BACKGROUND Hepatocellular carcinoma(HCC)is one of the most common malignant tumours of the digestive system worldwide.The expression of Ki-67 is crucial for the diagnosis,treatment,and prognostic evaluation of HCC.AIM...BACKGROUND Hepatocellular carcinoma(HCC)is one of the most common malignant tumours of the digestive system worldwide.The expression of Ki-67 is crucial for the diagnosis,treatment,and prognostic evaluation of HCC.AIM To construct a machine learning model for the preoperative evaluation of Ki-67 expression in HCC and to assist in clinical decision-making.METHODS This study included 164 pathologically confirmed HCC patients.Radiomic features were extracted from the computed tomography images reconstructed by superresolution of the intratumoral and peritumoral regions.Features were selected via the intraclass correlation coefficient,t tests,Pearson correlation coeffi-cients and least absolute shrinkage and selection operator regression methods,and models were constructed via various machine learning methods.The best model was selected,and the radiomics score(Radscore)was calculated.A nomogram incorporating the Radscore and clinical risk factors was constructed.The predictive performance of each model was evaluated via receiver operating characteristic(ROC)curves and calibration curves,and decision curve analysis was used to assess the clinical benefits.RESULTS In total,164 HCC patients,namely,104 patients with high Ki-67 expression and 60 with low Ki-67 expression,were included.Compared with the models in which only intratumoral or peritumoral features were used,the fusion model in which intratumoral and peritumoral features were combined demonstrated stronger predictive ability.Moreover,the clinical-radiomics model including the Radscore and clinical features had higher predictive performance than did the fusion model(area under the ROC curve=0.848 vs 0.780 in the training group,area under the ROC curve=0.830 vs 0.760 in the validation group).The calibration curve showed good consistency between the predicted probability and the actual probability,and the decision curve further confirmed its clinical benefit.CONCLUSION A machine learning model based on the radiomic features of the intratumoral and peritumoral regions on superres-olution computed tomography in conjunction with clinical factors can accurately evaluate Ki-67 expression.The model provides valuable assistance in selecting treatment strategies for HCC patients and contributes to research on neoadjuvant therapy for liver cancer.展开更多

Based on the concept of cost-effectiveness ratio, according to the newly formulated Johnson criterion and the view of discovery and destruction, this paper proposes to take the identification probability as the probability of being destroyed and uses mathematical formulas to calculate the cost of a single use decoy. On this basis, a cost-effectiveness ratio model is established, with the product of the increase in the survival probability of the target and the cost of the target as the benefit, and the sum of the product of the probability of being destroyed and the cost of the decoy and the cost of a single use as the consumption cost. Spatial transcriptomics technology provides novel insights into the spatial organization of gene expression during embryonic development.In this study,we propose a method that integrates analysis across both temporal ...Spatial transcriptomics technology provides novel insights into the spatial organization of gene expression during embryonic development.In this study,we propose a method that integrates analysis across both temporal and spatial dimensions to investigate spatial transcriptomics data from mouse embryos at different developmental stages.We quantified the spatial expression pattern of each gene at various stages by calculating its Moran’s I.Furthermore,by employing time-series clustering to identify dynamic co-expression modules,we identified several developmentally stage-specific regulatory gene modules.A key finding was the presence of distinct,stage-specific gene network modules across different developmental periods:Early modules focused on morphogenesis,mid-stage on organ development,and late-stage on neural and tissue maturation.Functional enrichment analysis further confirmed the core biological functions of each module.The dynamic,spatially-resolved gene expression model constructed in this study not only provides new biological insights into the programmed spatiotemporal reorganization of gene regulatory networks during embryonic development but also presents an effective approach for analyzing complex spatiotemporal omics data.This work provides a new perspective for understanding developmental biology,regenerative medicine,and related fields.展开更多

Methods: The study used the experimental control method. The study lasted from September to November 2022. The subjects of this study were 49 students of standardized training for resident doctors of traditional Chinese Medicine from grades 2020, 2021 and 2022 of Dazhou integrated TCM & Western Medicine Hospital. They were randomly divided into experiment group (25) and control group (24). The experiment group adopted flipped classroom combined with problem-based learning teaching method, and the control group adopted traditional teaching method. The teaching content was 4 basic clinical skill projects, including four diagnoses of traditional Chinese Medicine, cardiopulmonary resuscitation, dressing change procedure, acupuncture and massage. The evaluation method was carried out by comparing the students’ performance and a self-designed questionnaire was used to investigate the students’ evaluation of the teaching method. Results: The test scores of total scores in the experimental group (90.12 ± 5.89) were all higher than those in the control group (81.47 ± 7.96) (t = 4.53, P P Conclusions: The teaching process of the flipped classroom combined with problem-based learning teaching method is conducive to improving the efficiency of classroom teaching, cultivating students’ self-learning ability, and enhancing students’ willingness to learn.展开更多

Background:Ferroptosis is a type of regulated cell death characterized by iron-dependent lipid peroxidation,which has been linked to tumor progression and therapeutic resistance.However,the contribution of lactate met...Background:Ferroptosis is a type of regulated cell death characterized by iron-dependent lipid peroxidation,which has been linked to tumor progression and therapeutic resistance.However,the contribution of lactate metabolism and its receptor,hydroxycarboxylic acid receptor 1(HCAR1),in ferroptosis regulation in gastric cancer(GC)remains poorly understood.Focusing specifically on its effects on cell proliferation,ferroptosis regulation,and the disruption of lactate-mediatedmetabolic pathways,the study aimed to clarify the role ofHCAR1 in GC progression.Methods:Bioinformatics analysis identified prognostic genes associated with ferroptosis in GC.Receiver operating characteristic(ROC)curves were generated to assess the diagnostic potential of the predictive genes.The biological role of HCAR1 was investigated through gain and loss-of-function experiments in GC cell lines,followed by assessments of cell viability,oxidative stress indicators,gene/protein expression,and ferroptosis sensitivity under lactate stimulation or HCAR1modulation.Results:HCAR1 was significantly upregulated in GC tissues and linked to poor patient outcomes.Silencing HCAR1 inhibited GC cell growth and induced ferroptosis,as shown by increased levels of reactive oxygen species(ROS)and malondialdehyde(MDA),along with decreased expression of solute carrier family 7 member 11(SLC7A11)and glutathione peroxidase 4(GPX4).Conversely,HCAR1 overexpression or exposure to extracellular lactate inhibited ferroptosis and activated antioxidant defenses.Mechanistically,lactate activation of HCAR1 increases ATP levels,which in turn inactivates AMP-activated protein kinase(AMPK).It also upregulates stearoyl-CoA desaturase 1(SCD1)through the sterol regulatory element binding protein 1(SREBP1)signaling pathway.Blocking HCAR1 reversed these effects and restored ferroptosis sensitivity.Conclusion:HCAR1 mediates lactate-driven ferroptosis resistance in GC through the AMPK-SCD1 signaling pathway.Targeting the HCAR1-lactate axis may offer a promising strategy for overcoming metabolic adaptation and improving GC treatment outcomes.展开更多
BACKGROUND Single-port laparoscopic appendectomy is an advanced minimally invasive surgery that involves the use of upgraded instruments and equipment.We previously modified single-port laparoscopic appendectomy with ...BACKGROUND Single-port laparoscopic appendectomy is an advanced minimally invasive surgery that involves the use of upgraded instruments and equipment.We previously modified single-port laparoscopic appendectomy with needle-type grasping forceps(mSLAN)for patients with simple appendicitis,but the feasibility and safety of our modified procedure need further evaluation in a highquality clinical study.AIM To compare the short-term clinical outcomes of mSLAN with those of conventional three-port laparoscopic appendectomy(CLA)for patients with acute uncomplicated appendicitis.METHODS This single-center,single-blind,prospective,randomized controlled trial included patients who underwent emergency laparoscopic appendectomy for acute uncomplicated appendicitis at our center between April 2024 and August 2024.Patients were randomly divided into the mSLAN group or the CLA group via computergenerated randomization.The primary endpoint was the 24-hour postoperative visual analog scale(VAS)score,and the secondary endpoints included the operative time,24-hour postoperative inflammatory response biomarkers(including white blood cells,the neutrophil ratio,interleukin-6,and C-reactive protein),time to first postoperative exhaust,time to first out-of-bed activity,postoperative length of hospital stay,cost of hospitalization,and incidence of postoperative complications.RESULTS A total of 72 patients were enrolled and randomly divided into 2 groups:The mSLAN group(n=36)and the CLA group(n=36).The 24-hour VAS scores,24-hour postoperative inflammatory response marker levels,first postoperative exhaust times,first out-of-bed activity times,postoperative lengths of hospital stay,operative times,or hospitalization costs did not significantly differ between the two groups.No postoperative complications,including incision infection or hernia,abdominal abscess or intestinal obstruction,were observed during the 1-month postoperative follow-up in either group.CONCLUSION Compared with the CLA protocol,the mSLAN protocol for acute uncomplicated appendicitis yielded comparable short-term clinical outcomes,with a similar operative time and better cosmetic outcomes,indicating its potential for clinical application and superiority for patients with high cosmetic requirements.Further research is needed to evaluate the long-term outcomes.展开更多
Objective:Immune checkpoint inhibitors(ICIs)have demonstrated substantial efficacy in patients with advanced non-small cell lung cancer(NSCLC).However,the optimal duration of ICI therapy remains unclear,and limited re...Objective:Immune checkpoint inhibitors(ICIs)have demonstrated substantial efficacy in patients with advanced non-small cell lung cancer(NSCLC).However,the optimal duration of ICI therapy remains unclear,and limited real-world data are available.The aim of this study was to evaluate the relationship between ICI therapy duration and overall survival(OS)in patients who achieved varying best overall response(BOR)during ICI treatment,and to compare patients treated for 6 to 18 months vs.at least 18 months.Methods:This retrospective cohort study included adult patients diagnosed with advanced NSCLC who received ICI therapy at the Zhejiang Cancer Hospital between 2017 and 2022.Data collection ended on May 1,2024,and statistical analysis was performed between May and June 2024.Results:Using strict entry criteria,we screened 487 patients with advanced NSCLC and identified 134 eligible patients.Among these patients,the median durations of immunotherapy and follow-up were 24.57 months and 43.60 months,respectively.The objective response rate(ORR)was 58.2%,and the median progression-free survival(PFS)was 10.6 months.Median OS was not reached.At the last follow-up,54 patients had no disease progression,and 118 patients remained alive.Patients treated with ICI therapy for≥18 months had superior survival to those treated for 6 to 18 months(P=0.039).Further analysis revealed that the survival benefit was associated with BOR during ICI therapy.Specifically,patients achieving complete response/partial response(CR/PR)who received≥18 months of ICI therapy had a trend toward longer median OS than those treated for 6 to 18 months,but the difference did not reach statistical significance(P=0.177).Patients with stable disease(SD)who received≥18 months of ICI therapy had a statistically longer median OS than those treated for 6 to 18 months(P=0.019).Among patients treated with ICIs for≥18 months,24 continued with ICI-based therapy and achieved a median PFS2 of 6.67 months,an ORR of 33.3%,and a disease control rate(DCR)of 83.3%.Conclusions:This study provides real-world evidence and novel insights into the need for continuing ICI therapy beyond 18 months in patients with advanced NSCLC who do not exhibit progressive disease.For patients achieving SD during ICI therapy,a treatment duration of at least 18 months appears appropriate.For patients achieving CR/PR,treatment decisions should be individualized according to patient-specific circumstances.However,owing to the retrospective study design,potential selection bias and confounding factors might have influenced the results.Therefore,our findings require further validation in prospective clinical studies.展开更多

Background:Diabetic foot,a severe complication of diabetes,is characterized by chronic refractory wounds.Sanhuang Oil,a topical herbal formula,demonstrates significant therapeutic effects including antibacterial,anti-...Background:Diabetic foot,a severe complication of diabetes,is characterized by chronic refractory wounds.Sanhuang Oil,a topical herbal formula,demonstrates significant therapeutic effects including antibacterial,anti-inflammatory,and immunomodulatory activities.However,its active constituents and mechanisms of action against diabetic foot remain to be elucidated.Methods:In this study,the chemical constituents of Sanhuang Oil were identified using UPLC-QE-Orbitrap-MS.Subsequently,the mechanism by which Sanhuang Oil promotes diabetic foot ulcer healing was predicted by integrating network pharmacology and molecular docking.Additionally,diabetic mouse model was established in ICR mice using a combination of a high-fat diet(HFD)and streptozotocin(STZ)chemical induction.A full-thickness skin defect was created on the dorsum of the mice.Wound healing and the healing rate were observed following Sanhuang Oil intervention.The mechanism underlying Sanhuang Oil’s promotion of diabetic ulcer healing was further investigated using transcriptomics and histopathological examination(H&E staining).Results:A total of 97 active ingredients were identified from Sanhuang Oil.Network pharmacology analysis predicted 543 common targets,and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis identified 203 relevant pathways.Molecular docking further confirmed high binding affinity(binding energy≤−5.0 kcal/mol)between specific active components in Sanhuang Oil(e.g.,coptisine,phellodendrine,baicalein)and key targets associated with diabetic foot ulcers(e.g.,EGFR,AKT1,STAT3).In vivo experimental results demonstrated that the wound healing rate was significantly higher in Sanhuang Oil-treated groups compared to the model group(P<0.001).HE staining revealed that the high-dose Sanhuang Oil group exhibited more pronounced epithelial tissue coverage over the wound,reduced inflammatory cell infiltration,and increased collagen deposition and fibroblast proliferation.transcriptomic analysis identified Pdk4,Ttn,Csrp3,Actn2,Myoz2,Tnnc2,Myod1,Myog,Myot,and Myf6 as key regulatory proteins involved in promoting wound healing.Conclusion:Sanhuang Oil promotes wound healing in diabetic ulcer mice,potentially by mitigating inflammation and regulating key targets such as Pdk4 to enhance fibroblast function.These findings provide novel insights into the multi-target,multi-pathway mechanism of Sanhuang Oil for treating diabetic foot ulcers.展开更多

The rate-limiting enzyme in de novo serine biosynthesis, 3-phosphoglycerate dehydrogenase(PHGDH), has a well-established role in cellular metabolism, yet its specific role in macrophages remains unknown.Methods: Metabolomics assays were conducted to assess metabolite composition and dynamics in macrophages. Changes in polarization and immunosuppressive markers were validated with q RT-PCR. Bioinformatics was used to analyze immune cell subsets and associated metabolic pathways. Finally, Ch IP-q PCR and co-immunoprecipitation assays were performed to elucidate the downstream regulatory mechanisms of PHGDH.Results: Serine metabolism was found to be downregulated in TAMs in breast cancer. Functional studies revealed that PHGDH inhibition promotes an M2-like phenotype and immunosuppressive functions in macrophages. Furthermore, PHGDH was found to undergo nuclear translocation during macrophage polarization. Mechanistically, nuclear PHGDH was found to regulate GLUD1 and GLS2 transcription via interaction with the transcription factor STAT3. Rescue experiments demonstrated that glutamine supplementation and STAT3 inhibition reversed the effects of PHGDH on macrophage function.Conclusions: Our findings reveal a previously unrecognized non-canonical metabolic function of PHGDH, thus providing potential therapeutic targets in the tumor microenvironment for reversing malignant progression.展开更多

In such studies, robots are often required to possess capabilities in computation, storage, perception, and two-dimensional(2D)movement to execute predefined rules. Under these rules, the swarm can accomplish complex tasks, exhibit rich collective states, or demonstrate intriguing phase transition phenomena. However, this study demonstrates how a swarm of spin robots, which only respond to simple ambient light intensity, can be constructed into a collective system capable of performing practical swarm tasks such as phototactic motion, controllable folding, and object transport through weak coupling interactions between individuals. Furthermore, it is proven that this swarm exhibits strong system fault tolerance. This research aims to demonstrate that, beyond the common design of sophisticated individuals and excellent inter-individual interaction rules, appropriate structural and coupling designs can enable individuals without computational capabilities to generate complex collective behaviors and accomplish diverse swarm tasks through cooperation. This provides a research direction for experimental studies of active matter using robotic systems.展开更多
Objective Tumour cells in a hypoxic state are more invasive,have stronger self-renewal capabilities,and are difficult to treat because of their ability to promote tumour recurrence and metastasis.The glycolysis inhibi...Objective Tumour cells in a hypoxic state are more invasive,have stronger self-renewal capabilities,and are difficult to treat because of their ability to promote tumour recurrence and metastasis.The glycolysis inhibitor 3-bromopyruvic acid(3-BrPA)can completely inactivate glycolytic enzymes at extremely low drug concentrations,thereby exerting a strong inhibitory effect on the glucose energy metabolism of tumor cells.Therefore,we tested the inhibitory effect of 3-BrPA on hepatocellular carcinoma cells(HepG2)in vitro;then,we used the VX2 liver cancer model to study the antitumour effect of 3-BrPA combined with interventional embolization on liver cancer.Methods In vitro,a CCK-8 assay was used to detect the inhibitory effect of different concentrations of 3-BrPA on HepG2 cells,and light microscopy confirmed that the HepG2 cells were completely dead.Western blotting was used to detect the expression of key proteins involved in apoptosis.A total of 30 New Zealand white rabbits were used to establish a liver cancer model and were randomly divided into 3 groups 2 weeks after tumor establishment:the control group was perfused with saline in the hepatic artery;the transcatheter arterial embolization(TAE)group was given TAE;and the experimental group was perfused with 3-BrPA combined with TAE.The tumor-bearing rabbits were killed one week after surgery.The tumor volume and tumor necrosis ratio were calculated via the histopathological examination.Results In vitro,the inhibitory effect of 3-BrPA on HepG2 cells increased with increasing concentration.3-BrPA(100μmol/L)could induce the necrosis of HepG2 cells.Stimulation with 50μmol/L 3-BrPA could activate the tumor cell apoptosis pathway.3-BrPA combined with TAE treatment could significantly inhibit tumor growth and cause more complete tumor necrosis.Conclusion 3-BrPA not only has antitumour effects in vitro but can also significantly improve antitumour effects in the hypoxic microenvironment after embolization in vivo.展开更多
The CO2reforming of CH4is studied over MgO‐promoted Ni catalysts,which were supported on alumina prepared from hydrotalcite.This presents an improved stability compared with non‐promoted catalysts.The introduction o...The CO2reforming of CH4is studied over MgO‐promoted Ni catalysts,which were supported on alumina prepared from hydrotalcite.This presents an improved stability compared with non‐promoted catalysts.The introduction of the MgO promoter was achieved through the‘‘memory effect’’of the Ni‐Al hydrotalcite structure,and ICP‐MS confirmed that only0.42wt.%of Mg2+ions were added into the Ni‐Mg/Al catalyst.Although no differences in the Ni particle size and basicity strength were observed,the Ni‐Mg/Al catalyst showed a higher catalytic stability than the Ni/Al catalyst.A series of surface reaction experiments were used and showed that the addition of a MgO promoter with low concentration can promote CO2dissociation to form active surface oxygen arising from the formation of the Ni‐MgO interface sites.Therefore,the carbon‐resistance promotion by nature was suggested to contribute to an oxidative environment around Ni particles,which would increase the conversion of carbon residues from CH4cracking to yield CO on the Ni metal surface.?2018,Dalian Institute of Chemical Physics,Chinese Academy of Sciences.Published by Elsevier B.V.All rights reserved.展开更多

Background:When lowlanders rapidly ascend to altitudes>2500 m,they may develop acute mountain sickness(AMS).The individual susceptibility,ascending velocity,time spent at altitude,activity levels and altitude reach...Background:When lowlanders rapidly ascend to altitudes>2500 m,they may develop acute mountain sickness(AMS).The individual susceptibility,ascending velocity,time spent at altitude,activity levels and altitude reached are considered risk factors for AMS.However,it is not clear whether sex is a risk factor.The results have been inconclusive.We conducted a meta-analysis to test whether there were sex-based differences in the prevalence of AMS using Lake Louise Scoring System.Methods:Systematic searches were performed in August 2019 in EMBASE,PubMed,and Web of Science for prospective studies with AMS data for men and women.The titles and abstracts were independently checked in the primary screening step,and the selected full-text articles were independently assessed in the secondary screening step by the two authors(YPH and JLW)based on pre-defined inclusion criteria.The meta-analysis was performed using by the STATA 14.1 software program.A random-effects model was employed.Results:Eighteen eligible prospective studies were included.A total of 7669 participants(2639[34.4%]women)were tested.The results showed that there was a statistically significant higher prevalence rate of AMS in women than in men(RR=1.24,95%CI 1.09–1.41),regardless of age or race.However,the heterogeneity was significant in the analysis(Tau2=0.0403,Chi2=50.15,df=17;I2=66.1%,P=0.000),it was main caused by different numbers of subjects among the studies(coefficient=–2.17,P=0.049).Besides,the results showed that there was no evidence of significant publication bias in the combined studies on the basis of Egger’s test(bias coefficient=1.48,P=0.052)and Begg’s test(P=0.130).Conclusions:According to this study,the statistically significant finding emerging from this study was that women have a higher prevalence of AMS.However,the authors could not exclude studies where patients were on acetazolamide.Our analysis provided a direction for future studies of the relationship of sex and the risk of AMS,such as the pathological mechanism and prevention research.展开更多

文摘As the most critical part of post-graduate education,the Chinese government launched Standard Resident Training in 2013 to solve the regional inequality of medical quality and meet the increasing social requirement for better medical service.We integrated problem-based learning(PBL)and case-based learning(CBL)in the Endodontics Standard Resident Training.By evaluating with objective parameters including theoretical knowledge and clinical practice skill,and subjective parameters including questionnaire,it was found that PBL+CBL played a positive role in endodontic resident training with a significant difference(P<0.05).This combined training model is instructive for China’s resident training,and this result can provide a rudimentary reference to current postgraduate teaching reform.

文摘BACKGROUND Hepatocellular carcinoma(HCC)is one of the most common malignant tumours of the digestive system worldwide.The expression of Ki-67 is crucial for the diagnosis,treatment,and prognostic evaluation of HCC.AIM To construct a machine learning model for the preoperative evaluation of Ki-67 expression in HCC and to assist in clinical decision-making.METHODS This study included 164 pathologically confirmed HCC patients.Radiomic features were extracted from the computed tomography images reconstructed by superresolution of the intratumoral and peritumoral regions.Features were selected via the intraclass correlation coefficient,t tests,Pearson correlation coeffi-cients and least absolute shrinkage and selection operator regression methods,and models were constructed via various machine learning methods.The best model was selected,and the radiomics score(Radscore)was calculated.A nomogram incorporating the Radscore and clinical risk factors was constructed.The predictive performance of each model was evaluated via receiver operating characteristic(ROC)curves and calibration curves,and decision curve analysis was used to assess the clinical benefits.RESULTS In total,164 HCC patients,namely,104 patients with high Ki-67 expression and 60 with low Ki-67 expression,were included.Compared with the models in which only intratumoral or peritumoral features were used,the fusion model in which intratumoral and peritumoral features were combined demonstrated stronger predictive ability.Moreover,the clinical-radiomics model including the Radscore and clinical features had higher predictive performance than did the fusion model(area under the ROC curve=0.848 vs 0.780 in the training group,area under the ROC curve=0.830 vs 0.760 in the validation group).The calibration curve showed good consistency between the predicted probability and the actual probability,and the decision curve further confirmed its clinical benefit.CONCLUSION A machine learning model based on the radiomic features of the intratumoral and peritumoral regions on superres-olution computed tomography in conjunction with clinical factors can accurately evaluate Ki-67 expression.The model provides valuable assistance in selecting treatment strategies for HCC patients and contributes to research on neoadjuvant therapy for liver cancer.
文摘With the continuous application of new technologies in reconnaissance and attack, false camouflage plays a more important role in improving the survivability of targets, and the number of decoys plays a crucial role in the camouflaging effect. Based on the concept of cost-effectiveness ratio, according to the newly formulated Johnson criterion and the view of discovery and destruction, this paper proposes to take the identification probability as the probability of being destroyed and uses mathematical formulas to calculate the cost of a single use decoy. On this basis, a cost-effectiveness ratio model is established, with the product of the increase in the survival probability of the target and the cost of the target as the benefit, and the sum of the product of the probability of being destroyed and the cost of the decoy and the cost of a single use as the consumption cost. The model is calculated and analyzed, and the number of decoys that conform to the actual situation is obtained.

文摘Spatial transcriptomics technology provides novel insights into the spatial organization of gene expression during embryonic development.In this study,we propose a method that integrates analysis across both temporal and spatial dimensions to investigate spatial transcriptomics data from mouse embryos at different developmental stages.We quantified the spatial expression pattern of each gene at various stages by calculating its Moran’s I.Furthermore,by employing time-series clustering to identify dynamic co-expression modules,we identified several developmentally stage-specific regulatory gene modules.A key finding was the presence of distinct,stage-specific gene network modules across different developmental periods:Early modules focused on morphogenesis,mid-stage on organ development,and late-stage on neural and tissue maturation.Functional enrichment analysis further confirmed the core biological functions of each module.The dynamic,spatially-resolved gene expression model constructed in this study not only provides new biological insights into the programmed spatiotemporal reorganization of gene regulatory networks during embryonic development but also presents an effective approach for analyzing complex spatiotemporal omics data.This work provides a new perspective for understanding developmental biology,regenerative medicine,and related fields.
文摘Objective: To explore the application effect of flipped classroom combined with problem-based learning teaching method in clinical skills teaching of standardized training for resident doctors of traditional Chinese Medicine. Methods: The study used the experimental control method. The study lasted from September to November 2022. The subjects of this study were 49 students of standardized training for resident doctors of traditional Chinese Medicine from grades 2020, 2021 and 2022 of Dazhou integrated TCM & Western Medicine Hospital. They were randomly divided into experiment group (25) and control group (24). The experiment group adopted flipped classroom combined with problem-based learning teaching method, and the control group adopted traditional teaching method. The teaching content was 4 basic clinical skill projects, including four diagnoses of traditional Chinese Medicine, cardiopulmonary resuscitation, dressing change procedure, acupuncture and massage. The evaluation method was carried out by comparing the students’ performance and a self-designed questionnaire was used to investigate the students’ evaluation of the teaching method. Results: The test scores of total scores in the experimental group (90.12 ± 5.89) were all higher than those in the control group (81.47 ± 7.96) (t = 4.53, P P Conclusions: The teaching process of the flipped classroom combined with problem-based learning teaching method is conducive to improving the efficiency of classroom teaching, cultivating students’ self-learning ability, and enhancing students’ willingness to learn.

文摘Background:Ferroptosis is a type of regulated cell death characterized by iron-dependent lipid peroxidation,which has been linked to tumor progression and therapeutic resistance.However,the contribution of lactate metabolism and its receptor,hydroxycarboxylic acid receptor 1(HCAR1),in ferroptosis regulation in gastric cancer(GC)remains poorly understood.Focusing specifically on its effects on cell proliferation,ferroptosis regulation,and the disruption of lactate-mediatedmetabolic pathways,the study aimed to clarify the role ofHCAR1 in GC progression.Methods:Bioinformatics analysis identified prognostic genes associated with ferroptosis in GC.Receiver operating characteristic(ROC)curves were generated to assess the diagnostic potential of the predictive genes.The biological role of HCAR1 was investigated through gain and loss-of-function experiments in GC cell lines,followed by assessments of cell viability,oxidative stress indicators,gene/protein expression,and ferroptosis sensitivity under lactate stimulation or HCAR1modulation.Results:HCAR1 was significantly upregulated in GC tissues and linked to poor patient outcomes.Silencing HCAR1 inhibited GC cell growth and induced ferroptosis,as shown by increased levels of reactive oxygen species(ROS)and malondialdehyde(MDA),along with decreased expression of solute carrier family 7 member 11(SLC7A11)and glutathione peroxidase 4(GPX4).Conversely,HCAR1 overexpression or exposure to extracellular lactate inhibited ferroptosis and activated antioxidant defenses.Mechanistically,lactate activation of HCAR1 increases ATP levels,which in turn inactivates AMP-activated protein kinase(AMPK).It also upregulates stearoyl-CoA desaturase 1(SCD1)through the sterol regulatory element binding protein 1(SREBP1)signaling pathway.Blocking HCAR1 reversed these effects and restored ferroptosis sensitivity.Conclusion:HCAR1 mediates lactate-driven ferroptosis resistance in GC through the AMPK-SCD1 signaling pathway.Targeting the HCAR1-lactate axis may offer a promising strategy for overcoming metabolic adaptation and improving GC treatment outcomes.

文摘BACKGROUND Single-port laparoscopic appendectomy is an advanced minimally invasive surgery that involves the use of upgraded instruments and equipment.We previously modified single-port laparoscopic appendectomy with needle-type grasping forceps(mSLAN)for patients with simple appendicitis,but the feasibility and safety of our modified procedure need further evaluation in a highquality clinical study.AIM To compare the short-term clinical outcomes of mSLAN with those of conventional three-port laparoscopic appendectomy(CLA)for patients with acute uncomplicated appendicitis.METHODS This single-center,single-blind,prospective,randomized controlled trial included patients who underwent emergency laparoscopic appendectomy for acute uncomplicated appendicitis at our center between April 2024 and August 2024.Patients were randomly divided into the mSLAN group or the CLA group via computergenerated randomization.The primary endpoint was the 24-hour postoperative visual analog scale(VAS)score,and the secondary endpoints included the operative time,24-hour postoperative inflammatory response biomarkers(including white blood cells,the neutrophil ratio,interleukin-6,and C-reactive protein),time to first postoperative exhaust,time to first out-of-bed activity,postoperative length of hospital stay,cost of hospitalization,and incidence of postoperative complications.RESULTS A total of 72 patients were enrolled and randomly divided into 2 groups:The mSLAN group(n=36)and the CLA group(n=36).The 24-hour VAS scores,24-hour postoperative inflammatory response marker levels,first postoperative exhaust times,first out-of-bed activity times,postoperative lengths of hospital stay,operative times,or hospitalization costs did not significantly differ between the two groups.No postoperative complications,including incision infection or hernia,abdominal abscess or intestinal obstruction,were observed during the 1-month postoperative follow-up in either group.CONCLUSION Compared with the CLA protocol,the mSLAN protocol for acute uncomplicated appendicitis yielded comparable short-term clinical outcomes,with a similar operative time and better cosmetic outcomes,indicating its potential for clinical application and superiority for patients with high cosmetic requirements.Further research is needed to evaluate the long-term outcomes.

文摘Objective:Immune checkpoint inhibitors(ICIs)have demonstrated substantial efficacy in patients with advanced non-small cell lung cancer(NSCLC).However,the optimal duration of ICI therapy remains unclear,and limited real-world data are available.The aim of this study was to evaluate the relationship between ICI therapy duration and overall survival(OS)in patients who achieved varying best overall response(BOR)during ICI treatment,and to compare patients treated for 6 to 18 months vs.at least 18 months.Methods:This retrospective cohort study included adult patients diagnosed with advanced NSCLC who received ICI therapy at the Zhejiang Cancer Hospital between 2017 and 2022.Data collection ended on May 1,2024,and statistical analysis was performed between May and June 2024.Results:Using strict entry criteria,we screened 487 patients with advanced NSCLC and identified 134 eligible patients.Among these patients,the median durations of immunotherapy and follow-up were 24.57 months and 43.60 months,respectively.The objective response rate(ORR)was 58.2%,and the median progression-free survival(PFS)was 10.6 months.Median OS was not reached.At the last follow-up,54 patients had no disease progression,and 118 patients remained alive.Patients treated with ICI therapy for≥18 months had superior survival to those treated for 6 to 18 months(P=0.039).Further analysis revealed that the survival benefit was associated with BOR during ICI therapy.Specifically,patients achieving complete response/partial response(CR/PR)who received≥18 months of ICI therapy had a trend toward longer median OS than those treated for 6 to 18 months,but the difference did not reach statistical significance(P=0.177).Patients with stable disease(SD)who received≥18 months of ICI therapy had a statistically longer median OS than those treated for 6 to 18 months(P=0.019).Among patients treated with ICIs for≥18 months,24 continued with ICI-based therapy and achieved a median PFS2 of 6.67 months,an ORR of 33.3%,and a disease control rate(DCR)of 83.3%.Conclusions:This study provides real-world evidence and novel insights into the need for continuing ICI therapy beyond 18 months in patients with advanced NSCLC who do not exhibit progressive disease.For patients achieving SD during ICI therapy,a treatment duration of at least 18 months appears appropriate.For patients achieving CR/PR,treatment decisions should be individualized according to patient-specific circumstances.However,owing to the retrospective study design,potential selection bias and confounding factors might have influenced the results.Therefore,our findings require further validation in prospective clinical studies.
文摘Postoperative depression is a prevalent yet often overlooked complication following radical surgery for gastric cancer that significantly affects patient recovery and quality of life.Emerging evidence suggests that the gut microbiota has a crucial role in the gut-brain axis,influencing mood regulation and psychological resilience.In the study by Lu et al,Bifidobacterium triple viable bacteria combined with mirtazapine demonstrated marked efficacy:The observation group showed a 32%reduction in Self-Rating Depression Scale depression scores,significantly outperforming the control group.Neuroendocrine improvements included a 89%increase in dopamine and a 43%reduction in cortisol.Gut microbiota modulation was evident with Bifidobacterium colony counts rising by 22%,while Escherichia coli decreased by 28%.Quality of life scores improved by 39%,underscoring the therapy’s holistic benefits.These findings validate the synergistic potential of microbiota-targeted probiotics and psychotropic drugs.Early screening,multidisciplinary care,and personalized strategies integrating psychological support,dietary optimization,and lifestyle modifications are essential to enhance recovery and redefine postoperative management in gastric cancer patients.

文摘A larger counter-ion could change the environment of trigonal bipyramidal cobalt(Ⅱ)to some extent,diluting the cobalt-containing complex and slowing their magnetic relaxation.Considering this observation,we used a sulfur organic ligand NS_(3)^(tBu) to obtain two trigonal bipyramidal Co(Ⅱ)complexes,namely,[Co(NS_(3)^(tBu))Cl]Y,Y=PF_(6)^(-)(1),ClO_(4)^(-)(2).The resulting compound 1(with a larger counter-ion PF_(6)^(-))exhibits a longer relaxation time in comparison to compound 2 prepared with the smaller counter-ion ClO_(4)^(-),even though the presence of weak rhombicity decreases the energy barrier and speeds up the relaxation of the magnetization for the two compounds.Concurrently,we demonstrate that compound 1 has smaller effective energy barrier and displays slower magnetic relaxation rather than compound 2.A smaller dc magnetic field could almost suppress all the quantum tunneling of magnetization(QTM),direct and Raman processes in compound 1,but not in compound 2,which presents all the Orbach,QTM,direct and Raman processes.

文摘Background:Diabetic foot,a severe complication of diabetes,is characterized by chronic refractory wounds.Sanhuang Oil,a topical herbal formula,demonstrates significant therapeutic effects including antibacterial,anti-inflammatory,and immunomodulatory activities.However,its active constituents and mechanisms of action against diabetic foot remain to be elucidated.Methods:In this study,the chemical constituents of Sanhuang Oil were identified using UPLC-QE-Orbitrap-MS.Subsequently,the mechanism by which Sanhuang Oil promotes diabetic foot ulcer healing was predicted by integrating network pharmacology and molecular docking.Additionally,diabetic mouse model was established in ICR mice using a combination of a high-fat diet(HFD)and streptozotocin(STZ)chemical induction.A full-thickness skin defect was created on the dorsum of the mice.Wound healing and the healing rate were observed following Sanhuang Oil intervention.The mechanism underlying Sanhuang Oil’s promotion of diabetic ulcer healing was further investigated using transcriptomics and histopathological examination(H&E staining).Results:A total of 97 active ingredients were identified from Sanhuang Oil.Network pharmacology analysis predicted 543 common targets,and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis identified 203 relevant pathways.Molecular docking further confirmed high binding affinity(binding energy≤−5.0 kcal/mol)between specific active components in Sanhuang Oil(e.g.,coptisine,phellodendrine,baicalein)and key targets associated with diabetic foot ulcers(e.g.,EGFR,AKT1,STAT3).In vivo experimental results demonstrated that the wound healing rate was significantly higher in Sanhuang Oil-treated groups compared to the model group(P<0.001).HE staining revealed that the high-dose Sanhuang Oil group exhibited more pronounced epithelial tissue coverage over the wound,reduced inflammatory cell infiltration,and increased collagen deposition and fibroblast proliferation.transcriptomic analysis identified Pdk4,Ttn,Csrp3,Actn2,Myoz2,Tnnc2,Myod1,Myog,Myot,and Myf6 as key regulatory proteins involved in promoting wound healing.Conclusion:Sanhuang Oil promotes wound healing in diabetic ulcer mice,potentially by mitigating inflammation and regulating key targets such as Pdk4 to enhance fibroblast function.These findings provide novel insights into the multi-target,multi-pathway mechanism of Sanhuang Oil for treating diabetic foot ulcers.

文摘Objective: Tumor-associated macrophages(TAMs) exhibit heterogeneous properties including anti-tumorigenic and protumorigenic phenotypes. The rate-limiting enzyme in de novo serine biosynthesis, 3-phosphoglycerate dehydrogenase(PHGDH), has a well-established role in cellular metabolism, yet its specific role in macrophages remains unknown.Methods: Metabolomics assays were conducted to assess metabolite composition and dynamics in macrophages. Changes in polarization and immunosuppressive markers were validated with q RT-PCR. Bioinformatics was used to analyze immune cell subsets and associated metabolic pathways. Finally, Ch IP-q PCR and co-immunoprecipitation assays were performed to elucidate the downstream regulatory mechanisms of PHGDH.Results: Serine metabolism was found to be downregulated in TAMs in breast cancer. Functional studies revealed that PHGDH inhibition promotes an M2-like phenotype and immunosuppressive functions in macrophages. Furthermore, PHGDH was found to undergo nuclear translocation during macrophage polarization. Mechanistically, nuclear PHGDH was found to regulate GLUD1 and GLS2 transcription via interaction with the transcription factor STAT3. Rescue experiments demonstrated that glutamine supplementation and STAT3 inhibition reversed the effects of PHGDH on macrophage function.Conclusions: Our findings reveal a previously unrecognized non-canonical metabolic function of PHGDH, thus providing potential therapeutic targets in the tumor microenvironment for reversing malignant progression.

文摘The use of robotic swarms to study the properties of active matter is a common experimental approach. In such studies, robots are often required to possess capabilities in computation, storage, perception, and two-dimensional(2D)movement to execute predefined rules. Under these rules, the swarm can accomplish complex tasks, exhibit rich collective states, or demonstrate intriguing phase transition phenomena. However, this study demonstrates how a swarm of spin robots, which only respond to simple ambient light intensity, can be constructed into a collective system capable of performing practical swarm tasks such as phototactic motion, controllable folding, and object transport through weak coupling interactions between individuals. Furthermore, it is proven that this swarm exhibits strong system fault tolerance. This research aims to demonstrate that, beyond the common design of sophisticated individuals and excellent inter-individual interaction rules, appropriate structural and coupling designs can enable individuals without computational capabilities to generate complex collective behaviors and accomplish diverse swarm tasks through cooperation. This provides a research direction for experimental studies of active matter using robotic systems.

文摘Objective Tumour cells in a hypoxic state are more invasive,have stronger self-renewal capabilities,and are difficult to treat because of their ability to promote tumour recurrence and metastasis.The glycolysis inhibitor 3-bromopyruvic acid(3-BrPA)can completely inactivate glycolytic enzymes at extremely low drug concentrations,thereby exerting a strong inhibitory effect on the glucose energy metabolism of tumor cells.Therefore,we tested the inhibitory effect of 3-BrPA on hepatocellular carcinoma cells(HepG2)in vitro;then,we used the VX2 liver cancer model to study the antitumour effect of 3-BrPA combined with interventional embolization on liver cancer.Methods In vitro,a CCK-8 assay was used to detect the inhibitory effect of different concentrations of 3-BrPA on HepG2 cells,and light microscopy confirmed that the HepG2 cells were completely dead.Western blotting was used to detect the expression of key proteins involved in apoptosis.A total of 30 New Zealand white rabbits were used to establish a liver cancer model and were randomly divided into 3 groups 2 weeks after tumor establishment:the control group was perfused with saline in the hepatic artery;the transcatheter arterial embolization(TAE)group was given TAE;and the experimental group was perfused with 3-BrPA combined with TAE.The tumor-bearing rabbits were killed one week after surgery.The tumor volume and tumor necrosis ratio were calculated via the histopathological examination.Results In vitro,the inhibitory effect of 3-BrPA on HepG2 cells increased with increasing concentration.3-BrPA(100μmol/L)could induce the necrosis of HepG2 cells.Stimulation with 50μmol/L 3-BrPA could activate the tumor cell apoptosis pathway.3-BrPA combined with TAE treatment could significantly inhibit tumor growth and cause more complete tumor necrosis.Conclusion 3-BrPA not only has antitumour effects in vitro but can also significantly improve antitumour effects in the hypoxic microenvironment after embolization in vivo.

文摘The CO2reforming of CH4is studied over MgO‐promoted Ni catalysts,which were supported on alumina prepared from hydrotalcite.This presents an improved stability compared with non‐promoted catalysts.The introduction of the MgO promoter was achieved through the‘‘memory effect’’of the Ni‐Al hydrotalcite structure,and ICP‐MS confirmed that only0.42wt.%of Mg2+ions were added into the Ni‐Mg/Al catalyst.Although no differences in the Ni particle size and basicity strength were observed,the Ni‐Mg/Al catalyst showed a higher catalytic stability than the Ni/Al catalyst.A series of surface reaction experiments were used and showed that the addition of a MgO promoter with low concentration can promote CO2dissociation to form active surface oxygen arising from the formation of the Ni‐MgO interface sites.Therefore,the carbon‐resistance promotion by nature was suggested to contribute to an oxidative environment around Ni particles,which would increase the conversion of carbon residues from CH4cracking to yield CO on the Ni metal surface.?2018,Dalian Institute of Chemical Physics,Chinese Academy of Sciences.Published by Elsevier B.V.All rights reserved.

文摘Aiming at the problem that the consumption data of new ammunition is less and the demand is difficult to predict,combined with the law of ammunition consumption under different damage grades,a Bayesian inference method for ammunition demand based on Gompertz distribution is proposed.The Bayesian inference model based on Gompertz distribution is constructed,and the system contribution degree is introduced to determine the weight of the multi-source information.In the case where the prior distribution is known and the distribution of the field data is unknown,the consistency test is performed on the prior information,and the consistency test problem is transformed into the goodness of the fit test problem.Then the Bayesian inference is solved by the Markov chain-Monte Carlo(MCMC)method,and the ammunition demand under different damage grades is gained.The example verifies the accuracy of this method and solves the problem of ammunition demand prediction in the case of insufficient samples.

文摘Background:When lowlanders rapidly ascend to altitudes>2500 m,they may develop acute mountain sickness(AMS).The individual susceptibility,ascending velocity,time spent at altitude,activity levels and altitude reached are considered risk factors for AMS.However,it is not clear whether sex is a risk factor.The results have been inconclusive.We conducted a meta-analysis to test whether there were sex-based differences in the prevalence of AMS using Lake Louise Scoring System.Methods:Systematic searches were performed in August 2019 in EMBASE,PubMed,and Web of Science for prospective studies with AMS data for men and women.The titles and abstracts were independently checked in the primary screening step,and the selected full-text articles were independently assessed in the secondary screening step by the two authors(YPH and JLW)based on pre-defined inclusion criteria.The meta-analysis was performed using by the STATA 14.1 software program.A random-effects model was employed.Results:Eighteen eligible prospective studies were included.A total of 7669 participants(2639[34.4%]women)were tested.The results showed that there was a statistically significant higher prevalence rate of AMS in women than in men(RR=1.24,95%CI 1.09–1.41),regardless of age or race.However,the heterogeneity was significant in the analysis(Tau2=0.0403,Chi2=50.15,df=17;I2=66.1%,P=0.000),it was main caused by different numbers of subjects among the studies(coefficient=–2.17,P=0.049).Besides,the results showed that there was no evidence of significant publication bias in the combined studies on the basis of Egger’s test(bias coefficient=1.48,P=0.052)and Begg’s test(P=0.130).Conclusions:According to this study,the statistically significant finding emerging from this study was that women have a higher prevalence of AMS.However,the authors could not exclude studies where patients were on acetazolamide.Our analysis provided a direction for future studies of the relationship of sex and the risk of AMS,such as the pathological mechanism and prevention research.
