Integration of Problem-Based Learning and Case-Based Learning in Chinese Endodontics Standard Resident Training

As the most critical part of post-graduate education,the Chinese government launched Standard Resident Training in 2013 to solve the regional inequality of medical quality and meet the increasing social requirement for better medical service.We integrated problem-based learning(PBL)and case-based learning(CBL)in the Endodontics Standard Resident Training.By evaluating with objective parameters including theoretical knowledge and clinical practice skill,and subjective parameters including questionnaire,it was found that PBL+CBL played a positive role in endodontic resident training with a significant difference(P<0.05).This combined training model is instructive for China’s resident training,and this result can provide a rudimentary reference to current postgraduate teaching reform. 展开更多

Background The Registrar Clinical Encounters in Training(ReCEnT)project is an Australian general practice vocational training programme with integrated and interdependent education and research functions.Trainees(registrars)contemporaneously document in-consultation clinical experience and actions.Objectives Using a realist lens,we elucidate the mechanisms underpinning project outcomes to answer questions around programme effectiveness,impacts,sustainability and the lessons and findings that are translatable to other primary care training programmes.Methods The context,input,process and product framework was used.As a means to understand the interactions between each of the interdependent components,it allows for inferences regarding causal mechanisms for specific outcomes.Results Context:ReCEnT occurs within an apprenticeship-like model of general practice vocational training entailing a central supervisor/apprentice relationship.ReCEnT has demystified the content and characteristics of registrar consultations.Input:multiple stakeholder involvement is both advantageous and a logistical challenge,with the programme’s success dependent on registrars,practices and training providers providing detailed and accurate data,with prompt subsequent processing.Process:contemporaneous consultation data collection in different stages of training constitutes a component of registrars’programmatic assessment.Product:individualised feedback provides educational benefit through reflection.Clinical and educational research questions can be addressed with resulting research translation feeding back into the programme model and government policy.Clinical behaviour change is also evaluated.Conclusion ReCEnT is unique,globally,in its scope and longevity(2010–present).Creation of meaningful,individualised feedback facilitates reflection and provides both immediate educational benefits and the substrate for further research,programme and policy design and targeted formal teaching and learning. 展开更多

This paper studies a mirror-assisted rehabilitation training method based on a dual-arm robot,which aims to provide an effective rehabilitation training program for patients with upper limb dysfunction due to stroke or other causes.During the mirror training task scenario,the subjects are visually guided to perform the mirror movement of both arms,and the dual-arm robot is used to facilitate the mirror-assisted rehabilitation from the healthy side to the affected side.Adaptive imped-ance control and force field channel design ensure the stability and safety of the rehabilitation process.In the rehabilitation training,appropriate assistance forces are provided within the channel to correct trajectory deviations,ensuring that the subjects'movement path aligns with the predetermined trajectory.Outside the channel,the superposition of stiffness and correction force fields prevents the subjects from deviating from the predetermined trajectory,thus avoiding injuries.In addition,the adaptive impedance control is capable of dynamically adjusting the impedance parameters according to the real-time state of the subjects,providing a personalized rehabilitation training program.This method significantly enhances both the safety and effectiveness of the rehabilitation training.The experimental results showed that the subjects'motion flexibility and safety were significantly improved during the mirror-assisted rehabilitation training.This study offers a new approach for the future development of rehabilitation robotics with broad application potential. 展开更多

Objective: To develop a kind self active resistance movement training equipment for cervical spine, so as to restore the dynamic balance of the front and back muscle strength of the cervical spine in order to correct the abnormal physiological radian of the cervical spine, thereby relieving the clinical symptoms. Methods: The air spring with adjustable strength was used to provide the power of resistance active movement, and the neurological training cervical audio-visual synthesis training software was used to automatically guide patients to perform resistance active movement training of the cervical spine. Thirty two patients with cervical spondylosis were enrolled for treatment, and SPSS26 was used to statistically analyze the VAS, Lovett muscle strength, head and neck range of motion and C2-3 vertebral body curvature before and after the treatment. Result: There were significant differences in posterior cervical muscle strength (Lovett), dizziness, shoulder and neck pain (VAS) and C2-3 vertebral curvature before and after training in 32 patients (p˂0.001). Conclusion: The application of neurological training cervical vertebra self rehabilitation training instrument can effectively relieve clinical symptoms, restore head and neck range of motion, improve nape muscle strength, restore the dynamic balance of front and rear cervical muscle strength, and also can correct the abnormal physiological radian of the cervical vertebra. 展开更多

The article presents wide experience in controlling athletic training aimed at preparation of the Russian national teams for major international competitions. Experimental part of the study has been carried out in the course of training camps and competitions since 2010. Forty Russian national teams in 15 winter and 3 summer Olympic sports participated in the research. Theoretical concepts set forth in the works of Anthony Stafford Beer made a basis of a system of control and management developed for the Russian national winter sports teams. In particular, a five-level system was applied to control athletic training. The authors point out that a system of control over athletic training in elite sport teams should take into account a few important issues: (1) each sport, team and athlete has distinctive features; (2) various aspects of athletic training and their interrelation must be aimed at raising effectiveness of training according to individual training plans; (3) coaches and sport managers must be provided with real-time information necessary for development of long-term and short-term training plans and their timely correction. 展开更多

Non-enzymatic antioxidant blood plasma profile in the period of high training loads of elite speed skaters in the altitude

At the altitude,hypoxia and training load are key factors in the development of oxidative stress.Altitude-induced oxidative stress is developed due to the depletion of antioxidant potential.In the current study,we examined the non-enzymatic antioxidant profile of blood plasma in 7 males and 5 females specializing in speed skating at a 21-day training camp at 1850m above sea level.Training included:cycling,roller skating,ice skating,strength training,and special training.At the start point and the endpoint,total hemoglobin mass(tHb-mass),hemoglobin concentration,and circulating blood volume were determined.Antioxidant profiles,hypoxic doses,hypoxic impulses,and training impulses were assessed at 3,6,10,14,and 18 days.Antioxidant profiles consisting of“urate”and“thiol”parts were registered with chemiluminometry.In the training dynamics,antioxidant parameters changed individually,but in total there was a decrease in the“urate”capacity by a factor of 1.6(p=0.001)and an increase in the“thiol”capacity by a factor of 1.8(p=0.013).The changes in“urate”capacity positively correlated(r_(S)=0.40)and the changes in“thiol”capacity negatively correlated(r_(S)=−0.45)with changes in tHb-mass.Both exercise and hypoxic factors affect the antioxidant parameters bidirectionally.They correlated with a decrease in thiol capacity and with an increase in urate capacity.The assessment of the non-enzymatic antioxidant profile can be a simple and useful addition to screening the reactive oxygen species homeostasis and can help choose the personalized training schedule,individualize recovery and ergogenic support. 展开更多

In shipping,which is one of the drivers of the world’s economy,many marine accidents continue to occur,such as ship collisions and grounding.To reduce marine collision accidents,seafarers’skills must be improved through training.Therefore,the authors propose a ship handling training for collision avoidance(hereinafter referred to as“T for CA”)in which a group of several people discusses the ship handling for collision avoidance,assuming the situation of the collision avoidance.After T for CA implementation,anchoring training was done and the effect of T for CA was verified through comparison with a group where T for CA was not applied.Two instructors evaluated the anchoring training conducted with and without“T for CA”.The anchoring training experiment showed a difference of 27.5%in the achievement rate between the proposed training and previous training.T for CA maximises the effects of group work and resulted in good evaluations in the anchoring training experiments.The training was effective because the students themselves set the scenarios and devised ship handling strategies for collision avoidance.In addition,group work discussions helped deepen students’knowledge and skills. 展开更多

Memory Training and Task Specificity in 90-99-Year-Old Seniors with Mild Cognitive Impairment 被引量:3

Managing memory deficits is a central problem among older adults with mild cognitive impairment (MCI). This study examined the effects of memory training on memory performance in an understudied “oldest-old” population ranging in age from 90 to 99 years. Eighteen mild to moderately cognitive-impaired older seniors, 90 years and older were recruited from memory clinics established in senior living communities. Treatment sessions took place, on average, twice weekly, for 55 minutes. Memory intervention included nineteen computer-based exercises customized to focus on memory loss. The specificity of memory training was very clear;memory training produced significant effects (F(3,51) = 2.81, p = 0.05) on memory performance, especially after 6 months of training, while other outcome measures showed no effects as predicted. Based on the results, it can be concluded that interventions targeting cognition and memory in the oldest-old MCI population can significantly improve memory function and reduce cognitive deficits. 展开更多

Objective: To explore the application effect of flipped classroom combined with problem-based learning teaching method in clinical skills teaching of standardized training for resident doctors of traditional Chinese Medicine. Methods: The study used the experimental control method. The study lasted from September to November 2022. The subjects of this study were 49 students of standardized training for resident doctors of traditional Chinese Medicine from grades 2020, 2021 and 2022 of Dazhou integrated TCM & Western Medicine Hospital. They were randomly divided into experiment group (25) and control group (24). The experiment group adopted flipped classroom combined with problem-based learning teaching method, and the control group adopted traditional teaching method. The teaching content was 4 basic clinical skill projects, including four diagnoses of traditional Chinese Medicine, cardiopulmonary resuscitation, dressing change procedure, acupuncture and massage. The evaluation method was carried out by comparing the students’ performance and a self-designed questionnaire was used to investigate the students’ evaluation of the teaching method. Results: The test scores of total scores in the experimental group (90.12 ± 5.89) were all higher than those in the control group (81.47 ± 7.96) (t = 4.53, P P Conclusions: The teaching process of the flipped classroom combined with problem-based learning teaching method is conducive to improving the efficiency of classroom teaching, cultivating students’ self-learning ability, and enhancing students’ willingness to learn. 展开更多

Background:There is a growing body of experimental evidence examining the effects of plyometric jump training(PJT)on physical fitness attributes in basketball players;however,this evidence has not yet been comprehensively and systematically aggregated.Therefore,our objective was to meta-analyze the effects of PJT on physical fitness attributes in basketball players,in comparison to a control condition.Methods:A systematic literature search was conducted in the databases PubMed,Web of Science,and Scopus,up to July 2020.Peer-reviewed controlled trials with baseline and follow-up measurements investigating the effects of PJT on physical fitness attributes(muscle power,i.e.,jumping performance,linear sprint speed,change-of-direction speed,balance,and muscle strength)in basketball players,with no restrictions on their playing level,sex,or age.Hedge’s g effect sizes(ES)were calculated for physical fitness variables.Using a random-effects model,potential sources of heterogeneity were selected,including subgroup analyses(age,sex,body mass,and height)and single training factor analysis(program duration,training frequency,and total number of training sessions).Computation of metaregression was also performed.Results:Thirty-two studies were included,involving 818 total basketball players.Significant(p<0.05)small-to-large effects of PJT were evident on vertical jump power(ES=0.45),countermovement jump height with(ES=1.24)and without arm swing(ES=0.88),squat jump height(ES=0.80),drop jump height(ES=0.53),horizontal jump distance(ES=0.65),linear sprint time across distances≤10 m(ES=1.67)and>10 m(ES=0.92),change-of-direction performance time across distances≤40 m(ES=1.15)and>40 m(ES=1.02),dynamic(ES=1.16)and static balance(ES=1.48),and maximal strength(ES=0.57).The meta-regression revealed that training duration,training frequency,and total number of sessions completed did not predict the effects of PJT on physical fitness attributes.Subgroup analysis indicated greater improvements in older compared to younger players in horizontal jump distance(>17.15 years,ES=2.11;≤17.15 years,ES=0.10;p<0.001),linear sprint time>10 m(>16.3 years,ES=1.83;≤16.3 years,ES=0.36;p=0.010),and change-of-direction performance time≤40 m(>16.3 years,ES=1.65;≤16.3 years,ES=0.75;p=0.005).Greater increases in horizontal jump distance were apparent with>2 compared with≤2 weekly PJT sessions(ES=2.12 and ES=0.39,respectively;p<0.001).Conclusion:Data from 32 studies(28 of which demonstrate moderate-to-high methodological quality)indicate PJT improves muscle power,linear sprint speed,change-of-direction speed,balance,and muscle strength in basketball players independent of sex,age,or PJT program variables.However,the beneficial effects of PJT as measured by horizontal jump distance,linear sprint time>10 m,and change-of-direction performance time≤40 m,appear to be more evident among older basketball players. 展开更多

AIM: To evaluate if canine models are appropriate for teaching endoscopy fellows the techniques of endoscopic submucosal dissection (ESD). METHODS: ESD was performed in 10 canine models under general anesthesia, on artificial lesions of the esophagus or stomach marked with coagulation points. After ESD, each canine model was euthanized and surgical resection of the esophagus or stomach was carried out according to "The Principles of Humane Experimental Technique, Russel and Burch". The ESD specimens were fixed with needles on cork submerged in a formol solution with the esophagus or stomach, and delivered to the pathology department to be analyzed. RESULTS: ESD was completed without complications using the Hook-knife in five esophageal areas, with a procedural duration of 124 ± 19 min, a length of 27.4 ± 2.6 mm and a width of 21 ± 2.4 mm. ESD was also completed without complications using the IT-knife2 in five gastric areas, with a procedural duration of 92.6 ± 19 min, a length of 32 ± 2.5 mm and a width of 18 ± 3.7 mm. CONCLUSION: ESD is feasible in the normal esopha- gus and stomach of canine models, which are appropriate for teaching this technique. 展开更多

The development, planning, and implementation of such a programme are difficult due partly to the practical constraints related to the competitive schedule at the top level. The effective planning and organisation of training are therefore crucial to the effective delivery of the training stimulus for both individual players and the team. The aim of this article is to provide an overview of the principles of training that can be used to prepare players for the physical demands of soccer. Information relating to periodisation is supported by an outline of the strategies used to deliver the acute training stress in a soccer environment. Plyometrics training,can improve the club speed,ball speed and the hitting distance in swinging and hitting.According to the experimental method,15 professional golfers were randomly selected as the reference object,and 24 golfers joined the 8 weeks of plyometrics training.The conclusion was that after 8 weeks of plyometrics training,the experimental group had an increase of the club speed,ball speed and smash factor more obviously than that of the control group.Affected by training years,there is still a gap between the two and professional players.With the increase of club speed and ball speed,both groups have a certain increase in the carry length and total length.Compared to the control group,the experimental group has further hitting distance,but there are still significant differences between them affected by training years.It is suggested that during the teaching and training of golf techniques,practice of the stability of the lower limbs and the instability of the upper limbs should be increased,to improve the hitting distance. 展开更多

Since Harbin Welding Training Institute(WTI Harbin) was founded based on Sino-German cooperation in 1984, it has trained all kinds of welding personnel 20000 and it made important contribution to the up-to-date system introduction of international welding training to China. The common questions in Germany foreign aid programs are: when German experts were on site, the project was on the operation; when he/they left, only memory was left. AIM:To evaluate the effect of hands-on training of gastroenterology fellows in gastric polypectomy using an ex vivo simulator.METHODS:Eight gastroenterology fellows at Mackay Memorial Hospital,Taipei were evaluated in gastricpolypectomy techniques using a pig stomach with artificial polyps created by a rubber band ligation device.The performance of four second year(year-2)fellows who had undergone one year of clinical training was compared with that of four f irst year(year-1)fellows both before and after a 4-h workshop using the ex vivo simulator.The workshop allowed for hands-on train-ing in the removal of multiple artif icial polyps and the placement of hemoclips at the excision site.Evaluation included observation of technical skills,procedure time,and the fellows' conf idence scale.RESULTS:One week after the workshop,the year-1 fellows were re-evaluated and had significantly im-proved mean performance scores(from 17.9 ± 1.8 to 22.5 ± 0.7),conf idence scale(from 4.5 ± 1.0 to 7.8 ± 0.5)and procedure time(from 615.0 ± 57.4 s to 357.5 ± 85.0 s)compared with their baseline performance.After 4 h of training using the ex vivo simulator,the skills of the year-1 fellows were statistically similar to those of the year-2 fellows.CONCLUSION:Use of this ex vivo simulator significantly improved the endoscopic gastric polypectomy skills of gastroenterology fellows who had not had previous clinical training in gastric polypectomy. 展开更多
