms. Rarely, asymptomatic cases persist into the third trimester of pregnancy. In fact, the patients with uterine incarceration can be asymptomatic and normally carry their fetuses to the term because it mainly changes the cervix, which does not affect fetal growth in utero directly. Additionally, cesarean section is both a treatment and a direct method to clarify the diagnosis again, and low-molecular-heparin anticoagulant therapy should be considered immediately after surgery. Here, we present two cases clarifying that patients with uterine incarceration can be pregnant without any discomfort during pregnancy and provide a successful treatment plan.展开更多

In human, tapeworm species can cause a parasitic infection by ingestion of raw or under cooked beef (<em>T. saginata</em>) or pork (<em>T. solium</em> and <em>T. asiatica</em>). Taeniasis usually presents with vague symptoms or mild abdominal pain and discomfort. The intestinal complications are not commonly mentioned in the literatures.<strong> Case presentation:</strong> We present 65-year-old Egyptian woman who lives in Oman, who is known to have type two diabetes mellitus and hypertension. She presented to Emergency department with abdominal pain and constipation for a period of three days. On examination, she was dehydrated with tachycardia (pulse rate: 110/minute) blood pressure of 100/60 mmHg and patient was afebrile. Abdominal examination revealed, mildly distended abdomen and tense, no guarding or rigidity no palpable mass, exaggerated bowel sound and rectum is empty on per rectal examination. Emergency laparotomy was performed with intra-operative findings of large foreign body impacted in the distal jejunum about 8 × 3 cm, cylinder in shape completely obliterating the lumen, distal loops was collapsed. <strong>Conclusion: </strong>Although Taeniasis is a very rare infection, index of suspicion such as a potential in the differential diagnosis as cause of an acute abdomen with unusual surgical complications should be considered. The diagnosis of these rare circumstances is usually made intraoperatively, and surgery is recommended only for the treatment of complications.展开更多

Effectively used as a single agent or combined with a chemotherapy regimen to treat lymphoma, RTX is a significant step forward in the arsenal treatment of idiopathic thrombocytopenic purpura, systemic lupus erythematous, rheumatoid arthritis, and autoimmune hemolytic anemia. Side effects of RTX are commonly seen during the first infusion in up to 50% of patients and include fever, chills, and rigors. These side effects are generally transient and related to the tumor burden, probably due to a greater degree of complement activation and proinflammatory cytokine release. It addresses all nursing staff concerning children, adolescents or adults with cystic fibrosis (CF) either in an inpatient and outpatient setting or in rehab clinics. The guidelines evolve along with the increased life expectancy of cystic fibrosis patients and improved quality of life which is described by CF patients and can be observed by the therapeutic CF team. For example, the treatment of secondary diseases like CFDR (CF related diabetes) which comes along with an increased life expectancy has been added over the years. It is essential that within such a complex disease pattern, professional experience of care staff who worked in specialized CF care centers (50+ patients per year) for many years has to be transferred to other nursing stuff or beginners. This is especially important in the context of the heterogeneous characteristics of this disease. Often it can be seen that the quality of the care for CF patients depends on the know-how and professional experience of the interdisciplinary CF therapists team. Moreover, it depends on the knowledge which the patient has gathered by himself by experience or learning. And, of course, it depends also on the knowledge of the relatives especially parents or partner respectively.展开更多

The median age at diagnosis of IP by MPO-ANCA associated vasculitis was over 60 years, with a male predominance. Most patients died related to respiratory insufficiency. Despite the exuberance of this cutaneous eruption, it is usually auto-limited, reinforcing the importance of its knowledge, for eviction of unnecessary diagnostic tests and therapies. Mechanisms of pathogenesis postulated include ultraviolet A radiation and local heat. This photodermatosis presents as monomorphic, superficial, pustular, and non-pruritic folliculitis affecting the upper body but not the face, usually arising on neck, back, shoulders and upper trunk. The follicular pustules emerge 24 - 72 h after intense exposure to heat and/or sunlight and fade spontaneously in 5 - 10 days, without scarring. This patient showed a 48-hour latency period;the number of pustules and area of the body affected were proportional to the duration of the sunlight exposure;the eruption lasted approximately 10 days. Actinic superficial folliculitis has a specific histology with follicular subcorneal sterile pustules and a mixed inflammatory infiltrate around hair follicles, probably secondary to keratinocytes and Langerhans cells involvement in the immunomodulatory actions of ultraviolet radiation. Recurrence under identical conditions may occur, after a latency period of at least 4 weeks, but usually about 1 year. Actinic superficial folliculitis and related follicular conditions are probably underdiagnosed and subsequently there is insufficient scientific information available to clinicians. Being familiar with these entities is of the utmost importance, since it can be crucial for their management.展开更多

Three weeks later, the mass had increased and polyarthritis and erythema nodosum had developed in both legs. A core needle biopsy led to a diagnosis of granulomatous mastitis with arthritis and erythema nodosum. Low-dose prednisolone (10 mg) rapidly improved the arthritis and the erythema nodosum. Granulomatous mastitis is a very rare, chronic inflammatory disease and only nine patients with granulomatous mastitis with erythema nodosum have been described. We reviewed the literature to determine the clinicopathological features and treatment of granulomatous mastitis. Appropriate diagnosis and steroid therapy should result in excellent outcomes.展开更多

Atypical presentations and delayed early diagnosis can be significant challenges in managing NSTIs. The infectious process can start at any part of the body with rapid progression leading to limb amputation and high mortality rate. We present a case of a patient with NSTI, the sequelae and management of the chronic pain that developed. Aim: This case report looks to shed light on the importance of a plan for management of subacute and chronic pain in treating patients who present with Necrotizing soft tissue infection. Case presentation: A 53-year-old female who presented with septicemia and was subsequently admitted and treated for NSTI in 2014 resulting in amputation of her distal foot, toe digits, and now with ongoing chronic wound of the lower extremities along with chronic pain. Conclusion: Chronic pain as part of the sequelae of Necrotizing soft tissue infections needs to be anticipated by the health care management team in order to optimize patient care post operatively.展开更多

The patient was treated with wedge resection and has remained disease-free on surveillance imaging and endoscopy. On the second day of hospitalization, he developed symptoms of paraplegia, and we considered TEVAR, but we were concerned that TEVAR intervention in the acute phase might worsen the dissection, so we first placed a cerebrospinal drainage (CSFD) device, which resulted in improvement of his symptoms. Thereafter, although his lower limb mobility was fine, he underwent thoracic stent graft aortic repair (TEVAR) in the subacute phase due to worsening ULP. The patient had a good postoperative course and was discharged home unassisted. The initial placement of CSFD was effective in reducing the incidence of paraplegia as a complication of TEVAR and in bringing the timing of TEVAR intervention from the acute phase to the subacute phase.展开更多

The efficiency of these providers is directly related to their inpatient lengths of stay. The coronavirus epidemic has challenged the ability of hospitals in the United States to reduce stays and provide efficient care. This study described the impact of the epidemic on inpatient lengths of stay in the hospitals of Syracuse NY between March-November 2020 compared with the same periods in previous years. It demonstrated that, during this period, adult medicine lengths of stay increased by 4.5 percent and adult surgery stays increased by 5 - 6 percent. These increases were not large;however, they challenged the ability of hospitals to provide efficient care at a time when additional capacity was needed to deal with the epidemic. The results of the study suggested that the coronavirus epidemic should not limit the effectiveness of hospital programs that support efficiency and protect needed health care resources at the community level.展开更多

The teaching point of those cases is that this very frequent pathology can take a variety of forms, depending on the structures that are infiltrated. The key point in correctly diagnosing the intradural disc herniation is to look at the angle between the fragment and the dura. Is it pushing it (as in the usual disc herniation), embedded in it (those with the extra-arachnoid type) or does it go through to fall into the CSF (In the intra-arachnoid type)? It could be diagnosed on MRI preoperatively. Contrary to many published cases of intradural disc herniation, the two patients could be managed non-surgically.展开更多

This study reviewed the utilization of inpatient hospitals in the metropolitan area of Syracuse, New York during the most recent twelve-month periods. It focused on the onset and duration of respiratory disease utilization. The study evaluated this utilization by identifying numbers of hospital patients admitted with respiratory diseases to the combined Syracuse hospitals by month. By focusing on twelve-month periods for July-June of each year, it was possible to identify changes in hospital utilization for respiratory disease season. The study demonstrated that, during the five-year period, inpatient respiratory diseases were concentrated between January and March. The highest levels of respiratory disease occurred during January. The study also demonstrated that the onset of the respiratory disease season was rapid, demonstrated by increases in census levels and discharges between October-December and January-March. The study suggested that the highest demands for inpatient care in Syracuse increased by a community wide average of 15 to 20 patients during January. The data show that increased levels of respiratory diseases extended from January-March into April and May. This approach to the utilization of health care could be employed by other providers to address the onset and duration of respiratory disease at the community level.展开更多

Case Report: A 47-year-old postmenopausal and heavy smoker woman complained of localized sensitivity on the distal-anterior and medial surfaces of both tibiae, which had been ongoing for 1 month. She moved to a flat at 5th-floor without an elevator a year ago. Bilateral stress fractures were diagnosed by radiographs and confirmed by Magnetic Resonance Imaging (MRI). She was suggested to restrict activities and to use crutches until the symptoms subsided. Bilateral ankle joint pneumatic brace with full heel support was applied. The patient was followed one year after diagnosis. At the 2-month follow-up examination, the patient was pain free and planning to move to a new house. Conclusions: Several risk factors have been defined in the pathogenesis of stress fracture. In the present case, the acute phase had passed and diagnosis could be made from the direct radiographs. Although extremely rarely seen, tibia distal stress fracture should be kept in mind in the differential diagnosis of patients with the relevant risk factors who present with tibia distal localized pain.展开更多

The immunohistochemistry was similar in the parotid lesion and pulmonary metastases in autopsy specimens, but only the latter showed a cribriform pattern and comedolike necrosis. Although the parotid tumor was treated with concurrent chemoradiotherapy and adjuvant chemotherapy for over 21 months, the patient died of respiratory failure caused by advanced pulmonary metastases.展开更多

Extubation will be more worrisome if patient has difficult intubation, sleep apnea, surgical procedure involving airway or surrounding structures. In such extubation airway exchange catheters (AEC) are used, as they are simple to use and able to provide oxygen to the patient. Rarely AEC use can cause potential life threatening complications. We report a case of pneumothorax following use of AEC in post-thyroidectomy patient. Case: A 32 years old male patient was admitted to our ICU, with difficult intubation after thyroidectomy. He was a known case of obtructive sleep apnea, hypertension and large goiter. In ICU for proper visualization of vocal cords and resecuring the airway, AEC was used, but patient had hypoxia with bradycardia. He was recovered with Ambu bag ventilation and required brief cardiopulmonary resuscitation. Post resuscitation he had left impending tension pneumothorax and lung laceration requiring chest drain, which was removed after tracheostomy and weaned from the ventilator. After 9 days supra glottic edema subsided and vocal cords were moving;His trachea was decanulated and he was discharged home. Conclusion: Rarely AEC use can cause life threatening injuries. Pneumothorax following the use of AEC is not always due to tracheobronchial injury;it can also occur as a result of alveolar injury.展开更多

For example, the treatment of secondary diseases like CFDR (CF related diabetes) which comes along with an increased life expectancy has been added over the years. It is essential that within such a complex disease pattern, professional experience of care staff who worked in specialized CF care centers (50+ patients per year) for many years has to be transferred to other nursing stuff or beginners. This is especially important in the context of the heterogeneous characteristics of this disease. Often it can be seen that the quality of the care for CF patients depends on the know-how and professional experience of the interdisciplinary CF therapists team. Moreover, it depends on the knowledge which the patient has gathered by himself by experience or learning. And, of course, it depends also on the knowledge of the relatives especially parents or partner respectively.
文摘The interstitial pneumonia (IP) associated with myeloperoxidase anti-neutrophil cytoplasmic autoantibody (MPO-ANCA) showed characteristic histology dominated by usual IP pattern. We showed a 94-year-old woman, whose prognosis was poor by severity of IP by MPO-ANCA associated vasculitis. The median age at diagnosis of IP by MPO-ANCA associated vasculitis was over 60 years, with a male predominance. Most patients died related to respiratory insufficiency. Our case would be the oldest patient in IP by MPO-ANCA associated vasculitis in the literature.
文摘Rheumatic heart disease (RHD) is the most common cause of acquired heart disease in children and young adults worldwide and particularly developing countries continuing to experience a high incidence of this disease. The unexpected increase in the incidence of the disease in certain areas may explain the clinical and epidemiological characteristics of this disease. The key manifestation of RHD is the cardiac valvular abnormalities characterized principally by deforming the layered and avascular leaflet architecture due to inflammation and subsequent diffuse fibrosis. Mitral valve is mostly involved and pulmonary valve is rarely affected. Background of these case reports highlighted the increased incidence of rheumatic pulmonary valve disease in Thoothukudi region of India in Tamil Nadu state.
文摘The authors report the clinical case of a 29-year-old Caucasian woman who presented with clinicopathological findings and a previous outbreak all suggestive of actinic superficial folliculitis, a rarely reported and probably misdiagnosed phototoxic sun-induced dermatosis first described by Nieboer in 1985. Despite the exuberance of this cutaneous eruption, it is usually auto-limited, reinforcing the importance of its knowledge, for eviction of unnecessary diagnostic tests and therapies. Mechanisms of pathogenesis postulated include ultraviolet A radiation and local heat. This photodermatosis presents as monomorphic, superficial, pustular, and non-pruritic folliculitis affecting the upper body but not the face, usually arising on neck, back, shoulders and upper trunk. The follicular pustules emerge 24 - 72 h after intense exposure to heat and/or sunlight and fade spontaneously in 5 - 10 days, without scarring. This patient showed a 48-hour latency period;the number of pustules and area of the body affected were proportional to the duration of the sunlight exposure;the eruption lasted approximately 10 days. Actinic superficial folliculitis has a specific histology with follicular subcorneal sterile pustules and a mixed inflammatory infiltrate around hair follicles, probably secondary to keratinocytes and Langerhans cells involvement in the immunomodulatory actions of ultraviolet radiation. Recurrence under identical conditions may occur, after a latency period of at least 4 weeks, but usually about 1 year. Actinic superficial folliculitis and related follicular conditions are probably underdiagnosed and subsequently there is insufficient scientific information available to clinicians. Being familiar with these entities is of the utmost importance, since it can be crucial for their management.
文摘A 31-year-old woman was referred with a painful mass in the left breast that was initially treated with antibiotics. Three weeks later, the mass had increased and polyarthritis and erythema nodosum had developed in both legs. A core needle biopsy led to a diagnosis of granulomatous mastitis with arthritis and erythema nodosum. Low-dose prednisolone (10 mg) rapidly improved the arthritis and the erythema nodosum. Granulomatous mastitis is a very rare, chronic inflammatory disease and only nine patients with granulomatous mastitis with erythema nodosum have been described. We reviewed the literature to determine the clinicopathological features and treatment of granulomatous mastitis. Appropriate diagnosis and steroid therapy should result in excellent outcomes.

文摘Background: Necrotizing soft tissue infections (NSTIs) are potentially life threatening medical emergencies associated with devastating and rapidly spreading destruction of soft tissues. Atypical presentations and delayed early diagnosis can be significant challenges in managing NSTIs. The infectious process can start at any part of the body with rapid progression leading to limb amputation and high mortality rate. We present a case of a patient with NSTI, the sequelae and management of the chronic pain that developed. Aim: This case report looks to shed light on the importance of a plan for management of subacute and chronic pain in treating patients who present with Necrotizing soft tissue infection. Case presentation: A 53-year-old female who presented with septicemia and was subsequently admitted and treated for NSTI in 2014 resulting in amputation of her distal foot, toe digits, and now with ongoing chronic wound of the lower extremities along with chronic pain. Conclusion: Chronic pain as part of the sequelae of Necrotizing soft tissue infections needs to be anticipated by the health care management team in order to optimize patient care post operatively.
文摘Primary gastric synovial sarcoma is rare and challenging to diagnose. We present a case of a gastric ulcer that was diagnosed as primary gastric synovial sarcoma only after surveillance endoscopy with repeat biopsies. The diagnosis was established with the identification of the pathognomonic chromosomal translocation t(X;18)(p11;q11). The patient was treated with wedge resection and has remained disease-free on surveillance imaging and endoscopy. This case demonstrates the difficulty in diagnosing primary gastric synovial sarcoma and the benefits of early disease detection.
文摘The patient is 50-year-old man. He was admitted to our hospital with a strong back pain and diagnosed as an acute type B aortic dissection. On the second day of hospitalization, he developed symptoms of paraplegia, and we considered TEVAR, but we were concerned that TEVAR intervention in the acute phase might worsen the dissection, so we first placed a cerebrospinal drainage (CSFD) device, which resulted in improvement of his symptoms. Thereafter, although his lower limb mobility was fine, he underwent thoracic stent graft aortic repair (TEVAR) in the subacute phase due to worsening ULP. The patient had a good postoperative course and was discharged home unassisted. The initial placement of CSFD was effective in reducing the incidence of paraplegia as a complication of TEVAR and in bringing the timing of TEVAR intervention from the acute phase to the subacute phase.
文摘The need for efficiency has been a major challenge for hospitals in the United States. The efficiency of these providers is directly related to their inpatient lengths of stay. The coronavirus epidemic has challenged the ability of hospitals in the United States to reduce stays and provide efficient care. This study described the impact of the epidemic on inpatient lengths of stay in the hospitals of Syracuse NY between March-November 2020 compared with the same periods in previous years. It demonstrated that, during this period, adult medicine lengths of stay increased by 4.5 percent and adult surgery stays increased by 5 - 6 percent. These increases were not large;however, they challenged the ability of hospitals to provide efficient care at a time when additional capacity was needed to deal with the epidemic. The results of the study suggested that the coronavirus epidemic should not limit the effectiveness of hospital programs that support efficiency and protect needed health care resources at the community level.
文摘Intradural disc herniation is rare and often diagnosed during surgery. We present two patients with the disease, one with an intra-arachnoid type, the other with an extra-arachnoid location. The teaching point of those cases is that this very frequent pathology can take a variety of forms, depending on the structures that are infiltrated. The key point in correctly diagnosing the intradural disc herniation is to look at the angle between the fragment and the dura. Is it pushing it (as in the usual disc herniation), embedded in it (those with the extra-arachnoid type) or does it go through to fall into the CSF (In the intra-arachnoid type)? It could be diagnosed on MRI preoperatively. Contrary to many published cases of intradural disc herniation, the two patients could be managed non-surgically.
文摘In the United States, the respiratory disease season has challenged providers to care for populations at the community level. This study reviewed the utilization of inpatient hospitals in the metropolitan area of Syracuse, New York during the most recent twelve-month periods. It focused on the onset and duration of respiratory disease utilization. The study evaluated this utilization by identifying numbers of hospital patients admitted with respiratory diseases to the combined Syracuse hospitals by month. By focusing on twelve-month periods for July-June of each year, it was possible to identify changes in hospital utilization for respiratory disease season. The study demonstrated that, during the five-year period, inpatient respiratory diseases were concentrated between January and March. The highest levels of respiratory disease occurred during January. The study also demonstrated that the onset of the respiratory disease season was rapid, demonstrated by increases in census levels and discharges between October-December and January-March. The study suggested that the highest demands for inpatient care in Syracuse increased by a community wide average of 15 to 20 patients during January. The data show that increased levels of respiratory diseases extended from January-March into April and May. This approach to the utilization of health care could be employed by other providers to address the onset and duration of respiratory disease at the community level.
文摘Background: Stress fractures are fatigue-induced, very small cracks in the bone caused by repetitive application of force and most often seen in the weight-bearing bones of the lower extremities of athletes or military recruits. Case Report: A 47-year-old postmenopausal and heavy smoker woman complained of localized sensitivity on the distal-anterior and medial surfaces of both tibiae, which had been ongoing for 1 month. She moved to a flat at 5th-floor without an elevator a year ago. Bilateral stress fractures were diagnosed by radiographs and confirmed by Magnetic Resonance Imaging (MRI). She was suggested to restrict activities and to use crutches until the symptoms subsided. Bilateral ankle joint pneumatic brace with full heel support was applied. The patient was followed one year after diagnosis. At the 2-month follow-up examination, the patient was pain free and planning to move to a new house. Conclusions: Several risk factors have been defined in the pathogenesis of stress fracture. In the present case, the acute phase had passed and diagnosis could be made from the direct radiographs. Although extremely rarely seen, tibia distal stress fracture should be kept in mind in the differential diagnosis of patients with the relevant risk factors who present with tibia distal localized pain.
文摘Salivary duct carcinoma (SDC) is a rare, high-grade, aggressive malignancy, as having a morphologic resemblance to ductal carcinoma of the breast. We present here an atypical SDC of the parotid gland in a 63-year-old man that was diagnosed from immunohistochemical findings. The immunohistochemistry was similar in the parotid lesion and pulmonary metastases in autopsy specimens, but only the latter showed a cribriform pattern and comedolike necrosis. Although the parotid tumor was treated with concurrent chemoradiotherapy and adjuvant chemotherapy for over 21 months, the patient died of respiratory failure caused by advanced pulmonary metastases.
文摘Endotracheal intubation and extubation in intensive care unit (ICU) patients are difficult. Extubation will be more worrisome if patient has difficult intubation, sleep apnea, surgical procedure involving airway or surrounding structures. In such extubation airway exchange catheters (AEC) are used, as they are simple to use and able to provide oxygen to the patient. Rarely AEC use can cause potential life threatening complications. We report a case of pneumothorax following use of AEC in post-thyroidectomy patient. Case: A 32 years old male patient was admitted to our ICU, with difficult intubation after thyroidectomy. He was a known case of obtructive sleep apnea, hypertension and large goiter. In ICU for proper visualization of vocal cords and resecuring the airway, AEC was used, but patient had hypoxia with bradycardia. He was recovered with Ambu bag ventilation and required brief cardiopulmonary resuscitation. Post resuscitation he had left impending tension pneumothorax and lung laceration requiring chest drain, which was removed after tracheostomy and weaned from the ventilator. After 9 days supra glottic edema subsided and vocal cords were moving;His trachea was decanulated and he was discharged home. Conclusion: Rarely AEC use can cause life threatening injuries. Pneumothorax following the use of AEC is not always due to tracheobronchial injury;it can also occur as a result of alveolar injury.
