Addressing these risks is essential for enhancing patient safety and meeting global dialysis water quality standards. Advanced filtration technologies, such as titanium dioxide (TiO₂)-based nanoparticle filters, offer a promising approach to improve water purification processes in renal care. Objectives: This study aimed to develop and evaluate the effectiveness of a TiO₂-based nanoparticle microporous filtration system for hemodialysis water purification. The objectives included analyzing the system’s performance in reducing chemical contaminants (calcium, magnesium, aluminum, and lead) and microbiological contaminants (total viable count [TVC] and endotoxin units [EU]) across multiple renal centers. Methods: Water samples from three renal centers (RC1, RC2, and RC3) were analyzed pre- and post-filtration. TiO₂ nanoparticles were synthesized using the sol-gel method and characterized via Fourier Transform Infrared (FTIR) spectroscopy and Scanning Electron Microscopy with Energy Dispersive X-ray analysis (SEM/EDX). The microporous filter, fabricated with TiO₂ nanoparticles, silicon dioxide, and polyethylene glycol (PEG), was tested for its ability to remove contaminants. Analytical techniques included spectroscopy for chemical analysis and microbiological assays for contaminant quantification. Results: Post-treatment analysis revealed significant reductions in chemical contaminants, with removal efficiencies averaging 78% for calcium, 80% for magnesium, 81% for aluminum, and 76.6% for lead across all centers. Microbiological contamination was also substantially reduced, with 78–80% removal of TVC and 76–84.6% reduction in EU levels. FTIR analysis confirmed the presence of hydroxyl groups critical for adsorption, while SEM/EDX characterization revealed a crystalline structure with a particle size of 1.45 nm, pore size of 4.11 μm, filter height of 2.56 mm, and bulk density of 0.58 g/cm³. Conclusion: The TiO₂-based nanoparticle filtration system demonstrated high efficacy in removing chemical and microbiological contaminants, significantly improving water quality for hemodialysis. These results highlight its potential as a practical solution for renal centers, especially in resource-constrained settings. Further studies are needed to evaluate its long-term performance and feasibility for widespread adoption. Recommendation: Renal centers should consider adopting TiO2-based nanoparticle filters to address persistent water quality challenges. Pilot implementations across diverse settings can provide insights into operational feasibility. Additional research should explore scalability, maintenance requirements, and cost-effectiveness to optimize integration into healthcare systems. Significance Statement: This study introduces a practical and innovative solution to improve hemodialysis water purification. Syphilis and HIV are amongst the world’s most widespread diseases, particularly in low-income countries. Syphilis and HIV infections during pregnancy have been associated with numerous adverse pregnancy outcomes. Of ...Syphilis and HIV are amongst the world’s most widespread diseases, particularly in low-income countries. Syphilis and HIV infections during pregnancy have been associated with numerous adverse pregnancy outcomes. Of concern now are the rising rates of congenital syphilis and HIV in Cameroon. Cameroon only mandates testing pregnant women for syphilis and HIV during their first ANC visit. This study was aimed at determining the incidence of new syphilis and HIV infections and factors associated with pregnant women who previously tested negative during their first ANC visit. A cohort design was used, where 335 pregnant women were followed up for a period from December 2019 to August 2020. A blood sample was drawn and the serum was analyzed using the WANTAI ELISA and AIDTM HIV 1 + 2 Ag/Ab ELISAPlus test methods for syphilis and HIV respectively at three intervals. A questionnaire was used to identify risk factors. Data was analyzed using SPSS 23.0. Out of the 335 pregnant women who were followed up during this study, 49 (14.6%) were later diagnosed with syphilis (32 in 2<sup>nd</sup> trimester and 17 in 3<sup>rd</sup> trimester). 54 (16.1%) were diagnosed with HIV infection (13 at two months post-1<sup>st</sup>-trimester visit, 23 in the 2<sup>nd</sup> trimester and 18 in the 3rd trimester). Lastly, 10 (2%) were co-infected with syphilis and HIV of which 8 occurred during 2<sup>nd</sup> trimester and 2 in the 3<sup>rd</sup> trimester. The factors associated with contracting new syphilis infections include;younger age group aOR (1.302, 95% CI), leaving in an urban area aOR (3.158, 95% CI), lower level of education (Primary and no formal) with aOR of (9.055, 95% CI) (P = 0.001) and (6.764, 95% CI) (P = 0.006) respectively, inadequate knowledge on the diseases aOR (2.176, 95% CI), women unaware of their partner status aOR (3.190, 95% CI). Most factors associated with contracting new HIV infections were similar to the factors associated with contracting new syphilis infections post 1<sup>st</sup> ANC visit aOR (1.174, 95% CI) and pregnant women with more than one sexual partner aOR (7.342, 95% CI) were observed for HIV infection.. There is an increased incidence of new infection of syphilis and HIV post first ANC screening in the Buea Health District, Cameroon. The need for constant education on the identifiable factors and these diseases, and screening during every ANC visit irrespective of their previous laboratory results is warranted.展开更多

Introduction: Like healthcare workers, medical students are also exposed to a risk of accidental contamination during their hospital internships. However, they do not benefit from prevention services in the same way a...Introduction: Like healthcare workers, medical students are also exposed to a risk of accidental contamination during their hospital internships. However, they do not benefit from prevention services in the same way as workers in this sector, although they actively participate in the routine care provided to patients in the various hospital departments. The objective of our study is to determine the incidence of these accidents and their medical and administrative management among these victims (medical students) not classified in the category of workers. Materials and Method: This was a cross-sectional study carried out over three months from January 15 to March 14, 2023. The study population consisted of all medical students at the Faculty of Health Sciences at the University of Bangui. Regularly enrolled students from 2nd to 7th year, on placement in hospitals in the Central African Republic and who gave their informed consent were included. Data were collected through a self-administered questionnaire. Results: A cross-sectional study was carried out among medical students concerning the incidence and management of accidents involving exposure to blood in the Central African Republic. Of the 254 students included, three (9%) had already been correctly vaccinated against the viral hepatitis B virus, 198 (77.9%) reported still having access to personal protective equipment, 133 students (52.4%) reported being victims of OBE in the past 12 months. The main types of OBE were stings (62%), followed by liquid splashes (32%) and cuts (6%). Among the 133 victims, 92.9% carried out immediate hand disinfection or abundant eye washing, 41.9% obtained the serology of the source patient and 37% reported the accident to their supervisor. Conclusion: It is important that students benefit from better prevention and management of these accidents.展开更多
Until 2015,China had established 2740 nature reserves with a total area of 1.47million km^2,covering 14.8%of China’s terrestrial land surface.Based on remote sensing inversion,ecological model simulation and spatial ...Until 2015,China had established 2740 nature reserves with a total area of 1.47million km^2,covering 14.8%of China’s terrestrial land surface.Based on remote sensing inversion,ecological model simulation and spatial analysis methods,we analyzed the spatial and temporal variations of fractional vegetation coverage(FVC),net primary production(NPP),and human disturbance(HD)in habitats of typical national nature reserves(NNRs)during the first 15 years of the 21st century from 2000 to 2015.And then the three indicators were compared between different NNR types and varied climate zones.The results showed that(1)the average 5-year FVC of NNRs increased from 36.3%to 37.1%,and it improved in all types of NNRs to some extent.The annual average FVC increased by 0.11%,0.84%,0.21%,0.09%,0.11%and 0.08%in NNRs of forest ecosystem,plain meadow,inland wetland,desert ecosystem,wild animal and wild plant,respectively.(2)The NPP annually increased by 2.06 g·m^(-2),1.23 g·m^(-2),0.28 g·m^(-2)and 0.4 g·m^(-2)in NNRs of plain meadow,inland wetland,desert ecosystem and wild animal,respectively.However,it decreased by 3.45 g·m^(-2)and2.35 g·m^(-2)in NNRs of forest ecosystem and wild plant respectively.(3)In the past 15 years,besides the slight decreases in the NNRs located at the Qinghai-Tibet Plateau and the south subtropical zone,HD enhanced in most of NNRs,especially HD in the warm temperate humid zone increased from 4.7%to 5.35%.展开更多

Various recent reports on Tuberculosis have alarmed an increase in the patient class and subsequent death rates across the globe. Over and above the spread of more dangerous and fatal forms of tuberculosis like MDR-TB i.e. multiple-drug resistance tuberculosis, XDR-TB i.e. extensively-drug resistance tuberculosis & TDR-TB i.e. total-drug resistance tuberculosis has forwarded an urgent need to discover novel antitubercular agents. The current work is aimed at combining two previously well-known pharmacophores (pyrazoline and benzoxazole nucleus) in order to design and synthesize a series of novel benzoxazole-based pyrazoline derivatives. The synthesized target compounds were structurally confirmed by LCMS, 1H-NMR and 13C-NMR analysis. The target compounds were In vitro evaluated against M. tuberculosis H37Rv strain, multidrug-resistant TB (MDR-TB) and extensively drug-resistant TB (XDR-TB) strains. The In vitro screening results depicted that majority of the target compounds displayed potent activity with MIC in a range of ~0.8 to 6.25 μg/mL. Many compounds were found to be more potent than isoniazid against MDR-TB with MIC value 3.12 μg/mL and XDR-TB with MIC value 12.5 μg/mL. Cytotoxicity assay of these active compounds on VERO cell lines also displayed good selectivity index.展开更多

Method: Cross-sectio...Objective: To analyze maternal deaths, identify causes and dysfunctions leading to these deaths in order to contribute to the implementation of strategies to reduce maternal mortality at CHU Kara. Method: Cross-sectional descriptive study involving 65 cases of maternal deaths recorded at CHU-Kara from January 1, 2018 to June 30, 2021. Results: Our study focused on 65 cases of maternal deaths recorded at the maternity ward of CHU-Kara. The average age was 30 years, with a range of 15 to 45 years. They were mostly housewives (52.3%), uneducated (38.5%), multiparous (41.5%), and referred (86.2%). The causes were mainly direct obstetric causes (81.54%), with preeclampsia and its complications (28.30%) and immediate postpartum hemorrhage (20.75%) being the most common. However, uterine rupture (20.5%) and post-abortion sepsis (16.4%) were the most lethal etiologies. Delayed evacuation (46.43%), inadequate transportation (91%), and insufficient prenatal care (72.31%) were the dysfunctions before referral. Within the CHU Kara, delays in management (58.46%), unavailability of blood and labile products (18%), and insufficient monitoring were the dysfunctions identified. Ninety-five point four percent (95.4%) of the deaths were preventable. Conclusion: The magnitude of intrahospital maternal deaths, the various dysfunctions observed in the occurrence of maternal deaths before referral/evacuation and within the hospital highlight the importance of effectively implementing recommendations from audits in the fight against maternal mortality. The causes were mainly direct obstetric causes (81.54%), with preeclampsia and its complications (28.30%) and immediate postpartum hemorrhage (20.75%) being the most common. However, uterine rupture (20.5%) and post-abortion sepsis (16.4%) were the most lethal etiologies. Delayed evacuation (46.43%), inadequate transportation (91%), and insufficient prenatal care (72.31%) were the dysfunctions before referral. Within the CHU Kara, delays in management (58.46%), unavailability of blood and labile products (18%), and insufficient monitoring were the dysfunctions identified. Ninety-five point four percent (95.4%) of the deaths were preventable. Conclusion: The magnitude of intrahospital maternal deaths, the various dysfunctions observed in the occurrence of maternal deaths before referral/evacuation and within the hospital highlight the importance of effectively implementing recommendations from audits in the fight against maternal mortality. The majority of the deaths were preventable (95.38%).
