This study aimed to investigate the energy supply of athletes,who had a mild form of coronavirus infection(COVID-19)under cycling ergometric load(CEL)to substantiate the timing of recovery,as well as to determine the ...This study aimed to investigate the energy supply of athletes,who had a mild form of coronavirus infection(COVID-19)under cycling ergometric load(CEL)to substantiate the timing of recovery,as well as to determine the volume and intensity of physical activity.Eighty-seven athletes aged from 18 to 28 years old,involved in cyclic sports,were examined.Group I consisted of 52 athletes with COVID-19,and group Ⅱ consisted of 37 healthy self-isolating athletes.In addition to the comprehensive examination,the tested athletes underwent special examinations:the study of diagnostic material using methods of nucleic acid amplification,spirography and spiroergometry,electrocardiography(ECG),and CEL.The results of athletes from both groups did not differ significantly(p>0.5)in the 1^(st),2^(nd),and 3^(rd) examinations.The second examination revealed a discrepancy between functional reserves and the load performed,as evidenced by the difference in the recovery rate of most indicators,the results of the third examination in the long-term period(6 months)showed that the athletes of the first group did not have any violations of the parameters of the respiratory function(RF)and cardiovascular system at rest,after performing CEL,as well as in the recovery period.The results suggest that the full resumption of training loads and participation in competitions are possible only with the complete normalization of the functional state of the cardiorespiratory system.The most informative indicators are:minute ventilation(V E),heart rate(HR),oxygen pulse(OP),and coefficient of oxygen utilization(COU).展开更多

At the base of the pathogenesis of portal hypertension is the increase in hepatic vascular resistance to portal blood flow with subsequent development of hyperdynamic circulation, which, despite of the formation of collateral circulation, promotes progression of portal hypertension. An important role in its pathogenesis is played by the rearrangement of vascular bed and angiogenesis. As a result, strategic directions of the therapy of portal hypertension under liver cirrhosis include selectively decreasing hepatic vascular resistance with preserving or increasing portal blood flow, and correcting hyperdynamic circulation and pathological angiogenesis, while striving to reduce the hepatic venous pressure gradient to less than 12 mm Hg or 20% of the baseline. Over the last years, substantial progress in understanding the pathophysiological mechanisms of hemodynamic disorders under liver cirrhosis has resulted in the development of new drugs for their correction. Although the majority of them have so far been investigated only in animal experiments, as well as at the molecular and cellular level, it might be expected that the introduction of the new methods in clinical practice will increase the efficacy of the conservative approach to the prophylaxis and treatment of portal hypertension complications. The purpose of the review is to describe the known methods of portal hypertension pharmacotherapy and discuss the drugs that may affect the basic pathogenetic mechanisms of its development.展开更多

This st...Background: Postprandial hyperglycemia is an independent risk factor for diabetes-associated complications in individuals with type 2 diabetes. Dietary modification plays an important role in glycemic control. This study was to examine the efficacy of a diabetes specific formula (DSF) during a 4-hour postprandial meal tolerance test (MTT) in Russian subjects with type 2 diabetes receiving oral hypoglycemic medication. Methods: In a randomized, cross-over design, 168 eligible subjects from 11 study centers consumed, in a random order, the DSF (230 mL) or a common light hospital breakfast (i.e. standard meal) on two different occasions. The amounts of macronutrients were similar between the two meals providing ~200 kcal, 11 g protein, 26 g carbohydrate and 8 g fat. Capillary glucose levels were measured at baseline (before meal consumption), and post-meal consumption at 30, 60, 90, 120, 180 and 240 min. Results: The DSF was well tolerated in all subjects. There were 111 subjects completed the study per protocol (mean ± SEM: age: 58.6 ± 0.8 yr, BMI: 31.8 ± 0.42 kg/m2, waist circumference: 101 ± 1.3 cm, HbA1c: 8.0% ± 0.1%). Glucose levels reached peak values at 60 min (median) and the lowest levels at the end of the 240-min MTT test. The mean positive area under the curve (PAUC), the primary outcome, was significantly smaller after DSF consumption (mean ± SEM: 183.02 ± 18.74, median: 132.55) than the PAUC after consumption of the standard meal (mean ± SEM: 239.95 ± 23.11, median: 166.89;p = 0.027). The actual and adjusted peak glucose concentrations were similar between the two treatments. Conclusions: In patients with type 2 diabetes receiving oral hypoglycemic agents, compared to a hospital meal, the DSF improves postprandial glucose control. Combining results from earlier studies, long-term use of DSF may be beneficial to improve glucose management and decrease diabetes-associated complications.展开更多

Analytical expressions for wave function and energy spectrum are obtained. It is shown that at small values of the stretch angle of spherical sector the problem is reduced to the conical QD problem. Results: A study was conducted aimed at identifying the socio-psychological causes of conflicts and methods for their resolution in the organization of health care using the example of medical personnel. Conclusions: Conflict is an integral part of the functioning of any organization. A decrease of systolic and diastolic components of arterial pressure and pulse, and a normalization of systolic velocity of bloodstream (CVB) and of resistance index (RI) in orbital arteries in patients with IMD are noticed. This is one of the most important factors of the effective work of the institution, the full realization of the potential capabilities of people in the team. Leadership style, as an individual way of carrying out management activities, is mostly associated with the person of the manager. The management style is formed under the influence of the relationship between the manager and the team in the process of making and implementing management decisions, individual characteristics and preferences of the manager. The objective of the study was conflicts in the field of healthcare in the system “medical personnel of a healthcare institution—patients and relatives of patients”. The objective of the study was to determine management styles in conflict situations among medical workers. Materials and Methods: With the help of a direct individual survey, according to an anonymous questionnaire developed by us, the opinion of 582 medical workers (422 doctors and 160 nurses) of general hospitals and primary care centers of Chernivtsi and Chernivtsi region regarding the ceilings of behavior in conflict situations was studied. Results: In a conflict situation, the leading form of behavior for medics, both doctors and medical personnel, is the subordinate type (29.6% and 38.1%, respectively). The second most frequent method of conflict resolution is an authoritarian strategy (20.6% for doctors and 15% for nurses), in which personal aspirations and the achievement of one’s own goals come to the fore. Dependent style (17.8% for doctors and 13.75% for nurses) ranks third. A selfish style of behavior in a conflict situation is characteristic of 13.7% of doctors and 13.1% of nurses. A friendly style of behavior during the resolution of conflict situations is characteristic of medical workers at a rather low level (12.3% for doctors and 12.5% for nurses). Aggressive style (5.9% for doctors and 7.5% for nurses) is the least popular as a model of behavior in a conflict situation among respondents. Conclusion: The style of behavior of doctors and nurses in conflict situations is ambiguous, which is due to personal qualities, the degree of leadership, professional relationships and the specifics of the work environment.展开更多

Of the six major HCV genotypes (GT), the most predominant one is GT1, worldwide. The prevalence of HCV in Central Asia, which includes most of the Commonwealth of Independent States (CIS), has been estimated to be 5.8% of the total global burden. The predominant genotype in the CIS and Ukraine regions has been reported to be GT1, followed by GT3. Inadequate HCV epidemiological data, multiple socio-economic barriers, and the lack of regionspecific guidelines have impeded the optimal management of HCV infection in this region. In this regard, a panel of regional experts in the field of hepatology convened to discuss and provide recommendations on the diagnosis, treatment, and pre-, on-, and posttreatment assessment of chronic HCV infection and to ensure the optimal use of cost-effective antiviral regimens in the region. A comprehensive evaluation of the literature along with expert recommendations for the management of GT1-GT6 HCV infection with the antiviral agents available in the region has been provided in this review. This consensus document will help guide clinical decision-making during the management of HCV infection, further optimizing treatment outcomes in these regions.展开更多

In recent years, a progressive method of wavelet transformation of the electroencephalogram (EEG) has been developed, which enabled us to establish the fundamental possibility of direct objective registration of the human brain’s mental activity. Earlier, we created an experimental model and software for recognizing true and false mental responses of a person based on the EEG wavelet transformation and described it in the article. The developed experimental model and information software made it possible to compare the two mental states of brain activity by electroencephalographic indicators, one of which is false and the other is true. The goal is to develop a fundamentally new information technology for recognizing true and false states in the brain’s mental activity based on the wavelet transformation of the electroencephalogram. Results: It was revealed that the true and false states of the brain can be distinguished using the method of continuous wavelet transformation and calculation of the EEG wavelet energy. It is shown that the main differences between true and false mental responses are observed in the delta and alpha ranges of the EEG. In the EEG delta rhythm, the wavelet energy is reliably higher in case of a false answer compared to a true one. In the EEG alpha rhythm, the wavelet energy is significantly higher with a true answer than a false one. Practical significance of the research: The data obtained open up the fundamental possibility of identifying true and false mental states of the brain on the basis of continuous wavelet transformation and calculation of the EEG wavelet energy.展开更多

A well-known antitbrombotic supplement--acetylsalicylic acid has been added to improve hemocompatibility in the polymer. The results of our studies showed a controlled prolonged separation of acetylsalicylic acid from polymeric material in the blood. We studied in vitro the dynamics of liberation of acetylsalicylic acid from polymeric coatings. It was shown that the concentration of polyethylene glycol and the thickness of the polymer layer can affect the rate of diffusion of acetylsalicylic acid from polymer films.展开更多

The purpose of this study was the evaluation of efficacy of original oxidized form NAD-containing drug, NADCIN<sup>®</sup>, on hemodynamic reactions, baroreflex sensitivity (BRS) and reflex control of splanchnic sympathetic nerve activity (SSNA), level of redox-potential, activity of ALDH-2 and superoxide anion generation in aortic tissue in rat model of NTGT. Five groups (7 - 9 each) of male Wistar rats, including control, acute i.v. NTG (150 mcg/kg) administration, NTG tolerance NTGT treatment with NADCIN<sup>®</sup> 8 mg/kg and methylene blue (MB, 2.5 mg/kg) were used. NTGT in rats was accompanied with the greatly attenuation of hemodynamics reaction, BRS, the decreasing of the ability to reflex control of SSNA without pronounce overexpression of endothelin-1 in vessels (aorta). In NTGT rats i.v. NTG along induced less hypotensive reactions and alterations in heart period vs single NTG treated group, more expressively decreased BRS (-34%) and reflex control of SSNA (-18%). NADCIN<sup>®</sup> significantly inhibits tolerance-inducing properties of the prolonged nitroglycerin infusion (max decrease of blood pressure response to nitroglycerin injection, % of normal controls: NTGT 51.2%, NADCIN<sup>®</sup> 91.6%, MB 55.8%). NADCIN<sup>®</sup> in NTGT rats after NTG i.v. administration increased reduced BRS (+37.8%, p < 0,05), reflex control of SSNA (+29.4%, p < 0.05) and reversed the decreasing of NAD/NADH ratio, ALDH-2 activity and decreasing in superoxide generation in thoracic aortic tissue. Thus, course treatment with NADCIN<sup>®</sup> of NTGT rats restores hemodynamics changes, BRS and SSNA throughout the increasing of redox-potential NAD/NADH and cessates the NTGT developing.展开更多

Albumin, in reduced form (mercaptoalbumin, HMA), with antioxidant ability and alterations/deteriorations in the redox status of human serum albumin (HSA) under oxidative stress formation in infection diseases and its complications strongly modifies albumin antioxidant capacity. The aim of this study was the investigation of carbonyl/oxidative stress and pseudo-esterase activity of mercaptolbumin and oxidized HSA models. HSA (P. pastoris) purchased from MedChemExpress (USA) was used for study to model oxidative stress, HSA in reduced (intact) form was treated with H2O2, tert-butylhydroperoxide (t-BHP) and chloramine T (CT). The content of HSA-bound carbonyl groups decreased in under treatment with t-BPH- and CT-reduced HSA and more less extent in case of H2O2-treated. Fatty acid-free HSA and mercaptoalbumin (HMA) advanced oxidation protein products (AOPP) concentrations were significantly lower than in H2O2 loading reduced HSA by 123% and 235%, respectively. The total thiols level was lower in HMA + CT compared to reduced HMA by 51% and even increased after treatment of HMA with H2O2. Pseudo-esterase activity of HMA maintains >65% in the presence of hydroperoxide and occurs pronounced loss in the presence of chloramine T. Hydrogen peroxide at physiological concentration about 10 μM occurs less damage of reduced from of HSA then t-BPH and CT, and unlike t-BPH and CT, without significantly changes in pseudo-esterase activities.展开更多

BACKGROUND Breast cancer(BC)continues to occupy a leading position in terms of morbidity and mortality from malignant neoplasms among the female population.One of the promising markers associated with BC progression i...BACKGROUND Breast cancer(BC)continues to occupy a leading position in terms of morbidity and mortality from malignant neoplasms among the female population.One of the promising markers associated with BC progression is programmed death ligand 1(PD-L1).Previously,we investigated PD-L1 expression in BC via a new antibody against programmed cell death protein 1 ligand 1(PDCD1 LG1)and reported that high PDCD1 LG1 expression in tumor cells is an independent factor for a high risk of regional metastasis in patients with BC.However,the prognostic significance of PDCD1 LG1 expression in BC stromal cells has not been adequately studied.AIM To study the features of PDCD1 LG1 expression in BC stromal cells and its relationship with BC clinicopathological characteristics.METHODS In a prospective single-center observational study,tumor samples from 148 patients with newly diagnosed BC were examined.The tumor sections were immunohistochemically stained with antibodies against PDCD1 LG1.In the tumor samples,the PDCD1 LG1-positive lymphocyte(PDCD1 LG1+LF)score,presence of nuclear PDCD1 LG1 expression in the LFs,PDCD1 LG1 expression in polymorphic cell infiltrates(PDCD1 LG1+polymorphic cell infiltrates[PCIs]),and cells of the fibroblastic stroma and endothelial cells of the tumor microvessels were assessed.Statistical analyses were performed using Statistica 10.0 software.RESULTS A PDCD1 LG1+LF score≥3 was detected more often at stages N0 and N3 than at N1 and N2(P=0.03).Moderate and pronounced PDCD1 LG1+PCIs and the presence of PDCD1 LG1+fibroblastic stroma were associated with negative estrogen receptor status(P=0.0008 and P=0.03,respectively),human epidermal growth factor receptor 2-positive(HER2+)BC(P<0.00001 and P=0.0005),and luminal B HER2+,non-luminal HER2+and triple-negative BC(P<0.00001 and P=0.004).The risk of metastasis to regional lymph nodes(RLNs)depend on lymphovascular invasion(LVI)and the PDCD1 LG1+LF score.In the absence of LVI and a PDCD1 LG1+LF score<3 or≥3,metastases in RLNs were absent in 66.6%and 93.9%of patients with BC,respectively.In the presence of LVI and a PDCD1 LG1+LF score<3 or≥3,metastases in RLNs were detected in 82.6%and 92.7%of patients with BC,respectively.CONCLUSION The results indicated that the combined assessment of the PDCD1 LG1+LF score and LVI can improve the accuracy of predicting the risk of metastasis to RLNs in patients with BC.展开更多

文摘This study aimed to investigate the energy supply of athletes,who had a mild form of coronavirus infection(COVID-19)under cycling ergometric load(CEL)to substantiate the timing of recovery,as well as to determine the volume and intensity of physical activity.Eighty-seven athletes aged from 18 to 28 years old,involved in cyclic sports,were examined.Group I consisted of 52 athletes with COVID-19,and group Ⅱ consisted of 37 healthy self-isolating athletes.In addition to the comprehensive examination,the tested athletes underwent special examinations:the study of diagnostic material using methods of nucleic acid amplification,spirography and spiroergometry,electrocardiography(ECG),and CEL.The results of athletes from both groups did not differ significantly(p>0.5)in the 1^(st),2^(nd),and 3^(rd) examinations.The second examination revealed a discrepancy between functional reserves and the load performed,as evidenced by the difference in the recovery rate of most indicators,the results of the third examination in the long-term period(6 months)showed that the athletes of the first group did not have any violations of the parameters of the respiratory function(RF)and cardiovascular system at rest,after performing CEL,as well as in the recovery period.The results suggest that the full resumption of training loads and participation in competitions are possible only with the complete normalization of the functional state of the cardiorespiratory system.The most informative indicators are:minute ventilation(V E),heart rate(HR),oxygen pulse(OP),and coefficient of oxygen utilization(COU).

文摘Optogenetics has revolutionized the field of neuroscience by enabling precise control of neural activity through light-sensitive proteins known as opsins.This review article discusses the fundamental principles of optogenetics,including the activation of both excitatory and inhibitory opsins,as well as the development of optogenetic models that utilize recombinant viral vectors.A considerable portion of the article addresses the limitations of optogenetic tools and explores strategies to overcome these challenges.These strategies include the use of adeno-associated viruses,cell-specific promoters,modified opsins,and methodologies such as bioluminescent optogenetics.The application of viral recombinant vectors,particularly adeno-associated viruses,is emerging as a promising avenue for clinical use in delivering opsins to target cells.This trend indicates the potential for creating tools that offer greater flexibility and accuracy in opsin delivery.The adaptations of these viral vectors provide advantages in optogenetic studies by allowing for the restricted expression of opsins through cell-specific promoters and various viral serotypes.The article also examines different cellular targets for optogenetics,including neurons,astrocytes,microglia,and Schwann cells.Utilizing specific promoters for opsin expression in these cells is essential for achieving precise and efficient stimulation.Research has demonstrated that optogenetic stimulation of both neurons and glial cells-particularly the distinct phenotypes of microglia,astrocytes,and Schwann cells-can have therapeutic effects in neurological diseases.Glial cells are increasingly recognized as important targets for the treatment of these disorders.Furthermore,the article emphasizes the emerging field of bioluminescent optogenetics,which combines optogenetic principles with bioluminescent proteins to visualize and manipulate neural activity in real time.By integrating molecular genetics techniques with bioluminescence,researchers have developed methods to monitor neuronal activity efficiently and less invasively,enhancing our understanding of central nervous system function and the mechanisms of plasticity in neurological disorders beyond traditional neurobiological methods.Evidence has shown that optogenetic modulation can enhance motor axon regeneration,achieve complete sensory reinnervation,and accelerate the recovery of neuromuscular function.This approach also induces complex patterns of coordinated motor neuron activity and promotes neural reorganization.Optogenetic approaches hold immense potential for therapeutic interventions in the central nervous system.They enable precise control of neural circuits and may offer new treatments for neurological disorders,particularly spinal cord injuries,peripheral nerve injuries,and other neurodegenerative diseases.
文摘Male breast cancer(MBC)is rare,representing 0.5%–1%of all breast cancers,but its incidence is increasing due to improved diagnostics and awareness.MBC typically presents in older men,is human epidermal growth factor receptor 2(HER2)-negative and estrogen receptor(ER)-positive,and lacks routine screening,leading to delayed diagnosis and advanced disease.Major risk factors include hormonal imbalance,radiation exposure,obesity,alcohol use,and Breast Cancer Gene 1 and 2(BRCA1/2)mutations.Clinically,it may resemble gynecomastia but usually appears as a unilateral,painless mass or nipple discharge.Advances in imaging and liquid biopsy have enhanced early detection.Molecular mechanisms involve hormonal signaling,HER2/epidermal growth factor receptor(EGFR)pathways,tumor suppressor gene alterations,and epigenetic changes.While standard treatments mirror those for female breast cancer,emerging options such as cyclin-dependent kinase 4 and 6(CDK4/6),and poly(ADP-ribose)polymerase(PARP)inhibitors,immunotherapy,and precision medicine are reshaping management.Incorporating artificial intelligence,molecular profiling,and male-specific clinical trials is essential to improve outcomes and bridge current diagnostic and therapeutic gaps.
文摘Severe complications of liver cirrhosis are mostly related to portal hypertension. At the base of the pathogenesis of portal hypertension is the increase in hepatic vascular resistance to portal blood flow with subsequent development of hyperdynamic circulation, which, despite of the formation of collateral circulation, promotes progression of portal hypertension. An important role in its pathogenesis is played by the rearrangement of vascular bed and angiogenesis. As a result, strategic directions of the therapy of portal hypertension under liver cirrhosis include selectively decreasing hepatic vascular resistance with preserving or increasing portal blood flow, and correcting hyperdynamic circulation and pathological angiogenesis, while striving to reduce the hepatic venous pressure gradient to less than 12 mm Hg or 20% of the baseline. Over the last years, substantial progress in understanding the pathophysiological mechanisms of hemodynamic disorders under liver cirrhosis has resulted in the development of new drugs for their correction. Although the majority of them have so far been investigated only in animal experiments, as well as at the molecular and cellular level, it might be expected that the introduction of the new methods in clinical practice will increase the efficacy of the conservative approach to the prophylaxis and treatment of portal hypertension complications. The purpose of the review is to describe the known methods of portal hypertension pharmacotherapy and discuss the drugs that may affect the basic pathogenetic mechanisms of its development.
文摘Background: Postprandial hyperglycemia is an independent risk factor for diabetes-associated complications in individuals with type 2 diabetes. Dietary modification plays an important role in glycemic control. This study was to examine the efficacy of a diabetes specific formula (DSF) during a 4-hour postprandial meal tolerance test (MTT) in Russian subjects with type 2 diabetes receiving oral hypoglycemic medication. Methods: In a randomized, cross-over design, 168 eligible subjects from 11 study centers consumed, in a random order, the DSF (230 mL) or a common light hospital breakfast (i.e. standard meal) on two different occasions. The amounts of macronutrients were similar between the two meals providing ~200 kcal, 11 g protein, 26 g carbohydrate and 8 g fat. Capillary glucose levels were measured at baseline (before meal consumption), and post-meal consumption at 30, 60, 90, 120, 180 and 240 min. Results: The DSF was well tolerated in all subjects. There were 111 subjects completed the study per protocol (mean ± SEM: age: 58.6 ± 0.8 yr, BMI: 31.8 ± 0.42 kg/m2, waist circumference: 101 ± 1.3 cm, HbA1c: 8.0% ± 0.1%). Glucose levels reached peak values at 60 min (median) and the lowest levels at the end of the 240-min MTT test. The mean positive area under the curve (PAUC), the primary outcome, was significantly smaller after DSF consumption (mean ± SEM: 183.02 ± 18.74, median: 132.55) than the PAUC after consumption of the standard meal (mean ± SEM: 239.95 ± 23.11, median: 166.89;p = 0.027). The actual and adjusted peak glucose concentrations were similar between the two treatments. Conclusions: In patients with type 2 diabetes receiving oral hypoglycemic agents, compared to a hospital meal, the DSF improves postprandial glucose control. Combining results from earlier studies, long-term use of DSF may be beneficial to improve glucose management and decrease diabetes-associated complications.

文摘Algorithms for reconstruction of linear and circular birefringence-dichroism of optically thin anisotropic biological layers are presented.The technique of Jones matrix tomography of poly-crystalline films of biological fuids of various human organs has been developed and experimentally tested.The coordinate distributions of phase and amplitude anisotropy of bile films and synovial fuid taken from the knee joint are determined and statistically analyzed.Criteria(statistical moments of 3rd and 4th orders)of differential diagnostics of early stages of cholelithiasis and septic arthritis of the knee joint with excellent balanced accuracy were determined.Data on the diagnostic fficiency of the Jones matrix tomography method for polyerystalline plasma(liver disease),urine(albuminuria)and cytological smears(cervical cancer)are presented.

文摘The exactly solvable model of quasi-conical quantum dot, having a form of spherical sector, is proposed. Due to the specific symmetry of the problem the separation of variables in spherical coordinates is possible in the one- electron Sehrodinger equation. Analytical expressions for wave function and energy spectrum are obtained. It is shown that at small values of the stretch angle of spherical sector the problem is reduced to the conical QD problem. The comparison with previously performed works shows good agreement of results. As an application of the obtained results, the quantum transitions in the system are considered.
文摘Introduction: Conflicts are as natural a phenomenon in society as the seasons. It is difficult for a person to avoid them, but he can choose to take part in them or not. In any organization, including medical, this is a normal phenomenon inherent in the work of the workforce. The objective of the study was conflicts in the field of healthcare in the system “medical personnel of a healthcare institution, patients and relatives of patients”. Materials and Methods: The study of conflict in the team was conducted by medical and sociological methods using an anonymous questionnaire developed by us. Results: A study was conducted aimed at identifying the socio-psychological causes of conflicts and methods for their resolution in the organization of health care using the example of medical personnel. Conclusions: Conflict is an integral part of the functioning of any organization. The study confirmed the hypothesis that the conflict management process will be more effective when changing organizational processes as a whole.
文摘The study aims at the influence of THz radiation on 129.0 GHz of atmospheric oxygen on arterial pressure (A/P) and pulse, hemodynamic parameters of orbital arteries during the irradiation of biological active points of application in healthy volunteers and in patients with involutional macular degeneration (IMD). A decrease of systolic and diastolic components of arterial pressure and pulse, and a normalization of systolic velocity of bloodstream (CVB) and of resistance index (RI) in orbital arteries in patients with IMD are noticed. The results of the research are: 1) the method of THz influence on 129.0 GHz on molecular spectrum of radiation and absorbation of atmospheric oxygen is safe and doesn’t cause any negative side effects on common state in healthy volunteers and in patients with IMD;2) single influence of THz waves on 129.0 GHz on molecular spectrum of radiation and absorbation of atmospheric oxygen is caused to statistical important improvement in vascular system of eyeball.
文摘Introduction: Leadership style is a way, a system of methods of influence of the leader on subordinates. This is one of the most important factors of the effective work of the institution, the full realization of the potential capabilities of people in the team. Leadership style, as an individual way of carrying out management activities, is mostly associated with the person of the manager. The management style is formed under the influence of the relationship between the manager and the team in the process of making and implementing management decisions, individual characteristics and preferences of the manager. The objective of the study was conflicts in the field of healthcare in the system “medical personnel of a healthcare institution—patients and relatives of patients”. The objective of the study was to determine management styles in conflict situations among medical workers. Materials and Methods: With the help of a direct individual survey, according to an anonymous questionnaire developed by us, the opinion of 582 medical workers (422 doctors and 160 nurses) of general hospitals and primary care centers of Chernivtsi and Chernivtsi region regarding the ceilings of behavior in conflict situations was studied. Results: In a conflict situation, the leading form of behavior for medics, both doctors and medical personnel, is the subordinate type (29.6% and 38.1%, respectively). The second most frequent method of conflict resolution is an authoritarian strategy (20.6% for doctors and 15% for nurses), in which personal aspirations and the achievement of one’s own goals come to the fore. Dependent style (17.8% for doctors and 13.75% for nurses) ranks third. A selfish style of behavior in a conflict situation is characteristic of 13.7% of doctors and 13.1% of nurses. A friendly style of behavior during the resolution of conflict situations is characteristic of medical workers at a rather low level (12.3% for doctors and 12.5% for nurses). Aggressive style (5.9% for doctors and 7.5% for nurses) is the least popular as a model of behavior in a conflict situation among respondents. Conclusion: The style of behavior of doctors and nurses in conflict situations is ambiguous, which is due to personal qualities, the degree of leadership, professional relationships and the specifics of the work environment.
文摘Globally, 69.6 million individuals were infected with hepatitis C virus (HCV) infection in 2016. Of the six major HCV genotypes (GT), the most predominant one is GT1, worldwide. The prevalence of HCV in Central Asia, which includes most of the Commonwealth of Independent States (CIS), has been estimated to be 5.8% of the total global burden. The predominant genotype in the CIS and Ukraine regions has been reported to be GT1, followed by GT3. Inadequate HCV epidemiological data, multiple socio-economic barriers, and the lack of regionspecific guidelines have impeded the optimal management of HCV infection in this region. In this regard, a panel of regional experts in the field of hepatology convened to discuss and provide recommendations on the diagnosis, treatment, and pre-, on-, and posttreatment assessment of chronic HCV infection and to ensure the optimal use of cost-effective antiviral regimens in the region. A comprehensive evaluation of the literature along with expert recommendations for the management of GT1-GT6 HCV infection with the antiviral agents available in the region has been provided in this review. This consensus document will help guide clinical decision-making during the management of HCV infection, further optimizing treatment outcomes in these regions.
文摘Statement of the Problem: As you know, there exist two different states in the brain’s mental activity: true and false. In recent years, a progressive method of wavelet transformation of the electroencephalogram (EEG) has been developed, which enabled us to establish the fundamental possibility of direct objective registration of the human brain’s mental activity. Earlier, we created an experimental model and software for recognizing true and false mental responses of a person based on the EEG wavelet transformation and described it in the article. The developed experimental model and information software made it possible to compare the two mental states of brain activity by electroencephalographic indicators, one of which is false and the other is true. The goal is to develop a fundamentally new information technology for recognizing true and false states in the brain’s mental activity based on the wavelet transformation of the electroencephalogram. Results: It was revealed that the true and false states of the brain can be distinguished using the method of continuous wavelet transformation and calculation of the EEG wavelet energy. It is shown that the main differences between true and false mental responses are observed in the delta and alpha ranges of the EEG. In the EEG delta rhythm, the wavelet energy is reliably higher in case of a false answer compared to a true one. In the EEG alpha rhythm, the wavelet energy is significantly higher with a true answer than a false one. Practical significance of the research: The data obtained open up the fundamental possibility of identifying true and false mental states of the brain on the basis of continuous wavelet transformation and calculation of the EEG wavelet energy.
文摘With the aim of creating biodegradable materials for medical devices clinical appointments with high hemocompatibility we have developed a new polymer product.The basis of this product is plasticized by polyethylene glycol bacterial copolymer of hydroxybutyrate and oxovalerate. A well-known antitbrombotic supplement--acetylsalicylic acid has been added to improve hemocompatibility in the polymer. The results of our studies showed a controlled prolonged separation of acetylsalicylic acid from polymeric material in the blood. We studied in vitro the dynamics of liberation of acetylsalicylic acid from polymeric coatings. It was shown that the concentration of polyethylene glycol and the thickness of the polymer layer can affect the rate of diffusion of acetylsalicylic acid from polymer films.
文摘Continuous treatment with organic nitrates causes nitrate tolerance and provides evidence for a relationship between mitochondrial complex 1 activity and mitochondrial aldehyde dehydrogenase-2 (ALDH-2) with disturbances of the hemodynamics reaction during nitroglycerin (NTG) tolerance (NTGT). The purpose of this study was the evaluation of efficacy of original oxidized form NAD-containing drug, NADCIN<sup>®</sup>, on hemodynamic reactions, baroreflex sensitivity (BRS) and reflex control of splanchnic sympathetic nerve activity (SSNA), level of redox-potential, activity of ALDH-2 and superoxide anion generation in aortic tissue in rat model of NTGT. Five groups (7 - 9 each) of male Wistar rats, including control, acute i.v. NTG (150 mcg/kg) administration, NTG tolerance NTGT treatment with NADCIN<sup>®</sup> 8 mg/kg and methylene blue (MB, 2.5 mg/kg) were used. NTGT in rats was accompanied with the greatly attenuation of hemodynamics reaction, BRS, the decreasing of the ability to reflex control of SSNA without pronounce overexpression of endothelin-1 in vessels (aorta). In NTGT rats i.v. NTG along induced less hypotensive reactions and alterations in heart period vs single NTG treated group, more expressively decreased BRS (-34%) and reflex control of SSNA (-18%). NADCIN<sup>®</sup> significantly inhibits tolerance-inducing properties of the prolonged nitroglycerin infusion (max decrease of blood pressure response to nitroglycerin injection, % of normal controls: NTGT 51.2%, NADCIN<sup>®</sup> 91.6%, MB 55.8%). NADCIN<sup>®</sup> in NTGT rats after NTG i.v. administration increased reduced BRS (+37.8%, p < 0,05), reflex control of SSNA (+29.4%, p < 0.05) and reversed the decreasing of NAD/NADH ratio, ALDH-2 activity and decreasing in superoxide generation in thoracic aortic tissue. Thus, course treatment with NADCIN<sup>®</sup> of NTGT rats restores hemodynamics changes, BRS and SSNA throughout the increasing of redox-potential NAD/NADH and cessates the NTGT developing.
文摘Understanding the based-on drug or drug conjugates to reach the beneficial optimally recognized by the immune system requires multidisciplinary approaches and detailed of albumin as the key circulating a transporting/transmission and antioxidant protein of blood drug interaction. Albumin, in reduced form (mercaptoalbumin, HMA), with antioxidant ability and alterations/deteriorations in the redox status of human serum albumin (HSA) under oxidative stress formation in infection diseases and its complications strongly modifies albumin antioxidant capacity. The aim of this study was the investigation of carbonyl/oxidative stress and pseudo-esterase activity of mercaptolbumin and oxidized HSA models. HSA (P. pastoris) purchased from MedChemExpress (USA) was used for study to model oxidative stress, HSA in reduced (intact) form was treated with H2O2, tert-butylhydroperoxide (t-BHP) and chloramine T (CT). The content of HSA-bound carbonyl groups decreased in under treatment with t-BPH- and CT-reduced HSA and more less extent in case of H2O2-treated. Fatty acid-free HSA and mercaptoalbumin (HMA) advanced oxidation protein products (AOPP) concentrations were significantly lower than in H2O2 loading reduced HSA by 123% and 235%, respectively. The total thiols level was lower in HMA + CT compared to reduced HMA by 51% and even increased after treatment of HMA with H2O2. Pseudo-esterase activity of HMA maintains >65% in the presence of hydroperoxide and occurs pronounced loss in the presence of chloramine T. Hydrogen peroxide at physiological concentration about 10 μM occurs less damage of reduced from of HSA then t-BPH and CT, and unlike t-BPH and CT, without significantly changes in pseudo-esterase activities.

文摘BACKGROUND Breast cancer(BC)continues to occupy a leading position in terms of morbidity and mortality from malignant neoplasms among the female population.One of the promising markers associated with BC progression is programmed death ligand 1(PD-L1).Previously,we investigated PD-L1 expression in BC via a new antibody against programmed cell death protein 1 ligand 1(PDCD1 LG1)and reported that high PDCD1 LG1 expression in tumor cells is an independent factor for a high risk of regional metastasis in patients with BC.However,the prognostic significance of PDCD1 LG1 expression in BC stromal cells has not been adequately studied.AIM To study the features of PDCD1 LG1 expression in BC stromal cells and its relationship with BC clinicopathological characteristics.METHODS In a prospective single-center observational study,tumor samples from 148 patients with newly diagnosed BC were examined.The tumor sections were immunohistochemically stained with antibodies against PDCD1 LG1.In the tumor samples,the PDCD1 LG1-positive lymphocyte(PDCD1 LG1+LF)score,presence of nuclear PDCD1 LG1 expression in the LFs,PDCD1 LG1 expression in polymorphic cell infiltrates(PDCD1 LG1+polymorphic cell infiltrates[PCIs]),and cells of the fibroblastic stroma and endothelial cells of the tumor microvessels were assessed.Statistical analyses were performed using Statistica 10.0 software.RESULTS A PDCD1 LG1+LF score≥3 was detected more often at stages N0 and N3 than at N1 and N2(P=0.03).Moderate and pronounced PDCD1 LG1+PCIs and the presence of PDCD1 LG1+fibroblastic stroma were associated with negative estrogen receptor status(P=0.0008 and P=0.03,respectively),human epidermal growth factor receptor 2-positive(HER2+)BC(P<0.00001 and P=0.0005),and luminal B HER2+,non-luminal HER2+and triple-negative BC(P<0.00001 and P=0.004).The risk of metastasis to regional lymph nodes(RLNs)depend on lymphovascular invasion(LVI)and the PDCD1 LG1+LF score.In the absence of LVI and a PDCD1 LG1+LF score<3 or≥3,metastases in RLNs were absent in 66.6%and 93.9%of patients with BC,respectively.In the presence of LVI and a PDCD1 LG1+LF score<3 or≥3,metastases in RLNs were detected in 82.6%and 92.7%of patients with BC,respectively.CONCLUSION The results indicated that the combined assessment of the PDCD1 LG1+LF score and LVI can improve the accuracy of predicting the risk of metastasis to RLNs in patients with BC.
