BACKGROUND The recommended monitoring tools for evaluating nucleot(s)ide analogue renal toxicity,such as estimated glomerular filtration rate(eGFR)and phosphatemia,are late markers of proximal tubulopathy.Multiple ear...BACKGROUND The recommended monitoring tools for evaluating nucleot(s)ide analogue renal toxicity,such as estimated glomerular filtration rate(eGFR)and phosphatemia,are late markers of proximal tubulopathy.Multiple early markers are available,but no consensus exists on their use.AIM To determine the 24 mo prevalence of subclinical proximal tubulopathy(SPT),as defined with early biomarkers,in treated vs untreated hepatitis B virus(HBV)-monoinfected patients.METHODS A prospective,non-randomized,multicenter study of HBV-monoinfected patients with a low number of renal comorbidities was conducted.The patients were separated into three groups:Naïve,starting entecavir(ETV)treatment,or starting tenofovir disoproxil(TDF)treatment.Data on the early markers of SPT,the eGFR and phosphatemia,were collected quarterly.SPT was defined as a maximal tubular reabsorption of phosphate/eGFR below 0.8 mmoL/L and/or uric acid fractional excretion above 10%.The prevalence and cumulative incidence of SPT at month 24(M24)were calculated.Quantitative data were analyzed using analyses of variance or Kruskal-Wallis tests,whereas chi-squared or Fisher’s exact tests were used to analyze qualitative data.Multivariate analyses were used to adjust for any potential confounding factors.RESULTS Of the 196 patients analyzed,138(84 naïve,28 starting ETV,and 26 starting TDF)had no SPT at inclusion.At M24,the prevalence of SPT was not statistically different between naïve and either treated group(21.1%vs 30.7%,P<0.42 and 50.0%vs 30.7%,P=0.32 for ETV and TDF,respectively);no patient had an eGFR lower than 50 mL/min/1.73 m²or phosphatemia less than 0.48 mmoL/L.In the multivariate analysis,no explanatory variables were identified after adjustment.The cumulative incidence of SPT over 24 mo(25.5%,13.3%,and 52.9%in the naïve,ETV,and TDF groups,respectively)tended to be higher in the TDF group vs the naïve group(hazard ratio:2.283,P=0.05).SPT-free survival at M24 was 57.6%,68.8%,and 23.5%for the naïve,ETV,and TDF groups,respectively.The median survival time without SPT,evaluated only in the TDF group,was 5.9 mo.CONCLUSION The prevalence and incidence of SPT was higher in TDF-treated patients compared to naïve patients.SPT in the naïve population suggests that HBV can induce renal tubular toxicity.展开更多
This study at the Esmeralda Mine,part of the El Teniente Division of CODELCO,investigates optimizing hydraulic fracturing(HF)holes’spatial distribution to improve rock material production in one of the world's la...This study at the Esmeralda Mine,part of the El Teniente Division of CODELCO,investigates optimizing hydraulic fracturing(HF)holes’spatial distribution to improve rock material production in one of the world's largest copper-molybdenum deposits.Utilizing diverse data sources,including borehole,oriented borehole,and photogrammetry data,along with hang-up frequency and hydrofracturing details,we applied discrete fracture network(DFN)modeling to analyze in-situ block size distribution and fragmentation.These results are based on 12,000 realizations of discrete fracture network(DFN)models using R-Dis-Frag computer pacakge at real cave volumes of 200 m200 m200 m,with varying parameters,which significantly enhances their reliability.The incorporation of DFN modeling and geostatistical simulation allows for capturing the interaction berween several spatial variables and explaining the variations observed in the production results at the draw points.Keyfindings of spatio-statistical analysis highlight the significance of volumetric fracture intensity(P32)and extraction column height in reducing hang-up events and enhancing fragmentation efficiency.The study integrates HF-induced and natural fracture intensities,revealing that higher P32 values and higher draw columns correlate with fewer hang-ups and better fragmentation.We recommend non-regular HF patterns for high P32 zones to improve operational efficiency.This research provides insights into optimizing mining operations,acknowledging the limitations of HF propagation efficacy and paving the way for further exploration into the interplay between hydraulic fracturing and natural discontinuities.展开更多
Fe-based materials have received more and more interests in recent years as candidates to fabricate bioresorbable stents due to their appropriate mechanical properties and biocompatibility.However,the low degradation ...Fe-based materials have received more and more interests in recent years as candidates to fabricate bioresorbable stents due to their appropriate mechanical properties and biocompatibility.However,the low degradation rate of Fe is a serious limitation for such application.To overcome this critical issue,many efforts have been devoted to accelerate the corrosion rate of Fe-based stents,through the structural and surface modification of Fe matrix.As stents are implantable devices,the released corrosion products(Fe^(2+)ions)in vessels may alter the metabolism,by generating reactive oxygen species(ROS),which might in turn impact the biosafety of Fe-based stents.These considerations emphasize the importance of combining knowledge in both materials and biological science for the development of efficient and safe Fe-based stents,although there are still only limited numbers of reviews regarding this interdisciplinary field.This review aims to provide a concise overview of the main strategies developed so far to design Fe-based stents with accelerated degradation,highlighting the fundamental mechanisms of corrosion and the methods to study them as well as the reported approaches to accelerate the corrosion rates.These approaches will be divided into four main sections,focusing on(i)increased active surface areas,(ii)tailored microstructures,(iii)creation of galvanic reactions(by alloying,ion implantation or surface coating of noble metals)and(iv)decreased local pH induced by degradable surface organic layers.Recent advances in the evaluation of the in vitro biocompatibility of the final materials and ongoing in vivo tests are also provided.展开更多

Background:While malaria morbidity and mortality have declined since 2000,viral central nervous system infections appear to be an important,underestimated cause of coma in malaria-endemic Eastern Africa.We aimed to de...Background:While malaria morbidity and mortality have declined since 2000,viral central nervous system infections appear to be an important,underestimated cause of coma in malaria-endemic Eastern Africa.We aimed to describe the etiology of non-traumatic comas in young children in Benin,as well as their management and early outcomes,and to identify factors associated with death.Methods:From March to November 2018,we enrolled all HIV-negative children aged between 2 and 6 years,with a Blantyre Coma Score≤2,in this prospective observational study.Children were screened for malaria severity signs and assessed using a systematic diagnostic protocol,including blood cultures,malaria diagnostics,and cerebrospinal fluid analysis using multiplex PCR.To determine factors associated with death,univariate and multivariate analyses were performed.Results:From 3244 admissions,84 children were included:malaria was diagnosed in 78,eight of whom had a viral or bacterial co-infection.Six children had a non-malarial infection or no identified cause.The mortality rate was 29.8%(25/84),with 20 children dying in the first 24 h.Co-infected children appeared to have a poorer prognosis.Of the 76 children who consulted a healthcare professional before admission,only 5 were prescribed adequate antimalarial oral therapy.Predictors of early death were jaundice or increased bilirubin[odd ratio(OR)=8.6;95% confidential interval(CI):2.03-36.1]and lactate>5 mmol/L(OR=5.1;95%CI:1.49-17.30).Antibiotic use before admission(OR=0.1;95%CI:0.02-0.85)and vaccination against yellow fever(OR=0.2,95%CI:0.05-0.79)protected against mortality.Conclusions:Infections were found in all children who died,and cerebral malaria was by far the most common cause of non-traumatic coma.Missed opportunities to receive early effective antimalarial treatment were common.Other central nervous system infections must be considered in their management.Some factors that proved to be protective against early death were unexpected.展开更多
