背景老龄化背景下,失能老人照护需求不容忽视。全面了解失能老人照护与支持需求可为政府决策部门合理制定照护方案提供参考依据。目的系统评价失能老人照护与支持需求相关的质性研究。方法于2022-12-03—2023-02-26系统检索Web of Scie...背景老龄化背景下,失能老人照护需求不容忽视。全面了解失能老人照护与支持需求可为政府决策部门合理制定照护方案提供参考依据。目的系统评价失能老人照护与支持需求相关的质性研究。方法于2022-12-03—2023-02-26系统检索Web of Science、PubMed、Embase、Cochrane Library、中国知网、万方数据知识服务平台、中国生物医学文献服务系统从建库至2023-02-26相关的质性研究。采用澳大利亚JBI循证卫生保健中心质性研究质量评价标准进行文献质量评价,通过汇集性整合方法对原始质性研究结果进行归纳整合。结果本研究共纳入33篇质性研究,提炼出7个主题,共18个类别:日常生活相关需求(基础性日常生活活动、家务活动、照护特征),功能独立性需求(身体功能独立、自主决策独立),卫生保健相关需求(卫生保健服务内容、卫生保健服务体验、卫生保健服务需求表达),环境支持需求(客观环境支持、政策支持),正性情绪支持需求(情感慰藉、精神关怀),社会支持需求(社区支持性服务、活动参与、人际链接、信息链接),卫生技术相关需求(照护辅助设备、技术服务)。结论失能老人照护与支持需求复杂且多样。政府需要整合“家庭-社区-机构”资源以应对失能老人“身-心-社-医-环境”需求,从群体角度关注失能老人照护资源的分配公平,给予失能老人更多的社会福利。展开更多
The role of nm23H1 genetic instability is not limited to gastrointestinal malignancies.A similar close relationship exists between nm23H1 genetic instability and other non gastrointestinal systemic malignancies.For in...The role of nm23H1 genetic instability is not limited to gastrointestinal malignancies.A similar close relationship exists between nm23H1 genetic instability and other non gastrointestinal systemic malignancies.For instance,in oral malignant melanomas with lymphoid metastasis,the nm23H1 expression is significantly lower in contrast to tumors with no lymphoid metastasis.Similarly,increased metastasis is seen in non small cell lung cancers following down regulation of nm23H1 in conjunction with KAI-1 down regulation. There is an inverse relationship between tumor stage and metastasis and nm23H1 expression in individuals with prostate carcinomas and a similar relationship exists between microsatellite instability of the nm23H1 gene and ovarian carcinogenesis.For instance,nearly 70.5%of stageⅠ-Ⅱovarian tumors express nm23H1 in sharp contrast to only 25%of stageⅢ-Ⅳovarian tumors.As is clearly evident,nm23H1 has a major role in gastrointestinal and non-gastrointestinal carcinogenesis.The coming few years will hopefully see the development of new strategies by virtue of which we can alter nm23H1 expression and thus decrease the risk of metastasis in malignant tumors.展开更多

The authors have clearly shown that triptolide administration is associated with upregulation of the Bax gene, resulting in an attenuating effect on cell growth in gastrointestinal malignancies such as pancreatic carcinomas. The article by Zhou et al is all the more important because it highlights the rapidly increasing role of triplodide in the management of systemic malignancies. For instance, triptolide acts on the PI3K/Akt/NF-κB pathway, thereby enhancing apoptosis secondary to the administration of bortezomib in multiple myeloma cells. Similar synergisms are seen when triptolide is administered along with 5-fluoruracil for the management of colonic carcinomas. Similarly, triptolide causes down-regulation of the Bcl-2 gene, resulting in control of cell growth in tumors, such as glioblastoma multiformes.展开更多

To mitigate the performance d...In this paper a method that combines transmit antenna selection and reduced-constellation detection in spatially correlated Multi-Input Multi-Output (MIMO) fading channels is presented. To mitigate the performance degradation caused by the use of antenna selection that is based on correlation among columns, an iterative receiver scheme that uses only a subset of the constellation points close to the expected symbol vahle estimated in the previous iteration is proposed. The size of the subset can adapt to the maximum correlation of the sub-matrix after the simple antenna selection. Furthermore, the error rate performance of the scheme under linear Miniinutn Mean Square Error (MMSE) or Ordered Successive Interference Cancellation (OSIC) for the first run detection and different interleaver lengths is investigated while the transnlit antenna selection is considered. The simulation results show a significant advantage both for implementation complexity and for error rate performance under a fixed data rate.展开更多

In comparison to polyethylene glycol,lubiprostone offers other advantages and is increasingly being used as an adjunctive agent in diagnostic as well as management strategies not only in gastroenterology, but in other...In comparison to polyethylene glycol,lubiprostone offers other advantages and is increasingly being used as an adjunctive agent in diagnostic as well as management strategies not only in gastroenterology, but in other fields.For instance,lubiprostone exerts beneficial effects in cystic fibrosis tissues.It augmernts the chloride secretion in these cells by activating non-cystic fibrosis transmembrane regulator(CFTR) secretion of chloride by afflicted respiratory epithelia. Lubiprostone also seems to improve visualization of the gastrointestinal tract during procedures such as colonoscopy.This is especially true if the lubiprostone is administered prior to bowel cleansing with agents such as polyethylene glycol electrolyte(PEG-E). Lubiprostone also enhances and stimulates contraction in colonic as well as gastric muscles and may thus further contribute as a prokinetic agent.Besides these effects,lubiprostone also causes hyperpolarization in other tissues such as uterine muscle cells.This may prove to be of significant clinical benefit in the management of uterine pathologies in the near future.展开更多

Background The Study for Monitoring Antimicrobial Resistance Trends program monitors the activity of antibiotics against aerobic and facultative Gram-negative bacilli (GNBs) from intra-abdominal infections (IAIs) ...Background The Study for Monitoring Antimicrobial Resistance Trends program monitors the activity of antibiotics against aerobic and facultative Gram-negative bacilli (GNBs) from intra-abdominal infections (IAIs) in patients worldwide.Methods In 2011,1 929 aerobic and facultative GNBs from 21 hospitals in 16 cities in China were collected.All isolates were tested using a panel of 12 antimicrobial agents,and susceptibility was determined following the Clinical Laboratory Standards Institute guidelines.Results Among the Gram-negative pathogens causing IAIs,Escherichia coli (47.3%) was the most commonly isolated,followed by Klebsiella pneumoniae (17.2%),Pseudomonas aeruginosa (10.1%),and Acinetobacter baumannii (8.3%).Enterobacteriaceae comprised 78.8% (1521/1929) of the total isolates.Among the antimicrobial agents tested,ertapenem and imipenem were the most active agents against Enterobacteriaceae,with susceptibility rates of 95.1% and 94.4%,followed by amikacin (93.9%) and piperacillin/tazobactam (87.7%).Susceptibility rates of ceftriaxone,cefotaxime,ceftazidime,and cefepime against Enterobacteriaceae were 38.3%,38.3%,61.1%,and 50.8%,respectively.The leastactive agent against Enterobacteriaceae was ampicillin/sulbactam (25.9%).The extended-spectrum β-lactamase (ESBL) rates among E.coli,K.pneumoniae,Klebsiella oxytoca,and Proteus mirabilis were 68.8%,38.1%,41.2%,and 57.7%,respectively.Conclusions Enterobacteriaceae were the major pathogens causing IAIs,and the most active agents against the study isolates (including those producing ESBLs) were ertapenem,imipenem,and amikacin.Including the carbapenems,most agents exhibited reduced susceptibility against ESBL-positive and multidrug-resistant isolates.展开更多
