The aim of this study is to analyse preliminary data of the Limbe trauma registry, to describe the injury patterns and determine the outcome of care of the injured who were treated in the emergency department of the regional hospital Limbe in Cameroon. Methodology: This was a descriptive secondary analysis of prospective data collected on injury at Limbe Regional Hospital in two years, between October 2008 and October 2010. All cases of injury presenting to the emergency department of Limbe Regional Hospital were included in a prospective trauma registry. Data was collected with a pre-tested data entry form and included the age, sex, mechanism of injury, delay before arrival to hospital, body part injured, description of lesions and outcome at the emergency. Descriptive analyses were done using STATA 14. Comparisons between groups were evaluated using Chi-squared test or Kruskal-Wallis and p-values <0.05 were considered statistically significant. Results: A total of 2400 cases of injury were recorded during the study period with more males (68%) than females. The ages ranged from 0 to 90 years with a peak between 20 to 39 years. Road traffic crash was the most common cause of injury representing 60.81% of the population (n = 1434);other causes were burns, falls, domestic injuries, assaults, and labour accidents. The accidents mostly involved motor cycle alone (42.03%). Concerning the outcome of care at the emergency department, 88.73% (n = 1260) were discharged. Conclusion: The burden of road traffic injuries in south west Cameroon is high. Motorcycles are mostly involved alone or with pedestrians and/or cars.展开更多

Nine fish and one shrimp ...This study aimed at investigating the content of heavy metals in some marine organisms of the Cameroon Atlantic coast and assessing the health risk due to their consumption by the population. Nine fish and one shrimp species were collected for this study. These fish species were Ethmolosa fimbriata, Drepane africana, Dentex moroccanus, Arius latiscotatus, Scarus hoefleri, Cynoglossus browni, Sardinella madorensis, Pseudotolithus elongatus, Pseudotolithus typus. The shrimp species was Macrobranchium macrobranchium. The elements investigated in this study were: Fe, Co, Cu, Cd and Pb. For seven fish species it was found that the concentration of heavy metals followed the order Fe > Co > Cd > Cu > Pb. For most fish species the Target Hazard Quotient (THQ) value calculated showed the following trend Cd > Co > Pb > Fe ≥ Cu. Cadmium was found to be the main contributor to health risk that may result from the consumption of the fish species under study. The THQ for this element was in the range 0.7 - 1.2 while the Target Cancer Risk (TR) due to ingestion of this metal was in the range (3 × 10<sup>-</sup><sup>4</sup> - 6 × 10<sup>-</sup><sup>4</sup>). These values are slightly above the threshold limits established for both carcinogenic and non carcinogenic risks by the United States, Environmental Protection Agency. The estimated weekly intake of cadmium due to consumption of some fishes under study was greater or equal to the Provisional Tolerable Weekly Intake (PTWI) determined by Food and Agriculture Organization/World Health Organization.展开更多
BACKGROUND Thumb replantation following complete traumatic avulsion requires complex techniques to restore function,especially in cases of avulsion at the level of the metacarpophalangeal joint(MCP I)and avulsion of t...BACKGROUND Thumb replantation following complete traumatic avulsion requires complex techniques to restore function,especially in cases of avulsion at the level of the metacarpophalangeal joint(MCP I)and avulsion of the flexor pollicis longus(FPL)at the musculotendinous junction.Possible treatments include direct tendon suture or tendon transfer,most commonly from the ring finger.To optimize function and avoid donor finger complications,we performed thumb replantation with flexion restoration using brachioradialis(BR)tendon transfer with palmaris longus(PL)tendon graft.CASE SUMMARY A 20-year-old left-handed male was admitted for a complete traumatic left thumb amputation following an accident while sliding from the top of a handrail.The patient presented with skin and bone avulsion at the MCP I,avulsion of the FPL tendon at the musculotendinous junction(zone 5),avulsion of the extensor pollicis longus tendon(zone T3),and avulsion of the thumb’s collateral arteries and nerves.The patient was treated with two stage thumb repair.The first intervention consisted of thumb replantation with MCP I arthrodesis,resection of avulsed FPL tendon and implantation of a silicone tendon prosthesis.The second intervention consisted of PL tendon graft and BR tendon transfer.Follow-up at 10 months showed good outcomes with active interphalangeal flexion of 70°,grip strength of 45 kg,key pinch strength of 15 kg and two-point discrimination threshold of 4 mm.CONCLUSION Flexion restoration after complete thumb amputation with FPL avulsion at the musculotendinous junction can be achieved using BR tendon transfer with PL tendon graft.展开更多
Despite the promising outcomes observed in individual applications of biochar and polyvinyl alcohol(PVA)in soil,the impact of their combined usage remains inadequately understood.This study systematically explores the...Despite the promising outcomes observed in individual applications of biochar and polyvinyl alcohol(PVA)in soil,the impact of their combined usage remains inadequately understood.This study systematically explores the effects of concurrent biochar and PVA application on key soil parameters,including pH,water-holding capacity(WHC),and dynamic moisture content(MC),and the photosynthetic resilience and growth of the cyanobacterium Microcoleus vaginatus in a desert soil.Biochars,generated at different pyrolysis temperatures(300-600℃),were applied to the soil at varying rates(1%-6%),while PVA was introduced at a mass percentage of 0.05%.The photosynthetic resilience and biomass accumulation of M.vaginatus in different treatments were examined every 7 d during the 28-d exposure to dry conditions after 60-d water supply.The combined application of biochar and PVA resulted in a reduction of soil pH,coupled with significant improvements in WHC and dynamic MC.Moreover,this combined approach exhibited superior effects on the photosynthetic resilience and crust thickness(0.9-3.5 mm)of M.vaginatus compared to the application of biochar and PVA in isolation.Incremental increase in biochar application rate from 0% to 6% demonstrated a notable enhancement in the chlorophyll a content of M.vaginatus.Cyanobacterial crust thickness and exopolysaccharide content exhibited positive correlations with biochar application rate.Thus,combined application of biochar and PVA is cost-effective for enhancing soil properties and cyanobacterial biomass,which is of significance for combating desertification.展开更多

The aim of this study was to evaluate the radiological, the functional results and to determine the factors of poor postoperative prognosis of DRF treated with Newclip radial plates®. Methodology: This prospective cohort study evaluates the radiological and functional outcames of displaced radius fractures (DRFs) in patients ≥50 years old treated with Newclip® (locked anterior plates) at the Basse-Terre Hospital in Guadeloupe from 2022 to 2024. The patients were categorized into those with epiphyseal involvement (E1 - E4) and without epiphyseal involvement (E0) based on Laulan’s MEU classification. Radiological parameters (distal radio-ulnar index (DRUI), radial inclination frant view (IRF), radial inclination sagittal view (IRS) were assessed pre and post-operatively. Functional recovery was evaluated at 12 months using the QuickDash questionnaire. Results: Falls were the most common cause of fracture. Post-operatively, SRI was the least restored parameter. Poor prognostic factor for SRI improvement included posterior commimution and unstable fractures. Factors associated with higher QuickDash scores included unstable factures, unrestored DRUI, low plate position, metaphyseal features, and ulnar features. Conclusion: The anterior locking plate osteosynthesis is reliable treatment option with excellent functional outcomes.展开更多
Objective To develop a multimodal imaging atlas of a rat brain-computer interface(BCI)that incorporates brain,arterial,bone tissue and a BCI device using mixed reality(MR)for three-dimensional(3D)visualization.Methods...Objective To develop a multimodal imaging atlas of a rat brain-computer interface(BCI)that incorporates brain,arterial,bone tissue and a BCI device using mixed reality(MR)for three-dimensional(3D)visualization.Methods An invasive BCI was implanted in the left visual cortex of 4-week-old Sprague–Dawley rats.Multimodal imaging techniques,including micro-CT and 9.0 T MRI,were used to acquire images of the rat cranial bone structure,vascular distribution,brain tissue functional zones,and BCI device before and after implantation.Using 3D-slicer software,the images were fused through spatial transformations,followed by image segmentation and 3D model reconstruction.The HoloLens platform was employed for MR visualization.Results This study constructed a multimodal imaging atlas for rats that included the skull,brain tissue,arterial tissue,and BCI device coupled with MR technology to create an interactive 3D anatomical model.Conclusions This multimodal 3D atlas provides an objective and stable reference for exploring complex relationships between brain tissue structure and function,enhancing the understanding of the operational principles of BCIs.This is the first multimodal 3D imaging atlas related to a BCI created using Sprague–Dawley rats.展开更多

Its safety and efficacy has been demonstrated extensively in the literature, leading to increased use around the world over the last two decades. This has been demonstrated by the increase in Ponseti related Pub Med publications from many countries. We found evidence of Ponseti activity in 113 of 193 United Nations members. The contribution of many organizations which provide resources to healthcare practitioners in low and middle income countries, as well as Ponseti champions and modern communication technology, have helped to spread the Ponseti method around the world. Despite this, there are many countries where the Ponseti method is not being used, as well as many large countries in which the extent of activity is unknown. With its low rate of complication, low cost, and high effectiveness,this method has unlimited potential to treat clubfoot in both developed and undeveloped countries. Our listing of countries who have not yet shown presence of Ponseti activity will help non-governmental organizations to target those countries which still need the most help.展开更多

METHODS Eighty-seven patients with rotator cuff tears following arthroscopic treatment were divided i...AIM To compare the functional outcomes of traumatic and non-traumatic rotator cuff tears after arthroscopic repair. METHODS Eighty-seven patients with rotator cuff tears following arthroscopic treatment were divided into traumatic and non-traumatic tear groups. Postoperative muscle strength and outcomes using the modified University of California, Los Angeles score were evaluated. Sex, age, affected limb and dominant limb were correlated between groups. Muscle strength of the repaired and unaffected shoulders was compared. Rotator cuff injury size was measured. RESULTS Of the 87 patients who underwent rotator cuff repairs, 35 had traumatic tears and 52 had non-traumatic tears. In patients with non-traumatic tears, the average age was 59 years, 74.5% were female, 96.1% were righthand dominant and 92.3% had their dominant shoulder affected. Patients with traumatic tears were 59.5 yearsold on average, 51.4% were female, 91.4% were righthand dominant and 88.5% had their dominant shoulder affected. No difference existed in the mean modified University of California, Los Angeles score between patients with traumatic tears(33.7) compared with those with non-traumatic tears(32.8). No strength differences were observed between groups: The strength difference between the non-affected and affected sides was 1.21 kg in the non-traumatic group and 1.39 kg in the traumatic group(P = 0.576), while the strength ratio between the non-affected/affected sides was 0.805 in the nontraumatic group and 0.729 in the traumatic group(P = 0.224). CONCLUSION The functional results of traumatic rotator cuff repairs are similar to non-traumatic tears. Both outcomes are satisfactory.展开更多

Risk factors that contribute to defining thi...Complex ankle arthrodesis is defined as an ankle fusion that is at high risk of delayed and nonunion secondary to patient comorbidities and/or local ankle/hindfoot factors. Risk factors that contribute to defining this group of patients can be divided into systemic factors and local factors pertaining to co-existing ankle or hindfoot pathology. Orthopaedic surgeons should be aware of these risk factors and their association with patients' outcomes after complex ankle fusions. Both external and internal fixations have demonstrated positive outcomes with regards to achieving stable fixation and minimizing infection. Recent innovations in the application of biophysical agents and devices have shown promising results as adjuncts for healing. Both osteoconductive and osteoinductive agents have been effectively utilized as biological adjuncts for bone healing with low complication rates. Devices such as pulsed electromagnetic field bone stimulators, internal direct current stimulators and low-intensity pulsed ultrasound bone stimulators have been associated with faster bone healing and improved outcomes scores when compared with controls. The aim of this review article is to present a comprehensive approach to the management of complex ankle fusions, including the use of biophysical adjuncts for healing and a proposed algorithm for their treatment.展开更多

However, isolation of bone marrow-derived MSCs (BMSCs) is an invasive and...Studies have confirmed that bone marrow-derived mesenchymal stem cells (MSCs) can be used for treatment of several nervous system diseases. However, isolation of bone marrow-derived MSCs (BMSCs) is an invasive and painful process and the yield is very low. Therefore, there is a need to search for other alterative stem cell sources. Adipose-derived MSCs (ADSCs) have phenotypic and gene expression profiles similar to those of BMSCs. The production of ADSCs is greater than that of BMSCs, and ADSCs proliferate faster than BMSCs. To compare the effects of venous grafts containing BMSCs or ADSCs on sciatic nerve injury, in this study, rats were randomly divided into four groups: sham (only sciatic nerve exposed), Matrigel (MG; sciatic nerve injury + intravenous transplantation of MG vehicle), ADSCs (sciatic nerve injury + intravenous MG containing ADSCs), and BMSCs (sciatic nerve injury + intravenous MG containing BMSCs) groups. Sciatic functional index was calculated to evaluate the function of injured sciatic nerve. Morphologic characteristics of nerves distal to the lesion were observed by toluidine blue staining. Spinal motor neurons labeled with Fluoro-Gold were quantitatively assessed. Compared with sham-operated rats, sciatic functional index was lower, the density of small-diameter fibers was significantly increased, and the number of motor neurons significantly decreased in rats with sciatic nerve injury. Neither ADSCs nor BMSCs significantly improved the sciatic nerve function of rats with sciatic nerve injury,increased fiber density, fiber diameters, axonal diameters, myelin sheath thickness, and G ratios (axonal diameter/fiber diameter ratios) in the sciatic nerve distal to the lesion site. There was no significant difference in the number of spinal motor neurons among ADSCs, BMSCs and MG groups. These results suggest that neither BMSCs nor ADSCs provide satisfactory results for peripheral nerve repair when using MG as the conductor for engraftment.展开更多

Over the last decade, a growing body of evidence has accumulated suggesting a role for Teriparatide in the management of fractures. Studies in both normal and delayed healing models have shown improvement in callus volume and mineralisation, bone mineral content, rate of successful union and strength at fracture sites. However most of these results have been derived from animal studies. The majority of this research on humans has comprised low level evidence, with few randomised controlled trials, many case reports and case series. Nevertheless, the results from these studies seem to support research from animal models. This has led to a growing number of clinicians using Teriparatide "off license" to treat fractures and non-unions in their patients. This review presents a critical appraisal of the current evidence supporting the use of Teriparatide for fracture healing, delayed unions and non unions and in the setting of osteoporotic fractures, the studies producing this evidence and their transferability to human beings.展开更多

Aberrant vasculature resulting from tumor angiogenesis plays a critical role in the development of resistance to breast cancer treatments, via exacerbation of tumor hypoxia, decreased effective drug concentrations within tumors, and immune-related mechanisms. Antiangiogenic therapy can counteract these breast cancer resistance factors by promoting tumor vascular normalization. The combination of antiangiogenic therapy with chemotherapy, targeted therapy, or immunotherapy has emerged as a promising approach for overcoming drug resistance in breast cancer. This review examines the mechanisms associated with angiogenesis and the interactions among tumor angiogenesis, the hypoxic tumor microenvironment, drug distribution, and immune mechanisms in breast cancer. Furthermore, this review provides a comprehensive summary of specific antiangiogenic drugs, and relevant studies assessing the reversal of drug resistance in breast cancer. The potential mechanisms underlying these interventions are discussed, and prospects for the clinical application of antiangiogenic therapy to overcome breast cancer treatment resistance are highlighted.展开更多
OBJECTIVE: To observe the therapeutic effects of electro-acupuncture(EA) combined with psychological intervention on the symptom of somzatization or obsession and mental symptom of depression or anxiety and P50 of Aud...OBJECTIVE: To observe the therapeutic effects of electro-acupuncture(EA) combined with psychological intervention on the symptom of somzatization or obsession and mental symptom of depression or anxiety and P50 of Auditory Evoked Potential(AEP) on internet addiction disorder(IAD).METHODS: One hundred and twenty cases of IAD were randomly divided into an EA group, a psycho-intervention(PI) group and a comprehensive therapy(EA plus PI) group. Patients in the EA group were treated with EA. Patients in the PI group were treated with cognition and behavior therapy. Patients in the EA plus PI group were treated with electro-acupuncture plus psychological intervention. Scores of IAD, scores of the symptom checklist90(SCL-90), latency and amplitude of P50 of AEP were measured before and after treatment.RESULTS: The scores of IAD after treatment significantly decreased in all groups(P < 0.05), and thescores of IAD in the EA plus PI group were significantly lower than those in the other two groups(P < 0.05). The scores of SCL-90 assembled and each factor after treatment in the EA plus PI group significantly decreased(P < 0.05). After treatment in the EA plus PI group, the amplitude distance of S1P50 and S2P50(S1-S2) significantly increased(P < 0.05).CONCLUSION: EA combined with PI could relieve the mental symptoms of IAD patients, and the mechanism is possibly related to the increase of cerebrum sense perception gating function.展开更多

Also we used a trigonometric formula to predict the change of the femoral distal anatomical mechanical angle(AMA) after lengthening along the anatomical axis. RESULTS Lengthening along the anatomical axis of the femur by 10% of its original length results in reduction in the distal femoral AMA by 0.57 degrees. There is no objective experimental scientific data to prove that the Mechanical axis is passing via the center of the hip to the center of the knee. There is wide variation in normal anatomical axis for different populations. In deformity correction, surgeons try to reproduce the normal usual bone shape to regain normal function, which is mainly anatomical axis. CONCLUSION Lengthening of the femur along its anatomical axis results in mild reduction of the distal femoral AMA. This may partially compensate for the expected mechanical axis lateralisation and hence justify its minimal clinical impact.展开更多

In this paper, we study the characteristics of the second Hopf bifurcation in a driven square cavity by applying a consistent fourth-order compact finite difference scheme recently developed by us. We numerically identify the critical Reynolds number of the second Hopf bifurcation located in the interval of(11093.75, 11094.3604) by bisection. In addition, we find that there are two dominant frequencies in its spectral diagram when the flow is in the status of the second Hopf bifurcation, while only one dominant frequency is identified if the flow is in the first Hopf bifurcation via the Fourier analysis. More interestingly, the flow phase portrait of velocity components is found to make transition from a regular elliptical closed form for the first Hopf bifurcation to a non-elliptical closed form with self-intersection for the second Hopf bifurcation. Such characteristics disclose flow in a quasi-periodic state when the second Hopf bifurcation occurs.展开更多

Cadmium was found to be the main contributor to health risk that may result from the consumption of the fish species under study. The THQ for this element was in the range 0.7 - 1.2 while the Target Cancer Risk (TR) due to ingestion of this metal was in the range (3 × 10<sup>-</sup><sup>4</sup> - 6 × 10<sup>-</sup><sup>4</sup>). These values are slightly above the threshold limits established for both carcinogenic and non carcinogenic risks by the United States, Environmental Protection Agency. The estimated weekly intake of cadmium due to consumption of some fishes under study was greater or equal to the Provisional Tolerable Weekly Intake (PTWI) determined by Food and Agriculture Organization/World Health Organization.
文摘BACKGROUND Thumb replantation following complete traumatic avulsion requires complex techniques to restore function,especially in cases of avulsion at the level of the metacarpophalangeal joint(MCP I)and avulsion of the flexor pollicis longus(FPL)at the musculotendinous junction.Possible treatments include direct tendon suture or tendon transfer,most commonly from the ring finger.To optimize function and avoid donor finger complications,we performed thumb replantation with flexion restoration using brachioradialis(BR)tendon transfer with palmaris longus(PL)tendon graft.CASE SUMMARY A 20-year-old left-handed male was admitted for a complete traumatic left thumb amputation following an accident while sliding from the top of a handrail.The patient presented with skin and bone avulsion at the MCP I,avulsion of the FPL tendon at the musculotendinous junction(zone 5),avulsion of the extensor pollicis longus tendon(zone T3),and avulsion of the thumb’s collateral arteries and nerves.The patient was treated with two stage thumb repair.The first intervention consisted of thumb replantation with MCP I arthrodesis,resection of avulsed FPL tendon and implantation of a silicone tendon prosthesis.The second intervention consisted of PL tendon graft and BR tendon transfer.Follow-up at 10 months showed good outcomes with active interphalangeal flexion of 70°,grip strength of 45 kg,key pinch strength of 15 kg and two-point discrimination threshold of 4 mm.CONCLUSION Flexion restoration after complete thumb amputation with FPL avulsion at the musculotendinous junction can be achieved using BR tendon transfer with PL tendon graft.

文摘Despite the promising outcomes observed in individual applications of biochar and polyvinyl alcohol(PVA)in soil,the impact of their combined usage remains inadequately understood.This study systematically explores the effects of concurrent biochar and PVA application on key soil parameters,including pH,water-holding capacity(WHC),and dynamic moisture content(MC),and the photosynthetic resilience and growth of the cyanobacterium Microcoleus vaginatus in a desert soil.Biochars,generated at different pyrolysis temperatures(300-600℃),were applied to the soil at varying rates(1%-6%),while PVA was introduced at a mass percentage of 0.05%.The photosynthetic resilience and biomass accumulation of M.vaginatus in different treatments were examined every 7 d during the 28-d exposure to dry conditions after 60-d water supply.The combined application of biochar and PVA resulted in a reduction of soil pH,coupled with significant improvements in WHC and dynamic MC.Moreover,this combined approach exhibited superior effects on the photosynthetic resilience and crust thickness(0.9-3.5 mm)of M.vaginatus compared to the application of biochar and PVA in isolation.Incremental increase in biochar application rate from 0% to 6% demonstrated a notable enhancement in the chlorophyll a content of M.vaginatus.Cyanobacterial crust thickness and exopolysaccharide content exhibited positive correlations with biochar application rate.Thus,combined application of biochar and PVA is cost-effective for enhancing soil properties and cyanobacterial biomass,which is of significance for combating desertification.
文摘Background and Objectives: The distal radius fracture (DRF) is a major public health problem in northern countries. Its frequency is constantly increasing. The locked anterior plate with its well-established biomechanical properties, offers a reliable alternative. The aim of this study was to evaluate the radiological, the functional results and to determine the factors of poor postoperative prognosis of DRF treated with Newclip radial plates®. Methodology: This prospective cohort study evaluates the radiological and functional outcames of displaced radius fractures (DRFs) in patients ≥50 years old treated with Newclip® (locked anterior plates) at the Basse-Terre Hospital in Guadeloupe from 2022 to 2024. The patients were categorized into those with epiphyseal involvement (E1 - E4) and without epiphyseal involvement (E0) based on Laulan’s MEU classification. Radiological parameters (distal radio-ulnar index (DRUI), radial inclination frant view (IRF), radial inclination sagittal view (IRS) were assessed pre and post-operatively. Functional recovery was evaluated at 12 months using the QuickDash questionnaire. Results: Falls were the most common cause of fracture. Post-operatively, SRI was the least restored parameter. Poor prognostic factor for SRI improvement included posterior commimution and unstable fractures. Factors associated with higher QuickDash scores included unstable factures, unrestored DRUI, low plate position, metaphyseal features, and ulnar features. Conclusion: The anterior locking plate osteosynthesis is reliable treatment option with excellent functional outcomes.

文摘Objective To develop a multimodal imaging atlas of a rat brain-computer interface(BCI)that incorporates brain,arterial,bone tissue and a BCI device using mixed reality(MR)for three-dimensional(3D)visualization.Methods An invasive BCI was implanted in the left visual cortex of 4-week-old Sprague–Dawley rats.Multimodal imaging techniques,including micro-CT and 9.0 T MRI,were used to acquire images of the rat cranial bone structure,vascular distribution,brain tissue functional zones,and BCI device before and after implantation.Using 3D-slicer software,the images were fused through spatial transformations,followed by image segmentation and 3D model reconstruction.The HoloLens platform was employed for MR visualization.Results This study constructed a multimodal imaging atlas for rats that included the skull,brain tissue,arterial tissue,and BCI device coupled with MR technology to create an interactive 3D anatomical model.Conclusions This multimodal 3D atlas provides an objective and stable reference for exploring complex relationships between brain tissue structure and function,enhancing the understanding of the operational principles of BCIs.This is the first multimodal 3D imaging atlas related to a BCI created using Sprague–Dawley rats.
文摘The Ponseti method has become the gold standard for the treatment of idiopathic clubfoot. Its safety and efficacy has been demonstrated extensively in the literature, leading to increased use around the world over the last two decades. This has been demonstrated by the increase in Ponseti related Pub Med publications from many countries. We found evidence of Ponseti activity in 113 of 193 United Nations members. The contribution of many organizations which provide resources to healthcare practitioners in low and middle income countries, as well as Ponseti champions and modern communication technology, have helped to spread the Ponseti method around the world. Despite this, there are many countries where the Ponseti method is not being used, as well as many large countries in which the extent of activity is unknown. With its low rate of complication, low cost, and high effectiveness,this method has unlimited potential to treat clubfoot in both developed and undeveloped countries. Our listing of countries who have not yet shown presence of Ponseti activity will help non-governmental organizations to target those countries which still need the most help.
文摘Fracture of the radial head is a common injury. Over the last decades, the radial head is increasingly recognized as an important stabilizer of the elbow. In order to maintain stability of the injured elbow, goals of treatment of radial head fractures have become more and more towards restoring function and stability of the elbow. As treatment strategies have changed over the years, with an increasing amount of literature on this subject, the purpose of this article was to provide an overview of current concepts of the management of radial head fractures.
文摘AIM To compare the functional outcomes of traumatic and non-traumatic rotator cuff tears after arthroscopic repair. METHODS Eighty-seven patients with rotator cuff tears following arthroscopic treatment were divided into traumatic and non-traumatic tear groups. Postoperative muscle strength and outcomes using the modified University of California, Los Angeles score were evaluated. Sex, age, affected limb and dominant limb were correlated between groups. Muscle strength of the repaired and unaffected shoulders was compared. Rotator cuff injury size was measured. RESULTS Of the 87 patients who underwent rotator cuff repairs, 35 had traumatic tears and 52 had non-traumatic tears. In patients with non-traumatic tears, the average age was 59 years, 74.5% were female, 96.1% were righthand dominant and 92.3% had their dominant shoulder affected. Patients with traumatic tears were 59.5 yearsold on average, 51.4% were female, 91.4% were righthand dominant and 88.5% had their dominant shoulder affected. No difference existed in the mean modified University of California, Los Angeles score between patients with traumatic tears(33.7) compared with those with non-traumatic tears(32.8). No strength differences were observed between groups: The strength difference between the non-affected and affected sides was 1.21 kg in the non-traumatic group and 1.39 kg in the traumatic group(P = 0.576), while the strength ratio between the non-affected/affected sides was 0.805 in the nontraumatic group and 0.729 in the traumatic group(P = 0.224). CONCLUSION The functional results of traumatic rotator cuff repairs are similar to non-traumatic tears. Both outcomes are satisfactory.
文摘Complex ankle arthrodesis is defined as an ankle fusion that is at high risk of delayed and nonunion secondary to patient comorbidities and/or local ankle/hindfoot factors. Risk factors that contribute to defining this group of patients can be divided into systemic factors and local factors pertaining to co-existing ankle or hindfoot pathology. Orthopaedic surgeons should be aware of these risk factors and their association with patients' outcomes after complex ankle fusions. Both external and internal fixations have demonstrated positive outcomes with regards to achieving stable fixation and minimizing infection. Recent innovations in the application of biophysical agents and devices have shown promising results as adjuncts for healing. Both osteoconductive and osteoinductive agents have been effectively utilized as biological adjuncts for bone healing with low complication rates. Devices such as pulsed electromagnetic field bone stimulators, internal direct current stimulators and low-intensity pulsed ultrasound bone stimulators have been associated with faster bone healing and improved outcomes scores when compared with controls. The aim of this review article is to present a comprehensive approach to the management of complex ankle fusions, including the use of biophysical adjuncts for healing and a proposed algorithm for their treatment.

文摘Studies have confirmed that bone marrow-derived mesenchymal stem cells (MSCs) can be used for treatment of several nervous system diseases. However, isolation of bone marrow-derived MSCs (BMSCs) is an invasive and painful process and the yield is very low. Therefore, there is a need to search for other alterative stem cell sources. Adipose-derived MSCs (ADSCs) have phenotypic and gene expression profiles similar to those of BMSCs. The production of ADSCs is greater than that of BMSCs, and ADSCs proliferate faster than BMSCs. To compare the effects of venous grafts containing BMSCs or ADSCs on sciatic nerve injury, in this study, rats were randomly divided into four groups: sham (only sciatic nerve exposed), Matrigel (MG; sciatic nerve injury + intravenous transplantation of MG vehicle), ADSCs (sciatic nerve injury + intravenous MG containing ADSCs), and BMSCs (sciatic nerve injury + intravenous MG containing BMSCs) groups. Sciatic functional index was calculated to evaluate the function of injured sciatic nerve. Morphologic characteristics of nerves distal to the lesion were observed by toluidine blue staining. Spinal motor neurons labeled with Fluoro-Gold were quantitatively assessed. Compared with sham-operated rats, sciatic functional index was lower, the density of small-diameter fibers was significantly increased, and the number of motor neurons significantly decreased in rats with sciatic nerve injury. Neither ADSCs nor BMSCs significantly improved the sciatic nerve function of rats with sciatic nerve injury,increased fiber density, fiber diameters, axonal diameters, myelin sheath thickness, and G ratios (axonal diameter/fiber diameter ratios) in the sciatic nerve distal to the lesion site. There was no significant difference in the number of spinal motor neurons among ADSCs, BMSCs and MG groups. These results suggest that neither BMSCs nor ADSCs provide satisfactory results for peripheral nerve repair when using MG as the conductor for engraftment.
文摘Teriparatide is a recombinant form of the biologicallyactive component of Parathyroid hormone. It has been shown to increase bone mass and prevent fractures in osteoporotic bone. It is licensed by the Food and Drug Administration for the treatment of Osteoporosis. Over the last decade, a growing body of evidence has accumulated suggesting a role for Teriparatide in the management of fractures. Studies in both normal and delayed healing models have shown improvement in callus volume and mineralisation, bone mineral content, rate of successful union and strength at fracture sites. However most of these results have been derived from animal studies. The majority of this research on humans has comprised low level evidence, with few randomised controlled trials, many case reports and case series. Nevertheless, the results from these studies seem to support research from animal models. This has led to a growing number of clinicians using Teriparatide "off license" to treat fractures and non-unions in their patients. This review presents a critical appraisal of the current evidence supporting the use of Teriparatide for fracture healing, delayed unions and non unions and in the setting of osteoporotic fractures, the studies producing this evidence and their transferability to human beings.

文摘Angiogenesis is considered a hallmark pathophysiological process in tumor development. Aberrant vasculature resulting from tumor angiogenesis plays a critical role in the development of resistance to breast cancer treatments, via exacerbation of tumor hypoxia, decreased effective drug concentrations within tumors, and immune-related mechanisms. Antiangiogenic therapy can counteract these breast cancer resistance factors by promoting tumor vascular normalization. The combination of antiangiogenic therapy with chemotherapy, targeted therapy, or immunotherapy has emerged as a promising approach for overcoming drug resistance in breast cancer. This review examines the mechanisms associated with angiogenesis and the interactions among tumor angiogenesis, the hypoxic tumor microenvironment, drug distribution, and immune mechanisms in breast cancer. Furthermore, this review provides a comprehensive summary of specific antiangiogenic drugs, and relevant studies assessing the reversal of drug resistance in breast cancer. The potential mechanisms underlying these interventions are discussed, and prospects for the clinical application of antiangiogenic therapy to overcome breast cancer treatment resistance are highlighted.

文摘OBJECTIVE: To observe the therapeutic effects of electro-acupuncture(EA) combined with psychological intervention on the symptom of somzatization or obsession and mental symptom of depression or anxiety and P50 of Auditory Evoked Potential(AEP) on internet addiction disorder(IAD).METHODS: One hundred and twenty cases of IAD were randomly divided into an EA group, a psycho-intervention(PI) group and a comprehensive therapy(EA plus PI) group. Patients in the EA group were treated with EA. Patients in the PI group were treated with cognition and behavior therapy. Patients in the EA plus PI group were treated with electro-acupuncture plus psychological intervention. Scores of IAD, scores of the symptom checklist90(SCL-90), latency and amplitude of P50 of AEP were measured before and after treatment.RESULTS: The scores of IAD after treatment significantly decreased in all groups(P < 0.05), and thescores of IAD in the EA plus PI group were significantly lower than those in the other two groups(P < 0.05). The scores of SCL-90 assembled and each factor after treatment in the EA plus PI group significantly decreased(P < 0.05). After treatment in the EA plus PI group, the amplitude distance of S1P50 and S2P50(S1-S2) significantly increased(P < 0.05).CONCLUSION: EA combined with PI could relieve the mental symptoms of IAD patients, and the mechanism is possibly related to the increase of cerebrum sense perception gating function.
文摘Objective:To explore the effect of auricular point pressing combined with thunder-fire moxibustion in the treatment of abdominal distension and constipation after thoracic compression fracture.Methods:Totally 100 patients were randomly divided into 4 groups,the observation group 1(n=25)which were treated with conventional nursing combined with auricular piont pressing,observation group 2(n=25)treated with conventional nursing combined with thunder-fire moxibustion,observation group 3(n=25)treated with conventional nursing combined with auricular point pressing and thunder-fire moxibustion,and the control group(n=25)which adopted conventional nursing.First exhaust and defecation time after fracture in 4 groups were observed and compared.Results:The effect of observation group 3 was better than that of observation group 2,observation group 1 and control group(P<0.05).Conclusion:It is obvious that auricular point pressing combined with thunder-fire moxibustion has better result in treating abdominal distension and constipation after thoracic compression fracture.
文摘AIM To review and study the effect of lengthening along the anatomical axis of long bones and its relation to the mechanical axis deviation.METHODS We try in this review to calculate and discuss the exact clinical impact of lengthening along the anatomical axis of the femur on affecting the limb alignment. Also we used a trigonometric formula to predict the change of the femoral distal anatomical mechanical angle(AMA) after lengthening along the anatomical axis. RESULTS Lengthening along the anatomical axis of the femur by 10% of its original length results in reduction in the distal femoral AMA by 0.57 degrees. There is no objective experimental scientific data to prove that the Mechanical axis is passing via the center of the hip to the center of the knee. There is wide variation in normal anatomical axis for different populations. In deformity correction, surgeons try to reproduce the normal usual bone shape to regain normal function, which is mainly anatomical axis. CONCLUSION Lengthening of the femur along its anatomical axis results in mild reduction of the distal femoral AMA. This may partially compensate for the expected mechanical axis lateralisation and hence justify its minimal clinical impact.

文摘To date, there are very few studies on the second Hopf bifurcation in a driven square cavity, although there are intensive investigations focused on the first Hopf bifurcation in literature, due to the difficulties of theoretical analyses and numerical simulations. In this paper, we study the characteristics of the second Hopf bifurcation in a driven square cavity by applying a consistent fourth-order compact finite difference scheme recently developed by us. We numerically identify the critical Reynolds number of the second Hopf bifurcation located in the interval of(11093.75, 11094.3604) by bisection. In addition, we find that there are two dominant frequencies in its spectral diagram when the flow is in the status of the second Hopf bifurcation, while only one dominant frequency is identified if the flow is in the first Hopf bifurcation via the Fourier analysis. More interestingly, the flow phase portrait of velocity components is found to make transition from a regular elliptical closed form for the first Hopf bifurcation to a non-elliptical closed form with self-intersection for the second Hopf bifurcation. Such characteristics disclose flow in a quasi-periodic state when the second Hopf bifurcation occurs.
