All respondents were required to complete a questionnaire covering demographics, the Chinese version of the Rome Ⅲ survey, eating habits, life styles, and medical and family history. The response rate was 94.3% (8106/8600) with 4899 respondents qualified for analysis, including 1046 with offshore training and 3853 with onshore training. The prevalence of UD was higher in the offshore group than in the onshore group (12.6% vs. 6.9%, P〈0.001), with a general prevalence of 8.1%. The subjects with offshore training were more likely to suffer from UD and postprandial distress syndrome (OR= 1.955 95% CI 1.568-2.439, P〈0.001 and OR=1.789, 95% CI 1.403-2.303, P〈0.001, respectively). The mul- tivariate logistic regression analysis showed UD was associated with offshore training (OR=1.580, 95% CI 1.179-2.118, P=-0.002), family history (OR=1.765, 95% CI 1.186-2.626, P=0.005) and smoking (OR=1.270, 95% CI 1.084-1.488, P=0.003), but not with alcohol drinking. The association between dysentery history and UD was undetermined/borderline (P=0.056-0.069). In conclusion, we identified offshore training as a new factor associated with UD, and also confirmed 2 known associated factors, family history and smoking.展开更多

Here, in a single session of reward-based trace conditioning, we examine learning-induced synaptic plasticity in the dorsal CA1 hippocampus (dCA1). Local field-potential recording combined with selective optogenetic inhibition first revealed an increase of dCA1 synaptic responses to the conditioned stimulus (CS) induced during conditioning at both Schaffer collaterals to the stratum radiatum (Rad) and temporoammonic input to the lacunosum moleculare (LMol). At these dCA1 inputs, synaptic potentiation of CS-responding excitatory synapses was further demonstrated by locally blocking NMDA receptors during conditioning and whole-cell recording sensory-evoked synaptic responses in dCA1 neurons from naive animals. An overall similar time course of the induction of synaptic potentiation was found in the Rad and LMol by multiple-site recording;this emerged later and saturated earlier than conditioned behavioral responses. Our experiments demonstrate a cued memory-associated dCA1 synaptic plasticity induced at both Schaffer collaterals and temporoammonic pathways.展开更多

文摘Objective:Baicalein has been reported to have wide therapeutic effects that act through its antiinflammatory activity. This study examines the effect and mechanism of baicalein on sepsis-induced cardiomyopathy(SIC).Methods:A thorough screening of a small library of natural products, comprising 100 diverse compounds, was conducted to identify the most effective drug against lipopolysaccharide(LPS)-treated H9C2 cardiomyocytes. The core target proteins and their associated signaling pathways involved in baicalein's efficacy against LPS-induced myocardial injury were predicted by network pharmacology. Results:Baicalein was identified as the most potent protective agent in LPS-exposed H9C2 cardiomyocytes. It exhibited a dose-dependent inhibitory effect on cell injury and inflammation. In the LPSinduced septic mouse model, baicalein demonstrated a significant capacity to mitigate LPS-triggered myocardial deficits, inflammatory responses, and ferroptosis.Network pharmacological analysis and experimental confirmation suggested that hypoxia-inducible factor 1 subunit a(HIF1-a)is likely to be the crucial factor in mediating the impact of baicalein against LPS-induced myocardial ferroptosis and injury.By combining microRNA(miRNA)screening in LPS-treated myocardium with miRNA prediction targeting HIF1-a,we found that miR-299b-5p may serve as a regulator of HIF1-a.The reduction in miR-299b-5p levels in LPS-treated myocardium, compared to the control group, was reversed by baicalein treatment. The reverse transcription quantitative polymerase chain reaction,Western blotting, and dual-luciferase reporter gene analyses together identified HIF1-a as the target of miR-299b-5p in cardiomyocytes. Conclusion:Baicalein mitigates SIC at the miRNA level, suggesting the therapeutic potential of it in treating SIC through the regulation of miR-299b-5p/HIF1-a/ferroptosis pathway.
文摘BACKGROUND Most patients with non-alcoholic fatty liver disease(NAFLD)exhibit mild symptoms;however, without timely intervention, the condition may progress to cirrhosis or even liver cancer. The development of internet-based proactive follow-up management models has provided new avenues for medical services, allowing patients to access online consultations and enabling doctors to efficiently manage patient information. These models have particular significance for NAFLD patients,as they can enhance compliance and facilitate timely intervention.AIM To explore the effect of an internet-based proactive follow-up management model on the prognosis management of patients with NAFLD.METHODS This study collected data from 145 patients diagnosed with NAFLD.The patients were randomly divided into the internet-based proactive follow-up group(71 patients)and the traditional follow-up group(74 patients). The traditional followup group underwent routine outpatient and telephone follow-up,while the internet-based proactive follow-up group used the WeChat applet for active reminding, health education and follow-up.During the follow-up period, patients’compliance, disease control status, and quality of life score were recorded.RESULTS The follow-up compliance of patients in the internet-based proactive follow-up group was significantly higher than that in the traditional follow-up group(85.6%vs 68.4%,P<0.01),and the quality of life score was significantly improved(78.9±7.6 vs 72.5±8.4,P<0.01).CONCLUSION The internet-based proactive follow-up management model can provide personalized health management plans and reduce the risk of disease deterioration, making it worthy of promotion in a broader healthcare context.
文摘BACKGROUND The frequent occurrence of respiratory diseases in the island reef environment of the navy severely affects the health of personnel and the combat effectiveness of the troops.Current common screening methods can only indicate whether there is an infection with pathogenic microorganisms but not the degree of disease progression. Therefore,it is necessary to identify simple-to-operate and cost-effective methods that indicate the degree of disease progression, based on traditional screening methods.AIM To explores correlation between serum concentrations of nicotinamide phosphoribosyltransferase(NAMPT),nicotinamide nucleotide adenylyltransferase 1(NMNAT1),and the risk of upper respiratory infections in the island reef envirsonment.METHODS A total of 600 cases of upper respiratory infections among naval officers and soldiers were included.Types of infection were confirmed through sputum culture combined with multiplex polymerase chain reaction. The serum concentrations of NAMPT and NMNAT1 were measured using ELISA,and infection severity was assessed using the pneumonia severity index(PSI).Statistical analysis was performed using nonparametric tests and Spearman correlation analysis.RESULTS The serum concentrations of NAMPT and NMNAT1 in high-risk group patients with PSI were significantly lower than those in the medium and low-risk groups(P<0.05),and the concentrations increased in a stepwise manner with disease progression. However, within the same risk group, the differences in concentrations of NAMPT and NMNAT1among patients infected with different pathogens were not significant(P>0.05).CONCLUSIONConcentrations of NAMPT and NMNAT1 are closely related to severity of upper respiratory infections, and theircommon regulatory mechanisms provide new directions for development of broad-spectrum anti-infectionstrategies.
文摘BACKGROUND Patients with chronic hepatitis B(CHB)require long-term antiviral therapy. The effects of different antiviral drugs on kidney function are unclear. There is a lack of effective markers for monitoring early renal impairment.AIM To investigate the rate of abnormal renal function index and related potential hazards in patients with CHB.METHODS Clinical data of patients with CHB with urinaryβ2-microglobulin(β2-M)detec-tion,including demographic characteristics, hepatitis B virus(HBV)DNA,serum liver function(alanine aminotransferase, aspartate aminotransferase, total bilirubin, direct bilirubin),serum renal function(urea nitrogen, creatinine),blood lipid index(high density lipoprotein, low density lipoprotein,cholesterol,trigly-ceride),liver imaging, and other routine tests were retrospectively collected. The normal level of urinaryβ2-M and estimated glomerular filtration rate(eGFR)is defined as<0.173 mg/L and≥90 mL/min/1.73 m^(2),retrospectively.The pro-portion of patients with abnormal renal function index and related risk factors were analyzed.RESULTS A total of 500 patients with CHB were enrolled;these patients were aged 44.7±10.8 years,67.2%(336/500)were male,57.2%(286/500)were treated with anti-viral drugs, and 52.2%(261/500)had an HBV-related family history. In total,28.8%(144/500)of patients had fatty liver,35.0%(175/500)had liver fibrosis, and 13.2%(66/500)had cirrhosis. The proportion of patients with eGFR<90 mL/min/1.73 m^(2) was 43.2%(216/500),and the abnormal rate of urinaryβ2-M was 56.2%(281/500).There was no significant difference in the abnormal rate of urinaryβ2-M between the untreated group and the antiviral treated group(54.2%vs 57.7%;P=0.25).The abnormal rate ofβ2-M after long-term entecavir treatment(more than 1 year)was 54.6%(89/163).In the treatment group,56.4%(92/163)of patients with eGFR≥90 mL/min/1.73 m^(2) had abnormal urinaryβ2-M.CONCLUSION In patients with CHB,a higher proportion had greater urinaryβ2-M levels than eGFR for renal injury.Male patients should pay more attention to renal function and use antiviral regimens with a renal safety profile.
文摘A decentralized network made up of mobile nodes is termed the Mobile Ad-hoc Network(MANET).Mobility and a finite battery lifespan are the two main problems with MANETs.Advanced methods are essential for enhancing MANET security, network longevity, and energy efficiency.Hence, selecting an appropriate cluster. The cluster’s head further boosts the network’s energy effectiveness. As a result,a Hybrid Swallow Swarm Optimisation-Memetic Algorithm(SSO-MA)is suggested to develop the energy efficiency&of the MANET network. Then,to secure the network Abnormality Detection System(ADS)is proposed. The MATLAB-2021a platform is used to implement the suggested technique and conduct the analysis. In terms of network performance, the suggested model outperforms the current Genetic Algorithm,Optimised Link State Routing protocol, and Particle Swarm Optimisation techniques. The performance of the model has a minimum delay in the range of 0.82 seconds and a Packet Delivery Ratio(PDR)of 99.82%.Hence,the validation shows that the Hybrid SSO-MA strategy is superior to the other approaches in terms of efficiency.

文摘Governments worldwide have implemented non-pharmaceutical interventions(NPIs)to control the spread of coronavirus disease 2019(COVID-19),and it is crucial to accurately assess the effectiveness of such measures. Many studies have quantified the risk of infection transmission and used simulations to compare the risk before and after the implementation of NPIs to judge policies’effectiveness. However, the choice of metric used to quantify the risk can lead to different conclusions regarding the effectiveness of a policy. In this study,we analyze the correlation between different transmission-risk metrics, pedestrian environments, and types of infectious diseases using simulation-generated data. Our findings reveal conflicting results among five different metric types in specific environments.More specifically,we observe that, when the randomness of pedestrian trajectories in indoor spaces is low,the closeness centrality exhibits a higher correlation coefficient with infection-based metrics than with contact-based metrics.Furthermore, even within the same pedestrian environment, the likelihood of discrepancies between infection-based metrics and other metrics increases for infectious diseases with low transmission rates. These results highlight the variability in the measured effectiveness of NPIs depending on the chosen metric. To evaluate NPIs accurately, facility managers should consider the type of facility and infectious disease and not solely rely on a single metric. This study provides a simulation model as a tool for future research and improves the reliability of pedestrian-simulation-based NPI effectiveness analysis methods.

文摘The prevalence of mitral regurgitation(MR)is increasingly worldwide as a consequence of improved survival and ischemic heart disease.Severe MR is associated with worse prognosis, independent of left ventricular function and associated comorbidities.[1-3]Although surgical valve repair or replacement remains the standard treatment for patients with chronic MR,up to 50%of MR patients with symptomatic MR are not referred for surgery, mainly because of multiple comorbid conditions resulting in high surgical risk.[4]There is,therefore,a clear need for less invasive approaches for this patient group.

文摘Objective The geriatric nutritional risk index(GNRI)is widely used for nutritional assessment.Poor nutritional status is associated with complications and poor survival in cirrhotic patients. We aimed to investigate the value of the GNRI in predicting outcomes in cirrhotic patients. Methods This retrospective study included 420 cirrhotic patients from three centers between 2013 and 2017.Patients were divided into the high GNRI group(≥92)and low GNRI group(<92).Overall survival(OS)in the two groups was evaluated via the Kaplan‒Meier method.Cox proportional hazards model was used to estimate the value of the GNRI in predicting outcomes.Restricted cubic spline model was used to intuitively display the dose‒response associations between the GNRI and OS.A nomogram was constructed to predict OS.Results During the 2-year follow-up period,58(13.81%)patients died, and 262(62.38%)patients experienced episodes of complications.Compared with patients in the low GNRI group, those in the high GNRI group had lower mortality rates(18.73%vs.5.23%,P<0.001).The GNRI was an independent predictor of OS(hazard ratio[HR]=0.958,95%confidence interval[CI]0.929–0.988,P=0.007).The GNRI was associated with the cumulative incidence of ascites(HR=0.954,95%CI 0.940–0.969,P<0.001),spontaneous bacterial peritonitis(HR=0.928,95%CI 0.891–0.966,P<0.001),hepatic encephalopathy(HE;HR=0.944,95%CI 0.920–0.968,P<0.001),and hepatorenal syndrome(HRS)(HR=0.916,95%CI 0.861–0.974,P=0.005).Furthermore,6 independent factors were included to construct the nomogram for OS prediction, including GNRI,age, total bilirubin, serum sodium, history of HE and HRS.The C statistics of our model were 0.83(95%CI 0.75–0.90)and 0.80(95%CI 0.73–0.86)at 1 and 2 years, respectively.Patients whose GNRI score decreased within 3 and 6 months had poorer outcomes(P<0.001).Conclusions The lower GNRI score was associated with the higher cumulative incidence of complications and poorer OS of cirrhotic patients. The GNRI could be a helpful tool for assessing nutritional status and prognosis of these patients.

文摘Osteosarcoma(OS)is the most prevalent type of primary malignant bone cancer and currently lacks effective targeted treatments.Increasing evidence indicates that SOX2 overexpression is a primary driver of OS.By screening a small-molecule kinase inhibitor library,we identified AKT as a kinase essential for robust SOX2 expression in OS cells.AKT was found to be frequently overexpressed in OS and positively correlated with SOX2 protein levels. We demonstrated that AKT has no effect on SOX2 transcription but promotes SOX2 protein stability.Mechanistically,AKT binds to and phosphorylates SOX2 at T116,preventing SOX2 ubiquitination and proteasome-dependent degradation by ubiquitin E3 ligases UBR5 and STUB1.Moreover,we found that AKT-SOX2 axis is a significant modulator of cancer stemness and chemoresistance and that the combination of AKT inhibitor MK2206 and cisplatin resulted in a synergistic and potent inhibition of OS tumor growth in the PDX model. In conclusion,we identified a critical role for AKT in promoting SOX2 overexpression, tumor stemness, and chemoresistance in OS,and provided evidence that targeting AKT combined with chemotherapy may hold promise for treating refractory OS.

文摘Modifications in fuel spray characteristics fundamentally influence fuel–air mixing dynamics in diesel engines, thereby significantly affecting combustion performance and emission profiles. This study explores the operational behavior of RP-5 aviation kerosene/diesel blended fuels in marine diesel engines.A spray visualization platform based on Mie scattering technology was developed to comparatively analyze the spray characteristics, ignition behavior, and soot emissions of RP-5 aviation kerosene,conventional-35#diesel,and their blends at varying mixing ratios(D100H0,D90H10,D70H30,D50H50,D30H70,D0H100).The findings demonstrate that, under constant injection pressure, aviation kerosene combustion results in a more uniform temperature field, characterized by lower core flame temperatures, broader high-temperature regions, and reduced soot concentrations with spatially homogeneous distribution and no pronounced peaks. In terms of spray dynamics, increasing the proportion of aviation kerosene leads to a marked widening of the spray cone angle.Meanwhile, spray penetration length exhibits a non-monotonic trend—initially decreasing and subsequently increasing—as the kerosene blending ratio rises.

文摘Zn–Ag alloys are highly promising materials for fabricating biodegradable orthopedic implants.Nonetheless, they suffer from inferior strength.A Zn–2.5Ag alloy was alloyed with different contents of Mg(0.08,0.5,and 1 wt.%)and then processed by equal channel angular pressing.Tensile tests and microstructure observation were conducted to investigate the impact of Mg addition on the microstructure and mechanical properties of Zn–2.5Ag alloy.Zn–2.5Ag alloy exhibits an ultrahigh elongation(EL)of 120.4%but a poor yield strength(YS)of 90.1 MPa,because of phase boundary sliding(PBS).Mg addition inhibits PBS and thus dramatically enhances YS but lowers EL.Specifically,YS of Zn–2.5Ag alloys containing Mg of 0.08%,0.5%,and 1%is 257.0,291.8,and 322.6 MPa,respectively. The alloys with 0.08%and 0.5%Mg possess an EL of around 30%,while the alloy with 1%Mg has an EL of only 11.0%.YS and EL of Zn–2.5Ag–0.5Mg alloy surpass that needed by orthopedic implants by 45.9%and 106.0%,respectively.Grain refinement strengthening is the main contributor to high strength. It is speculated that deformation twinning suppression and<c+a>pyramidal slip activation contribute to good ductility.
文摘Objectives:Although Yes-associated protein 1(YAP1)is an important oncogene in hepatocellular carcinoma(HCC)progression, its nuclear localization prevents it from being considered a potential therapeutic target.Recently, studies have reported that coatomer protein complex subunit beta 2(COPB2)also plays a critical role in HCC development;however its mechanism of action is unclear. This study aimed to investigate the role of COPB2 and YAP1 in the progression of HCC and to elucidate the underlying mechanisms.Methods:COPB2 and YAP1 expression in HCC tissues were first analyzed by database searches and immunohistochemistry.Nomogram and artificial neural network models were established based on COPB2 and YAP1 expression.Cell proliferation was detected by cell counting kit-8 and clone formation assay, while cell migration and invasion were assessed using Transwell assays. Finally, the potential mechanisms underlying COPB2 regulation of YAP1 nuclear translocation were explored by immunofluorescence assay and Western blot.Results:COPB2 combined with YAP1 expression was associated with overall postoperative survival in HCC patients and was an independent prognostic factor.High expression of both COPB2 and YAP1 in patients may reduce the efficacy of postoperative transarterial chemoembolization therapy. In vitro experiments revealed that COPB2 affected the sensitivity of HCC cells to Cisplatin(DDP)by regulating YAP1 nuclear translocation. Conclusions:Our findings suggest that COPB2/YAP1 affects the drug sensitivity of HCC cells to DDP and that targeting COPB2/YAP1 may be a promising strategy for the precision treatment of HCC.
文摘Urban expansion in India’s metropolitan regions has led to significant alterations in land surface composition, which directly affect local thermal environments.Vegetation loss and the emergence of bare land surfaces are increasingly recognized as key contributors to urban heat, yet comparative,multi-city studies addressing their combined effects remain scarce. This study analyzes the influence of vegetation and bare land on land surface temperature(LST)across three major Indian cities—Delhi,Lucknow, and Ahmedabad in 2023.Satellite imagery was used to extract Normalized Difference Vegetation Index(NDVI),Normalized Difference Bareness Index(NDBaI),and LST values.Statistical correlation and spatial analysis techniques were applied to evaluate thermal variations across land cover types.NDVI was negatively correlated with LST(r=−0.68 to−0.81),indicating the cooling role of vegetation, whereas NDBaI showed a positive correlation with LST(r=0.59 to 0.74),highlighting the warming effect of bare surfaces.Delhi exhibited the highest maximum LST(47.45℃),while Lucknow recorded the highest minimum(38.63℃).Across all cities and timeframes, vegetated areas consistently showed lower surface temperatures compared to bare or built-up regions. The findings emphasize the importance of vegetation in reducing urban heat and the thermal risk posed by increasing bare land.Strengthening green infrastructure and minimizing exposed soil in urban areas can serve as effective strategies for enhancing thermal comfort and climate resilience in Indian cities.

文摘Learning-associated functional plasticity at hippocampal synapses remains largely unexplored. Our experiments demonstrate a cued memory-associated dCA1 synaptic plasticity induced at both Schaffer collaterals and temporoammonic pathways.

文摘Data compression plays a vital role in datamanagement and information theory by reducing redundancy. However,it lacks built-in security features such as secret keys or password-based access control, leaving sensitive data vulnerable to unauthorized access and misuse. With the exponential growth of digital data, robust security measures are essential.Data encryption,a widely used approach, ensures data confidentiality by making it unreadable and unalterable through secret key control.Despite their individual benefits, both require significant computational resources.Additionally, performing them separately for the same data increases complexity and processing time.Recognizing the need for integrated approaches that balance compression ratios and security levels, this research proposes an integrated data compression and encryption algorithm, named IDCE,for enhanced security and efficiency.Thealgorithmoperates on 128-bit block sizes and a 256-bit secret key length. It combines Huffman coding for compression and a Tent map for encryption.Additionally,an iterative Arnold cat map further enhances cryptographic confusion properties.Experimental analysis validates the effectiveness of the proposed algorithm, showcasing competitive performance in terms of compression ratio, security, and overall efficiency when compared to prior algorithms in the field.
