METHODS: Between July 2007 and March 2009, 27 consecutive superficial esophageal squamous cell neop...AIM: To evaluate the efficacy of endoscopic submucosal dissection for superficial esophageal squamous cell neoplasms. METHODS: Between July 2007 and March 2009, 27 consecutive superficial esophageal squamous cell neoplasms in 25 enrolled patients were treated by endoscopic submucosal dissection. The therapeutic efficacy, complications, and follow-up results were assessed. RESULTS: The mean size of the lesions was 21 ± 13 mm (range 2-55 mm); the mean size of the resection specimens was 32 ± 12 mm (range 10-70 mm). The enblock resection rate was 100% (27/27), and en block resection with tumor-free lateral/basal margins was 88.9% (24/27). Perforation occurred in 1 patient who was managed by conservative medical treatments. None of the patients developed local recurrence or distant metastasis in the follow-up period. CONCLUSION: Endoscopic submucosal dissection is applicable to superficial esophageal squamous cell neoplasms with promising results.展开更多

In the report, we defined the tree-like appearance (TLA) on the images of abnormal blood vessels which resembled branches from the trunk of a tree in the shiny mucosa, in which the glandular structure was lost. The 67-year-old female was diagnosed with gastric MALT lymphoma. The patient received eradication therapy for H. pylori. Conventional endoscopy revealed multiple ill-delineated brownish depressions in the stomach and cobblestonelike mucosa was observed at the greater curvature to the posterior wall of the upper gastric body 7 mo after successful eradication. Unsuccessful treatment of gastric MALT lymphoma was suspected on conventional endoscopy. Conventional endoscopic observations found focal depressions and cobblestone-like appearance, and these lesions were subsequently observed using magnified endoscopy combined with narrow band imaging to identify abnormal vessels presenting with a TLA within the lesions. Ten biopsies were taken from the area where abnormal vessels were present within these lesions. Ten biopsies were also taken from the lesions without abnormal vessels as a control. A total of 20 biopsy samples were evaluated to determine whether the diagnosis of MALT lymphoma could be obtained histologically from each sample. A positive diagnosis was obtained in 8/10 TLA (+) sites and in 2/10 TLA(-) sites. Target biopsies of the site with abnormal blood vessels can potentially improve diagnostic accuracy of gastric MALT lymphoma.展开更多

Narrow band imaging(NBI) is a new image enhancement system employing optic digital methods to enhance images of blood vessels on mucosal surfaces,allowing improved visualization of mucosal surface structures.Studies h...Narrow band imaging(NBI) is a new image enhancement system employing optic digital methods to enhance images of blood vessels on mucosal surfaces,allowing improved visualization of mucosal surface structures.Studies have progressed over the last several years,and the clinical usefulness has been demonstrated.NBI has become frequently applied for preoperative diagnosis before endoscopic submucosal dissection(ESD) of digestive tract cancers,as well as for assessment of the range of ESD for en-bloc resection of large lesions.Consensus has been reached with regard to the usefulness of NBI for detecting micro-lesions of esophageal squamous cell carcinoma indicated for ESD,for the diagnosis of the range and depth.NBI has also been attracting attention for diagnosing gastric cancer based on the observation of micro blood vessels on the mucosal surface and mucosal surface microstructures.The usefulness of NBI has been reported in relation to various aspects of colon cancer,including diagnoses of the presence,quality,range,and depth of lesions.However,as NBI has not surpassed diagnostic methods based on magnifying observation combined with the established and widely employed dye method,its role in ESD is limited at present.Although NBI is very useful for the diagnosis of digestive tract cancers,comprehensive endoscopic diagnosis employing the combination of conventional endoscopy including dye spraying,EUS,and NBI may be important and essential for ESD.展开更多

In the early invasive area of a rectal adenocarcinoma reported here, an obvious transition between the muscularis mucosa and the bundles of eosinophilic stromal cells observed in the carcinomatous stroma was demonstrated both in morphology and in their cytoskeletal phenotype, which conceivably suggests that the smooth muscle cells of the muscularis mucosa could convert to the eosinophilic stromal cells, namely myofibroblasts. Moreover, type I procollagen was demonstrated in both protein and mRNA levels in the areas of eosinophilic stromal cells with a lesser degree of differentiated smooth muscle phenotype that showed a transition from the muscularis mucosa, implying that the myofibroblastic cells converted from smooth muscle cells of the muscularis mucosa could be responsible for type I collagen production. These findings suggest that the muscularis mucosae may not be a passive barrier through which colorectal carcinomas infiltrate into the submucosa, but may play an active role in the formation and remodeling of tumor stroma.展开更多

METHODS: This ...AIM: To investigate whether magnifying endoscopy with narrow band imaging (ME-NBI) is useful for evaluating the area of superficial, depressed- or flat-type differentiated adenocarcinoma of the stomach. METHODS: This procedure was performed in Saitama Medical University International Medical Center, Japanese Red Cross Kumamoto Hospital and Kitakyushu Municipal Medical Center. The subjects were 31 patients in whom biopsy findings, from superficial, depressed- or flat-type gastric lesion, suggested differentiated adenocarcinoma in the above 3 hospitals between January and December 2009. Biopsy was performed on the lesion and non-lesion sides of a boundary (imaginary boundary) visualized on ME-NBI. The results were pathologically investigated. We evaluated the accuracy of estimating a demarcation line (DL) on ME-NBI in comparison with biopsy findings as a gold standard. RESULTS: The DL that could be recognized at 2 points on the orifice and anal sides of each lesion during ME-NBI was consistent with the pathological findings in 22 patients with 0-IIc lesions, 7 with 0-IIb lesions, and 2 with 0-IIb + IIc lesions, showing an accuracy of 100%. CONCLUSION: The results suggest the usefulness of ME-NBI for evaluating the area of superficial, depressed- and flat-type differentiated adenocarcinoma of the stomach.展开更多

ESD is an invasive endoscopic surgical procedure, requiring extensive knowledge, skill, and specialized equipment. ESD starts with evaluation of the lesion, as accurate assessment of the depth and margin of the lesion is essential. The devices and strategies used in ESD vary, depending on the nature of the lesion. Prior to the procedure, the operator must be knowledgeable about the treatment strategy(ies), the device(s) to use, the electrocautery machine settings, the substances to inject, and other aspects. In addition, the operator must be able to manage complications, should they arise, including immediate recognition of the complication(s) and its treatment. Finally, in case the ESD treatment is not successful, the operator should be prepared to apply alternative treatments. Thus, adequate knowledge and training are essential to successfully perform ESD.展开更多

The COVID-19 pandemic is a novel virus that is highly infectious and has several negative psychological impacts on individuals globally. </span><b><span style="font-family:Verdana;">Aim:</span></b><span style="font-family:Verdana;"> Coronavirus disease 2019 (COVID-19) has been reported to increase the incidence rate of depression. We investigated the prevalence rate and associated factors of postpartum depression (PPD) among women in Jeddah, Saudi Arabia, during the COVID-19 period, thereby attempting to determine whether and how COVID-19 affected PPD. </span><b><span style="font-family:Verdana;">Methods:</span></b><span style="font-family:Verdana;"> This is a cross-sectional study that was conducted on women at one week to six months postpartum using an online questionnaire. Regarding the level of depression, there were 60.7% reported the presence of depression;the depression was affected by some demographics of females such as the education level (P = 0.021) and other factors related to the COVID-19 pandemic such as visiting the doctor during quarantine (P = 0.049), diagnosis with depression by the doctor (P = 0.006), the impact of the pandemic on depression (P = 0.035), experiencing symptoms of depression during pregnancy (P = 0.005), family history (P = 0.043), and difficulties during childbirth during the pandemic period (P = 0.03). </span><b><span style="font-family:Verdana;">Conclusion:</span></b><span style="font-family:Verdana;"> There was a high prevalence of PPD among women during the COVID-19 period, and it was higher than the prevalence of PPD before the COVID-19 period.</span></span></span></span>展开更多
BACKGROUND Among the most frequent hip fractures are trochanteric fractures,which usually occur from low-energy trauma like minor falls,especially in older people with osteoporotic bones.AIM To evaluate the treatment ...BACKGROUND Among the most frequent hip fractures are trochanteric fractures,which usually occur from low-energy trauma like minor falls,especially in older people with osteoporotic bones.AIM To evaluate the treatment efficacy of dynamic condylar screws(DCS)and proximal femoral nails(PFN)for unstable intertrochanteric fractures.METHODS To find pertinent randomized controlled trials and retrospective observational studies comparing PFN with DCS for the management of unstable femoral intertrochanteric fractures,a thorough search was carried out.For research studies published between January 1996 and April 2024,PubMed,EMBASE,Scopus,Web of Science,Cochrane Library,and Google Scholar were all searched.The complete texts of the papers were retrieved,vetted,and independently examined by two investigators.Disputes were settled by consensus,and any disagreements that persisted were arbitrated by a third author.RESULTS This study included six articles,comprising a total of 173 patients.Compared to the DCS,the PFN had a shorter operation time[mean difference(MD):-41.7 min,95%confidence interval(95%CI):-63.04 to-20.35,P=0.0001],higher success rates with closed reduction techniques[risk ratio(RR):34.05,95%CI:11.12-104.31,P<0.00001],and required less intraoperative blood transfusion(MD:-1.4 units,95%CI:-1.80 to-1.00,P<0.00001).Additionally,the PFN showed shorter fracture union time(MD:-6.92 wk,95%CI:-10.27 to-3.57,P<0.0001)and a lower incidence of reoperation(RR:0.37,95%CI:0.17-0.82,P=0.01).However,there was no discernible variation regarding hospital stay,implant-related complications,and infections.CONCLUSION Compared to DCS,PFN offers shorter operative times,reduces the blood transfusions requirements,achieves higher closed reduction success,enables faster fracture healing,and lowers reoperation incidence.展开更多

Methods: We measured that pain scores and the use of additional analgesic drugs w...We investigated the efficacy of ultrasound-guided pectoral nerves (PECS) block for modified radical mastectomy surgery retrospectively. Methods: We measured that pain scores and the use of additional analgesic drugs were recorded in the postoperative care unit within 24 hours after the operation. Postoperative complications (i.e., nausea and vomiting) were noted. Results: Patients who received the PECS block under general anesthesia (PECS group) reported lower visual analog scale pain scores at 0, 1, 2, 4, 6, 12, 24 hours after the operation than patients who did not receive PECS block under general anesthesia (control group). Moreover, the use of additional analgesic drugs during the first 24 hours after surgery was lower in the PECS group than in the control group. While in the postoperative care unit, the PECS group had less nausea and vomiting than the control group. Conclusion: The PECS block provides effective postoperative analgesia within the first 24 hours after breast cancer surgery.展开更多
AIM:To study usefulness of high-potency sucralfate(HPS)in a patient with chemoradiation mucositis and discuss its mechanism of action.METHODS:HPS,a non-covalently cross-link of sucralfate,cations and bidentate anionic...AIM:To study usefulness of high-potency sucralfate(HPS)in a patient with chemoradiation mucositis and discuss its mechanism of action.METHODS:HPS,a non-covalently cross-link of sucralfate,cations and bidentate anionic chelators,has a maintains a surface concentration of sucralfate 3 h following administration that is 7-23 fold that possible with standard-potency sucralfate.The accelerated mucosal healing and pain alleviation of HPS in patients with erosive esophageal reflux,prompted its use in this patient with chemoradiation mucositis of the oropharynx and alimentary tract.A literature-based review of the immuno-modulatory effects of sucralfate is discussed.RESULTS:Within 48 h of intervention:(1)there was complete disappearance of oral mucositis lesions;tenderness with(2)patient-reported disappearance of pain,nausea and diarrhea;patient required(3)no opiate analgesia and(4)no tube-feeding supplements to regular diet.Dysgeusia and xerostomia persisted.A modified Naranjo Questionnaire score of 10 supported the likelihood that HPS intervention caused the observed clinical effects.No adverse reactions noted.CONCLUSION:In this patient HPS was useful to treat chemo-radiation mucositis of the oropharynx and alimentary tract.HPS may directly or indirectly facilitate an immunomodulatory mechanism involving accelerated growth factor activation,which may be a new target for therapeutic intervention in such patients.展开更多

Clients provided informed consent to veterinarians to prescribe ...A potency-enhanced polyanionic phyto-saccharide of elm mucilage (PEPPS) was prescribed by 197 small animal veterinarians in an open-labeled field trial. Clients provided informed consent to veterinarians to prescribe PEPPS to 3952 dogs and 2248 cats. A 2 day/4 dose response rate, determined by veterinarians’ consensus, provided clinical threshold for a significant clinical outcome. Data was collected through phone interviews conducted over a period of 3.5 years from June 2003 through December 2006. 82% of 1928 vomiting dogs and 77% of 1064 vomiting cats responded to PEPPS within 2 days or four doses. 93% of 2024 dogs and 79% of 1184 cats with diarrhea responded to PEPPS within 2 days or four doses. PEPPS appears useful for managing vomiting and diarrhea in dogs and cats. However, a randomized blinded placebo controlled trial is needed to quantify true clinical efficacy.展开更多

The cleavage rates of formed 2 PN zygotes were compared with blastocyst and hatching blastocyst stage in drops of T6 medium with or without HGF or FGF (0 ng/ml, 10 ng/ml, 20 ng/ml, 50 ng/ml, 1 000 ng/ml). Results HGF and FGF enriched embryo culture media promotea the aevetopment from 2 PN stage embryos to blastocyst. Adding 20 ng/ml of FGF or HGF to the culture medium significantly increased the percentage of 2PN embryos that developed into blastocysts (P〈0.05), but culture of embryos in drops of T6 medium with HGF (20 ng/ml) had no improvement in in-vitro hatching.Conclusion Exogenous HGF and FGF at low concentrations promote in-vitro mouse blastocyst formation.展开更多
Objective:To understand the dietary intake of high-risk population with hypertension in Haikou,and to analyze the relationship between dietary intake and blood pressure,especially the relationship between sodium intak...Objective:To understand the dietary intake of high-risk population with hypertension in Haikou,and to analyze the relationship between dietary intake and blood pressure,especially the relationship between sodium intake and blood pressure,so as to provide a scientific basis for salt reduction.Methods:A multi-stage cluster sampling 2021 was used from July to December to collect 3-4 cscs from each of the four districts in Haikou.A total of 15 cscs were collected,295 permanent residents aged 55-74 were recruited from 15 community service centers,with 20 eligible subjects from each community.The subjects underwent general physical examination,laboratory tests,and a dietary survey using the self-developed Android phone APP“Nutrition Assistant”(registration number:2021SR1547832).SPSS 21.0 was used 2 for t test,x^(2) test and multiple Regression analysis.Result:In Haikou,69.4%of the subjects had energy intake above energy requirement,and 44.8%had fat intake above the acceptable range of macronutrients.There were only differences in energy,carbohydrate,vitamin B 6 and sodium intake(t=-2.174,-1.990,2.333,-5.442,P=0.031,0.048,0.021,<0.001,respectively).There were significant differences in BMI,family history of chronic diseases,systolic blood 2 pressure and diastolic blood pressure(f value or x^(2) value were 4.260,19.045,139.916,36.864,P value were 0.015,<0.001,<0.001,<0.001,respectively).Multiple Regression analysis analysis showed that 24h sodium excretion was significantly correlated with systolic and diastolic blood pressure(t=12.964,P<0.001),systolic and diastolic blood pressure increased by 0.135 mmhg and 0.068 mmhg,respectively.Conclusion:The dietary intake of the middle-aged and elderly people at high risk of hypertension in the Haikou community was unbalanced,and the intake of sodium salt was strongly correlated with blood pressure,it is necessary to continue to promote salt reduction as one of the key ways to prevent and control hypertension.展开更多

The last two compounds were suggested as activators of two transcription factors, PPARα and PPARγ, in C2C12 myocytes, which participate in β-oxidation of fatty acids and insulin sensitivity. However, the involvement of the hepatocytic and adipocytic PPARs in the effects of C. ficifolia has not yet been explored. This research aimed to determine the effects of C. ficifolia on PPARα, PPARγ, and inflammatory cytokines in streptozotocin (STZ)-induced diabetes mice, HepG2 hepatocytes and 3T3-L1 adipocytes, implicating two additional cell types associated with metabolism of carbohydrates and lipids. STZ-induced diabetes mice received C. ficifolia (200 mg/kg/day) for 30 days, measuring serum cytokines (TNF-α and IL-6). RNA was extracted from liver. Besides, HepG2 and 3T3-L1 cells were incubated (24 h) with C. ficifolia (0.078 mM DCI), using pioglitazone or fenofibrate as controls. RNA was also extracted from cells and PCR in real-time was performed to determinate PPARα and PPARγ expression. In diabetic animals, C. ficifolia decreased glycemia and body weight, decreasing the expression level of TNF-α and IL-6. In addition, C. ficifolia increased PPARα expression in liver of diabetic animals, in HepG2 and 3T3-L1 cells;PPARγ expression only significantly increased in HepG2 cells. The data suggest that the effects on the glycemia and lipids of C. ficifolia and its anti-inflammatory effects imply, besides skeletal muscle cells, hepatic and adipocytic PPARα activation, without affectation on PPARγ. PPARs regulation by C. ficifolia may improve the metabolic dysfunctions associated with metabolic disease, controlling the intake, activation, and oxidation of fatty acids and lipid storage.展开更多

文摘目的:动态光学乳腺成像技术(dynamic optical breast imaging,DOBI)对乳腺癌诊断指标进行初步探讨。材料和方法:接受DOBI及乳腺活检的患者共52例,均为女性。乳腺癌组19例,非乳腺癌组33例。结果:非乳腺癌感兴趣区内的"蓝色病灶"85.42%为漂移或发散,而乳腺癌68.42%为聚焦;非乳腺癌的"蓝色病灶"代谢曲线86.46%为平缓下降或呈波浪状,而乳腺癌以直线下降为多,达57.37%;64.58%非乳腺癌感兴趣区内的"蓝色病灶"代谢曲线与非蓝区代谢曲线相同,78.95%乳腺癌患者的代谢曲线与非蓝区代谢曲线不同;乳腺癌患者"蓝色病灶"代谢值(平均值为-5.77±2.13)的绝对值明显高于非乳腺癌患者(平均值为-3.34±0.87;P<0.05)。结论:DOBI空间特征的局限、聚焦且稳定、代谢曲线呈陡直下降对恶性或可疑恶性病变诊断价值更大,其次为代谢值的绝对值较大(多大于|-5|)。
文摘AIM: To evaluate the efficacy of endoscopic submucosal dissection for superficial esophageal squamous cell neoplasms. METHODS: Between July 2007 and March 2009, 27 consecutive superficial esophageal squamous cell neoplasms in 25 enrolled patients were treated by endoscopic submucosal dissection. The therapeutic efficacy, complications, and follow-up results were assessed. RESULTS: The mean size of the lesions was 21 ± 13 mm (range 2-55 mm); the mean size of the resection specimens was 32 ± 12 mm (range 10-70 mm). The enblock resection rate was 100% (27/27), and en block resection with tumor-free lateral/basal margins was 88.9% (24/27). Perforation occurred in 1 patient who was managed by conservative medical treatments. None of the patients developed local recurrence or distant metastasis in the follow-up period. CONCLUSION: Endoscopic submucosal dissection is applicable to superficial esophageal squamous cell neoplasms with promising results.
文摘Recently, we reported a case of gastric mucosaassociated lymphoid tissue (MALT) lymphoma presenting with unique vascular features. In the report, we defined the tree-like appearance (TLA) on the images of abnormal blood vessels which resembled branches from the trunk of a tree in the shiny mucosa, in which the glandular structure was lost. The 67-year-old female was diagnosed with gastric MALT lymphoma. The patient received eradication therapy for H. pylori. Conventional endoscopy revealed multiple ill-delineated brownish depressions in the stomach and cobblestonelike mucosa was observed at the greater curvature to the posterior wall of the upper gastric body 7 mo after successful eradication. Unsuccessful treatment of gastric MALT lymphoma was suspected on conventional endoscopy. Conventional endoscopic observations found focal depressions and cobblestone-like appearance, and these lesions were subsequently observed using magnified endoscopy combined with narrow band imaging to identify abnormal vessels presenting with a TLA within the lesions. Ten biopsies were taken from the area where abnormal vessels were present within these lesions. Ten biopsies were also taken from the lesions without abnormal vessels as a control. A total of 20 biopsy samples were evaluated to determine whether the diagnosis of MALT lymphoma could be obtained histologically from each sample. A positive diagnosis was obtained in 8/10 TLA (+) sites and in 2/10 TLA(-) sites. Target biopsies of the site with abnormal blood vessels can potentially improve diagnostic accuracy of gastric MALT lymphoma.
文摘Tamarindus[Tamarindus indica L.(T.Indira)],belongs to the family Leguminosae(Fabaceae),commonly known as Tamarind tree,is one of the fruit tree species that is used as traditional medicine.The aim of this article is to review the current literatue on health related effect of T.indir.a.Literature review about this plant was conducted between 2003 and 2014 through Pubmed and Google.The keywords Tamarind,T.indica were used for search.Only the health related articles selected.Tamarind tree is found especially in the Indian subcontinent,Africa,Pakistan,Bangladesh,Nigeria and most of the tropical countries.It is preferred to be used for abdominal pain,diarrhea and dysentery,some bacterial infections and parasitic infestations,wound healing,constipation and inflammation.It is a rich source of most of the essential amino acids and phytochemicals,and hence the plant is reported to possess antidiabetic,antimicrobial,antivenomic,antioxidant,antimalarial,cardioprotective,hepatoprotective,antiasthmatic,laxative and anti-hyperlipidemir activity.T.indica has ameliorative effects on many diseases.It can also be preferred as a nutritious support for malnourished patients as it is cheap and easy to access.Those effects should be clarified with further research.
文摘Narrow band imaging(NBI) is a new image enhancement system employing optic digital methods to enhance images of blood vessels on mucosal surfaces,allowing improved visualization of mucosal surface structures.Studies have progressed over the last several years,and the clinical usefulness has been demonstrated.NBI has become frequently applied for preoperative diagnosis before endoscopic submucosal dissection(ESD) of digestive tract cancers,as well as for assessment of the range of ESD for en-bloc resection of large lesions.Consensus has been reached with regard to the usefulness of NBI for detecting micro-lesions of esophageal squamous cell carcinoma indicated for ESD,for the diagnosis of the range and depth.NBI has also been attracting attention for diagnosing gastric cancer based on the observation of micro blood vessels on the mucosal surface and mucosal surface microstructures.The usefulness of NBI has been reported in relation to various aspects of colon cancer,including diagnoses of the presence,quality,range,and depth of lesions.However,as NBI has not surpassed diagnostic methods based on magnifying observation combined with the established and widely employed dye method,its role in ESD is limited at present.Although NBI is very useful for the diagnosis of digestive tract cancers,comprehensive endoscopic diagnosis employing the combination of conventional endoscopy including dye spraying,EUS,and NBI may be important and essential for ESD.
文摘Endoscopic submucosal dissection (ESD) is an advanced therapeutic endoscopic technique,which allowsresection of larger superficial tumors in the esophagus,stomach,and colon.Precise diagnosis of the boundary between tumor and the non-tumorous surrounding portion is especially important before starting ESD,because too much resection can potentially take more time and can induce a higher complication rate,while too little resection can result in a non-curative resection.The boundary diagnosis is often difficult for early gastric cancer,mainly because of the underlying condition of chronic gastritis.Due to recent developments in endoscopy,including magnified endoscopy and narrow band endoscopy,the boundary diagnosis is becoming easy and more accurate.We have also applied magnified endoscopy combined with narrow band imaging to fresh specimens immediately after resection using thetiling method and XY stage.
文摘The origin of myofibroblasts or myofibroblastic cells in the desmoplastic stroma associated with carcinoma invasion has been controversial. In the early invasive area of a rectal adenocarcinoma reported here, an obvious transition between the muscularis mucosa and the bundles of eosinophilic stromal cells observed in the carcinomatous stroma was demonstrated both in morphology and in their cytoskeletal phenotype, which conceivably suggests that the smooth muscle cells of the muscularis mucosa could convert to the eosinophilic stromal cells, namely myofibroblasts. Moreover, type I procollagen was demonstrated in both protein and mRNA levels in the areas of eosinophilic stromal cells with a lesser degree of differentiated smooth muscle phenotype that showed a transition from the muscularis mucosa, implying that the myofibroblastic cells converted from smooth muscle cells of the muscularis mucosa could be responsible for type I collagen production. These findings suggest that the muscularis mucosae may not be a passive barrier through which colorectal carcinomas infiltrate into the submucosa, but may play an active role in the formation and remodeling of tumor stroma.
文摘AIM: To investigate whether magnifying endoscopy with narrow band imaging (ME-NBI) is useful for evaluating the area of superficial, depressed- or flat-type differentiated adenocarcinoma of the stomach. METHODS: This procedure was performed in Saitama Medical University International Medical Center, Japanese Red Cross Kumamoto Hospital and Kitakyushu Municipal Medical Center. The subjects were 31 patients in whom biopsy findings, from superficial, depressed- or flat-type gastric lesion, suggested differentiated adenocarcinoma in the above 3 hospitals between January and December 2009. Biopsy was performed on the lesion and non-lesion sides of a boundary (imaginary boundary) visualized on ME-NBI. The results were pathologically investigated. We evaluated the accuracy of estimating a demarcation line (DL) on ME-NBI in comparison with biopsy findings as a gold standard. RESULTS: The DL that could be recognized at 2 points on the orifice and anal sides of each lesion during ME-NBI was consistent with the pathological findings in 22 patients with 0-IIc lesions, 7 with 0-IIb lesions, and 2 with 0-IIb + IIc lesions, showing an accuracy of 100%. CONCLUSION: The results suggest the usefulness of ME-NBI for evaluating the area of superficial, depressed- and flat-type differentiated adenocarcinoma of the stomach.
文摘Endoscopic submucosal dissection (ESD) is now the most common endoscopic treatment in Japan for intramucosal gastrointestinal neoplasms (non-metastatic). ESD is an invasive endoscopic surgical procedure, requiring extensive knowledge, skill, and specialized equipment. ESD starts with evaluation of the lesion, as accurate assessment of the depth and margin of the lesion is essential. The devices and strategies used in ESD vary, depending on the nature of the lesion. Prior to the procedure, the operator must be knowledgeable about the treatment strategy(ies), the device(s) to use, the electrocautery machine settings, the substances to inject, and other aspects. In addition, the operator must be able to manage complications, should they arise, including immediate recognition of the complication(s) and its treatment. Finally, in case the ESD treatment is not successful, the operator should be prepared to apply alternative treatments. Thus, adequate knowledge and training are essential to successfully perform ESD.
文摘The field of endoscopy has progressed markedly and become widespread in recent years, and the role of minimally invasive endoscopic treatment has become increasingly more important with the increase in the number of patients in whom gastric cancer is detected at an early stage. In addition, the characteristics of early gastric cancer, which can be curably treated by mucosal resection alone just as by surgical cancer resection, were clarified, and endoscopic submucosal dissection (ESD) was developed as a highly curable, minimally invasive treatment, that is gaining popularity. In this paper, we describe the technical details and complications of ESD for early gastric cancer, including their management.

The COVID-19 pandemic is a novel virus that is highly infectious and has several negative psychological impacts on individuals globally. </span><b><span style="font-family:Verdana;">Aim:</span></b><span style="font-family:Verdana;"> Coronavirus disease 2019 (COVID-19) has been reported to increase the incidence rate of depression. We investigated the prevalence rate and associated factors of postpartum depression (PPD) among women in Jeddah, Saudi Arabia, during the COVID-19 period, thereby attempting to determine whether and how COVID-19 affected PPD. </span><b><span style="font-family:Verdana;">Methods:</span></b><span style="font-family:Verdana;"> This is a cross-sectional study that was conducted on women at one week to six months postpartum using an online questionnaire. There were 30.7% reported being primigravida. Regarding the level of depression, there were 60.7% reported the presence of depression;the depression was affected by some demographics of females such as the education level (P = 0.021) and other factors related to the COVID-19 pandemic such as visiting the doctor during quarantine (P = 0.049), diagnosis with depression by the doctor (P = 0.006), the impact of the pandemic on depression (P = 0.035), experiencing symptoms of depression during pregnancy (P = 0.005), family history (P = 0.043), and difficulties during childbirth during the pandemic period (P = 0.03). </span><b><span style="font-family:Verdana;">Conclusion:</span></b><span style="font-family:Verdana;"> There was a high prevalence of PPD among women during the COVID-19 period, and it was higher than the prevalence of PPD before the COVID-19 period.</span></span></span></span>
文摘BACKGROUND Among the most frequent hip fractures are trochanteric fractures,which usually occur from low-energy trauma like minor falls,especially in older people with osteoporotic bones.AIM To evaluate the treatment efficacy of dynamic condylar screws(DCS)and proximal femoral nails(PFN)for unstable intertrochanteric fractures.METHODS To find pertinent randomized controlled trials and retrospective observational studies comparing PFN with DCS for the management of unstable femoral intertrochanteric fractures,a thorough search was carried out.For research studies published between January 1996 and April 2024,PubMed,EMBASE,Scopus,Web of Science,Cochrane Library,and Google Scholar were all searched.The complete texts of the papers were retrieved,vetted,and independently examined by two investigators.Disputes were settled by consensus,and any disagreements that persisted were arbitrated by a third author.RESULTS This study included six articles,comprising a total of 173 patients.Compared to the DCS,the PFN had a shorter operation time[mean difference(MD):-41.7 min,95%confidence interval(95%CI):-63.04 to-20.35,P=0.0001],higher success rates with closed reduction techniques[risk ratio(RR):34.05,95%CI:11.12-104.31,P<0.00001],and required less intraoperative blood transfusion(MD:-1.4 units,95%CI:-1.80 to-1.00,P<0.00001).Additionally,the PFN showed shorter fracture union time(MD:-6.92 wk,95%CI:-10.27 to-3.57,P<0.0001)and a lower incidence of reoperation(RR:0.37,95%CI:0.17-0.82,P=0.01).However,there was no discernible variation regarding hospital stay,implant-related complications,and infections.CONCLUSION Compared to DCS,PFN offers shorter operative times,reduces the blood transfusions requirements,achieves higher closed reduction success,enables faster fracture healing,and lowers reoperation incidence.
文摘We investigated the efficacy of ultrasound-guided pectoral nerves (PECS) block for modified radical mastectomy surgery retrospectively. Methods: We measured that pain scores and the use of additional analgesic drugs were recorded in the postoperative care unit within 24 hours after the operation. Postoperative complications (i.e., nausea and vomiting) were noted. Results: Patients who received the PECS block under general anesthesia (PECS group) reported lower visual analog scale pain scores at 0, 1, 2, 4, 6, 12, 24 hours after the operation than patients who did not receive PECS block under general anesthesia (control group). Moreover, the use of additional analgesic drugs during the first 24 hours after surgery was lower in the PECS group than in the control group. While in the postoperative care unit, the PECS group had less nausea and vomiting than the control group. Conclusion: The PECS block provides effective postoperative analgesia within the first 24 hours after breast cancer surgery.

文摘AIM:To study usefulness of high-potency sucralfate(HPS)in a patient with chemoradiation mucositis and discuss its mechanism of action.METHODS:HPS,a non-covalently cross-link of sucralfate,cations and bidentate anionic chelators,has a maintains a surface concentration of sucralfate 3 h following administration that is 7-23 fold that possible with standard-potency sucralfate.The accelerated mucosal healing and pain alleviation of HPS in patients with erosive esophageal reflux,prompted its use in this patient with chemoradiation mucositis of the oropharynx and alimentary tract.A literature-based review of the immuno-modulatory effects of sucralfate is discussed.RESULTS:Within 48 h of intervention:(1)there was complete disappearance of oral mucositis lesions;tenderness with(2)patient-reported disappearance of pain,nausea and diarrhea;patient required(3)no opiate analgesia and(4)no tube-feeding supplements to regular diet.Dysgeusia and xerostomia persisted.A modified Naranjo Questionnaire score of 10 supported the likelihood that HPS intervention caused the observed clinical effects.No adverse reactions noted.CONCLUSION:In this patient HPS was useful to treat chemo-radiation mucositis of the oropharynx and alimentary tract.HPS may directly or indirectly facilitate an immunomodulatory mechanism involving accelerated growth factor activation,which may be a new target for therapeutic intervention in such patients.

文摘A potency-enhanced polyanionic phyto-saccharide of elm mucilage (PEPPS) was prescribed by 197 small animal veterinarians in an open-labeled field trial. Clients provided informed consent to veterinarians to prescribe PEPPS to 3952 dogs and 2248 cats. A 2 day/4 dose response rate, determined by veterinarians’ consensus, provided clinical threshold for a significant clinical outcome. Data was collected through phone interviews conducted over a period of 3.5 years from June 2003 through December 2006. 82% of 1928 vomiting dogs and 77% of 1064 vomiting cats responded to PEPPS within 2 days or four doses. 93% of 2024 dogs and 79% of 1184 cats with diarrhea responded to PEPPS within 2 days or four doses. PEPPS appears useful for managing vomiting and diarrhea in dogs and cats. However, a randomized blinded placebo controlled trial is needed to quantify true clinical efficacy.
文摘Objective To explore the effect of hepatocyte growth factor (HGF) and fibroblast growth factor (FGF) on preimplantation embryo development of mouse.Methods Mated mice were killed by cervical dislocation to collect two pronucleous (2 PN) zygotes from oviduct of pregnant 1 d NMRI mice and were cultured to the hatching blastocyst stage and the number of embryo in different stages was recorded under an invert microscope. The cleavage rates of formed 2 PN zygotes were compared with blastocyst and hatching blastocyst stage in drops of T6 medium with or without HGF or FGF (0 ng/ml, 10 ng/ml, 20 ng/ml, 50 ng/ml, 1 000 ng/ml). Results HGF and FGF enriched embryo culture media promotea the aevetopment from 2 PN stage embryos to blastocyst. Adding 20 ng/ml of FGF or HGF to the culture medium significantly increased the percentage of 2PN embryos that developed into blastocysts (P〈0.05), but culture of embryos in drops of T6 medium with HGF (20 ng/ml) had no improvement in in-vitro hatching.Conclusion Exogenous HGF and FGF at low concentrations promote in-vitro mouse blastocyst formation.

文摘Objective:To understand the dietary intake of high-risk population with hypertension in Haikou,and to analyze the relationship between dietary intake and blood pressure,especially the relationship between sodium intake and blood pressure,so as to provide a scientific basis for salt reduction.Methods:A multi-stage cluster sampling 2021 was used from July to December to collect 3-4 cscs from each of the four districts in Haikou.A total of 15 cscs were collected,295 permanent residents aged 55-74 were recruited from 15 community service centers,with 20 eligible subjects from each community.The subjects underwent general physical examination,laboratory tests,and a dietary survey using the self-developed Android phone APP“Nutrition Assistant”(registration number:2021SR1547832).SPSS 21.0 was used 2 for t test,x^(2) test and multiple Regression analysis.Result:In Haikou,69.4%of the subjects had energy intake above energy requirement,and 44.8%had fat intake above the acceptable range of macronutrients.There were only differences in energy,carbohydrate,vitamin B 6 and sodium intake(t=-2.174,-1.990,2.333,-5.442,P=0.031,0.048,0.021,<0.001,respectively).There were significant differences in BMI,family history of chronic diseases,systolic blood 2 pressure and diastolic blood pressure(f value or x^(2) value were 4.260,19.045,139.916,36.864,P value were 0.015,<0.001,<0.001,<0.001,respectively).Multiple Regression analysis analysis showed that 24h sodium excretion was significantly correlated with systolic and diastolic blood pressure(t=12.964,P<0.001),systolic and diastolic blood pressure increased by 0.135 mmhg and 0.068 mmhg,respectively.Conclusion:The dietary intake of the middle-aged and elderly people at high risk of hypertension in the Haikou community was unbalanced,and the intake of sodium salt was strongly correlated with blood pressure,it is necessary to continue to promote salt reduction as one of the key ways to prevent and control hypertension.
文摘Diabetes mellitus control in Mexico is practiced by using antidiabetic agents and, by empirical way using medicinal plants. Cucurbita ficifolia (C. ficifolia) has been attributed with hypoglycemic, hypotriglyceridemic, and anti-inflam- matory effects, and D-chiro-inositol (DCI), 4-hydroxybenzoic acid, and β-sitosterol were proposed as active principles.
