Background Mental health problems are prevalent among patients seeking primary healthcare.However,traditional patient interviews often fail to identify and manage these problems.Therefore,an improved patient interview...Background Mental health problems are prevalent among patients seeking primary healthcare.However,traditional patient interviews often fail to identify and manage these problems.Therefore,an improved patient interview process is needed to assess and treat mental health problems in primary care settings.Aims To assess the validity,reliability,sensitivity and specificity of a 5-Step Patient Interview approach for the screening,diagnosis and treatment of mental health problems.Design and setting This study compared a 5-Step Patient Interview approach(AlKhathami approach)with expert psychiatric interviews based on the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders,fifth edition,the Patient Health Questionnaire-9(PHQ-9)and the Generalised Anxiety Disorder-7(GAD-7)questionnaire from 1 January 2020 to 30 April 2020 in family practice or primary healthcare centres.Methods A total of 760 participants from five Saudi Arabian regions were selected using a multiclustered random sample of every third patient aged≥18 years,drawn from the clinics’patient list.Results A total of 732 patients agreed to participate,with a response rate of 96.3%;396(54.1%)were women;the mean(Standard Deviation)age was 41.28(14.30)years(ranging from 18 to 84 years).Mental health problems were suspected in 40%of the participants.The 5-Step Patient Interview approach was shown to be comparable to an expert interview by a psychiatrist.Moreover,it was more accurate than the PHQ-9 and GAD-7 self-administered questionnaires in screening for stress and classifying it as mild,moderate to severe or severe.Additionally,the 5-Step Patient Interview approach improved physician-patient communication by encouraging the exploration of patients’perspectives.Conclusions The 5-Step Patient Interview approach is a valid,reliable tool that can aid the integration of mental healthcare into primary healthcare and family practice.Future studies should evaluate the implementation outcomes of the 5-Step Patient Interview approach.展开更多
Morphological analyses are key outcome assessments for nerve regeneration studies but are historically limited to tissue sections.Novel optical tissue clearing techniques enabling three-dimensional imaging of entire o...Morphological analyses are key outcome assessments for nerve regeneration studies but are historically limited to tissue sections.Novel optical tissue clearing techniques enabling three-dimensional imaging of entire organs at a subcellular resolution have revolutionized morphological studies of the brain.To extend their applicability to experimental nerve repair studies we adapted these techniques to nerves and their motor and sensory targets in rats.The solvent-based protocols rendered harvested peripheral nerves and their target organs transparent within 24 hours while preserving tissue architecture and fluorescence.The optical clearing was compatible with conventional laboratory techniques,including retrograde labeling studies,and computational image segmentation,providing fast and precise cell quantitation.Further,optically cleared organs enabled three-dimensional morphometry at an unprecedented scale including dermatome-wide innervation studies,tracing of intramuscular nerve branches or mapping of neurovascular networks.Given their wide-ranging applicability,rapid processing times,and low costs,tissue clearing techniques are likely to be a key technology for next-generation nerve repair studies.All procedures were approved by the Hospital for Sick Children’s Laboratory Animal Services Committee(49871/9)on November 9,2019.展开更多

The potential treatments have focused...Despite many decades of researches and large numbers of clinical trials, there remain no effective treatments for Alzheimer’s disease, a major degenerative ageing brain disorder. The potential treatments have focused on targeting the accumulation of amyloid beta-peptide in the brains of patients, but without success in slowing the disease. Many studies have now identified a large range of pathological changes (<em>i.e.</em> altered immune activity, mitochondrial impairment, abnormal microbiome), and links to the external environment (<em>i.e.</em> associations with infections, the influence of air pollution). While the concept of One Health (which considers links between the environment and human disease) has traditionally been applied to the understanding of the human infectious disease, it is argued here that the One Health approach should be adopted for Alzheimer’s disease. This would provide a far more holistic understanding of the disease, and its relationship to a growing number of exogenous factors, as well as could potentially lead to new treatment options targeted at the confluence of external influences, and internal molecular pathways.展开更多

Axonal regeneration following surgical nerve repair is slow and often incomplete,resulting in poor functional recovery which sometimes contributes to lifelong disability.Currently,there are no FDA-approved therapies a...Axonal regeneration following surgical nerve repair is slow and often incomplete,resulting in poor functional recovery which sometimes contributes to lifelong disability.Currently,there are no FDA-approved therapies available to promote nerve regeneration.Tacrolimus accelerates axonal regeneration,but systemic side effects presently outweigh its potential benefits for peripheral nerve surgery.The authors describe herein a biodegradable polyurethane-based drug delivery system for the sustained local release of tacrolimus at the nerve repair site,with suitable properties for scalable production and clinical application,aiming to promote nerve regeneration and functional recovery with minimal systemic drug exposure.Tacrolimus is encapsulated into co-axially electrospun polycarbonate-urethane nanofibers to generate an implantable nerve wrap that releases therapeutic doses of bioactive tacrolimus over 31 days.Size and drug loading are adjustable for applications in small and large caliber nerves,and the wrap degrades within 120 days into biocompatible byproducts.Tacrolimus released from the nerve wrap promotes axon elongation in vitro and accelerates nerve regeneration and functional recovery in preclinical nerve repair models while off-target systemic drug exposure is reduced by 80%compared with systemic delivery.Given its surgical suitability and preclinical efficacy and safety,this system may provide a readily translatable approach to support axonal regeneration and recovery in patients undergoing nerve surgery.展开更多

The aims of the study were to id...Schizophrenia is classified as a priority mental disorder by the World Health Organization (WHO) and accounts for around 35% of diagnoses at the Bingerville Psychiatric Hospital (HPB). The aims of the study were to identify the cost drivers for hospitalization and to calculate the costs of managing schizophrenia in hospital, with a view to planning household expenditure on care. This pilot cross-sectional study involved 31 patients with schizophrenia who had been hospitalized in the various third-category wards at the HPB between 1st January 2019 and 31st May 2020. Sampling was accidental. The methods used to estimate costs were based on the actual costs of drugs, hospitalization and additional examinations which prices were known, and on patients’ estimations for certain expenses such as food and transport. Results: The sex ratio was 3.42, the mean age was 29.52 years. The mean length of stay was 46.19 days, and the most frequent clinical forms were paranoid schizophrenia (41.9%) and schizoaffective disorder (29%). The combination of haloperidol and chlorpromazine was the most common medications for initial treatment (67.8%) and maintenance treatment (41.9%). The average cost of hospitalization at HPB for schizophrenia was XOF 164,412 (€249.90). The average direct medical cost was XOF 105,412 (€160.226) and the average direct non-medical cost was XOF 59,000 (€89.68). The average daily cost of antipsychotic treatment was XOF 795/day (€1.2084). The high cost of drugs as a proportion of hospitalization costs suggested the need of a reflection on the simplification of prescribing practices, assistance in psychiatric emergencies and the development of other alternatives to psychiatric hospitalization in Côte d’Ivoire.展开更多

The purpose of this study was to evaluate the characteristic features of psychotropic drug prescriptions for patients with schizophrenia in Pakistan. Methods: Three centers in Pakistan participated in a large scale collaborative study known as Research on Asian Prescription Pattern (REAP). The 2016 REAP survey included centers from 15 countries in Asia and used a unified research protocol. The design of the study was quantitative and of descriptive epidemiology. Analysis was made on the data collected from three centers i.e., Lahore, Karachi and Islamabad. The data collected in Pakistan were compared with those from other Asian countries. The details of REAP were presented on the homepage of REAP (http://www.REAP.Asia). Results: From Pakistan, 298 patients were included. Patients with schizophrenia in Pakistan received higher rate of antipsychotic polypharmacy and a higher rate of co-prescription of mood stabilizers and anxiolytics when compared with participants from other South Asian countries (India and Bangladesh). Conclusion: The main findings of the study were that a majority of the patients were prescribed antipsychotic polypharmacy drugs in Pakistan. Insufficient training on the use of psychotropic drugs and a lack of treatment guideline are considered to be the major contributing factors. Further education and training on the proper use of psychotropic drugs are recommended to psychiatrists in Pakistan. Few previous studies have investigated the prevalence of somatic diseases in depression or the prescription pattern of antidepressants in comorbidly depressed patients in Asia. This study aimed to investigate the prevalence of somatic comorbidity (SC) in depression and compared the prescriptions of antidepressants in depressed patients with and without SC. Methods: A total of 2320 patients treated with antidepressants in 8 Asian countries were examined, and a diagnosis was based on the International Classification of Disease, 10^th revision. We listed 17 common chronic somatic diseases. Patients' socio-demographic and clinical characteristics and psychotropic drug prescriptions were recorded using a standardized protocol and data collection procedure. Results: Of the patients examined, 1240 were diagnosed with depression and 30% of them (n = 375) had SC. The most common comorbid condition was diabetes (23.7%). The patients with SC were more likely to seek help at a general hospital (74.7% vs. 47.2%), and had a higher incidence of symptoms involving sadness, disturbed sleep, and poor appetite. Noradrenergic and specific serotonergic antidepressant was prescribed more for patients with SC than for those without SC (30.4% vs, 22.9%). Conclusions: SC is common in depressed Asian patients. It is important to strengthen the recognition of depression, especially in general hospitals and when patients report some somatic discomfort. It is also a matter of urgency to establish evidence-based guidelines for the use of new antidepressants in depressed patients with SC.展开更多
Introduction Hematopoietic stem and progenitor cells(HSPC)are responsible for the reconstitution of blood cells and thought to contribute to peripheral tissue repair.Following acute exercise in young adults,HSPC are m...Introduction Hematopoietic stem and progenitor cells(HSPC)are responsible for the reconstitution of blood cells and thought to contribute to peripheral tissue repair.Following acute exercise in young adults,HSPC are mobilized from their niche in bone marrow into circulation,however the kinetics of mobilization following exercise is not well understood in older adults.In the present study,we aimed to investigate how exercise intensity influences mobilization of specific subpopulations of HSPC,and how mobilization is affected by aging.Methods Healthy older men(OM;69.9±2.0 years)and young men(YM;21.5±0.8 years)performed three separate bouts of exercise on a cycle ergometer:70%of their peak work rate(WRpeak)until volitional fatigue,30%of their WRpeak work matched to the 70%WRpeak trial,and a high intensity interval training(HIIT)trial.Blood samples were collected before,immediately post,and 10,30,and 60 min post-exercise.Total blood cells,hematocrit,and mononuclear cells isolated by density gradient centrifugation were quantified using flow cytometry.Results Mononuclear cells,CD34+,CD34+/CD38+,CD34+/CD110+,CD3+/CD(16+56)+,CD11c+/CD123-,and CD11c-/CD123+cells per millilitre of blood increased immediately post-exercise,but predominately in the 70%WRpeak trial in both OM and YM.Conclusion Taken together,our evidence suggests that higher intensity exercise but also volume is necessary for the mobili-zation of HSPC.Furthermore,the pattern of mobilization was nearly identical in OM as compared to YM though to a lesser extent than their younger counterparts.展开更多

文摘Introduction Hematopoietic stem and progenitor cells(HSPC)are responsible for the reconstitution of blood cells and thought to contribute to peripheral tissue repair.Following acute exercise in young adults,HSPC are mobilized from their niche in bone marrow into circulation,however the kinetics of mobilization following exercise is not well understood in older adults.In the present study,we aimed to investigate how exercise intensity influences mobilization of specific subpopulations of HSPC,and how mobilization is affected by aging.Methods Healthy older men(OM;69.9±2.0 years)and young men(YM;21.5±0.8 years)performed three separate bouts of exercise on a cycle ergometer:70%of their peak work rate(WRpeak)until volitional fatigue,30%of their WRpeak work matched to the 70%WRpeak trial,and a high intensity interval training(HIIT)trial.Blood samples were collected before,immediately post,and 10,30,and 60 min post-exercise.Total blood cells,hematocrit,and mononuclear cells isolated by density gradient centrifugation were quantified using flow cytometry.Results Mononuclear cells,CD34+,CD34+/CD38+,CD34+/CD110+,CD3+/CD(16+56)+,CD11c+/CD123-,and CD11c-/CD123+cells per millilitre of blood increased immediately post-exercise,but predominately in the 70%WRpeak trial in both OM and YM.Conclusion Taken together,our evidence suggests that higher intensity exercise but also volume is necessary for the mobili-zation of HSPC.Furthermore,the pattern of mobilization was nearly identical in OM as compared to YM though to a lesser extent than their younger counterparts.
