Objective To investigate the impact of the deeplearning-based CT fractional flow reserve(CT-FFR)on clinical decision-making and long-term prognosisin patients withobstructive coronaryyheartdisease.Methods In this sing...Objective To investigate the impact of the deeplearning-based CT fractional flow reserve(CT-FFR)on clinical decision-making and long-term prognosisin patients withobstructive coronaryyheartdisease.Methods In this single-center retrospective cohort study,consecutive patients with obstructive coronary heart disease(with at least one stenosis≥50%)on their first coronary computed tomography angiography(CCTA)in Beijing Anzhen Hospital from February 2017 to July 2018 were included.Baseline clinical and CT characteristics were collected.Deep-learning-based CT-FFR and Leiden CCTA risk score were calculated.All patients enrolled were followed up for at least 5 years.The study endpoint was major adverse cardiovascular events(MACE),defined as the composite of cardiac death,nonfatal myocardial infarction,unstable angina requiring hospitalization,and unplanned revascularization.Receiver operating characteristic(ROC)curves were drawn to define the optimal cut-off point of the Leiden score in predicting the 5-year MACE,and survival analysis and Cox regression were performed to explore the related factors of MACE.Results A total of 622 patients,aged 61(54,66)years with 407(65.4%)males,were included.Diagnostic coronary angiography was performed in 78 patients after their baseline CCTA,and 34(43.6%)patients had CT-FFR>0.80.During a follow-up time of 2181(2093,2355)days,155 patients(24.9%)suffered from MACE.Optimal cut-off point of Leiden score for predicting MACE was 15.48.Survival analysis found that male patients,Leiden risk score>15 and CT-FFR≤0.80 had worse prognosis.Multivariate Cox regression analysis identified CT-FFR≤0.80 as a robust and independent predictor of MACE(HR=4.98,95%CI 3.15-7.86,P<0.001).Conclusion Deep-learning-based CT-FFR aids in clinical decision-making and the evaluation of long-term prognosis in patients with obstructive coronary heart disease.展开更多
Objective To investigate the correlation between serum plasmamembranevesicle-associated protein(PLVAP)levels and metabolism-associated fatty liver disease(MAFLD)in patients with T2DM.Methods A total of 255 patients wi...Objective To investigate the correlation between serum plasmamembranevesicle-associated protein(PLVAP)levels and metabolism-associated fatty liver disease(MAFLD)in patients with T2DM.Methods A total of 255 patients with type 2 diabetes mellitus(T2DM)treated in Gansu Provincial Hospital from September 2023 to March 2024 were selected in this study,and were divided into T2DM alone group(n=141)and the T2DM combined with MAFLD group(n=114)according to whether they were combined with MAFLD or not.Spearman correlation analysis was used to analyze the correlation between serum PLVAP and other indexes,multiple linear regression analysis was used to analyze the influencing factors of serum PLVAP,and Logistic regression analysis was used to analyze the influencing factors of MAFLD in T2DM patients.The early diagnostic value of serum PLVAP on MAFLD in patients with T2DM by analyzing the receiver's operating characteristics(ROC)curve.Results Compared with the MAFLD group,BMI,WC,FPG,C-P,HOMA-IR,TG,AST,ALT,GGT,hepatic steatosis index(HSI)and phytohaemagglutinin-like oxidized low-density lipoprotein receptor 1(LOX-1)were decreased in the T2DM group(P<0.05).DM duration,,HDL-C,AST/ALT and PLVAP were elevated(P<0.05).Spearman correlation analysis showed that PLVAP was positively correlated with AST/ALT(P<0.05)and negatively correlated with WC,DBP,C-P,ALT,GGT and HSI(P<0.05).Multiple linear regression analysis showed that WC was an influencing factor for PLVAP.Logistic regression analysis showed that DM duration,WC,TG,PLVAP,LOX-1 were influencing factors for MAFLD.Analysis of the subjects'work characteristics(ROC)curve showed that the area under the ROC curve was 0.785,with a sensitivity of 81.6%and a specificity of 67.4%.Conclusion Reduced PLVAP is a risk factor for comorbid MAFLD in patients with T2DM and is important for predicting the co-existence of MAFLD in patients with T2DM.展开更多
Objective To compare the effects of standard cognitive behavioral therapy for insomnia(CBT-I)and enhanced cognitive behavioral therapy for insomnia(CBT-I Plus)in patients with chronic insomnia disorder comorbid anxiet...Objective To compare the effects of standard cognitive behavioral therapy for insomnia(CBT-I)and enhanced cognitive behavioral therapy for insomnia(CBT-I Plus)in patients with chronic insomnia disorder comorbid anxiety or depressive symptoms.Methods This prospective study included 148 patients with chronic insomnia disorder and anxiety/depression symptoms who were treated at the Sleep Disorder clinic of Shanghai Mental Health Center between July 2020 and August 2023.Participants(56 males,92 females;aged 18-65 years,mean age 35.08±10.30 years)were randomly assigned in a 1:2 ratio to the CBT-I group(n=54)or CBT-I Plus group(n=94).The CBT-I Plus group received additional treatments targetinganxiety and depressive symptoms.Treatment lasted 8 weeks,with assessment conducted at baseline,weeks 2,4,and 8.Depression severityy was measured using the 17-item Hamilton Depression Rating Scale(HAMDiz),anxiety severity with the Hamilton Anxiety Scale(HAMA),and sleep quality with the Pittsburgh Sleep Quality Index(PSQI).Paired sample t-tests were used to evaluate within-group changes,repeated-measures ANOVA compared treatment effects between groups,and ANCOVA was employed to adjust for confounding variables.Results Significant reductions in PSQI,HAMDi7,and HAMA scores were observed in both groups after treatment:CBT-I group:PSQI((14.15±2.54)us.(7.50±3.35),t=13.25),HAMD_(17)((14.70±4.09)us.(7.40±4.61),t=9.33),and HAMA((14.94±4.11)vs.(5.56±3.67),t=12.38)(all P<0.001).CBT-I Plus group:PSQI((14.87±3.01)vs.(7.19±3.86),t=18.75),HAMD_(17)((16.84±3.91)us.(6.84±4.79),t=17.42),andHAMA((15.57±3.93)vs.(6.10±4.57),t=18.39)(all P<0.001).After adjusting for HAMD_(17)scores and medication use,no statistically significant between-group differences were observed in changes in PSQI,HAMD_(17),and HAMA scores(P>0.05).A significant time-by-group interaction was found for the PSQI daytime dysfunction subscale(F=4.87,P<0.01).Conclusion Both CBT-I and CBT-I Plus improve sleep and emotional symptoms in patients with chronic insomnia disorder and comorbid anxiety/depression symptoms.However,CBT-I Plus has no significant advantages over standard CBT-I.Further studies are needed to refine the timing and content of interventions.展开更多

文摘Objective To investigate the clinical phenotype and gene mutation in a child with developmental disorders caused by CTNNB1 gene mutation.Methods Clinical data of a child with CTNNB1 gene mutation who was admitted to Xiamen Hospital of Fudan University Affiliated Pediatric Hospital in May 2017 were collected,whole exome sequencing technology was applied to verify the family lineage of the child,and the pathogenicity of mutation site was analyzed.Results The patient was a6 years and 1 month old male,with a clinical phenotype including mental retardation,motor developmental disorders,speech disorders,visual disorders(internal strabismus),microcephaly,and behavioral problems(social withdrawal,overdependence,etc.).

文摘Objective JWH133,a cannabinoid type 2 receptor agonist,was tested for its ability to protect mice from bleomycin-induced pulmonary fibrosis.Methods By using a random number generator,24 C57BL/6J male mice were randomly divided into the control group,model group,JWH133 intervention group,and JWH133+a cannabinoid type-2 receptor antagonist(AM630)inhibitor group,with 6 mice in each group.A mouse pulmonary fibrosis model was established by tracheal instillation of bleomycin(5 mg/kg).
