Influence of psychological care on anxiety and depression in older adult patients with coronary heart disease complicated by arrhythmia

BACKGROUND Coronary heart disease(CHD)has shown a consistent upward trend in global incidence in recent years.Notably,older adults with CHD complicated by arrhy-thmia exhibit significantly higher susceptibility to psychological distress com-pared with the general CHD population.This increased vulnerability has garn-ered growing clinical and research interest in the potential therapeutic benefits of structured psychological interventions for alleviating comorbid depressive and anxiety symptoms in this high-risk demographic.AIM To evaluate the efficacy of psychological care in reducing anxiety and depressive symptoms among older adult patients with CHD and comorbid arrhythmia.METHODS This retrospective analysis included 100 patients with CHD and arrhythmia admitted to the First Affiliated Hospital of Jinzhou Medical University from June 2024 to December 2024.Of these,49 patients in the control group received routine care,whereas 51 patients in the observation group received psychological care in addition to routine care.Therapeutic outcomes were compared between the two groups.Psychological distress was assessed before and after providing nursing care.A treatment compliance scale developed by the hospital was used to assess adherence.Complication rates were also compared.Quality of life was evaluated using the Short Form-36 Health Survey after providing nursing care.Patient satisfaction with nursing care was assessed using a self-designed questionnaire.RESULTS The observation group demonstrated a higher overall treatment effectiveness compared with the control group(P<0.05).After nursing care,both groups showed reduced scores on the Self-Rating Anxiety Scale and Self-Rating Depression Scale compared with baseline(P<0.05),with significantly greater improvements in the observation group(P<0.05).Treatment compliance was higher and complication rates were lower in the observation group(P<0.05).Additionally,the observation group demonstrated better quality of life after 1 month of care and higher satisfaction with nursing services(P<0.05).CONCLUSION Psychological care for patients with CHD and comorbid arrhythmia effectively enhanced therapeutic outcomes,reduced anxiety and depression,improved treatment compliance and quality of life,and lowered the risk of complications.These findings support the broader implementation of psychological care for patients with CHD in clinical practice. 展开更多

BACKGROUND Steroids remain the primary treatment for severe alcohol-associated hepatitis(AAH),though there is little available tools to predict patient response to steroids.It was hypothesized that phosphatidylethanol(PEth)value will inversely corre-late with response to steroid therapy based on Lille score in AAH.AIM To assess the relationship of patient factors,focusing on pre-steroid therapy PEth value,to steroid therapy response in AAH.METHODS A retrospective case control study was performed on patients who received≥4 days of steroid therapy for AAH at our hospital between July 1,2019 and June 30,2022.A total of 2087 patients were screened for AAH and those treated with steroids were included for statistical analysis utilizing independent sample t-test and for categorical variables using the χ^(2) test.RESULTS No correlation was found between PEth value,pre-steroids abstinence length,or number of drinks per week pre-steroids and response to steroids.Non-responder status significantly correlated with older age(P=0.024),lower albumin(P=0.003),and higher bilirubin(P=0.010)pre-steroids.Our study suggests that age,pre-steroid albumin,and pre-steroid bilirubin levels may predict nonresponse to steroid therapy.Non-responders have increased incidence of death and higher medical costs.CONCLUSION Identifying non-responders through these identified factors should prompt early referral for liver transplantation.Future prospective studies with larger population size are needed to assess the efficacy of combined pre-steroid age,albumin,bilirubin and other biochemical markers as predictors of steroid response. 展开更多

Guided by the theoretical processes of mindfulness and psychological well-being,this study examined serenity as a mediator between mindfulness facets and psychological well-being,as indexed by depressive symptoms and life satisfaction.Participants were 133 mindfulness practitioners who took part in a 3-day transnational meditation event in Hong Kong.Upon informed consent,participants completed a self-report questionnaire.The findings from structural equation modeling showed that serenity mediated the relation between two facets of mindfulness,including describing and nonreacting to inner experience,and life satisfaction.Serenity also mediated the relation between the mindfulness facet of describing and depressive symptoms.Direct associations were indicated between two mindfulness facets,including observing and nonjudging of inner experience,and depressive symptoms.Taken together,the findings revealed mindfulness facets as major correlates of serenity and psychological outcomes among Chinese meditation practitioners.To foster psychological well-being,researchers,educators,and practitioners should pay attention to the role of serenity,describing,and nonreacting to inner experience in mental health. 展开更多

Neuroimaging studies of cognitive remediation in schizophrenia:A systematic and critical review 被引量:2

AIM To examine the effects of cognitive remediation therapies on brain functioning through neuroimaging procedures in patients with schizophrenia.METHODS A systematic, computerised literature search was conducted in the PubM ed/Medline and PsychI nfo databases. The search was performed through February 2016 without any restrictions on language or publication date. The search was performed using the following search terms: [("cogniti*" and "remediation" or "training" or "enhancement") and("fMRI" or "MRI" or "PET" or "SPECT") and(schizophrenia or schiz*)]. The search was accompanied by a manual online search and a review of the references from each of the papers selected, and those papers fulfilling our inclusion criteria were also included.RESULTS A total of 101 studies were found, but only 18 of them fulfilled the inclusion criteria. These studies indicated that cognitive remediation improves brain activation in neuroimaging studies. The most commonly reported changes were those that involved the prefrontal and thalamic regions. Those findings are in agreement with the hypofrontality hypothesis, which proposes that frontal hypoactivation is the underlying mechanism of cognitive impairments in schizophrenia. Nonetheless,great heterogeneity among the studies was found. They presented different hypotheses, different results and different findings. The results of more recent studies interpreted cognitive recovery within broader frameworks, namely, as amelioration of the efficiency of different networks. Furthermore, advances in neuroimaging methodologies, such as the use of wholebrain analysis, tractography, graph analysis, and other sophisticated methodologies of data processing, might be conditioning the interpretation of results and generating new theoretical frameworks. Additionally, structural changes were described in both the grey and white matter, suggesting a neuroprotective effect of cognitive remediation. Cognitive, functional and structural improvements tended to be positively correlated.CONCLUSION Neuroimaging studies of cognitive remediation in patients with schizophrenia suggest a positive effect on brain functioning in terms of the functional reorganisation of neural networks. 展开更多

Animal experiments have confirmed that mesenchymal stem cells can inhibit motor neuron apoptosis and inflammatory factor expression and increase neurotrophic factor expression. Therefore, mesenchymal stem cells have been shown to exhibit prospects in the treatment of amyotrophic lateral sclerosis. However, the safety of their clinical application needs to be validated. To investigate the safety of intrathecal injection of Wharton's jelly-derived mesenchymal stem cells in amyotrophic lateral sclerosis therapy, 43 patients(16 females and 27 males, mean age of 57.3 years) received an average dose of 0.42 × 106 cells/kg through intrathecal administration at the cervical, thoracic or lumbar region depending on the clinical symptoms. There was a 2 month interval between two injections. The adverse events occurring during a 6-month treatment period were evaluated. No adverse events occurred. Headache occurred in one case only after first injection of stem cells. This suggests that intrathecal injection of Wharton's Jelly-derived mesenchymal stem cells is well tolerated in patients with amyotrophic lateral sclerosis. This study was approved by the Bioethical Committee of School of Medicine, University of Warmia and Mazury in Olsztyn, Poland(approval No. 36/2014 and approval No. 8/2016). Emerging adulthood(EA)is a critical stage of life to develop and sustain a healthy lifestyle,which is also a time of vulnerability to poor physical and mental health outcomes.In this study,we conducted a path analysis(N=1326)to examine associations among four dimensions of EA,levels of regular physical activity(PA),self-control,MPA tendency and irrational procrastination.Results found:1)higher levels of PA predicted both MPA tendency(β=−0.08,95%CI:−0.11 to−0.06,p<0.001)and irrational procrastination(β=−0.01,95%CI:−0.17 to−0.008,p<0.01)indirectly via self-control;2)Instability(β=0.13,95%CI:0.08 to 0.18,p<0.01)and Responsibility(β=−0.06,95%CI:−0.10 to−0.08,p=0.03)exerted direct effects on irrational procrastination and Instability also indirectly predicted irrational procrastination via MPA tendency(β=0.03,95%CI:0.02 to 0.05,p<0.01).These findings proved that perceived features of EA are linked to behavioral problems and supported that regular PA plays a crucial role to protect mental health. 展开更多

The objective of this study was to evaluate the effects of a Pilates exercise program on postural stability and balance confidence in people with IPD. Ten IPD patients (Hoehn & Yahr Stage 1-3) with a history of falls or nearfalls had the following assessments before and after completion of a 6-week supervised Pilates exercise program: Activities-Specific Balance Confidence Scale (ABC);Berg Balance Scale (BBS);Schwab and England Scale (SES);Unified Parkinson’s Disease Rating Scale (UPDRS);pull-test;timed-up-and-go (TUG);5-metre walk;static and dynamic posturography. There were significant improvements in BBS score, 5-metre walk time and TUG after the training program, as well as improvement trends in some posturographic measures. Participants also reported improved balance confidence with Activities of Daily Living (ADLs). Our findings suggest that Pilates therapy can be beneficial in IPD and warrants further evaluation in a larger study. 展开更多

Traditional Chinese Medicine (TCM), a theoretical medical system founded about three thousand years ago, originated from the richness of Chinese culture with the support and development of ancient astronomy and the Ta... Traditional Chinese Medicine (TCM), a theoretical medical system founded about three thousand years ago, originated from the richness of Chinese culture with the support and development of ancient astronomy and the Taoist philosophy of heaven and man. It has revealed many mysteries and illuminated interactions between nature and human beings. Those mysteries have not been easily understood by the public because TCM's conceptual system differs in form and essence from modern sciences. In modern times, some of the mysterious processes have been gradually discovered or confirmed by an advanced science: quantum physics. Quantum science is considered to break through basic concepts of mechanistic science, thus providing an entry to understanding of Chinese culture and TCM theories. Some experimental results of contemporary quantum physics shake the foundations of conventional scientific concepts. For example, quantum mechanics theory helps expand the limits of three-dimensional observations. It helps explain the phenomena that the consciousness with certain human functions interfere some experimental results. Viewing new things in a high-dimensional time-space framework helps people understand the real and virtual characteristics and their interaction;this leads to understanding the function of advanced human wisdom and intelligence in dealing with more complex objects (systems) such as the universe, the human body, life itself and consciousness. It may help people in developing simple models of the complex world and is believed more conducive to grasping the essence of human life and gradually removing the veil over many wonderful phenomena of Chinese culture and TCM. For example, TCM's 'Imagery Analogism' is attained in the quiet state of well-trained meditation practitioners. Their intuition and inspiration have significant advantages in understanding complexity of objects such as the universe, human body, life, and consciousness. The methodology of thinking helps to derive simple models so as to better grasp the essence of complex objects. Many phenomena of Chinese culture and TCM acupuncture remain incompletely understood. This paper selects a few examples to discuss, including human consciousness and sub-consciousness, human body's energy system, invisible qi, functional phenomena as well as special functions of the heart and Triple Energizer (Three Jiao) in six Fu organs. The 'new finding of an unrecognized interstitium in human tissues' as declared by a group of Western scholars is actually referring to Triple Energizer that had already been revealed and written in Huang Di Nei Jing (Yellow Emperor's Classic of Internal Medicine) three thousand years ago. Both the concept and functions of Triple Energizer have been utilized throughout TCM clinical practices. Through the discussion, the authors attempt to explain the superiority of TCM theories, to strengthen the confidence for TCM innovation and Chinese culture because they believe that offering an integration of TCM practice into the public health and life science would advance human wellbeing and pave a right path for TCM mission and development. 展开更多
