Dexamethasone intravitreal implant monotherapy in naive patients with macular edema secondary to retinal vein occlusion:long term follow-up retrospective cohort study

AIM:To investigate the efficacy and safety of repeated dexamethasone implants with real-life data in eyes with naive retinal vein occlusion(RVO)with macular edema(ME)at a minimum of 60mo follow-up.METHODS:In this retrospective cohort study,the data about best corrected visual acuity(BCVA),central macular thickness(CMT),serous macular detachment(SMD),hard exudate,hyperreflective foci(HRF),cystoid degeneration,pearl necklace sign,epiretinal membrane(ERM),disorganization of retinal inner layers(DRIL),ellipsoid zone and external limiting membrane(EZ-ELM)integrity,intraocular pressure(IOP)and lens condition were recorded.RESULTS:Thirty-eight eyes of 38 patients were included in the study.Thirteen patients presented with central RVO(CRVO)and 25 with branch RVO(BRVO).The mean follow-up time was 69.9±15.8mo,and the mean number of injections was 7.9±4.0.The mean BCVA gain was 25.0±36 letters,and this difference was statistically significant(P=0.021).The BCVA gain was 19.4±20.4 letters in the CRVO group,and 26.5±38.6 letters in the BRVO group(P=0.763).Besides,21(55.2%)of the patients achieved≥15 letters improvement.At the end of the follow-up period,SMD was not observed in any of the patients(P=0.016).Hard exudate,HRF number were decreased;while DRIL,ERM and EZ-ELM defects were increased but not significantly.CONCLUSION:Intravitreal dexamethasone monotherapy is an effective and safe treatment option for the treatment-naive RVO-ME patients in the long-term follow-up. 展开更多

We present a novel protocol for deterministic secure quantum communication by using the lour-qubit cluster state as quantum channel. It is shown that two legitimate users can directly transmit the secret messages based on Bellbasis measurements and classical communication. The present protocol makes use of the ideas of block transmission and decoy particle checking technique. It has a high capacity as each cluster state can carry two 5its of information, and has a high intrinsic efficieney 5ecause almost all the instances except the decoy checking particles (its numSer is negligible) are useful. Background:Potassium(K)deficiency has become a common field production problem following the widespread adoption of Bacillus thuringiensis(Bt)transgenic cotton(Gossypium hirsutum L.)worldwide.The purpose of this study was to clarify whether the introduction of Bt gene directly reduces the K-use efficiency of cotton to induce K deficiency.Results:The cotton variety,Jihe 321(wild type,WT)and its two Bt(Cry1Ac)-transgenic overexpression lines(OE-29317,OE-29312)were studied in field with low soil-test K+(47.8 mg·kg^(−1)).In the field with low soil-test K+,only OE-29317 had less biomass and K+accumulation than the WT at some growth stages.Both Bt lines produced similar or even greater seed cotton yield than WT in the field.When the Bt gene(~70%)in OE-29317 and OE-29312 plants was silenced by virus-induced gene silencing(VIGS),the VIGS-Bt plants did not produce more biomass than VIGSgreen fluorescent protein(control)plants.Conclusions:The introduction of Bt gene did not necessarily hinder the K use efficiency of the cotton lines under this study. 展开更多

During COVID-19,the repercussions on scholar dynamics,functioning and structure were inevitable,causing an impact on groups that,historically,are minority in relation to the scope of school inclusion policies,such as students diagnosed with autism spectrum disorder(ASD).This scenario is followed by educational phenomena in the face of which School Psychology has collaborated with the advancement of research and knowledge production.This qualitative study,based on Historical-Cultural Psychology and Critical School Psychology,analyzed the role of the school psychologist in monitoring students diagnosed with ASD in the context of regular school during the pandemic.The following procedures were conducted:application of an online questionnaire;and semi-structured interviews with four school psychologists.As a result,three zones of shared meanings were built:the claim for the right to education with support and adaptations;the psychologist as the great savior of inclusion;the non-total inclusion that denounces the structure of injustice by school.The information suggests that professional performance was hampered by different obstacles in the pandemic,such as the difficulty imposed by school,overload of demands,and impasses in the implementation of inclusion historically experienced at school,revealing that the school structure still carries historical consequences of exclusion.These shared meanings have demanded a professional performance with more effort,resistance,and strategies that accept the demands of school inclusion,providing collective mobilization,together with school actors,capable of breaking down barriers,especially those related to capacitist concepts. 展开更多

The biocompatibility of orthopedic implants is closely related to their elastic modulus and surface properties.The objective of this study was to determine the effects of cold rolling,recrystallization and laser surface melting(LSM)on the microstructure and mechanical properties of a biphase(α″+β)Ti-30Nb-4Sn alloy.X-ray diffraction(XRD)texture analysis of the cold-rolled substrate revealed the[302]α″//ND texture component,while analysis of the recrystallized substrate showed the[302]α″//ND and[110]α″//ND components.Theβ-phase texture could not be directly measured by XRD,but the presence of the[111]β//ND texture component was successfully predicted by considering the orientation relationship between theα″andβphases.Nanoindentation measurements showed that the elastic modulus of the cold-rolled substrate(63GPa)was lower than that of the recrystallized substrate(74GPa).Based on the available literature and the results presented here,it is suggested that this difference is caused by the introduction of crystal defects during cold deformation.The combined nanoindentation/EBSD analysis showed that the nanoindentation results are not affected by crystal orientation.LSM of the deformed alloy produced changes in hardness,elastic modulus and crystallographic texture similar to those produced by recrystallization heat treatment,creating a stiffness gradient between surface and substrate. 展开更多

Bacillus spp.has been proven to be critical for the quality of many fermented foods,but as one of the domain bacteria,its effect on the quality of Yibin Yacai has not been fully addressed.Herein,the effects of inoculation of Bacillus subtilis Y4 on the physicochemical properties,flavor metabolism and microbial community changes of Yibin Yacai for three-month solid-state fermentation were investigated.The results showed that inoculation of B.subtilis Y4 could reduce the pH and salinity of Yibin Yacai,accelerate the consumption of nitrite,promote the growth of Lactobacillus and the production of sweet amino acid,and inhibit pathogenic microorganisms at the late fermentation stage.During solid-state fermentation,the dominant fungus Cystofilobasidium was positively correlated with most volatile flavor substances,which were indirectly affected by B.subtilis Y4.Furthermore,compared to spontaneous fermented Yibin Yacai,inoculation of B.subtilis Y4 promoted the production of alkenes and made the peak of ester content appear earlier.These results indicated that inoculation of B.subtilis Y4 for solid-state fermentation could improve the quality and taste of Yibin Yacai,and shorten the fermentation time. 展开更多

Carbon Footprint and Economic Analysis of LNG-fueled Fishing Vessel Using Real Engine Performance Simulation

Analysis of the environmental and economic performance of fishing vessels has received limited attention compared with other ship types despite their notable contribution to global greenhouse gas(GHG)emissions.This study evaluates the carbon footprint(CF)and economic viability of a liquefied natural gas(LNG)-fueled fishing vessel,using real engine operation simulations to provide precise and dynamic evaluation of fuel consumption and GHG emissions.Operational profiles are obtained through the utilization of onboard monitoring systems,whereas engine performance is simulated using the 1D/0D AVL Boost^(TM)model.Life cycle assessment(LCA)is conducted to quantify the environmental impact,whereas life cycle cost assessment(LCCA)is performed to analyze the profitability of LNG as an alternative fuel.The potential impact of the future fuel price uncertainties is addressed using Monte Carlo simulations.The LCA findings indicate that LNG has the potential to reduce the CF of the vessel by 14%to 16%,in comparison to a diesel power system configuration that serves as the baseline scenario.The LCCA results further indicate that the total cost of an LNG-powered ship is lower by 9.5%-13.8%,depending on the share of LNG and pilot fuels.This finding highlights the potential of LNG to produce considerable environmental benefits while addressing economic challenges under diverse operational and fuel price conditions. 展开更多

Electroacupuncture improves psychosocial outcomes in rectal cancer patients with bowel dysfunction

BACKGROUND Low anterior resection syndrome(LARS)is a prevalent and debilitating complication following sphincter-preserving surgery for rectal cancer.Evidence-based interventions for the concurrent psychological burden are limited.Electroacupuncture has been proposed as a potential adjunctive therapy,but its psychological benefits remain inadequately studied.AIM To investigate the therapeutic effect of electroacupuncture on emotional recovery and gastrointestinal function in patients with moderate to severe LARS,and to explore its potential advantages in psychologically vulnerable subgroups.METHODS We conducted a retrospective,controlled study involving 100 patients with moderate to severe LARS(LARS score≥21)treated at two tertiary hospitals in China between January 2022 and December 2024.Patients received either standard postoperative care alone(n=50)or in combination with a standardized 4-week electroacupuncture protocol(n=50).Psychological and functional outcomes were assessed using validated instruments including Hospital Anxiety and Depression Scale(HADS),Body Image Scale(BIS),General Self-Efficacy Scale,Perceived Social Support Scale(PSSS),LARS score,and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 at four time points.The primary endpoint was emotional remission,defined as a≥3-point reduction in HADS-Anxiety subscale(HADS-A).Analyses included repeated-measures comparisons,Kaplan-Meier survival curves,Cox regression models,and subgroup-interaction testing.RESULTS At baseline,demographic,surgical,and psychosocial characteristics were comparable among groups.By week 4,patients receiving electroacupuncture demonstrated significantly greater reductions in anxiety(HADS-A:4.8±2.6 vs 7.3±3.0;P<0.001),depression,and body-image disturbance(BIS:8.7±3.6 vs 11.9±4.2;P<0.001),alongside enhanced coping capacity(Brief Coping Orientation to Problems Experienced),perceived social support(PSSS),and bowel function(LARS score).Emotional remission-defined as a≥3-point HADS-A reduction-was achieved more rapidly in the electroacupuncture group,as confirmed by Kaplan-Meier analysis(log-rank P<0.001;odds ratio=4.7).Multivariate Cox regression identified higher baseline LARS and BIS scores as independent predictors of delayed emotional recovery.Subgroup analyses revealed significantly amplified treatment benefits in patients with high baseline anxiety(HADS-A≥8),elevated body-image disturbance(BIS≥12),or low perceived social support(PSSS<60),with consistent interaction effects(P for interaction<0.05 across subgroups).CONCLUSION Electroacupuncture may accelerate emotional recovery and improve functional and psychosocial outcomes in patients with LARS.Its integration into postoperative care may offer particular benefits for psychologically vulnerable subgroups. 展开更多

This paper investigates the reliability of internal marine combustion engines using an integrated approach that combines Fault Tree Analysis(FTA)and Bayesian Networks(BN).FTA provides a structured,top-down method for identifying critical failure modes and their root causes,while BN introduces flexibility in probabilistic reasoning,enabling dynamic updates based on new evidence.This dual methodology overcomes the limitations of static FTA models,offering a comprehensive framework for system reliability analysis.Critical failures,including External Leakage(ELU),Failure to Start(FTS),and Overheating(OHE),were identified as key risks.By incorporating redundancy into high-risk components such as pumps and batteries,the likelihood of these failures was significantly reduced.For instance,redundant pumps reduced the probability of ELU by 31.88%,while additional batteries decreased the occurrence of FTS by 36.45%.The results underscore the practical benefits of combining FTA and BN for enhancing system reliability,particularly in maritime applications where operational safety and efficiency are critical.This research provides valuable insights for maintenance planning and highlights the importance of redundancy in critical systems,especially as the industry transitions toward more autonomous vessels. 展开更多

Bacillus subtilis plays an important role in fundamental and applied research,and it has been widely used as a cell factory for the production of enzymes,antimicrobial materials,and chemicals for agriculture,medicine,and industry.However,genetic manipulation tools for B.subtilis have low efficiency.In this work,our goal was to develop a simple recombineering system for B.subtilis.We showed that genome editing can be achieved in B.subtiliis 1A751 through co-expression of YqaJ/YqaK,a native phage recombinase pair found in B.sub-tilis 168,and the competence master regulator ComK using a double-stranded DNA substrate with short ho-mology arms(100 bp)and a phosphorothioate modification at the 5′-end.Efficient gene knockouts and large DNA insertions were achieved using this new recombineering system in B.subtilis 1A751.As far as we know,this is the first recombineering system using the native phage recombinase pair YqaJ/YqaK in B.subtilis.In conclusion,this new recombineering system provides a simple and fast tool for genetic manipulation of B.sub-tilis,and it will promote studies of genome function,construction of production strains,and genome mining in B.subtilis. 展开更多

The emergence of metal nanoclusters with atomically precise compositions and structures provides an opportunity for in-depth investigation of catalysis mechanisms and structure−property correlations at the nanoscale.However,a serious problem for metal nanocluster catalysts is that the ligands inhibit the catalytic activity through deactivating the surface of the nanoclusters.Here,we introduce a novel catalytic mode for metal nanoclusters,in which the nanoclusters initiate the catalysis via single electron transfer(SET)without destroying the integrity of nanoclusters,providing a solution for the contradiction between activity and stability of metal nanoclusters.We illustrated that the novel activation mode featured low catalyst loading(0.01 mol%),high TOF,mild reaction conditions,and easy recycling of catalyst in alkyne hydroborylation,which often suffered from poor selectivity,low functional group tolerance,etc.Furthermore,the catalyst[Au_(1)Cu_(14)(TBBT)_(12)(PPh_(3))_(6)]^(+)(TBBTH:p-tert-butylthiophenol)can be applied in highly efficient tandem processes such as hydroborylation−deuteration and hydroborylation−isomerization,demonstrating the utility of the introduced activation mode for metal nanoclusters. 展开更多

Newly diagnosed patients with high-risk acute graft-versus-host disease(aGVHD)often experience poor clinical outcomes and low complete remission rates.Ruxolitinib with corticosteroids showed promising efficacy in improving response and failure free survival in our phase I study.This study(ClinicalTrials.gov:NCT04061876)sought to evaluate the safety and effectiveness of combining ruxolitinib(RUX,5 mg/day)with corticosteroids(1 mg/kg/day methylprednisolone,RUX/steroids combined group)versus using methylprednisolone alone(2 mg/kg/day,steroids-only group).Newly diagnosed patients with intermediate-or high-risk aGVHD were included,with risk levels classified by either the Minnesota aGVHD Risk Score or biomarker assessment.Patients were randomized in a ratio of 1:1 into 2 groups:99 patients received RUX combined with methylprednisolone,while the other 99 received methylprednisolone alone as the initial treatment.The RUX/steroids group showed a significantly higher overall response rate(ORR)on day 28(92.9%)compared to the steroids-only group(70.7%,Odds Ratio[OR]=5.8;95%Confidence Interval[CI],2.4-14.0;P<0.001).Similarly,the ORR on day 56 was higher in the RUX/steroids group(85.9%vs.46.5%;OR=7.07;95%CI,3.36-15.75;P<0.001).Additionally,the 18-month failure-free survival was significantly better in the RUX/steroids group(57.2%)compared to the steroids-only group(33.3%;Hazard Ratio=0.46;95%CI,0.31-0.68;P<0.001).Adverse events(AEs)frequencies were comparable between both groups,with the exception of fewer grade 4 AEs in the RUX/steroids group(26.3%vs.50.5%P=0.005).To our knowledge,this study is the first prospective,randomized controlled trial to demonstrate that adding ruxolitinib to the standard methylprednisolone regimen provides an effective and safe first-line treatment for newly diagnosed high-risk acute GVHD. 展开更多

数量守恒作为儿童认知发展的重要里程碑,反映了儿童对数量关系的理解能力及其逻辑思维的初步形成。为探究学前儿童数量守恒概念发展水平并验证皮亚杰的研究结论,以幼儿园小、中、大班幼儿为研究对象,以皮亚杰经典数量守恒任务为基础,从... 数量守恒作为儿童认知发展的重要里程碑,反映了儿童对数量关系的理解能力及其逻辑思维的初步形成。为探究学前儿童数量守恒概念发展水平并验证皮亚杰的研究结论,以幼儿园小、中、大班幼儿为研究对象,以皮亚杰经典数量守恒任务为基础,从外部形态和空间排列两个维度设计了12个关于数量守恒的问题,采用临床法收集实验数据,利用SPSS24进行描述和推断统计,对学前儿童的数量守恒发展水平进行了测量和比较。研究发现,小、中、大班的幼儿均能够克服物体的大小、颜色、形状的干扰实现对数量的正确理解,即物体的外部形态不影响幼儿对数量守恒的正确感知;而从空间排列维度考察,小班幼儿尚无法完成数量守恒任务。中班是幼儿守恒思维的快速发展期,不同年级幼儿的数量守恒发展水平具有显著性差异。不同年级和不同语言发展水平的幼儿在数量守恒概念发展水平上均表现出统计学差异,不同性别幼儿在数量守恒概念发展水平上没有统计学差异。 展开更多

This paper focuses on rural music education in the context of rural revitalization,deeply analyzes its current resource situation,and points out that tangible resources face shortages of teachers,inappropriate textbooks,and outdated teaching facilities,while intangible resources are confronted with the loss of folk music culture and the weakening of educational foundations due to cultural ecological changes.Resources are characterized by dispersion,scarcity,and inefficient utilization.In response to these difficulties,this paper proposes integration paths for teachers,textbooks,teaching facilities,and folk music culture resources.Additionally,it constructs a diversified development model that combines rural tourism with music activities,develops characteristic music education industries,and integrates rural cultural construction.Simultaneously,it identifies funding shortages and outdated concepts as major obstacles and proposes targeted strategies such as expanding funding channels and changing inherent concepts.The aim is to promote the development of rural music education and contribute to the revitalization of rural culture. 展开更多

Maximal steered coherence(MSC)is a noteworthy resource measure in the field of quantum information,and it is defined under the framework of coherence measure and the formalism of quantum steering ellipsoids(QSEs).Here,we explore the MSC of a two-qubit X state under the common influences of reservoir and noise.The results disclose that the introduction of auxiliary qubits can give rise to enhancement of the MSC in both the strong and weak coupling regimes.Moreover,more auxiliary qubits can decrease the oscillation period of the MSC,and also suppress the oscillation amplitude of the MSC in the strong coupling regime.In contrast,the increases in auxiliary qubits result in the oscillation of the MSC for the setting of the initially weak coupling regime.Of particular interest is that the improvement effects of more auxiliary qubits on the MSC in the initially weak coupling regime are significantly stronger than that in the initially strong coupling regime. 展开更多

Erectile function after laparoscopic versus robotic-assisted radical prostatectomy:A systematic review and meta-analysis

Objective:Prostate cancer is a common malignancy in men over 50 years old,and radical prostatectomy,particularly via laparoscopic and robotic-assisted techniques,significantly impacts quality of life,especially in terms of erectile dysfunction.This systematic review and meta-analysis aimed to evaluate the preservation of erectile function following robotic-assisted and laparoscopic radical prostatectomy,with a separate analysis of randomized clinical trials and non-randomized studies.Methods:This review was carried out using randomized and non-randomized studies involving adult patients diagnosed with localized prostate cancer undergoing radical prostatectomy,according to Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines and registered in PROSPERO.Applicable literature from PubMed,Cochrane,Embase,and the Latin American and Caribbean Health Sciences Literature database was analysed.The bias in randomized clinical trials was assessed using the Cochrane Risk of Bias 2.0 tool,and observational studies were evaluated via the Newcastle-Ottawa Scale.The statistical analysis was performed using Review Manager version 5.4.Results:Our analysis included 13 studies involving 6281 patients.Comparative meta-analysis of non-randomized studies demonstrated that robotic techniques were significantly more effective in preserving erectile function at 3 months(risk difference[RD]0.05,95%confidence interval[CI]0.00-0.11;p=0.040),6 months(RD 0.10,95%CI 0.03-0.17;p=0.006),and 12 months postoperatively(RD 0.06,95%CI 0.02-0.10;p=0.002).Conclusion:Robotic-assisted surgery showed greater preservation of erectile function 3 months,6 months,and 12 months after radical prostatectomy.However,additional studies with meticulous methodological criteria are necessary for future analysis. 展开更多

Analysis of Apsidal Motion and Light Curves of Three Eccentric Eclipsing Binaries:V398 Lac,V2544 Cyg and V785 Cas

In this study,the apsidal motion analyses of the eclipsing binary systems V398 Lac,V2544 Cyg,and V785 Cas are presented.The(O-C)diagrams of the selected systems were constructed using all available and reliable times of minimum light compiled from the literature.As a result,the apsidal motion periods were determined to be 353±50 yr,43.3±5.1 yr and 83.4±8.5 yr,respectively.Furthermore,the TESS light curves of the three systems were analyzed to derive their photometric and absolute parameters.The component masses were determined as follows:for V398 Lac,M_(1)=3.83±0.35 M_(⊙)and M2=3.27±0.35 M_(⊙);for V2544 Cyg,M_(1)=1.75±0.38 M_(⊙)and M2=1.40±0.38 M_(⊙);for V785 Cas,M_(1)=5.64±0.41 M_(⊙)and M2=5.18±0.41 M_(⊙).The parameters related to apsidal motion,as well as the observational and theoretical internal structure constants(K_(2,obs) and K_(2,teo))were calculated for studied binaries.The relativistic contribution to the observed apsidal motion rate was found to be relatively small,constituting less than approximately 5%of the total rate in each system.The theoretical internal structure constants were derived from the evolutionary models,assuming a standard chemical composition of(X,Z)=(0.70,0.01). 展开更多

In an era defined by complex,interconnected challenges like climate change,pandemics,and resource depletion,the traditional siloed approach to science education is proving increasingly insufficient.Interdisciplinary project-based learning represents a promising path forward in science education,fostering integrated and holistic learning experiences that move beyond isolated subject learning.Grounded in philosophical ideas of holism,pragmatism,constructivism,and transcendentalism,this article presents a case project illustrating the practical application of interdisciplinary project-based learning.This project engages students in integrating concepts from biology,chemistry,earth science,engineering,and social studies.Through phased activities-research and planning,data collection,implementation,and presentation-students develop a decent understanding of real-world problems while fostering skills in collaboration,problem-solving,and a sense of civic responsibility.Additionally,strategies are proposed to navigate the challenges associated with implementing interdisciplinary project-based learning,including aligning projects with standards,investing in professional development,leveraging community resources,and building support from stakeholders. 展开更多
