The gut microbiota(GM)is a highly dynamic ecology whose density and com-position can be influenced by a wide range of internal and external factors.Thus,“How do GM,which can have commensal,pathological,and mutualistic rela-tionships with us,affect human health?”has become the most popular research issue in recent years.Numerous studies have demonstrated that the trillions of microorganisms that inhabit the human body can alter host physiology in a variety of systems,such as metabolism,immunology,cardiovascular health,and neurons.The GM may have a role in the development of a number of clinical disorders by producing bioactive peptides,including neurotransmitters,short-chain fatty acids,branched-chain amino acids,intestinal hormones,and secondary bile acid conversion.These bioactive peptides enter the portal circulatory system through the gut-liver axis and play a role in the development of chronic liver diseases,cirrhosis,and hepatic encephalopathy.This procedure is still unclear and quite complex.In this study,we aim to discuss the contribution of GM to the development of liver diseases,its effects on the progression of existing chronic liver disease,and to address the basic mechanisms of the intestinal microbiota-liver axis in the light of recent publications that may inspire the future. 展开更多

From health literacy to organ donation and transplantation awareness:The path through self-control

BACKGROUND Organ donation and transplantation are vital for saving lives,yet awareness remains limited.Health literacy,the ability to understand and use health information,and self-control,which influences decision-making,are crucial factors in promoting awareness.Understanding their relationship can guide efforts to increase participation in organ donation.AIM To investigate the effect of health literacy on organ donation and transplantation awareness,with self-control acting as a mediator,and to emphasize its importance.METHODS The study comprised 345 adults recruited from five different cities in Türkiye.The sample included 208 women(60.29%)and 137 men(39.71%).The age range of the sample group varies between 18 and 49(median age=24.13;SD=8.81).Selfreport data were collected including psychometric measures assessing organ donation and transplantation awareness,health literacy,and self-control.Reliability analysis,descriptive statistics,correlational analysis,and mediational analysis of the data were carried.RESULTS The analysis showed that health literacy was positively correlated with organ donation and transplantation awareness(r=0.340)and self-control(r=0.376).Self-control was positively related to organ donation and transplantation awareness(r=0.531).Mediating analysis supported a mediation model where health literacy was found to influence organ donation and transplantation awareness via self-control(bootstrap=0.158,95%confidence interval=0.105-0.215).CONCLUSION The results indicate that self-control mediates the positive relationship between health literacy and organ donation and transplantation awareness.Enhancing individuals’health literacy may therefore increase awareness of organ donation and transplantation through improvements in self-control. 展开更多

BACKGROUND Knowledge about organ donation and transplantation plays a crucial role in shaping individuals'health behaviors and perceptions,potentially impacting their health-related quality of life(HRQoL).Future anxiety,defined as the anticipatory worry individuals experience regarding potential negative events and outcomes in their future,may further influence these outcomes.AIM To investigate the effect of such knowledge on HRQoL and to examine whether future anxiety mediates this relationship.METHODS A cross-sectional study was conducted with 659 participants aged 18 to 65 years.Participants completed the Organ Tissue Donation and Transplantation Knowledge Scale,the Dark Future Scale,and the European Health Interview Survey-Quality of Life 8.Correlation analyses were performed,followed by Structural Equation Modeling to test the proposed mediation model.RESULTS The findings indicated that greater knowledge about organ donation and transplantation was positively associated with higher HRQoL and negatively associated with future anxiety.Future anxiety was negatively correlated with HRQoL.Structural Equation Modeling analysis indicated that knowledge directly enhanced HRQoL and reduced future anxiety.Additionally,future anxiety negatively affected HRQoL,mediating the relationship between knowledge and HRQoL.The mediation effect was significant,as confirmed by bootstrapping(bootstrap coefficient=0.068,95%CI:0.046-0.093).CONCLUSION The study concludes that future anxiety partially mediates the positive impact of knowledge about organ donation and transplantation on HRQoL.These results suggest that increasing public knowledge in this area may reduce future anxieties and enhance quality of life. 展开更多

BACKGROUND Stapled hemorrhoidopexy(SH)is one of the most commonly used surgical techniques for hemorrhoidal disease,being particularly effective for grade III and IV hemorrhoids.The procedure is associated with better short-term outcomes,including less postoperative pain,shorter operative time,faster return to work,and higher patient satisfaction.However,there is a risk-benefit debate surrounding SH due to significant complications from the procedure,such as anal stenosis,rectovaginal fistula,fecal incontinence,and recurrence.AIM To evaluate recurrence rates and factors influencing surgical outcomes following SH in patients with grade III and IV hemorrhoids.METHODS This retrospective,single-center study enrolled a total of 77 patients with grade III/IV hemorrhoids for analysis.Early(less than 7 days after SH)and late(7 or more days after SH)complications were analyzed.Recurrence rates were calculated as well.RESULTS Patients were categorized by hemorrhoid grade and showed no differences in demographic data between the two groups.Recurrence was observed in 4 patients(23.6%)with grade IV hemorrhoids,and no recurrence was noted in patients with grade III hemorrhoids.Postoperative bleeding,incomplete defecation,urgent defecation,incontinence,skin tags,and anal fissure were complications reported by both groups.CONCLUSION Due to the high recurrence rate,SH is not an appropriate treatment option for patients with grade IV hemorrhoids.Open surgery may be a more suitable option for these patients. 展开更多

Trends and predictors of diabetes mellitus after living kidney transplantation: A retrospective study

BACKGROUND Post-transplant diabetes mellitus(PTDM)is a common metabolic adverse event following kidney transplantation,negatively impacting graft function and patient outcomes.AIM To evaluate the frequency of PTDM and to determine predictive factors in living donor individuals who have undergone kidney transplantation.METHODS A retrospective analysis was conducted on 1200 living donor kidney transplant recipients treated between 2016 and 2023.Demographic,clinical,and treatment data were collected,and PTDM was identified based on American Diabetes Association criteria.Statistical analysis included logistic regression analysis to determine independent predictors of PTDM.RESULTS PTDM was diagnosed in 162 patients(13.5%).Risk factors included older age[odds ratio(OR)1.03,P=0.03],increased body mass index(OR 1.08,P=0.02),a genetic predisposition to diabetes(OR 1.95,P=0.001),and corticosteroid use(OR 1.30,P=0.04).Most PTDM cases(61.7%)occurred during the initial 6 months after transplant.Tacrolimus-based regimens were more commonly associated with PTDM compared to other protocols.Renal function at 12 months was com-parable between PTDM and non-PTDM groups.CONCLUSION PTDM remains a significant concern in kidney transplantation,particularly among patients with modifiable risk factors.Optimizing immunosuppressive regimens,implementing early metabolic monitoring,and addressing modifiable risks such as BMI may help reduce PTDM incidence.Additional research is required to evaluate extended-term results and refine preventive strategies. 展开更多

Computational Analysis on the Hydrodynamics of a Semisubmersible Naval Ship

Semisubmersible naval ships are versatile military crafts that combine the advantageous features of high-speed planing crafts and submarines.At-surface,these ships are designed to provide sufficient speed and maneuverability.Additionally,they can perform shallow dives,offering low visual and acoustic detectability.Therefore,the hydrodynamic design of a semisubmersible naval ship should address at-surface and submerged conditions.In this study,Numerical analyses were performed using a semisubmersible hull form to analyze its hydrodynamic features,including resistance,powering,and maneuvering.The simulations were conducted with Star CCM+version 2302,a commercial package program that solves URANS equations using the SST k-ωturbulence model.The flow analysis was divided into two parts:at-surface simulations and shallowly submerged simulations.At-surface simulations cover the resistance,powering,trim,and sinkage at transition and planing regimes,with corresponding Froude numbers ranging from 0.42 to 1.69.Shallowly submerged simulations were performed at seven different submergence depths,ranging from D/LOA=0.0635 to D/LOA=0.635,and at two different speeds with Froude numbers of 0.21 and 0.33.The behaviors of the hydrodynamic forces and pitching moment for different operation depths were comprehensively analyzed.The results of the numerical analyses provide valuable insights into the hydrodynamic performance of semisubmersible naval ships,highlighting the critical factors influencing their resistance,powering,and maneuvering capabilities in both at-surface and submerged conditions. 展开更多

Effect of rare earth doping on structural,optical,dielectric,and humidity properties of Cu-Mg-Zn ferrites

Rare earth(RE)doped ferrites with the chemical formula Cu_(0.3)Zn_(0.3)Mg_(0.4)T_(x)Fe_(2-x)O_(4)(x=0,0.1;T=La,Ce,Sr)were synthesized by chemical co-precipitation method.The structural,optical,electrical and humidity sensing properties of Cu-Mg-Zn ferrites with rare earth element doping were investigated.Single-phase cubic spinel structure was confirmed via X-ray diffraction(XRD),and the crystal size ranges fro m 22.12 to 63.17 nm according to the Scherrer formula and from 25.66 to 67.46 nm according to the Williamson-Hall method.Po rous structure and elemental characterization of the samples were investigated by scanning electron microscopy(SEM).The optic band gap varies between 2.21 and 2.49 eV.Electrical measurements were conducted in the frequency range of 1 Hz-20 MHz and temperature range of 25-400℃.It has been determined that the dielectric results are consistent with the Maxwell-Wagner method and exhibit a non-Debye relaxation model,as observed from the Nyquist plots.At a minimum frequency value of 1 Hz,the dielectric constants for pure,Ce,Sr,and La samples are 9×10^(4),5×10^(4),1×10^(8),and 2×10^(5) at 25℃,and 1.85×10^(8),1.34×10^(8),1.15×10^(10),and 4.4×10^(8)at 400℃.In the same order,for the maximum frequency value of 20 MHz,the dielectric constants at 25℃are 169,166,3799,and 60,while at 400℃they are 734,624,12108,and 774.The La doped sample's low dielectric loss makes it suitable for high-frequency applications.Humidity measurements were performed at room temperature and in the 5%-95%relative humidity range.The humidity properties of the samples were investigated through humidity mapping,sensitivity,hysteresis,and long-term stability tests.Compared to other samples,the results indicate that Ce exhibits better humidity performance with 99%sensitivity and the highest repeatability(91.2%).These results show that Ce-doped ferrite can be used as a low-cost,high-performance humidity sensor. 展开更多

This letter to the editor critically appraises the study by Luo et al.While the study provides valuable insights into imaging-pathology correlations in pancreatic can-cer,we identify several opportunities for enhancing its clinical relevance.Notably,the exclusion of magnetic resonance cholangiopancreatography and positron emission tomography/computed tomography imaging limits the study’s diag-nostic scope,as these modalities offer superior capabilities in differentiating benign from malignant lesions and assessing metabolic tumor activity.Addi-tionally,the retrospective,cross-sectional design restricts the potential for dyna-mic insights into disease progression.We also highlight the untapped potential of radiomics-based analyses,which could significantly improve diagnostic accuracy and prognostic assessments.We recommend integrating these advanced imaging modalities,adopting longitudinal study designs,and leveraging radiomics app-roaches in future research to enhance the diagnostic frameworks in pancreatic cancer imaging. 展开更多

Comparison of complication rates after early and late closure of loop ileostomies:A retrospective cohort study

BACKGROUND In the treatment of rectal cancer,a temporary loop ileostomy(TLI)is created after low anterior resection to protect bowel function in the postoperative period.Dehydration and kidney failure are significant potential complications in loop ileostomies.Compared to late closure(3-6 months),early closure(10-14 days)of the TLI may facilitate faster patient recovery and shorter hospital stays.AIM To compare early and late closure of TLIs and demonstrate that early stoma closure can be performed without increasing morbidity.METHODS This study included patients who underwent TLI for rectal cancer,with data collected prospectively between June 2016 and October 2024 and analyzed retrospectively.Patients whose stomas were closed in the late period(3-6 months)between June 2016 and October 2022(group A)were compared with those who underwent early closure(10-14 days)between October 2022 and 2024(group B),with the primary outcome being complication rate and the secondary outcome being quality of life.RESULTS A total of 270 TLIs were created(70.9%).Of these,120(44.4%)were closed in the late period(group A),and 150(55.6%)were closed in the early period(group B).There was no statistically significant difference between group A and group B in terms of demographic and clinicopathological characteristics(P>0.05).Perioperative(anesthesia management,operative time,blood loss,surgical technique)and postoperative findings(anastomotic leak,infection)were similar between the two groups and were not statistically significant(P>0.05).There were no statistically significant differences in complication rates as the primary outcome between the two groups.Quality of life as a secondary outcome was significantly higher in the early closure group(P<0.05).CONCLUSION No statistically significant difference was found between early and late loop ileostomy closure in terms of perioperative and postoperative morbidity.Early closure accelerated patients’psychological and social recovery. 展开更多

Mediating role of self-esteem in the relationship between parental disapproval of the child and alexithymic personality

BACKGROUND Alexithymia is defined as difficulties in identifying,expressing,and understanding emotions.An unapproving environment during childhood is defined as the child not receiving emotional approval from their parents,being punished,or having their emotions devalued.The formation of self-esteem is shaped by the influence of parental behavior during childhood.The commu-nication that the child establishes with their parents contributes to their increased self-esteem and sense of importance.The absence of this may play a role in developmental psychopathology.AIM To examine the mediating role of self-esteem in the relationship between a disapproving environment in childhood and alexithymia.METHODS The research in the relational screening model was conducted with 404 participants.Demographic Information Form,Disapproving Environment Scale in Childhood,Toronto Alexithymia Scale,and Rosenberg Self-Esteem Scale were used as data collection tools.Hayes’bootstrapping technique was used in the analysis of the data obtained from the research.RESULTS As a result of the analyses,parental disapproval positively predicted the level of alexithymia(rmother=0.51,rfather=0.52,P<0.05)and negatively predicted selfesteem(rmother=-0.75,rfather=-0.67,P<0.05).Additionally,self-esteem negatively predicted alexithymia(r=-0.58,P<0.05).It was observed that self-esteem had a partial mediating effect on the relationship between parental disapproval and alexithymia.Family types were analyzed separately as chaotic,approving,perfect,and typical.Of these,the chaotic family type positively predicted alexithymia(r=0.33,P<0.05)and negatively predicted self-esteem(r=-0.60,P<0.05).The approving family type negatively predicted alexithymia(r=-0.43,P<0.05)and positively predicted self-esteem(r=0.61,P<0.05).The perfect family type negatively predicted alexithymia(r=-0.27,P<0.05)and positively predicted self-esteem(r=-0.45,P<0.05).CONCLUSION The type of family in which the child grows up in and parental disapproval during childhood affected the child’s self-esteem and caused alexithymic personality traits. 展开更多

BACKGROUND Supracondylar humeral fractures(SCHF)are the second most common fractures in childhood and can lead to short-and long-term complications.Despite their prevalence,the anatomical factors that predispose children to SCHF remain unclear.This study aimed to determine whether there are significant morpho-logical differences in the elbow by comparing the radiographic angular measure-ments of the contralateral elbows of patients with SCHF to those of patients with distal radius fractures(DRF)and a healthy control group.We sought to explore if these morphological variations contribute to the occurrence of SCHF.Radiographs of 78 SCHF patients were analyzed for angular measurements of the contralateral elbow.Two control groups were used:98 healthy children and 40 patients with DRF.Angular measurements included Baumann angle(BA),hu-meroulnar angle(HUA),humerus metaphysis-diaphysis angle(HMDA),humerus shaft-condylar angle(HSCA),and lateral capitellohumeral angle.Only BA,HUA,and HMDA were measured in the DRF group.Statistical analysis was performed to compare differences among groups.RESULTS Significant differences were found in elbow measurements between SCHF and control groups(P<0.05).However,the mean values for all groups fell within the ranges described in the literature.CONCLUSION While statistically significant differences were found in elbow morphology between SCHF patients and controls,these differences don't translate into clinically meaningful morphological deviations. 展开更多

BACKGROUND Traumatic spinal cord injury(SCI)is a life-altering condition that results in long-term complications,including progressive neurodegeneration and cord atrophy.It presents a significant unmet medical need with extensive social and economic burdens.AIM To evaluate the safety and preliminary efficacy of allogeneic mesenchymal stem cells derived from Wharton’s jelly(WJ-MSCs)in patients with chronic complete SCI.The primary objective was to assess whether WJ-MSCs could facilitate neurological recovery and improve the quality of life in this patient population.METHODS This open-label,multicenter phase I study investigated the effects of administering WJ-MSCs via three delivery routes:Intrathecal(for localized spinal targeting);intramuscular(for targeting end organ);and intravenous(for systemic immunomodulation).While all three routes were used concurrently to enhance therapeutic synergy,neurological,sensory,and functional scales were used to assess overall efficacy.Participants with chronic SCI(duration of at least 6 months)who had significant impairment and disability were eligible for inclusion.WJ-MSCs were administered twice monthly for 2 months,with each route receiving a dose of 1×10^(6)cells/kg.Patients were closely monitored for 1 year following treatment.RESULTS At baseline,participants displayed considerable functional deficits,as indicated by the following scores:Functional independence measure of 77.5±2.26;Modified Ashworth Scale of 15.83±4.83;American Spinal Injury Association(ASIA)Motor score of 1.67±2.66;ASIA Light Touch and Pin-Prick scores of 62±18.42 each;Wexner Incontinence Score of 20;and Qualiveen Short Form,a validated questionnaire specifically designed to assess the impact of urinary dysfunction on quality of life in individuals with SCI,score of 32.Following WJ-MSC therapy,significant improvements were observed in all neurological functions over the 1-year follow-up.Notably,the ASIA Motor score improved significantly(χ^(2)=23.938,P<0.001),and Qualiveen Short Form scores demonstrated a substantial enhancement in quality of life(z=-2.214,P<0.05).CONCLUSION This phase I study,conducted without a control group,suggests that the administration of WJ-MSCs through multiple routes is both safe and potentially effective in patients with chronic complete SCI.However,the observed neurological improvements cannot be solely attributed to WJ-MSC therapy,as concurrent pharmacological and rehabilitative interventions were not controlled.These findings indicated that WJ-MSC therapy may offer a promising approach for enhancing neurological function and quality of life in this challenging patient population.Further research with larger cohorts and extended follow-up is necessary to validate these preliminary results. 展开更多

The performance characteristics,particularly the starting performance of direct line-fed induction motors,which are mainly influenced by the design of the rotor,are crucial considerations for end-users.It is quite a challenging issue for motor manufacturers to enhance the starting performance of existing mass-produced motors with minimal modifications and expenses.In this paper,a simple and cost-effective method to improve the starting performance of a commercial squirrel-cage induction motor(SCIM)is proposed.The influence of geometric parameters of the end-ring on the performance characteristics,including starting(locked rotor)torque,pull-up and break down torque,starting current,rotor electric parameters,current density,power losses,and efficiency have been comprehensively investigated.It has been revealed that among the other end-ring design parameters,the ring thickness has a significant effect on the performance characteristics.An optimal end-ring thickness is determined,and its performance characteristics have been compared to those of its initial counterpart.Numeric and parametric analyses have been conducted using a 2D time-stepping finite element method(FEM).The FEM results were validated using experimental measurements obtained from an 11 kW SCIM prototype. 展开更多

I read the study by Zhao et al with great interest.Although the study design was quite complicated,it was successful in raising awareness of science and relevant researchers.Thirty patients with liver cirrhosis and portal hypertension secondary to chronic hepatitis B were included in the study.They were treated for variceal bleeding and underwent trans-jugular intrahepatic portosystemic shunt to prevent the recurrence of variceal bleeding and to reduce portal pressure.The authors evaluated the effects of changes in gut microbiota(GM)on hepatic encephalopathy secondary to portocaval bypass.The GM is greatly affected by local and general factors,including herbal and medical drugs,a person's dietary characteristics(carnivorous,vegan,vegetarian),supplementary foods,drinking water sources,and living in a city center or town.Therefore,I congratulate Zhao et al for their concise and comprehensive study on a multifactorial subject. 展开更多

Objective:To evaluate the effect of ureteral access sheath(UAS)use and calibration change on stone-free rate and complications of retrograde intrarenal surgery(RIRS).Methods:Data from 568 patients undergoing RIRS for kidney or upper ureteral stones were retrospectively included.Firstly,patients were compared after 1:1 propensity score matching,according to UAS usage during RIRS(UAS used[+]87 and UAS non-used[−]87 patients).Then all UAS+patients(n=481)were subdivided according to UAS calibration:9.5-11.5 Fr,10-12 Fr,11-13 Fr,and 13-15 Fr.Primary outcomes of the study were the success and complications of RIRS.Results:Stone-free rate of UAS+patients(86.2%)was significantly higher than UAS−patients(70.1%)after propensity score matching(p=0.01).Stone-free rate increased with higher caliber UAS(9.5-11.5 Fr:66.7%;10-12 Fr:87.0%;11-13 Fr:90.6%;13-15 Fr:100%;p<0.001).Postoperative complications of UAS+patients(11.5%)were significantly lower than UAS−patients(27.6%)(p=0.01).Complications(8.7%)with 9.5-11.5 Fr UAS was lower than thicker UAS(17.2%)but was not statistically significant(p=0.09).UAS usage was an independent factor predicting stone-free status or peri-and post-operative complications(odds ratio[OR]3.654,95%confidence interval[CI]1.314-10.162;OR 4.443,95%CI 1.350-14.552;OR 4.107,95%CI 1.366-12.344,respectively).Conclusion:Use of UAS in RIRS may increase stone-free rates,which also increase with higher caliber UAS.UAS usage may reduce complications;however,complications seemingly increase with higher UAS calibration. 展开更多

In today’s manufacturing industries,hard competition between rival firms makes it compulsory for researchers to design lighter and cheaper machine components due to the megatrends of cost-effectiveness and anti-pollution.At this point,aluminum syntactic foams(ASFs)are new-generation engineering composites and come into the upfront as a problem-solver.Owing to their features like low density,sufficient elongation,and perfect energy absorption ability,these advanced foams have been considerably seductive for many industrial sectors nowadays.In this study,an industrial-oriented automatic die casting technology was used for the first time to manufacture the combination of AA7075/porous expanded clay(PEC).Micro evaluations(optical and FESEM)reveal that there is a homogenous particle distribution in the foam samples,and inspections are compatible with the other ASF studies.Additionally,T6 aging heat treatment was operated on one half of the produced foams to explore the probable impact of aging on the compressive responses.Attained results show that PEC particles can be an alternative to expensive hollow spheres used in the previous works.Besides,a favorable relationship is ascertained between the aging treatment and mechanical properties such as compression strength and plateau strength. 展开更多

The main aims of the current study are to determine the morphological features of the periglacial landforms(non-sorted step,mud circle,stony earth circle,thufur,and congeliturbation)located on the Ilgaz Mountains,examine the physicochemical and mineralogical properties with pedological processes of the soils,and assess of the effects of climatic conditions controlling the development of landforms.The Ilgaz Mountains(2587 m a.s.l.),located in the Western Black Sea Region,within the Anatolian Mountains,are important in terms of periglacial landforms(mud circles,stony earth circles,thufurs,non-sorted steps,non-sorted stripes,congeliturbation deposits,and block currents).The descriptive statistics of 123 periglacial landforms measured by fieldworks were analyzed.The distribution of freezing and thawing in the Ilgaz Mountains throughout the year was evaluated,and it was found that freezing takes place between December and March,freezing-thawing takes place in April,May,October and November,and thawing takes place between June and October.Accoding to soil properties,organic matter content changes from 1.88%to 12.72%in non-sorted step soils,while it is between 2.03%and 12.24%in stony earth circle soils.The organic matter is observed to be close to congeliturbation deposits at lower ratios compared to non-sorted steps,stony earth circles and mud circles.The soil reactions on stony earth circles and non-sorted steps vary between slightly acidic and slightly alkaline.On the other hand,soil samples taken from the mud circles are different from those taken from the non-sorted steps and stony earth circles.Their soil reaction is acidic,and pH changes between 4.86 and 6.25.The lime content also varies between 2.81%and 32.08%,with an average of 12.02%.The texture properties of soils are dominantly loam and clay loam,as in the non-sorted steps,stony earth circles,and mud circles.Considering their mineralogical properties,the XRD study was carried out to determine the primer mineral types and abundance degrees of soils of periglacial landforms.Quartz,muscovite and albite minerals were found in soils in the stony earth circle,while quartz,muscovite,orthoclase and albite minerals were determined as primary minerals in soils formed on the thufur landforms. 展开更多
