Aim: To assess for the first time the occurrence of Y chromosomal microdeletions and partial deletions of the Azoospermia Factor c (AZFc) region in Sri Lankan men and to correlate them with clinical parameters. Methods: In a retrospective study, we analyzed 96 infertile men (78 with non-obstructive azoospermia) and 87 controls with normal spermatogenesis. AZFa, AZFb, AZFc and partial deletions within the AZFc region were analyzed by multiplex polymerase chain reaction (PCR) according to established protocols. Results: No AZFa, AZFb or AZFc deletions were found in the control group. Seven patients in the group of infertile men were found to have deletions as following: one AZFa, two AZFc, two AZFbc and two AZFabc. The relative distribution of these patterns was significantly different compared with that found in the German population. Extension analysis confirmed that the deletions occurred according to the current pathogenic model, gr/gr deletions were found to be equally present both in the patients (n = 4) and in the control group (n = 4). One b2/b3 deletion was found in the patient group. Conclusion: These results suggest that the frequency and pattern of microdeletions of the Y chromosome in Sri Lankan men are similar to those found in other populations and confirm that gr/gr deletions are not sufficient to cause spermatogenetic failure. During mammalian fertilisation, the zona pellucida (ZP) matrix surrounding the oocyte is responsible for the binding of the spermatozoa to the oocyte and induction of the acrosome reaction (AR) in the ZP-bound spermatozoon. The AR is crucial for the penetration of the ZP matrix by spermatozoa. The ZP matrix in mice is composed of three glycoproteins designated ZP1, ZP2 and ZP3, whereas in humans, it is composed of four (ZP1, ZP2, ZP3 and ZP4). ZP3 acts as the putative primary sperm receptor and is responsible for AR induction in mice, whereas in humans (in addition to ZP3), ZP1 and ZP4 also induce the AR. The ability of ZP3 to induce the AR resides in its C-terminal fragment. O-linked glycans are critical for the murine ZP3-mediated AR. However, N-linked glycans of human ZP1, ZP3 and ZP4 have important roles in the induction of the AR. Studies with pharmacological inhibitors showed that the ZP3-induced AR involves the activation of the Gi-coupled receptor pathway, whereas ZP1- and ZP4-mediated ARs are independent of this pathway. The ZP3-induced AR involves the activation of T-type voltage-operated calcium channels (VOCCs), whereas ZP1- and ZP4-induced ARs involve both T- and L-type VOCCs. To conclude, in mice, ZP3 is primarily responsible for the binding of capacitated spermatozoa to the ZP matrix and induction of the AR, whereas in humans (in addition to ZP3), ZP1 and ZP4 also participate in these stages of fertilisation. 展开更多

Methods: Initial screening was made on the basis of ram cauda epididymal sperm immobilization immediately after addition of extracts. The most potent extract was selected and was evaluated on both ram and human spermatozoa. To unravel its mode of action several sperm functional tests were carried out, namely viability of cells, hypo-osmotic swelling test for membrane integrity and assays of membrane-bound enzyme 5'-nucleotidase and acrosomal marker enzyme acrosin. Results: The crude aqueous extract of the bulb of Allium sativum L. showed the most promising results by instant immobilization of the ram epididymal sperm at 0.25 g/mL and human ejaculated sperm at 0.5 g/mL. Sperm immobilizing effects were irreversible and the factor of the extract responsible for immobilization was thermostable up to 90 癈. On boiling at 100 癈 for 10 minutes, this activity was markedly reduced. Moreover, this extract was able to cause aggregation of ram sperms into small clusters after 30 minutes of incubation at 37 癈. However this property was not found in human spermatozoa. More than 50 % reduction in sperm viability and hypo-osmotic swelling occurred in treated sperm as compared with the controls, indicating the possibility of plasma membrane disintegration which was further supported by the significant reduction in the activity of membrane bound 5'-nucleotidase and acrosomal acrosin. Conclusion: The crude aqueous extract of A. sativum bulb possesses spermicidal activity in vitro. 展开更多

Water pollution of the Yangtze River in China became one of challenges that the government is facing today. Increasing numbers of petrochemical plants were built along the river in past decades, and numbers of organic chemicals were discharged into the river. Our goal was to establish in vitro system on rat sertoli cells, spermatogenic cells and leydig cells to investigate the reproductive toxicity potential induced by organic extracts from petrochemical effluents. Our results showed that the organic extract depressed the viability (p 〈 0.01) and destroyed the plasma membrane integrity of sertoli cells and spermatogenic cells to a certain degree. Accordingly, proportion of early apoptotic sertoli cells and late apoptotic spermatogenic cells increased significantly. Although significant morphological changes were not detected for leydig cells, the extract was observed to inhibit their testosterone production (p 〈 0.01). Sertoli cells and sperrnatogenic cells appeared to be more sensitive and maybe the main targets of the key toxins. These results suggested that the in vitro system on rat testicular cells may be useful to predicate reproductive toxicity potential of organic extracts from petrochemical effluents. More attention should be paid to the petrochemical effluents, because long-term accumulation of these organic compounds in organisms may cause spermatogenesis malfunction and testosterone reduction. 展开更多

Semen from 10 932 male partners of infertile couples was analysed and sperm parameter trends were evaluated at the Reproduction Biology Laboratory of the University Hospital of Marseille (France) between 1988 and 2007. After 3-6 days of abstinence, semen samples were collected. Measurements of seminal fluid volume, pH, sperm concentration, total sperm count, motility and detailed morphology of spermatozoa were performed. Sperm parameters were analysed on the entire population and in men with normal total numeration ( 〉~ 40 million per ejaculate). The whole population demonstrated declining trends in sperm concentration (1.5% per year), total sperm count (1.6% per year), total motility (0.4% per year), rapid motility (5.5% per year) and normal morphology (2.2% per year). In the group of selected samples with total normal sperm count, the same trends of sperm quality deterioration with time were observed. Our results clearly indicate that the auality of semen decreased in this population over the study period. 展开更多

After several rounds of proliferation and spermatogonial differentiation, the male germ cells will eventually undergo meiosis to form haploid spermatids. Disturbance of the molecular regulation of spermatogenesis can lead to spermatogenic arrest, in humans often during meiosis,1,2 and subsequent azoospermia. Methods: ELF was collected by micropuncture from twelve sites along the epididymal duct. The elementalcontents was analyzed with inductively coupled plasma (ICP) emission spectroscopy, a microanalytical technique thatcan simultaneously measure many elements in minute volumes of sample. The Na and K concentrations were determinedby flame photometry. Results: ICP spectroscopy showed the presence of copper, calcium, nickel, iron, magne-sium, chromium, titanium and zinc in ELF, with fluctuating levels at different sites along the length of the epididymis.Cadmium, cobalt, lead and manganese were not fotmd. The Na^+ / K^+ ratio was seen to be higher at the initial seg-ments of the epididymis and lower at the distal. Conclusion: It is proposed that the observed characteristic distribu-tion of elements in ELF may have far reaching implications in sperm maturation and storage known to occur in the epi-didymis. OBJECTIVE Midkine (MK),a new member of the heparin-binding growth factor family,was found recently to have a highexpression level in many carcinoma specimens,including thoseof the esophagus,gall bladder,pancreas,colorectum,breast andlung.Estrogen receptor beta (ER-β),a recently cloned estrogenreceptor subtype,was also found to be highly expressed in lungtumor tissue,in contrast to a lower level of expression in normallung tissue.However,few relevant studies on these proteins havebeen published.The aims of our study were to investigate theexpression of midkine and ER-β proteins in non-small cell lungcancer (NSCLC) and to examine the relationship between theirexpression and the clinicopathologic data as well as to analyse thecorrelation of their expression in NSCLC.METHODS By immunohistochemistry,MK and ER-β were ex-amined in 24 surgically resected cases of NSCLC with their corre-sponding paraneoplastic and normal lung tissues.RESULTS MK and ER-β were overexpressed in NSCLC.Thelevels of MK and ER-β expression in NSCLC were found to be sig-nificantly negatively correlated with the pathological classification(P=0.042 and 0.021,respectively),and their expression decreasedwith a raise in the classification.Spearman's correlation analysisshowed that the correlation of their expression in NSCLC wasstrong (correlation coefficient[r_s]= 0.535,P=0.007<0.01).CONCLUSION The expression levels of MK and ER-β to someextent reflect the malignant degree of NSCLC,and their combineddetection may be of great value in early diagnosis,treatments ofpatients with NSCLC and can predict the prognoses. 展开更多

Growing evidence suggests that oxidative stress is a major cause of male infertility.Spermatozoa are highly sensitive cells due to the vulnerability of their membrane fatty acids and the diminished antioxidant defense.Reactive oxygen species(ROS)impact multiple signaling pathways involved in mitochondrial dysfunction,hormonal unbalance and semen quality decline.The origin of ROS is diverse,including generated normal cellular metabolism,environmental exposure,advanced paternal age and inflammation.Research has indicated that antioxidant supplementation can improve oxidative stress and reduce the risk of chronic diseases.Moreover,it is widely known that antioxidant supplementation can enhance sperm quality and increase the pregnancy rate in couples undergoing fertility treatments.The current study highlights the deleterious effect of ROS and its impact on semen function.In addition,it contributes to the development of a clinical approach for the use of antioxidants in the management of male infertility. 展开更多

Varicocele is the first etiology of infertility in men worldwide and affects almost half of the men dealing with infertility in Senegal. Varicocele is associated with a high rate of sperm DNA fragmentation so this test is being more and more recommended especially when patients have to benefit from an ART (Assisted Reproductive Technology) procedure. The aim of our study was to assess if our patients with varicocele are concerned by higher DNA fragmentation rate in spermatozoon. Sperm samples were collected (95 cases of varicocele and 30 controls) and analyzed between January 2013 and June 2016 in a prospective study. Sperm DNA damage, characterized by the rate of DNA fragmentation was evaluated by TUNEL assay using fluorescence microscopy. There was a significant higher percentage of DNA fragmentation in patients with varicocele (p < 0.001), compared to controls. We plan to study a bigger cohort to assess this tendency and study the benefits of surgery in varicocele by evaluating DNA fragmentation after the procedure. 展开更多

The presence of DNA fragmentation in semen is associated with poor results in ART (Assisted Re-productive Technology). This has led to the introduction of sperm DNA integrity testing in the clinical assessment of male infertility. The TUNEL assay is one of the most commonly used tests to measure DNA fragmentation in research studies, but it is now being optimized for routine clinical use. So, the objectives of our study were to install the TUNEL assay in our laboratory and to use it to assess the degree of DNA fragmentation in our patients dealing with couple infertility. Forty seven (47) and 25 samples from patients followed up for infertility and known fertile men were analyzed between August 2013 and July 2015 respectively. The sperm samples were evaluated according to the World Health Organization Guidelines. Sperm DNA damage was assessed by the TUNEL assay, with a standardized protocol that we adapted for reading using fluorescence microscopy instead of flow cytometry. The Sperm DNA Fragmentation Index was significantly higher in patients with infertility compared to the controls (22.2% ± 5.6% vs. 16.7% ± 0.7%) with a p < 0.05. This Index was also high in patients with varicocele compared to the control group (22.8% ± 9.2 vs. 16.7% ± 0.7%) (p < 0.01). After establishing the ROC curve (Receiver Operating Characteristics curve), we found a 15% threshold of DNA fragmentation, separating the patients with infertility from the control group. In our study, a 15% threshold positivity of DNA fragmentation using the TUNEL assay was highly associated with infertility. The results indicate that high level DNA fragmentation is associated with infertility. 展开更多

However, there is no evidence on the impact of aging on semen quality in men residing in Libreville, Gabon. Objective: To determine the effects of age on semen quality of patients visiting the Reproductive Laboratory at the Academic Hospital Mère Enfant located in Libreville, Gabon. Methods: This descriptive and prospective study was performed between the 1st of October 2021 and 30st of September 2022. This study involved patients who had semen analysis as part of fertility check-up. Semen analysis was performed following the World Health Organization, 2010 guidelines. Semen parameters such as volume, leucocytes count, sperm concentration, sperm motility, norml morphology and vitality were used in the current study. Descriptive statistics were performed. The Chi-square test and the Fisher test were used as statistical tools for the analysis of data. The ROC curve was used to illustrate the data. Results: A total of 148 patients were included in the study. Male infertility prevalence was 77.03%. The average age was 41 years with minimum and maximum of 24 and 61 years respectively. Secondary infertility was predominant (66.9%). Patients displaying asthenozoospermia (22.3%) followed by those displaying oligoasthenozoospermia (OAT) isolated (1.4%) or not (11.6%) were the most frequent semen parameter abnormalities. Men aged more than 45 years were 4.4 times likely to have abnormal semen parameters. Odd = 4.4 (IC 95% = [3.3 - 14.7]). Staphylococcus haemolyticus infection was more prevalent. Conclusion: Male age was found to significantly influence semen quality. 展开更多

Objective: To determine whether Oxiplex/AP Gel (FzioMed, San Luis Obispo, CA) was safe and preliminarily effective in reducing postsurgical adhesions after adnexal surgery by laparoscopy. Design: Prospective, multicenter, double-blind, randomized, U.S. Food and Drug Administration-monitored feasibility study. Setting: University and private clinics. Patient (s): Patients undergoing laparoscopic surgery with pelvic adhesions, tubal occlusion, endometriosis, and/or dermoids were randomized to receive Oxiplex/AP Gel or no further treatment after surgery. Intervention(s): A blinded, parallelgroup design was conducted at six centers. Patients (aged 18-46 years) underwent laparoscopic surgery, with secondlook surgery 6-10 weeks later. Surgeries were videotaped. Oxiplex/AP Gel was used to cover adnexa and adjacent tissue. Main Outcome Measure(s): Blinded reviews of videotapes were quantitated with the American Fertility Society adhesion score (AFS score). Result(s): In 18 treatment patients, surgery was performed on 29 adnexa. Application of Oxiplex/AP Gel required approximately 90 seconds. In 10 control patients, surgery was performed on 18 adnexa. The mean baseline AFS score for each group was 8.0. At second look, treated adnexa had the same score (8.1), whereas in control adnexa the score increased (from 8.0 to 11.6). Thirty-four percent of treated adnexa increased in adhesion score, in contrast to 67%of control adnexa. There were no device-related adverse events. Conclusion(s): In this pilot study, Oxiplex/AP Gel was safe, easy to use with laparoscopy, and produced a reduction in the increase of adnexal adhesion scores. 展开更多
