The objective of this study is to develop a nanocrystal formulation to enhance transdermal delivery of MLX. MLX nanocrystals were successfully prepared by the nanoprecipitation technique based on acidbase neutralization. With poloxamer 407 and Tween 80(80/20, w/w) as mixed stabilizers,MLX nanocrystals with particle size of 175 nm were obtained. The crystalline structure of MLX nanocrystals was confirmed by both differential scanning calorimetry and X-ray powder diffractometry. However, the nanoprecipitation process reduced the crystallinity of MLX.Nanocrystals increased both in vitro and in vivo transdermal permeation of MLX compared with the solution and suspension counterparts. Due to the enhanced apparent solubility and dissolution as well as the facilitated hair follicular penetration, nanocrystals present a high and prolonged plasma MLX concentration. And 2.58-and 4.4-fold increase in AUC0 →2 4 h was achieved by nanocrystals comparing with solution and suspension, respectively. Objective:Interdigital infection,as a neglected disease,is frequently misdiagnosed as simple tinea pedis which caused by purely dermatophyte induced.It remains intractable to treatments because of high recurrence rate...Objective:Interdigital infection,as a neglected disease,is frequently misdiagnosed as simple tinea pedis which caused by purely dermatophyte induced.It remains intractable to treatments because of high recurrence rates and potential complications.However,the optimal diagnosis and treatment regimens have not been firmly established.This study aimed to investigate the presentation,etiology,predisposing factors,occurrence regularity,and therapeutic approaches of interdigital infection in Shanghai,China.Methods:A cross-sectional study was conducted at Shanghai Dermatology Hospital in 2019,and data from 57 patients with acute interdigital inflammation were collected.Clinical features and medical histories including blood,bacterial,and mycological examination,culture test,drug susceptibility test results,and follow-up data were analyzed for pathogenic agents.Patients received etiological therapy and were followed up for 1 year.McNemar’s tests were used for comparison of positive rates of direct microscopy and cultures.Results:We found Pseudomonas aeruginosa(40.35%),Staphylococcus aureus(36.84%),and other bacteria(22.80%).The rate of positive fungus culture after bacterial infection recovery was significantly higher than that at the first visit(86.67%vs.36.00%,P=0.001).Compared to other bacteria,Pseudomonas aeruginosa interdigital infection was more inclined to co-infect with fungal agents and was frequently seen in spring and autumn and in patients aged 60-69 years.Interdigital infection caused by Staphylococcus aureus occurred more frequently in the summer and in patients aged 31-40 years.We found that levofloxacin had excellent therapeutic effects for interdigital infection.Conclusion:Our findings may provide important information for establishing diagnostic guidelines and effective treatment,and subsequently,help reduce the rate of recurrence and improve patient outcomes following interdigital infection.展开更多

By modulating the compositions of the lipid bilayers and adding polymers or ligands, both the stability and permeability of liposomes can be greatly improved for oral drug delivery. This study was performed to identify the fecal microbial composition of psoriasis patients, and investigat...Psoriasis is an autoimmune disease and gut microbiota participate in the establishment of intestinal immunity. This study was performed to identify the fecal microbial composition of psoriasis patients, and investigated the influence of subgroup(type and severity) on the fecal microbial composition, and to define the key microbiota in the pathogenesis of psoriasis. Fecal samples from 35 psoriasis patients and 27 healthy controls were sequenced by 16 S rRNA and then analyzed by informatics methods. We found that the microbiota of the psoriasis group differed from that of the heathy group. The relative abundances of Firmicutes and Bacteroidetes were inverted at the phylum level, and 16 kinds of phylotype at the genus level were found with significant difference. No microbial diversity and composition alteration were observed among the four types of psoriasis. The microbiota of psoriasis patients in the severe state differs from those of psoriasis patients with more mild conditions and also the healthy controls. The veillonella in fecal microbiota showed a positive relationship with h-CRP in blood. This research proved that psoriasis patients have a significant disturbed microbiota profiles. Further study of psoriasis based on microbiota may provide novel insights into the pathogenesis of psoriasis and more evidence for the prevention and treatment of psoriasis.展开更多

Methods: The BALB/c mice were randomized into a normal group, a control group, a roller-needle(RN) group, a mild plum-blossom needle(MP) group, and a heavy plum-blossom needle(HP) group. An area of hair was removed by external application of 8% sodium sulfide on BALB/c mice. The hair regrowth, hair follicle changes, and local inflammatory factor changes after cutaneous acupuncture were observed. Results: After treated with sodium sulfide, the hair was completely removed, the local hair follicles reached the catagen phase, and the expressions of interleukin(IL)-1α, IL-1β, IL-2, tumor necrosis factor(TNF)-α, interferon(IFN)-γ, and IL-17 were increased. Mice intervened by RN achieved the same hair growth rating as the controls but with thicker hair shafts; mice in the MP group had incomplete and uneven hair growth but thicker hair shafts; mice in the HP group didn't show hair growth. Pathological analysis revealed significant inflammatory infiltration into the local follicle bulbs and increased catagen-phase follicles in the control group, while RN and MP groups showed significantly increased anagen-phase follicles, coarser individual hairs, and obvious hair shafts. Meanwhile, most of the hair follicles in the HP group were in telogen phase and showed obvious surrounding inflammatory infiltration. RN, MP, and HP significantly down-regulated the increased IL-2, IL-1α, IL-1β, TNF-α, and IFN-γ levels(P〈0.05), but didn't notably affect the increased CD34 expression(P〈0.05). Conclusion: Cutaneous acupuncture with heavy stimulation intensity can inhibit hair growth in hair removal mice, while RN, with the lightest stimulation intensity, is unlikely to affect hair growth but may make hair shafts thicker and follicles larger.展开更多
Metastasis is a major cause of chemotherapeutic failure and death.Degradation of a specific component of the extracellular matrix(ECM)by matrix metalloproteinases(MMPs)affects the physical barrier of the tumor microen...Metastasis is a major cause of chemotherapeutic failure and death.Degradation of a specific component of the extracellular matrix(ECM)by matrix metalloproteinases(MMPs)affects the physical barrier of the tumor microenvironment(TME)and induces metastasis.Here,lysolipid-containing thermosensitive liposomes(LTSLs)were prepared to deliver an MMP inhibitor,marimastat(MATT),to the TME to inhibit MMP activity and expression.LTSLs rapidly released their payloads at 42°C.Compared with the saline control,MATT-LTSLs exhibited enhanced accumulation in the tumor and a 20-fold decrease in tumor growth in 4T1 tumor-bearing mice;moreover,MATT-LTSLs reduced MMP-2 and MMP-9 activity by 50%and 43%,respectively,and downregulated MMP-2 and MMP-9 expression in vivo by 30%and 43%,respectively.Most importantly,MATT-LTSL treatment caused a 7-fold decrease in metastatic lung nodules and a 6-fold reduction in microvessels inside the tumor.We believe this study provides an effective approach for the suppression of metastasis,and the use of a cytotoxic agent in combination with MATT is a potential strategy for metastatic cancer treatment.展开更多

Background:Type 1 leprosy reaction,also known as“reversal reaction”,is related to cellular immune responses to Mycobacterium leprae antigens.The risk factors that trigger type 1 leprosy reactions are poorly understo...Background:Type 1 leprosy reaction,also known as“reversal reaction”,is related to cellular immune responses to Mycobacterium leprae antigens.The risk factors that trigger type 1 leprosy reactions are poorly understood.Leprosy with concurrent tetanus is rare,and there are no publicly available reports of a leprosy patient infected with tetanus that induced type 1 leprosy reactions.Case presentation:A 56-year-old Chinese Han female presented to our hospital with symptoms of erythematous plaques and pain over her left upper limb for 2 days and foreign object sensation in her throat for 3 days.The patient had a 6-year history of leprosy.Type 1 leprosy reactions were initially considered,followed by treatment with methylprednisolone.Two days later,the patient’s symptoms were aggravated,with neck muscle tension and difficulty in opening her mouth,and the erythematous plaques had spread over most of her left upper limb.After further careful examinations,we confirmed the diagnosis of tetanus with concurrent type 1 leprosy reactions.The patient was given anti-tetanus treatment for 12 days and anti-leprosy reaction treatment for 4 months;the diseases were eventually controlled.Conclusions:This report suggests that tetanus infection may be a trigger for type 1 leprosy reactions.展开更多

文摘To The Editor:Acne vulgaris is a chronic inflammatory skin disease involving the sebaceous glands.It has an incidence of approximately 9.4%,often occurring in adolescents.[1]The disease itself and the scars left after treatment can cause psychological stress in adolescents and seriously affect patients’quality of life.[2]At present,skin care products,either alone or in combination with medical drugs,have become important means for treating acne.This single-center,single-blind(clinician-blind),masculine-paralleled,randomized study(Registration number:ChiCTR2100051398)evaluated the efficacy and safety of a cream containing octyl salicylic acid,salicylic acid,linoleic acid,nicotinamide,and piroctone olamine(Duo+,La Roche-Posay,Paris,France)alone and in combination with benzoyl peroxide(BPO,Galderma,Paris,France)for the treatment of mild-to-moderate acne.The research was approved by the Ethics Committee of Shanghai Skin Disease Hospital,China(No.2020-13 ke).All participants provided written informed consent.

The possible side effects of vitiligo or vitiligo-like hypopigmentation associated with imiquimod treatment have rarely been reported in the open literature. We herein presented 6 cases of imiquimod-induced genital hypopigmentation.

文摘Background:Type 1 leprosy reaction,also known as“reversal reaction”,is related to cellular immune responses to Mycobacterium leprae antigens.The risk factors that trigger type 1 leprosy reactions are poorly understood.Leprosy with concurrent tetanus is rare,and there are no publicly available reports of a leprosy patient infected with tetanus that induced type 1 leprosy reactions.Case presentation:A 56-year-old Chinese Han female presented to our hospital with symptoms of erythematous plaques and pain over her left upper limb for 2 days and foreign object sensation in her throat for 3 days.The patient had a 6-year history of leprosy.Type 1 leprosy reactions were initially considered,followed by treatment with methylprednisolone.Two days later,the patient’s symptoms were aggravated,with neck muscle tension and difficulty in opening her mouth,and the erythematous plaques had spread over most of her left upper limb.After further careful examinations,we confirmed the diagnosis of tetanus with concurrent type 1 leprosy reactions.The patient was given anti-tetanus treatment for 12 days and anti-leprosy reaction treatment for 4 months;the diseases were eventually controlled.Conclusions:This report suggests that tetanus infection may be a trigger for type 1 leprosy reactions.
文摘Atopic dermatitis(AD)is a common disease clinically characterized by chronic recurrent eczematous lesions,dry skin,and pruritus.AD can negatively impact patients’quality of life.The prevalence of AD in China has been increasing during the past few decades.Based on the most recent advances in the treatment of AD,we updated the 2014 version of the Guidelines for Diagnosis and Treatment of Atopic Dermatitis in China regarding the definition,epidemiology,pathogenesis,clinical classification,diagnosis,prevention,and treatment of AD.
文摘Introduction The reported incidence of cutaneousmetastasis from internal carcinoma ranges from 0.7%to 10%,with wide variability across different studies.Although cervical carcinoma is a very common malignant tumor in women,the frequency of cutaneous metastasis from cervical carcinoma is less than 2%.1 Usually,metastasis occurs at a site adjacent to the primary carcinoma,and the most common clinical presentation of the metastasis is a nodule.Cervical carcinomas almost always metastasize to the abdomen wall,vulva,or anterior chestwall.To the best of our knowledge,reports of a facial cellulitis-like metastasis of cervical carcinoma are relatively rare in the published literature.Herein,we report the case of aChinesewoman,whohad a twoyearshistory of cervical carcinoma and a developed facial metastasis presenting as a cellulitis-like lesion.
文摘Allergic contact dermatitis is a common clinical allergic disease.The patch test is the gold standard for finding and clarifying contact allergens.With the deepening understanding of the epidemiology,pathogenesis,and clinical manifestations of contact dermatitis,as well as the increased requirements for the standardized application of patch tests,the relevant consensus needs to be continuously updated.This consensus is updated on the basis of the version issued in 2015.In this revised version,it refines the selection of clinical indications,improves the judgment end interpretation of results,adds delayed detection and patient education,and so on,which aims to further standardize clinical applications and improve the value of patch test.
