The clinicpathologic features and MRI manifestations of the thoracic lumbar tuberculosis

Objective: To explore the clinicpathologic features and MRI manifestations of the thoracic lumbar tuberculosis by underwenting MRI and pathological examinations. Methods: 34 cases of Thoracic lumbar tuberculosis were collected which underwent MRI examination and confirmed by pathology or treatment of anti-TB drugs, the clinical cure from June 2008 to June 2012 in our hospital. The first MRI was performed on all patients, to determine the MRI findings, and surgical treatment for pathological examination in order to determine the pathological features. Results: 34 patients with 81 vertebral involvement, in which two adjacent vertebral involvement is the common, accounting for 72.25%, vertebral showed uneven long T1, long T2 signal. Intervertebral disc abnormalities accounted for 90.17%, the performance showed long T1, long T2 signal changes in the intervertebral disc damage, often accompanied by disc space narrowing or disappear. Paraspinal abscess accounted for 90.67%, often more than vertebra, up and down across one or more vertebral bodies showed long T1, long T2 signal. The vertebrae were damaged kyphosis or (and) the posterior longitudinal ligament abscess caused by the dural sac and spinal column with pressure accounted for 74.72%. In enhanced scan, vertebrae, intervertebral discs showed heterogeneous enhancement, paraspinal abscess was curved or ring enhancement. The pathological diagnosis may be obvious to confirm the degree of swelling and lesions. Conclusion: By doing the MRI and pathologic examinations, it can determine the clinical and pathological features and MRI findings of spinal tuberculosis patients with Thoracic lumbar spinal tuberculosis, and lay a reliable foundation for the treatment of the subsequent treatment. 展开更多

BACKGROUND Fibrous dysplasia(FD)is a common benign intramedullary fibro-osseous lesion.Involvement of the spine is rare,with the literature including only case reports,and cases of monostotic FD(MFD)in the sacrum are extremely rare.A correct preoperative diagnosis of spinal MFD is important for clinicians to select proper treatment.CASE SUMMARY We retrospectively assessed a case report of MFD in the sacrum.This patient was examined by computed tomography(CT)and magnetic resonance imaging(MRI),and the diagnosis was confirmed by pathology.A review of the literature was performed to analyze the imaging characteristics and differential diagnoses of spinal MFD.For our patient,the CT scan showed the lesion to be expansile,with ground glass opacity and a sclerotic rim.On MRI,the lesion showed iso-low signal intensity on T1WI and iso-high signal intensity on T2WI.A low signal rim was found on T1WI and T2WI.Our patient was treated by posterior focal excision,decompression,bone grafting,fusion and pedicle screw fixation.A satisfactory result was achieved,with pain disappearance.No complications had occurred at the 1-year follow up.CONCLUSION MFD is an expansile osteolytic change.Ground glass opacity and a sclerotic margin are obvious characteristics.The lesion often involves the vertebral body and posterior element.Knowledge of these imaging characteristics of spinal FD could be helpful for diagnosis and prevent unnecessary procedures. 展开更多

BACKGROUND: ^31p magnetic resonance spectroscopy (^31p MRS) can be used to non-injuredly and dynamicly detect vadous metabolites including phosphorus in organis and reflect changes of phospholipid metabolism and energy metabolism in tissue and pH value in cells. OBJECTIVE: To observe changes of pH value, phospholipid metabolism and energy metabolism of various cerebral tumors and normal brain tissue with sip MRS. DESIGN: Semi-quantitative contrast observation PARTICIPANTS : A total of 44 patients with cerebral tumor diagnosed with surgery operation were selected from the Department of Magnetic Resonance, Central South Hospital, Wuhan University from September 2004 to June 2006. All the subjects had complete sip MRS data before steroid and operation. Among them, 16 patients had glioma of grade Ⅱ-Ⅲ, 12 spongioblastoma and 16 meningioma. The mean age was (45±6) years. Another 36 subjects without focus on cerebral MRI were regarded as normal group, including 19 males and 18 females, and the mean age was (41±4) years. Included subjects were consent. METHODS: Eclipse1.5T MRS (Philips Company) was used to collect wave spectrum; jMRUI(1.3) was used to analyze expenmental data and calculate pH value in voxel and ratios of phosphocreatine (PCr)/inorganic phosphate (Pi), PCr/phosphodiesterase (PDE) and phosphomonoesterase (PME)/β-adenosine triphosphate (β-ATP) of vadous metabolites. ^31p MRS results were compared with t test between tumor patients and normal subjects. MAIN OUTCOME MEASURES: Changes of phospholipid metabolism (PME./PDE), energy metabolism (PCr/ATP) and pH value of various cerebral tumors and normal brain tissues. RESULTS : A total of 44 cases with cerebral tumor and 36 normal subjects were involved in the final analysis. pH value and semi-quantitative measurements of normal brain tissues and various cerebral tumors: ① pH value at top occipital region and temple occipital region of normal brain tissue was 7.04 ±0.02; PCr/β-ATP was 1.51 ±0.03; PCr/Pi was 2.85±0.20; PDE/β-ATP was 2.70±0.40; PME/β-ATP was 1.01 ±0.03. Giioma of grade Ⅱ - Ⅲ : Tumor tissue was alkalized. The pH value was 7.09±0.01, which was higher than that in normal group (t=-1.823, P 〈 0.05); PCr/β-ATP, PME/β-ATP and PCr/ Pi were 1.50±0.04, 1.04±0.07 and 2.49±0.21, respectively, which was close to that in normal group (P 〉 0.05); PDE/β-ATP was 1.73±0.20, which was lower than that in normal group (t=-2.168, P 〈 0.05). ③ Spongioblastoma: Tumor tissue was alkalized. The pH value was 7.12±0.02, which was higher than that in normal group (t=-2.170, P 〈 0.05); PCr/β-ATP and PCr/Pi were 1.48±0.05 and 2.39±0.23, respectively, which was lower than that in normal group, but the difference was not significant (P 〉 0.05); PDE/β-ATP was 1.61±0.25, which was lower than that in normal group (t=-3.582, P 〈 0.01); PME/β-ATP was 1.64±0.05, which was higher than that in normal group (t=-3.930, P 〈 0.01). ④ Meningioma: Tumor tissue was alkalized. The pH value was 7.16±0.03, which was higher than that in normal group (t=-2.978, P 〈 0.01); PCr/β-ATP, PCr/Pi and PDE/β-ATP were 0.46±0.04, 1.08±0.16 and 0.82±0.10, respectively, which was lower than that in normal group (t=-3.132, 3.983, 3.985, P 〉 0.01); PME/β-ATP was 2.05±0.03, which was higher than that in normal group (t=-3.007, P〈 0.01). CONCLUSION: ^31p MRS can provide informations about pH value of cerebral tumor, phospholipid metabolism and energy metabolism. Changes of pH value, PCr, PME and PDE can reflect metabolic characteristics of various cerebral tumors. 展开更多

The amount of work need to displace the blood in the systemic circulation, exceeds the work done by the left ventricle. With this, blood recovers increased flow resistance between the heartbeats with the Womersley number alterations in the rhythm of the accompanying electrocardiogram (ECG). Viscoelastic transformation is heavily expressed in coagulation. There must be a relationship between the ECG and blood transient flow resistance. The influence of the electromagnetic field on blood coagulation was studied. Venous blood was affected by the oscillated electromagnetic field (500 - 5000 Hz), with the square wave input signal in 25 healthy individuals (15 males, 10 females in the age 18 - 57 years). Electromagnetic irradiation (EMI) time of the sample 3 - 10 min. Hypocoagulation in normal blood samples was revealed (decreased quantity of Platelets up to 10 - 23 × 10<sup>9</sup>/L, Prothrombin index up to 9% - 10%, Fibrinogen concentration up to 0.20 - 0.21 g/L) and thrombolysis after the blood stasis. Ac electric field from the myocardial depolarization initiates electroacoustic phenomena. An emerging repulsing electromagnetic force acts on the red blood cells (RBC) and in addition to the pulse pressure from the heart, promotes blood motion and viscoelastic changes. The alterations of the blood inertial and elasticity, in addition to hemodynamics, are facilitated by the magnetic features of the hemoglobin. The external electromagnetic signal can manage the blood coagulation process, including thrombolysis. 展开更多
